    The CAF three key steps - Preparation

    Talk to the child/parent and talk to others involved(normally with consent)

    Check if a common assessment already exists

    With the child/parent, decide if a common assessmentwould be helpful (or needs updating if it already exist)

    The pre assessment checklist can support professionaljudgement in making this decision

    If a common assessment would be helpful, seek theconsent of the child/parent to proceed

    The discussion need not be a big event Follow local procedures for recording

    Add to or update an existing common assessment orundertake a new one with the child/parent

    Undertake the assessment together, in a child centredway and with consent

    Work togetherto understand issues/develop solutions

    Focus on strength as well as need

    Consider the needs of the individual child

    The CAF discussion and form completion

    1. Explain the purpose of the common assessment

    8. Identify conclusions, solutions and actions, includingwho will do what and when progress will be reviewed

    2. Gather basic details of the child being assessed (p1)

    4. Gather details of current family and home situation (p2)

    Likely common assessment outcomes:

    - Concern resolved or

    -Actions agreed for single agency with child/parent, or

    -Actions require multi-agency response

    If more than one agency is involved:

    - Identify a lead professional-Agree a multi-agency plan (record on the CAF form)

    - Make referrals or broker access to other services ifneeded (use the CAF form as evidence of need)

    The CAF and the lead professional

    LP core functions:

    Act as a single pointof contact for thechild or family

    Coordinate thedelivery of actionsagreed by thepractitionersinvolved

    Reduce overlap andinconsistency inservice delivery

    Key Accountabilities A LP is:

    Accountable to their homeagency for delivery of their partof the action plan and delivery of

    core functions Not responsible or accountablefor the actions of otherpractitioners / services

    Responsible for gathering people

    together to review progress but itis up to the individuals to deliveron their agreed actions

    Cross-agency management frameworks and escalationroutes are required to support the lead professional

    Need can be identified through common assessment -

    this may sometimes mean a specialist assessment is not

    The CAF cannot replace specialist assessments

    preventative services (and if concerned about harm)

    Consent why it is important, how to gain it, who can

    give (or refuse) it and when it is not required

    What to share and how to share it The law in relation to information sharing

    Information sharing is vital for
