SUMMARY
Pediatric esophageal motility disorders cover a wide spectrum of disorders, varying in prevalence,
severity and symptom burden. This thesis covers two ends of the spectrum: gastroesophageal reflux
(GER) and pediatric GER disease (GERD) on the one hand and on the other hand achalasia, a rare
clinical entity in both children and adults. PART I comprises of studies on the (patho)physiology
underlying GER episodes and (presumed) GER related extraesophageal symptoms. The value of
novel diagnostic techniques in GER and motility disorders and the treatment of pediatric GERD with
acid suppression are explored in PART II. The final part of the thesis describes a study on the pre -
valence, characteristics and management of pediatric achalasia in the Netherlands.
PART I – (Patho)physiology
Transient lower esophageal sphincter relaxations (TLESR) account for up to 90% of GER episodes
both in non-hiatus hernia GERD patients and healthy controls.1–4 TLESR are relaxations of the lower
esophageal sphincter (LES) mediated by a vago-vagal pathway, initiated by tension receptors in the
proximal stomach (e.g. by gastric distension after a meal).5,6 However, TLESRs are also triggered by
changes in body positioning and stimuli far too small to induce gastric distension.7–12 This indicates
a more complex mechanism of TLESR triggering then can be explained by gastric distension alone.
In Chapter 1, the effect of esophagogastric junction (EGJ) distension on the triggering of TLESRs
after consummation of carbonated soft drink was assessed. We demonstrated an overall enhanced
triggering of TLESRs during distension of the EGJ with a balloon-inflated catheter compared to
the non-inflated state. Furthermore, EGJ distension induced significantly more belches compared
to a non-inflated state, especially after consumption of the carbonated drink. This, and the rise in
EGJ pressure after balloon inflation alone, cannot solely be explained by the excitation of common
known tension receptors in the proximal stomach. We propose a role for vago-vagal afferents in and
around the EGJ in the ‘sensing’ of gastric contents that could for example lower the threshold for
distension induced vago-vagal triggering of TLESR.
In GERD patients and healthy adults, TLESRs and GER have been shown to occur more frequently in
right lateral position (RLP) compared to left lateral position (LLP).11,13,14 Moreover, it was shown that
TLESRs occur more frequently and are more likely to cause liquid GER in RLP in healthy premature
infants, despite more rapid gastric emptying.15 Additionally, minute volumes of a liquid meal caused
a significant faster onset of TLESR triggering after the start of a meal infusion in RLP compared
to LLP.11 In Chapter 2, the mechanisms associated with TLESR triggering in altered body positions,
the activation of mechanoreceptors by distension and accommodation of the stomach, distribution
of gastric contents within the stomach and the rate of gastric emptying, were assessed. Ten adult
GERD patients and ten healthy volunteers were studied for 90min (30min test-meal infusion, 30min
post-prandial in either RLP or LLP [randomly assigned] and 30 min in alternate position). The study
was repeated on a separate day in reverse position. In GERD patients, TLESRs, GER, distension of
the proximal stomach and gastric emptying are increased in RLP compared to LLP. This effect was

not seen in healthy volunteers. This study suggests that the interaction of the factors regulating the
number of TLESRs may be disrupted in GERD patients.
Apnea is the cessation of respiratory airflow and can be central (no respiratory effort), obstructive
(respiratory effort present but not resulting in airflow due to upper airway obstruction) or mixed in
nature. Clinically significant apneas last >20 seconds, or for a period of >— 2 breaths accompanied
with significant desaturation, hypoxemia or bradycardia.16 Apneas, irrespective of their nature, are
proposed to be GER-related. The persuasion that GER and apneas are causally related is embedded
in guidelines for the treatment of GER disease (GERD) and pathological apneas.17,18 We systematically
reviewed articles from PubMed, Embase, and the Cochrane Database for this association between
GER and apnea in infants in Chapter 3. Of 1959 abstracts found, only 6 studies met the inclusion
criteria. All studies had poor methodological quality and extracted data were too heterogeneous to
be pooled. A total of 289 infants were included. We found insufficient evidence for an association
between GER and apnea in infants, with only one of the included six studies finding a positive asso-
ciation between apneas and GER in infants.
The aim of Chapter 4 was to assess to what extent IL-8 responses of bronchial epithelial cells are
modulated when exposed to gastric juice (GJ) collected from children with short-term or chronic
acid suppression treatment.19,20 The higher occurrence of respiratory tract infections during PPI
therapy has been hypothesized to be caused by the direct harmful effect of aspirated nonacid
refluxate.21,22 We exposed cultured bronchial cells from a NCI-H292 cell line and brushed pediatric
primary bronchial epithelial cells (PBECs) to GJ samples. Samples were collected from 16 pediatric
long-term proton pump inhibitor (PPI) users (LT), 16 controls and 9 children admitted to the pedi-
atric intensive care unit (PICU), before and 72h after start of PPI therapy. We found that GJ of
chronic PPI users (<18 years) is similar pro-inflammatory and cytotoxic to bronchial tissue compared
to controls. Gastric juice, both with and without acid suppression, inhibits IL-8 response to additional
stimulation with TNF-α, which could lead to inadequate anti-bacterial responses in both groups. This
queries the relevance of PPI treatment for reducing inflammation and cytotoxicity. Gastric juice
from PICU patients on PPI therapy induced more IL-8 in NCI-H292 cells, was more cytotoxic to PBECs
of multiple donors, and reduced the IL-8 response to additional stimulation with TNF-α even more
compared to PICU patients before therapy. Moreover, we found altered bile acids composition of
GJ in LT compared to controls (more conjugated taurocholate) and after start PPI therapy in PICU
children (more deconjugated cholate). Concentrations of total bile salts have been shown to damage
esophageal and respiratory mucosa in animal models and is associated with chronic rejection (bron-
chiolitis obliterans syndrome) in patients who underwent lung transplantation.23,24 Acid suppression
therapy in pediatric patients might not prevent extra-esophageal reflux symptoms and potentially
increases the risk of (secondary) respiratory infections, especially in PICU patients. The use of PPIs
in pediatric patients should thus be well founded, evaluated regularly and phased out if possible.

Diagnosing GERD in pediatric patients is difficult, since no combination of symptoms is conclusive.
GER episodes occur in all age groups and in infancy GER is often visible, as effortless regurgitation.
Symptoms are generally mild and self-limiting.25 If symptoms become troublesome or complications
occur, GER is referred to as GER disease (GERD). However, quantifying “troublesome” is a subjec-
tive measure, especially in infants, when parents report the symptoms. A broad set of diagnostic
tests is available to quantify or qualify GERD, however their diagnostic accuracy is unclear. Chapter 5
systematically reviews the most commonly used diagnostic tools in pediatric GERD (pH-metry,
pH-impedance metry, barium swallow studies, esophagogastroscopy, barium contrast study, scin-
tigraphy, and empirical treatment as diagnostic tool) as compared to the current ‘gold standard’:
history taking and physical examination. We included six studies in this review, all investigating the
additional value of pH-metry to medical history and physical exam. Two studies assessed the addi-
tional value of endoscopy as well. In total, 408 patients (age 1 month-13.6 years) and 145 control
subjects (age 1 month-16.9 years) were included. The range of reported sensitivity and specificity
was broad and unreliable because of poor methodological quality and inadequate study design.
Therefore, the additional value of diagnostic tests in children suspected of GERD remains unclear.
pH-impedance metry is recognized to be superior to conventional pH-metry because of its ability
to detect liquid or gaseous bolus flow (swallows or GER), regardless of acidity.26 Despite existing
criteria for scoring gastroesophageal reflux (GER) on combined esophageal multichannel pH-impe-
dance tracings, intra- and interobserver variability is large and agreement with automated anal-
ysis is poor.27 Therefore, in Chapter 6, 21 experts from 10 countries were asked to mark presence
(yes/no) of 79 difficult GER patterns selected from pediatric and adult pH-impedance tracings in an
online pre-assessment. During a subsequent consensus meeting, experts voted on the patterns that
did not reach majority consensus (>70% agreement) in the pre-assessment. Consensus outcome was
considered the gold standard. Based on the 79 difficult impedance patterns, eight characteristics
of GER on pH-impedance were formulated. Combining these in multiple logistic regression ana -
lysis did not result in an algorithm for optimal sensitivity and specificity for the detection of GER.
However, the presence of a retrograde propagation pattern is the best indicator of GER in difficult
pH-impedance patterns. Apparently, achieved consensus among international experts is based on
visual pattern recognition, which is not easily described by one or more parameters, except for the
presence of retrograde flow.
Chapter 7 describes the results of an intra- and interobserver variability study for the diagnosis of
esophageal motility disorders in a pediatric cohort using high resolution manometry (HRM). The
introduction of HRM with esophageal pressure topography (EPT) allows for better characterization
of esophageal motor function. The 2012 Chicago Classification (CC) algorithm v2.0 for esophageal
motility, with application through interactive analysis software, facilitates this diagnostic interpreta-
tion of pressure recordings and has been shown to be reliable and reproducible to the adult popu-
lation.28 Its applicability to the pediatric population is unknown. In our study, 6 expert raters and 5
raters naïve to HRM interpretation completed analysis of 30 HRM measurements twice (minimum of
7 days between sessions). Software-placed anatomical landmarks and EPT metrics were manually

adjusted or removed. Automatically generated and raters’ subjective CC diagnosis were assigned to
each measurement. Automated software based CC diagnosis of pediatric esophageal motility disor-
ders showed high inter- and intra-rater reliability amongst both experts and non-experts for most
motility disorders. However, the Chicago Classification v2.0 is less reliable and influenced by rater’s
expertise for the diagnosis of esophageal motility disorders such as diffuse esophageal spasm and
achalasia. This could have important consequences, both for clinical and research use of the CC
classification.
The use of acid suppressive medication for the treatment of pediatric GERD, both in the Netherlands
and internationally, has steadily increased over the past decade. However, these data were extra-
polated from adult studies. Current guidelines recommend a conservative policy concerning the use
of acid suppressive medication.17,18 However, a thorough review of the available evidence was lacking.
Therefore, in Chapter 8, we systematically searched PubMed, Embase, and the Cochrane Database
for randomized controlled trials and crossover studies investigating efficacy and safety of the acid
suppressive agent PPI in children with GERD. Twelve studies were included with data from children
aged 0–17 years. Five studies considered infants, 5 included children and 2 adolescents. Data extrac-
tion showed that PPIs are not effective in reducing GERD symptoms in infants. If the primary aim
is to treat GER-related symptoms in infants, PPIs should not be prescribed. Although placebo-con-
trolled studies are lacking in older children and adolescents, randomized trials have shown that PPIs
are equally effective in reducing GERD symptoms compared with their controls (alginates, ranitidine,
different-dosage PPIs).
The last chapter (Chapter 9) of this thesis reports the symptoms, diagnosis, management, and
clinical course of a cohort of achalasia patients in the Netherlands, diagnosed at a pediatric age.
Medical charts of all registered Dutch patients (<18 years) diagnosed between January 1990 and
December 2013, were retrospectively reviewed for data on presenting symptoms, treatment and
relapses. Prospectively, data on severity of achalasia symptoms and quality of life were obtained
via administration of validated questionnaires. The mean incidence of pediatric achalasia in The
Netherlands is 0.1 per 100,000 children/year and the prevalence in 2012 was 0.9/100,000 children.
Primary presenting symptoms included dysphagia (83%), regurgitation (69%), weight loss (44%)
and chest pain (37%). Manometry was incorporated in the diagnostic evaluation in only 86% of
cases. In eleven patients (14%) the diagnosis was based on barium swallow and/or upper endoscopy
only, without a documented manometry recording.
Pneumatic dilation was the predominant initial treatment of choice over (laparoscopic or open)
Heller’s Myotomy. However, pneumatic dilation required far more re-treatment due to relapse of
symptoms compared to initial Heller’s myotomy. The high number of reported complications after
Heller’s myotomy, on the other hand, indicates the need for international, prospective, randomized
studies evaluating both treatment approaches. Despite the risk of serious late complications, many
children had no documented clinical follow up after treatment. Prospectively, nearly half of the
patients were still symptomatic years after their last follow up visit. Disease-related and general
quality of life was lower in achalasia with long-standing disease compared to patients who were still
<18 years at time of administration of the questionnaire.

In this thesis, the (patho)physiology, diagnosis and management of a common (gastroesophageal
reflux disease, [GERD]) and a rare (achalasia) esophageal motility disorder in children is addressed.
Neuroregulatory mechanisms behind the triggering of transient lower esophageal sphincter relaxa-
tions (TLESRs) are not yet completely understood, as was shown when distending the EGJ prior and
post the intake of a carbonated drink. Besides the known TLESR regulators, i.e. tension receptors in
the proximal stomach, future research should focus on novel described mechanoreceptors in and
around the EGJ and their role in GER disease. One approach could be making use of the EndoFLIP®
device, which allows detailed identification of the role of different triggers (distension, solid and
liquid meals) on EGJ distensibility and compliance in GERD patients and healthy volunteers.
We described in detail that the effect of lateral position on GER and its underlying mechanisms
(TLESRs, gastric emptying and gastric distension) is significantly different in GERD patients
compared to healthy volunteers. These effects might be related to more rapid delivery of nutrients
to the duodenum in GERD patients and possibly by pooling of gastric contents in the fundus in
right lateral positioning. In line with other adult studies,29,30 our results support the possibility of an
acid pool (so called ‘acid pocket’) floating on top of the gastric contents, from which GER is more
likely to be acid when a TLESR occurs. This acid pool seems to be dependent on body position, as
we observed a distinctly different pattern of 99mTc-pertechnetate pooling in right vs. left lateral
positioning. Our experimental setting was not specifically designed to measure pooling of gastric
acid and, therefore, future research should assess the exact position of the acid pool in relation to
the EGJ and body position. It is yet unknown whether such an acid pocket exists in infants as well,
and whether it might play a role in the generation of GER symptoms. The existence of a body posi-
tion-dependent acid pocket is relevant to infants, whose body position during the day is different
from that of adults. It is however unlikely that if it is ethically possible to evaluate the existence of an
acid pocket in infants, since invasive techniques and high radiation exposure are needed to visualize
this acid pocket.
In this thesis, we found that the composition of gastric juice from infants and children is altered
during proton pump inhibitor (PPI) treatment and the pro-inflammatory components of non-acid
gastric juice induce altered IL-8 responses in bronchial tissue. This argues for critical re-evaluation of
the use of PPI for the prevention of pulmonary complications. Furthermore, future research should
focus on clarifying the presumed predominantly transcriptional mechanism by which non-acid
gastric juice modulates IL-8 responses.
The diagnosis of GERD in children, and especially in infants, is challenging. When no clear alarm
symptoms are present, it is difficult to diagnose if symptoms are actually signs of a disease which
requires treatment. Alternatively, symptoms might represent a transient phase of physiological GER
for which conservative therapy is sufficient. There is no well-defined symptom complex of GERD,
and current diagnostic tools are falling short, as was systematically reviewed in this thesis. There is
an urgent need to distinguish physiologic GER from pathologic GERD in children, to avoid unneces-

sary treatment, reassure parents/caregivers and reduce health care costs. One of the problems is
that normative values for traditional diagnostic tests are hard to acquire in a pediatric population,
as it would be regarded unethical to perform invasive measurements in healthy children. However,
new trial designs are being increasingly used to overcome ethical difficulties and they could clear
the way to in fact perform valid research. In the case of the additional value of diagnostic tests for
pediatric GERD, this could be a randomized controlled trial in which one group receives treatment
based on the outcome of a diagnostic tool for GERD and the control group receives treatment based
on history and physical examination. By monitoring symptom improvement, stratifying for age, and
taking in account confounders such as the factor of time, diagnostic value could be determined.
Because of the lack of adequate diagnostic tests for pediatric GERD, we aimed to characterize
different parameters of difficult pH-impedance patterns and facilitate the analysis of this diagnostic
tool. However, experts cooperating in this study were shown to rely on visual pattern recognition
to classify pH-impedance patterns as GER, which is not easily incorporated into an algorithm for
automated analysis. The adaptation of the automated analysis in such way that each retrograde
pattern is highlighted, followed by visual-manual scoring of each of those patterns, could aid the
reproducibility and reliability of analysis of pH-impedance tracings. Studies applying this approach
are needed in the future.
Alternatively, new non-invasive diagnostic tools should be explored. The potential of an office-based,
non-invasive test for the diagnosis of pediatric GERD could improve the management of infants
and children with GER symptoms. A test that could reassure parents and/or care-givers that there
is no need for pharmacological intervention and that symptoms will subside spontaneously (high
negative predictive value) would likely decrease the number of prescriptions for acid-suppressive
medication. At the same time, non-invasive diagnostic tests could possibly pinpoint a subgroup of
patients with pathologic GERD, who indeed might benefit from acid-suppressive medication. Such a
possible new diagnostic test tool could be the detection of e.g. pepsin concentration in saliva. Tests
like these have not yet been studied properly in infants and children with GERD or healthy infants.
This thesis shows that Chicago Classification criteria (v2.0) for esophageal motility disorders are
highly reliable and reproducible in children (Chapter 7). In the very recently updated Chicago Clas-
sification (v.3.0), it was emphasized that normal cut-off values differ per type of catheter.31 In addi-
tion, esophageal pressure topography (EPT) metrics seem to be influenced by age and size.32 Up
to date, it is unknown how age-adjusted catheter sizes influence EPT metrics in healthy children.
To accurately diagnose pediatric esophageal motility disorders, it would of course be preferable to
acquire normative HRM data in all age groups. Ethical committees, however, are unlikely to allow
manometric studies in healthy infants to obtain such data. On the other hand, one might speculate
whether applying non-evidence based invasive techniques in pediatric patients is ethically accept-
able. Insertion of esophageal catheters during general anesthetics might overcome ethical barriers
in future research. This method is not ideal since it is unknown what effect general anesthetics have
on esophageal motility in children. However, it could provide the much needed normative data to aid
diagnosis and treatment of esophageal motility disorders in children.

The unsatisfactory efficacy and safety of current available treatment of pediatric GERD with proton
pump inhibitors, as systematically reviewed in this thesis, suggest the need to expand our horizons
to other treatment strategies. Reduction of TLESRs might be induced in other ways than inhibitory
agents or surgery. For example, recently, electrical stimulation of the LES region has shown to lessen
GER in symptomatic adult GERD patients.33–35 Alternatively, treatment shift might focus to other
aspects of the refluxate, besides acidity.36 Refluxate might contain varying concentrations of pepsin,
gas, or contents of duodenal reflux (such as bile acid and pancreatic enzymes). This, and other
characteristics, such as volume and proximal extent,37 have been shown to have impact on symptom
perception.19,38–42
We looked further into the prevalence, treatment and outcome rates of the most severe of esopha-
geal motility disorders: achalasia. From our data it became clear that there is a need for ran -
domized trials comparing initial pneumatic balloon dilation and Heller’s myotomy (+/- fundoplication)
in children with achalasia. Sufficiently large patients groups should be included in such prospective
studies. Since achalasia is a rare disorder, international collaboration is required to ensure adequate
group sizes. Furthermore, to optimize treatment and ensure quality of life, there is a need for stan-
dardized follow-up regimes to improve clinical outcome and transfer to adult care in pediatric acha-
lasia patients.
The results of the presented studies in this thesis indicate the need to rethink our current diag-
nostic and treatment approach towards pediatric GERD and the need to develop novel diagnostic
tools and treatment strategies. We provided a basis for more clinical and laboratory-based research,
looking further into the mechanisms behind TLESR triggering and the relationship between proton
pump inhibitor treatment and respiratory complications. Finally, the data on diagnosis and clinical
management of a Dutch pediatric achalasia show there is a need to internationally cooperate and
ensure optimal management of this severe motility disorder at a pediatric age.

SAMENVATTING
Slokdarm-motiliteitstoornissen op de kinderleeftijd omvatten een breed spectrum van aandoe-
ningen, variërend in ernst. In dit proefschrift worden de twee uiteinden van dit spectrum besproken:
het veel voorkomende verschijnsel gastro-oesofageale reflux (GOR) en GOR-ziekte (GORZ), en de
zeldzame slokdarm-motiliteitstoornis achalasie. DEEL I omvat studies over de (patho)fysiologie
achter GOR en extra-oesofageale symptomen die (mogelijk) GOR-gerelateerd zijn. De diagnose van
GORZ en andere motiliteitstoornissen met conventionele en nieuwe technologieën en de behande-
ling van GORZ op de kinderleeftijd met zuurremmende medicatie wordt onderzocht in DEEL II. Dit
proefschrift sluit af met een studie naar achalasia op de kinderleeftijd in Nederland.
De slokdarm transporteert vloeibare en vaste bolussen van de mond naar de maag. De wand van de
slokdarm bestaat uit dwarsgestreept (bovenste 1/3) en glad spierweefsel, longitudinaal en circulair
gerangschikt. De slokdarm wordt aan beide uiteinden begrensd door twee spieren: de bovenste en
onderste slokdarmsfincter (OSS, ook wel aangeduid als ‘maagklepje’ in lekentaal), welke openen na
het slikken van een bolus. Als de OSS opent zonder een voorafgaande slik, noemen we dat een spon-
tane relaxatie van de onderste slokdarmsfincter. Deze spontane relaxaties van de OSS faciliteren het
ventileren van overtollig gas uit de maag (een boer). Er kan tegelijkertijd ook terugvloed van maag-
inhoud in de slokdarm optreden: vloeibare gastro-oesofageale reflux (GOR). Spontane relaxaties en
GOR episodes zijn fysiologisch en komen voor op alle leeftijden. We spreken van gastro-oesofageale
refluxziekte (GORZ) als er hinderlijke symptomen en/of complicaties ontstaan van de GOR-episodes.
DEEL I - (patho) fysiologie
Tot 90% van alle GOR-episodes wordt voorafgegaan door een spontane relaxatie van de OSS. Dit
geldt zowel voor patiënten met GORZ als voor gezonde vrijwilligers.1-4 Deze spontane relaxaties
worden vago-vagaal gemedieerd en worden geïnitieerd als onder andere de rekreceptoren in de
proximale maag worden geprikkeld (bijvoorbeeld door het uitzetten van de maag na een maaltijd).5,6
Echter, het is bekend dat deze spontane relaxaties ook worden geïnitieerd door stimuli die veel te
klein zijn om de rekreceptoren te kunnen activeren.7-12 Dit geeft aan dat het mechanisme achter het
uitlokken van spontane relaxaties van de OSS complexer is dan kan worden verklaard door rekrecep-
toren alleen.
In hoofdstuk 1 onderzochten we het effect van een kleine distensie van de OSS op het uitlokken van
de spontane relaxaties in gezonde vrijwilligers, voor en na het drinken van koolzuurhoudende fris-
drank. Wij zagen dat meer spontane relaxaties werden uitgelokt als de OSS minimaal gedistendeerd
werd met een ballon-katheter vergeleken met de niet-gedistendeerde toestand. Ook ver oorzaakt de
distensie meer boeren in vergelijking met een niet gedistendeerde toestand, vooral na het drinken
van de koolzuurhoudende drank. Dit, en de stijging van de OSS-druk alleen al na het opblazen van
de ballon, kan niet alleen worden verklaard door de activatie van de rekreceptoren, die bij deze
marginale distensie nog niet worden geactiveerd. Vago-vagale afferente receptoren rondom de OSS
zouden een rol kunnen spelen in de ‘detectie’ van de inhoud van de maag. Deze sensoren kunnen

vervolgens bijvoorbeeld de drempel voor rek-geïnduceerde vago-vagaal gemedieerde transiente
relaxaties verlagen.
Bij GORZ-patiënten en gezonde volwassenen is aangetoond dat spontane relaxaties vaker voorkomen
bij personen gelegen in rechter zijligging (RZ) vergeleken met linker zijligging (LZ).11,13,14 Bovendien
werd aangetoond dat bij premature zuigelingen in RZ de spontane relaxaties vaker vloeibare GOR
veroorzaken, ondanks een snellere maaglediging in RZ vergeleken met LZ.15 Tot slot toonde onze
onderzoeksgroep eerder aan dat infusie van kleine volumes (enkele milliliters) in de maag, signifi-
cant sneller spontane relaxaties uitlokt in RZ vergeleken met LZ.11 Het optreden van spontane re-
laxaties, de distributie van maaginhoud in de maag en de snelheid van maaglediging na een veran-
derde lichaamshouding werd verder onderzocht in hoofdstuk 2. Tien volwassen GORZ-patiënten
en tien gezonde vrijwilligers werden gedurende 90 min gemeten (30 min infusie testmaaltijd via
neus-maagsonde, 30min postprandiaal in RZ of LZ [willekeurig toegewezen] en 30 min in alter-
natieve positie). De studie werd herhaald op een andere dag in omgekeerde positie. Bij patiënten
met GORZ zagen we meer spontane relaxaties, meer GOR, meer uitzetting van de proximale maag
en een snellere maaglediging in RZ vergeleken met LZ. Deze verschillen zagen we niet bij gezonde
vrijwilligers, wat suggereert dat de interactie van de factoren die het aantal spontane relaxaties
bepaalt, verstoord is bij GORZ patiënten.
Een apneu is gedefinieerd als een tijdelijke stop van ademhaling. Deze stop kan centraal (geen adem-
halingsbeweging), obstructief (ademhalingsbewegingen die niet leiden tot luchtstroom als gevolg
van bovenste luchtwegobstructie) en/of gemengd van aard zijn. Een klinisch relevante (patholo-
gische) apneu duurt >20 seconden, of >— 2 ademteugen, en gaat gepaard met aanzienlijke desaturatie,
hypoxemie of bradycardie.16 Apneus, van welke aard dan ook, worden verondersteld gerelateerd te
zijn aan GOR-episodes. De overtuiging dat GOR en apneus causaal gerelateerd zijn, is ingebed in
internationale richtlijnen voor de behandeling van de GORZ en pathologische apneus in kinderen.17,18
Om te achterhalen of deze aannames ook gestaafd worden door bewijs, hebben we in hoofdstuk
3 een systematisch literatuuronderzoek (in PubMed, Embase, en de Cochrane Database) gedaan
naar de associatie tussen GOR en apneus bij zuigelingen. Van de 1959 abstracts die uit de search
kwamen, voldeden er slechts 6 aan onze inclusiecriteria. Al deze studies hadden een slechte meth-
odologische kwaliteit en waren te heterogeen om te worden samengevoegd voor meta-analyse. In
totaal werden 289 kinderen geïncludeerd. In onze review vonden we onvoldoende bewijs voor een
associatie tussen GOR en apneus bij zuigelingen: slechts één van de zes studies vond een positief
verband tussen GOR en apneus.
Indien GOR tot proximaal in de slokdarm reikt, kan het worden geïnhaleerd: aspiratie. Aspiratie van
maagsap kan een ontstekingsreactie in de longen veroorzaken, welke voornamelijk wordt geme-
dieerd door het cytokine Interleukine-8 (IL-8). Recent onderzoek bij volwassenen heeft aangetoond
dat het vaker voorkomen van respiratoire infecties bij protonpomp-zuurremmergebruik mogelijk
wordt veroorzaakt door het direct schadelijke effect van niet-zuur maagsap op longepitheel.19-22 Het
doel van hoofdstuk 4 was om te achterhalen in welke mate de IL-8 respons gemoduleerd wordt
wanneer bronchiale epitheelcellen worden blootgesteld aan maagsap van kinderen die wel of geen
zuurremmers gebruiken. Gekweekte bronchiale cellen van een H292 cellijn en gebrushte primaire

bronchiale epitheelcellen (PBECs) van kinderen werden blootgesteld aan maagsap-monsters. De
monsters werden zowel vóór als 72 uur ná het starten van protonpompremmers (PPRs) verzameld,
bij 9 kinderen opgenomen op de intensive care afdeling (PICU). Bij 16 kinderen met chronisch PPR
gebruik (LT) en bij 16 controles (C) werd 1x maagsap afgenomen. Maagsap van chronische PPR
gebruikers (<18 jaar) blijkt net zo pro-inflammatoir en cytotoxisch te werken op bronchiale epith-
eelcellen, als het maagsap van controles. Maagsap remt de IL-8 respons na extra stimulatie met
TNF-α, zowel met als zonder PPR, in gelijke mate, wat kan leiden tot inadequate antibacteriële
respons in beide groepen (LT en C). Dit zet een vraagteken bij een PPR behandeling met als doel het
verminderen van respiratoire infecties en cytotoxiciteit bij kinderen met aspiratie-risico. Bij patienten
opgenomen op de intensive care, induceert maagsap na de start van PPR-therapie zelfs meer IL-8
in H292 cellen vergeleken met vóór de start van PPR, en is het meer cytotoxisch voor PBECs. Ook is
de IL-8 respons na extra stimulatie met TNF-α sterker verminderd na de start van PPR ten opzichte
van vóór behandeling. Naast deze bevindingen, vonden we een veranderde galzout-samenstelling
van het maagsap van LT gebruikers in vergelijking met controles (meer geconjugeerde taurocho-
laat) en na de start PPR- therapie bij PICU kinderen vergeleken met voor PPR (meer gedeconju-
geerd cholaat). Galzouten zijn schadelijk voor de slokdarm en respiratoire mucosa in diermodellen
en worden geassocieerd met chronische afstoting (bronchiolitis obliterans) bij patiënten die long-
transplantatie ondergingen.23,24 Onze studie toont aan dat zuurremmende therapie bij kinderen de
extra-oesofageale refluxsymptomen waarschijnlijk niet vermindert en mogelijk de kans op (secun-
daire) luchtweginfecties vergroot, vooral bij PICU patiënten. Het gebruik van PPRs bij kinderen moet
dus gegrond zijn, regelmatig worden geëvalueerd en worden afgebouwd indien mogelijk. Bij PICU
patiënten dient PPR-therapie vermeden te worden indien mogelijk.
DEEL II - Diagnose en behandeling
Het vaststellen van GORZ bij kinderen is moeilijk, omdat er geen specifieke combinatie van symp-
tomen bestaat. GOR komt voor op alle leeftijden, en symptomatische GOR (regurgitatie) is normaal
bij zuigelingen. De symptomen van GOR zijn dan over het algemeen mild en gaan vanzelf over.25
Als de symptomen hinderlijk worden of als er complicaties optreden, spreken we van GOR-ziekte of
‘refluxziekte’. Echter, de term ‘hinderlijk’ is een subjectieve maat en is deels afhankelijk van de inter-
pretatie van ouders, vooral bij jonge kinderen. Een scala van diagnostische tests is beschikbaar om
de GOR te kwantificeren of te kwalificeren, maar hun diagnostische nauwkeurigheid is onduidelijk.
In Hoofdstuk 5 evalueren we systematisch de literatuur voor de meest gebruikte diagnostische
instrumenten voor het vaststellen van GORZ op de kinderleeftijd (pH-metrie, pH-impedantie metrie,
bariumslikfotos, oesofagogastroscopie, bariumcontraststudie, scintigrafie en empirische behan-
deling als diagnostisch hulpmiddel) in vergelijking met de huidige ‘gouden standaard’: anamnese
en lichamelijk onderzoek. Zes studies maakten deel uit van deze review, die allen de toegevoegde
waarde van de pH-metrie met anamnese en lichamelijk onderzoek vergeleken. Twee studies onder-
zochten daarnaast de additionele waarde van gastroscopie. In totaal werden 408 patiënten (leeftijd
1 maand - 13,6 jaar) en 145 controles (leeftijd 1 maand - 16,9 jaar) geïncludeerd. De spreiding van de
gerapporteerde sensitiviteit en specificiteit was breed en onbetrouwbaar vanwege de slechte meth-

odologische kwaliteit van de studies. Daarom is de toegevoegde waarde van diagnostische tests bij
kinderen met vermoedelijk GORZ nog altijd onduidelijk.
Gecombineerde oesofageale multichannel pH-impedantiemetrie is superieur gebleken aan pH-metrie
alleen, vanwege het vermogen om vloeibare of gasvormige bolus-stromen te detecteren (antegraad
en retrograad: een slik of GOR), ongeacht de zuurgraad.26 Ondanks de bestaande criteria voor het
identificeren van GOR op pH-impedantie metingen, is de intra- en interobservervariatie groot en
komt deze slecht overeen met de geautomatiseerde analyse.27 Zoals beschreven wordt in hoofdstuk
6, hebben we daarom 21 pH-impedantie experts uit 10 landen gevraagd om online 79 moeilijke impe-
dantie-patronen te beoordelen op de aanwezigheid van GOR. Alle episoden werden geselecteerd uit
volwassen en pediatrische metingen (pre-assessment). Vervolgens hebben de experts tijdens een
consensusbijeenkomst gestemd over de patronen die in de pre-assessment geen meerderheids-
consensus (>70% overeenkomst) hebben gehaald. De consensus-uitkomst werd beschouwd als de
gouden standaard. Gebaseerd op de 79 moeilijke impedantie patronen, werden acht kenmerken
van GOR op een pH-impedantie meting geformuleerd. Het combineren van deze kenmerken in een
regressieanalyse resulteerde niet in een algoritme met optimale sensitiviteit en specificiteit voor
de detectie van GOR. Alleen de aanwezigheid van een retrograad propagatiepatroon is een goede
indicator voor GOR in moeilijke pH-impedantiepatronen. Deze consensus-uitkomst toont aan dat
experts terugvallen op visuele patroonherkenning voor het scoren van GOR, en dit proces laat zich
niet gemakkelijk vangen in een algoritme om GOR te herkennen met een automatische analyse.
Hoofdstuk 7 beschrijft de resultaten van een intra- en interbeoordelaar variabiliteitstudie naar
het diagnosticeren van slokdarm-motiliteitstoornissen in een kindercohort met een relatief nieuwe
techniek: hogeresolutiemanometrie (HRM). De introductie van HRM met oesofagus-druk-topografie
(ODT) heeft ervoor gezorgd dat de motorische functies van de slokdarm beter in beeld kunnen
worden gebracht. De Chicago Classificatie (V2.0, 2012), een algoritme voor de slokdarm-motiliteit
gemeten met HRM toegepast in interactieve analyse-software, faciliteert deze diagnostische inter-
pretatie. De Chicago Classificatie is bij volwassenen betrouwbaar en reproduceerbaar gebleken.
Echter, de toepasbaarheid ervan in de pediatrische populatie is niet bekend.28 In onze studie analy-
seerden 6 ervaren HRM-beoordelaars en 5 niet-ervaren HRM-beoordelaars twee keer een set van
30 HRM-metingen (minimaal 7 dagen tussen de twee sessies). De beoordelaars verschoven of
verwijderden naar eigen inzicht de door de software automatisch geplaatste oriëntatiepunten en
ODT-metrics. Vervolgens werd per meting een geautomatiseerde Chicago Classificatie-diagnose
geregistreerd, alsmede een subjectieve diagnose van de beoordelaar. De geautomatiseerde diag-
nose toonde een hoge inter- en intrabeoordelaar betrouwbaarheid bij zowel ervaren als onervaren
beoordelaars. Echter, voor de motiliteitstoornis diffuse slokdarmspasmen en achalasie bleek de
semi-automatische software het minst betrouwbaar en het meest afhankelijk van de deskundigheid
van de beoordelaar. Dit kan belangrijke gevolgen hebben, voor zowel klinische als onderzoekstoe-
passingen van de Chicago Classificatie.
Het gebruik van zuurremmende medicatie voor de behandeling van GORZ op de kinderleeftijd is
gestaag toegenomen in de afgelopen tien jaar, zowel binnen als buiten Nederland. De argumenten
om deze medicatie voor te schrijven zijn grotendeels voortgekomen uit studies bij volwassenen.

De huidige GORZ-richtlijnen adviseren een terughoudend beleid met betrekking tot het gebruik
van zuurremmende medicatie bij kinderen.17,18 Echter, een grondig overzicht van de beschikbare
evidence bij kinderen ontbrak. Daarom wordt in hoofdstuk 8 een systematische literatuurstudie
gepresenteerd, waarvoor we de databases PubMed, Embase en de Cochrane Database systematisch
hebben doorzocht. We includeerden gerandomiseerde gecontroleerde studies en crossover-studies
naar de werkzaamheid en veiligheid van PPRs bij kinderen met GORZ. Twaalf studies werden geïn-
cludeerd met gegevens van kinderen van 0-17 jaar. Vijf studies hadden betrekking op zuigelingen,
5 op kinderen en 2 op adolescenten. Analyse van gegevens toonde dat PPRs niet effectief zijn in
het verminderen van GORZ-symptomen bij zuigelingen. Hoewel placebogecontroleerde studies
ontbreken bij kinderen en adolescenten, lieten de geïncludeerde studies zien dat PPRs even effec-
tief zijn in het verminderen van GORZ-symptomen in vergelijking met alternatieve medicamenteuze
behandelingen (alginaten, ranitidine, andere dosering PPRs). Concluderend stellen we dat PPRs
niet moeten worden voorgeschreven als het voornaamste behandeldoel is om GORZ-symptomen bij
kinderen te verminderen.
Dit proefschrift sluit af met een onderzoek (hoofdstuk 9) naar de symptomen, diagnostiek, behan-
deling en klinisch beloop van een cohort van achalasie-patiënten in Nederland, gediagnosticeerd op
de kinderleeftijd. Achalasie is een zeldzame, ernstige motiliteitstoornis van de slokdarm, gekenmerkt
door de afwezigheid van peristaltiek en gefaalde relaxatie van de OSS. Dit leidt tot progressieve
dysfagie en stase van voedsel, door de verminderde bolusstroom naar de maag. Medische dossiers
van alle geregistreerde Nederlandse achalasiepatiënten (<18 jaar), gediagnosticeerd tussen januari
1990 en december 2013, werden opgevraagd en gegevens over de symptomen bij diagnose, behande-
ling en recidieven werd retrospectief geëxtraheerd. Prospectief werden gegevens verzameld over de
ernst van achalasiesymptomen en kwaliteit van leven, door middel van het afnemen van gevalideerde
vragenlijsten bij de retrospectief geïdentificeerde patiënten. De gemiddelde incidentie van acha-
lasie op de kinderleeftijd is 0,1 per 100.000 kinderen/jaar en de prevalentie in 2012 was 0,9/100.000
kinderen. Symptomen bij diagnose waren dysfagie (83%), regurgitatie (69%), gewichtsverlies (44%)
en pijn op de borst (37%). Manometrie werd in 86% van de gevallen opgenomen in diagnostische
evaluatie. Elf (14%) van de patiënten werden alleen gediagnosticeerd op basis van een gastroscopie,
zonder gedocumenteerde slokdarm-manometrie.
Pneumodilatatie van de OSS bleek de initiële geprefereerde behandeling, boven (laparoscopische
of open) Heller’s myotomie van de OSS. Echter, pneumodilatatie vereist veel meer herbehandeling
als gevolg van terugkerende symptomen in vergelijking met Heller’s myotomie. Het hoge aantal
gerapporteerde complicaties na Heller’s myotomie, aan de andere kant, onderstreept de noodzaak
van het uitvoeren van internationale, prospectieve, gerandomiseerde studies die Heller’s myotomie
als initiële behandeling vergelijken met pneumodilatatie bij kinderen met achalasie. Ondanks het
risico op ernstige late complicaties, hebben veel kinderen geen gedocumenteerd klinisch beloop
na de behandeling. Prospectief blijkt dat bijna de helft van hen nog symptomatisch is, jaren na het
laatste vervolgbezoek. Ziekte-gerelateerde en algemene kwaliteit van leven is lager bij patiënten
waarbij de achalasie al langer bestaand is, vergeleken met patiënten die <18 jaar waren ten tijde van
het afnemen van de vragenlijsten.

ALGEMENE DISCUSSIE & TOEKOMSTPERSPECTIEF
Dit proefschrift heeft betrekking op de (patho)fysiologie, diagnostiek en behandeling van kinderen
met slokdarmaandoeningen. Neuroregulatoire mechanismen achter het ontstaan van transiënte
relaxaties van de onderste slokdarmsfincter (OSS) worden nog niet volledig begrepen. Dit blijkt uit
onze studie waarbij we de OSS distendeerden voor en na het drinken van een koolzuurhoudende
drank. Toekomstig onderzoek naar de mechanismen achter het optreden van transiënte relaxaties
moet zich niet alleen richten op de bekende rekreceptoren in de maag, maar ook op mogelijk nieuwe
receptoren die zich rondom de OSS bevinden en het optreden van spontane relaxaties reguleren.
Eén van de onderzoeksmethoden die gebruikt zou kunnen worden, is de EndoFLIP®, een ballon-
katheter met elektrodenparen op de as. Met de EndoFLIP® kan men de verschillende invloeden
(distentie, vaste en vloeibare maaltijden) op de distenstibiliteit en compliantie van de OSS nauwke-
urig onderzoeken bij GORZ-patiënten en gezonde vrijwilligers.
We hebben beschreven dat het effect van zijligging op GOR en de achterliggende mechanismen (tran-
siënte relaxaties van de OSS, maaglediging en maagdistentie) anders werken bij GORZ-patiënten
vergeleken met gezonde vrijwilligers. Deze effecten zijn mogelijke gerelateerd aan een snellere
maaglediging bij GORZ-patiënten en het ‘poolen’ van de maaginhoud in de fundus in rechter zijlig-
ging. De resultaten van onze studie komen overeen met studies in volwassenen, en ondersteunen
de mogelijkheid van een drijvende zure laag op de maaginhoud, een zogenaamde ‘acid pocket’.29,30
Optredende GOR uit een acid pocket zou eerder zuur van aard kunnen zijn. Uit onze studie blijkt
dat de positie van de acid pocket waarschijnlijk afhankelijk is van de lichaamshouding, omdat we
een ander patroon van 99mTc-pertechnetateverdeling vonden in linker- en rechterzijligging. Echter,
omdat onze studie niet ontworpen was om het poolen van zuur in de maag te analyseren, is het
nodig deze hypothese te bevestigen in toekomstig onderzoek. Het is onbekend of bij zuigelingen ook
een acid pocket bestaat en of deze een rol speelt bij het ontstaan van GOR-klachten. Het bestaan van
een houdingsafhankelijk acid pocket is mogelijk interessant voor zuigelingen, omdat zij gedurende
de dag een andere houding hebben dan volwassenen. Helaas is het onduidelijk of onderzoek naar
een acid pocket bij kinderen zal kunnen worden uitgevoerd, omdat de noodzakelijke metingen
invasief van karakter zijn en de acid pocket alleen gevisualiseerd kan worden middels een hoge
stralingsbelasting.
In dit proefschrift hebben we beschreven dat de samenstelling van maagsap van zuigelingen en
kinderen verandert door het gebruik van protonpompremmers (PPRs) en dat pro-inflammatoire
componenten van niet-zuur maagsap de IL-8 respons van bronchiale cellen verandert. Deze bevin-
dingen vragen om een kritische herevaluatie van het gebruik van PPRs voor kinderen, bij wie men
respiratoire complicaties wilt voorkomen.
Het diagnosticeren van GORZ bij kinderen, en in het bijzonder bij zuigelingen, is niet makkelijk.
Wanneer er geen alarmsymptomen aanwezig zijn, kan men zich zelfs afvragen of er wel echt sprake
is van ziekte die behandeling behoeft, of slechts van een voorbijgaande fase van meer fysiologische
GOR waarbij een conservatieve benadering voldoende is. Er is geen goed gedefinieerd complex
van symptomen voor GORZ, en de huidige diagnostische middelen schieten tekort, zoals wordt

beschreven in dit proefschrift. Er is dringend behoefte aan een diagnosticum dat fysiologische GOR
van pathologische GORZ onderscheidt bij kinderen om onnodige behandeling te vermijden, ouders/
verzorgers gerust te stellen en de kosten voor de gezondheidszorg te reduceren. Eén van de pro-
blemen is dat normaalwaarden voor traditionele diagnostische tests moeilijk te verkrijgen zijn bij
kinderen, omdat het onethisch wordt geacht invasieve metingen bij gezonde kinderen uit te voeren
ter referentie. Steeds vaker worden nieuwe studiemethoden gebruikt om ethische problemen,
bijvoorbeeld bij gezonde controlegroepen, te omzeilen. In het geval van de toegevoegde waarde van
diagnostische tests voor pediatrische GORZ, zou een gerandomiseerde gecontroleerde trial kunnen
worden uitgevoerd waarin de ene groep behandeling krijgt op basis van de uitkomst van een diag-
nostisch hulpmiddel voor GORZ, en de controlegroep behandeling krijgt op basis van anamnese en
lichamelijk onderzoek. Door het monitoren van verbetering van de symptomen, met een stratificatie
voor leeftijd en rekening houdend met verstorende variabelen zoals de factor tijd, kan de additionele
diagnostische waarde van een test voor GORZ toch worden bepaald.
Vanwege het ontbreken van goede diagnostische tests voor GORZ, hebben we geprobeerd om een
van de huidige diagnostische tests, pH-impedantie meting, te verfijnen. Het doel van onze studie
was om moeilijke pH-impedantie patronen te karakteriseren en de analyse ervan te vergemakke-
lijken. Echter, de experts die aan deze studie deelnamen, bleken vooral op basis van patroonherken-
ning een GOR-episode te definiëren op pH-impedantiemetingen. Dit is niet gemakkelijk te vatten in
een algoritme voor automatische analyse. Deze automatische analyse zou in toekomstige studies
zo aangepast kunnen worden, dat alle retrograde patronen worden herkend. Vervolgens behoeven
deze patronen één voor één goed- of afkeuring van diegene die de analyses uitvoert. Dit kan de
herhaalbaarheid en betrouwbaarheid van pH-impedantie analyses verbeteren.
Als alternatief moeten nieuwe, niet-invasieve diagnostische tests worden onderzocht. Een simpele,
snelle en niet-invasieve test die ouders en hun verzorgers verzekert dat er geen noodzaak voor
farmacologische interventie is en dat de symptomen spontaan zullen overgaan (hoge negatieve
voorspellende waarde), zal vermoedelijk het aantal voorschriften van zuurremmers doen dalen.
Tegelijkertijd kan het een subgroep van patiënten met pathologische GORZ identificeren die wel
zouden profiteren van zuurremmende medicijnen. Een voorbeeld van een niet-invasieve test is het
kwantificeren van de hoeveelheid pepsine, een verteringsenzym, in speeksel. Tot op heden zijn er
onvoldoende studies verricht naar pepsinebepalingen in speeksel van gezonde kinderen en kinderen
met GOR-gerelateerde klachten.
In dit proefschrift hebben we aangetoond dat analyse van HRM metingen met behulp van de Chicago
Classificatie criteria (v2.0) zeer betrouwbaar en reproduceerbaar is. In de onlangs verschenen
Chicago Classificatie update (v3.0) wordt beschreven dat de HRM normaalwaarden bij volwassenen
verschillen per type en grootte van de katheter.31 Bovendien lijken oesofagus-druk-topografie (ODT)
metrics, waarop de Chicago Classificatie gebaseerd is, beïnvloed te worden door leeftijd en lengte.
Het is nog onbekend of en hoe leeftijdgecorrigeerde katheters de ODT metrics beïnvloeden bij
gezonde kinderen. Om slokdarmmotiliteitstoornissen nauwkeurig te diagnosticeren, zouden ideal-

iter normaalwaarden beschikbaar moeten zijn. Het is echter onwaarschijnlijk dat medisch-ethische
commissies toestemming geven om manometrische studies te verrichten bij gezonde kinderen. Aan
de andere kant kan het ook onethisch worden beschouwd om, zoals nu het geval is, invasieve onder-
zoeken uit te voeren bij kinderen die niet evidence-based zijn. Toekomstig onderzoek moet derhalve
gericht zijn op het verwerven van normaalwaarden voor HRM-data. Het inbrengen van een HRM
katheter onder algehele anesthesie zou de ethische barrières in de toekomst kunnen overkomen.
Hoewel de invloed van algehele anesthesie op de slokdarm-motiliteit bij kinderen onbekend is, zou
dit soort onderzoek de data kunnen genereren die hard nodig zijn om de diagnose en behandeling
van motiliteitstoornisssen bij kinderen te vergemakkelijken.
De onbevredigende effectiviteit en de grotendeels onbekende veiligheid van de huidige beschikbare
zuurremmende behandeling voor GORZ op de kinderleeftijd, onderstrepen de noodzaak om onze
horizon te verbreden en andere behandelingsstrategieën te exploreren. Het verminderen van tran-
siënte relaxaties van de OSS zou uitkomst kunnen bieden voor GORZ, maar de huidige medicamen-
teuze of chirurgische aanpak is onvoldoende. Onlangs is aangetoond dat elektrische stimulatie van
de regio waarin de OSS zich bevindt, een gunstig effect heeft op symptomen van GOR bij volwassen
GORZ-patiënten.33–35 Een andere behandelmogelijkheid is de verschuiving van therapie naar de
andere aspecten van het refluxaat, naast de zuurgraad.36 Therapie kan worden gefocust op verschil-
lende componenten van refluxaat, waarvan de relatie met klachten bekend is: pepsine, hoeveelheid
gasreflux, duodenale reflux (galzuur en pancreasenzymen), proximale extent van reflux.19,38–42
We hebben de prevalentie, behandeling en uitkomsten van de behandeling beschreven van de meest
ernstige slokdarm-motiliteitstoornis, achalasie. Uit onze data werd duidelijk er een behoefte bestaat
aan prospectieve studies die de verschillende behandelopties (pneumodilataties en Heller’s myot-
omie) met elkaar vergelijken. Om dit soort studies voldoende statistische power te geven, moeten
voldoende patiënten worden geïncludeerd. Voor een zeldzame, ernstige motiliteitstoornis als acha-
lasie, is het onontbeerlijk dat voor dit soort onderzoek internationale samenwerking wordt gezocht
om een voldoende groepsgrootte te garanderen. Naast deze studies om de behandeling te opti-
maliseren is er behoefte aan gestandaardiseerde follow-up-regimes om de klinische uitkomst te
verbeteren en de transitie naar volwassenenzorg soepel te laten verlopen.
De resultaten van de studies in dit proefschrift onderstrepen de noodzaak om bestaande diagnos-
tische en behandeltechnische benaderingen van GORZ op de kinderleeftijd te herevalueren en
nieuwe diagnostische tests en behandelingen te ontwikkelen. We hebben een basis gelegd voor
verder klinisch en basaal onderzoek, wat focust op de werkingsmechanismen achter transiënte
relaxaties van de onderste slokdarmsfincter en de relatie tussen protonpompremmer-therapie
en respiratoire complicaties. Tot slot, de data van de studie naar de diagnose en behandeling van
Nederlandse achalasiepatiënten onder 18 jaar, onderstrepen dat het onontbeerlijk is internationale
samenwerking te zoeken om een zeldzame aandoening als deze optimaal te kunnen behandelen in
de toekomst.
