This guide is a compilation of all information we deem that you should know as a student of the university. LOCKER RENTALPOINT PERSON: SAM CINCO

During the regular registration, a council booth is set up where students can rent lockers for the semester.

Should a student miss the chance to rent a locker during the above mentioned time, he or she may contact the Secretariat Councilor or their respective batch representatives to see if there are still any lockers available.

Once rented, a student is to provide their own padlock for their locker.

A student is to vacate their locker at the end of the semester.

*Each locker rents for P150 for one semester.

Ask availability of the council room from the Secretariat Councilor at least one (1) day before the planned use of the room.

When available, the Secretariat Councilor shall reserve the said time and date for your use.

Prior to your scheduled use, obtain the key from the Secretariat Councilor or any council member she assigns.

After your use of the room, return the key to the Secretariat Councilor or to any council member.

Please maintain cleanliness and orderliness of the council room.

*There is a weekly scheduled use of the council room for all SEAC organizations. A separate key is provided for this.

***The Council Room may be used by any stundet currently enrolled in UPSE.

Send the message you wish to be brigged to the Secretariat Councilor at 0917.405.7265 (11:59 pm the night before you wish the brig to be sent out).

All text brigades are still subject to the council’s approval. The Secretariat Councilor shall inform you whether or not your brig has been approved.

When approved, just wait for the text dissemination.

The Online Econ Kit is a one stop shop for all academic services and resources, compiled by the Academic Alliance. The PDF file is available in all batch groups.

REQUEST FOR TUTORIALPOINT PERSON: CARA LATINAZO

Tutorials for certain required subjects are automatically provided by the Academic Alliance without request. These are Math 17, 100, 102, and Econ 11, 101, 102, 106, 131. Business Econ subjects such as Econ 122, 132, and 162 are also offered.

A tutorial for any other Econ elective not listed above may be opened if a group of at least 8 students requests for it.

To request for a tutorial, fill up the google form that is linked in the Online Econ Kit. A response from the person in charge can be expected within 36 hours.

You may also approach the Education and Research Councilor, your org’s Academic heads, or your Batch Reps for concerns.

A list of the all books in the council room library are available in the Online Econ Kit. You may borrow one for a period of three (3) days at a time.

To borrow a book, simply fill out the google form that is linked in the Online Econ Kit. You may also contact the Education and Research Councilor at 0917.843.7787.

Write a letter addressed to the college secretary (Cielo D. Magno) with the following details:

1. Name and address of the organization2. Officers3. Purpose and schedule of activities

Submit the letter to SE 101 (thru Tita Evelyn) at least three working days before the event.

Wait for the approval of SE 101.

Once approved, you may now post your posters.

Be sure to remove them after your event.

RENTING A ROOM/ADMIN EQUIPMENTPOINT PERSON: TITA EVELYN

Write a letter to the college secretary requesting use of facilities/equipment. Make sure to include the name of the organization requesting use, which facility/equipment to be used, date and time, and purpose. Wait for approval.

Acquire a copy of the request for use of facilities and equipment form in SE 208 and accomplish the needed information.

Get payment form and pay at SE 232.

Go back to SE 208 and present receipt for recording.

RENT CHAIRSPOINT PERSON: HEAD COLLEGE LIBRARIAN

Write a letter addressed to the head college librarian stating the purpose, time, date, and quantity of chairs to be borrowed.

Have the letter endorsed by SE 101 at least three (3) working days before the event. Submit the letter to the head college librarian.

Settle with them the date and time of pick up and return of chairs.

Make sure to take care of school property borrowed.

FILING FOR A LEAVE OF ABSENCEPOINT PERSON: TITA CONNIE, DEAN, COLLEGE SECRETARY, SE 101 REGISTRATION ASSISTANTS, SE-RVC

1. Fill out the online application form found in your CRS account. A student wishing to be officially on leave must submit his/her application on or before the set deadline.*

2. Have your LOA application approved and printed by Tita Connie at SE 101. Make sure to have this signed by the Dean\ and the College Secretary.

3. Secure all permits/clearance from the SE Library and the Student Disciplinary Tribunal (SDT) at Vinzon’s Hall.

4. Proceed to the Cashier’s Office to pay the LOA fee, currently at Php 150.00. You are officially on leave once the fee is paid.

*Instructors need to submit the class standing of the student if he is applying for LOA during or after the midterm date.

REGISTRATION INFORMATION

DROPPING A SUBJECTPOINT PERSON: TITA CONNIE, YOUR CLASS INSTRUCTOR FOR THE CLASS TO BE DROPPED, SE 101 REGISTRATION ASSISTANTS, SE-RVC

1. Proceed to the CRS Module for Dropping. Indicate your reason for dropping the subject. Make sure to read the rules regarding dropping, maximum residence, and graduation with honors carefully before dropping. (You can find them in the RVC primer given during your Freshmen Advanced Registration, or at SE 101)

2. Advise your class instructor that you have submitted an application for dropping. Your class instructor will need to submit their consent in order for your application to move on to the next stage. Note that your class instructor may choose to withhold their consent, in which case your dropping application will be considered terminated.

3. Once approved by the class instructor, proceed to SE 101 to have Tita Connie print your Form 26 (aka dropping slip), and have it signed by her as well.

4. Once signed, proceed to the Cashier’s Office to pay for the dropping fine, currently at Php 30.00. Check the CRS Dropping module once payment has been made to confirm that the process has been successful.

5. At the end of the semester, you will see a grade of DRP for the class only when your class instructor officially submits your grade at the end of the semester.

*You may cancel an active (i.e. unpaid) dropping application anytime. Canceling an application cannot be undone. You may however, submit another dropping application should you decide to proceed with dropping the class.

**Once your instructor withholds their consent for your dropping application, then it is considered terminated. However, if you manage to convince your instructor otherwise, you may then submit a new application.

ADD OF MATRICULATIONPOINT PERSON: TITA CONNIE, ASSESSORS AT RM 105/312, SE-RVC

1. Have your form 5 and proceed to the respective college where you will enlist.

2. Enlist in that class.

3. Go back to SE 113 (SE 101 for late registration) to have your additional transaction validated.

4. Go to SE 105 (SE 312 for late registration) to have your transaction form (Form 26A) printed.

5. Go back to SE 113 (SE 101 for late registration) to get the stamps needed for your form.

6. Pay the additional fees at the payment centers.

1. Have your form 5 and proceed to the respective college where you will enlist.

2. Cancel the class you wish to cancel and enlist in your new class. Note that if you cancel a class, you must enlist in another class or you will have a pending changemat transaction in your CRS.

3. Go back to SE 113 (SE 101 for late registration) to have your changemat transaction validated.

4. Go to SE 105 (SE 312 for late registration) to have your changemat transaction form (Form 26A) printed.

5. Go back to SE 113 (SE 101 for late registration) to get the stamps needed for your form.

1. Check your eligibility for enrollment. If you are on probation, proceed to the Undergrad Committee for advising. If you are dismissed, go to the college secretary for advising. Have yourself untagged after advising. If you are ineligible because of inaccountabilities, kindly settle them first.

2. “Are you satisfied with your schedule?” Check if you have all the subjects you need. If not, you may proceed to enlistment in the respective colleges.

3. After enlisting, have your Form 5A validated at SE 111.

4. Have your Form 5A assessed at SE 105. You will be given a printed Form 5 afterwards. Fill up the information correctly using black pen. An usher will check your form 5 afterwards.

5. Pay your tuition fees at the respective payment centers (may differ per semester).

GET A NEW IDPOINT PERSON: OUR/ISSI

Processing of Photo-ID of incoming freshmen starts one week before the start of regular registration. Processing for other students starts one week after the last day for late registration of every semester and ends on the last day of final examinations.

> During registration period, secure an appointment slip by presenting to the ID personnel the following :

1. official receipt of Photo-ID fee of P130 (pay at ISSI)

2. original copy of current UP Form 5

3. one 2”x2” recently-taken photograph

4. properly accomplished OUR Form 21 (Personal Data Sheet for Students)

> Proceed to your respective college library for your temporary library card.

> Report for photo-ID card processing on the date and time specified on your appointment slip. Bring with you your current UP Form 5 and Photo-ID fee receipt.

> Proceed to your respective college library for validation and ID sticker .

GET A PINK CARDPOINT PERSON: UPSE LIBRARIAN

> Have your ID validated at the UPSE Library.

> The Librarian will give you a pink card good for the semester.

*The pink card gives a students 20 hours of free internet use in the computer lab.

*No new pink card will be given when lost.

*If a students loses his/her pink card or has used up all of his/her free internet use, it will cost P20/hour to rent a unit at the computer lab.

APPLYING FOR A CAR STICKERPOINT PERSON: OVCCA

> Secure a car sticker application from the Office of the Vice Chancellor for Community Affairs (OVCCA) found behind Quezon Hall.

> Bring to the office a photocopies of the following:

1. Application form

2. LTO Car Certificate of Registration

3. LTO Official Receipt

4. Driver’s License

5. Form 5

6. UPD ID

7. Certificate of Residency (must match address on LTO CoR) and your car.

> Wait for the office to process the request.

> They will be the ones to place the sticker on your car.

*Quezon Hall is the building behind the Oblation overlooking University Avenue.

**The sticker costs P300. The sticker is valid for one school year.

***After the form is passed and requirements are checked, your sticker will be placed on your car on the same day.
