The Great Aussie Crunch vegetable grants - APPLICATION While parents are encouraged to supply vegetables for their children, we understand that this might not always be possible. To ensure all students are able to participate in Great Aussie Crunch activities, registered primary schools are eligible to receive a grant of up to $65.00 for the purchase of fresh vegetables. Grant payment: Grant monies are reimbursed to the school, parent association or canteen after the purchase of items. Schools are required to supply a Tax Invoice with Australian Business Number and proof of purchase (e.g. till receipt). A cheque or electronic funds transfer will then be arranged. Personal cheques cannot be issued. Relevant documents must be received before the 31 st October 2015 to be eligible for reimbursement. Items able to be purchased: Fresh vegetables only. Terms and conditions

Grant amount:

A maximum of $65.00

Grant payment:

Grant monies are reimbursed to the school, parent association or canteen after the purchase of items. Schools are required to supply a Tax Invoice with Australian Business Number and proof of purchase (e.g. till receipt). A cheque or electronic funds transfer will then be arranged. Personal cheques cannot be issued.

The Tax Invoice with Australian Business Number and proof of purchase can be submitted by email, fax or post (see details below). Original documents are not required.

Items able to be purchased:

The only food items eligible for reimbursement will be fresh vegetables for consumption at the Great Aussie Crunch event.

Additional:

It is recommended that the school registrar or financial officer be consulted before purchasing items to adhere with school buying and invoicing requirements.

Submission of grant payment documents:

After purchase of items, the following must be submitted to Cancer Council WA to ensure reimbursement:

1. Tax Invoice made out to Cancer Council WA from the school, parent association or canteen. The amount invoiced must not exceed the grant amount (e.g. the invoice must be equal to or less than $65).

1. Australian Business Number (ABN) of the school, parent association or canteen.

1. Proof of purchase e.g. till receipt. The proof of purchase may be more than the grant amount.

The above documentation must be received before the 31st October 2015 to be eligible for reimbursement.
