(8) Summary: A Consultant in Medical Education and Healthcare Quality Performance with more than 20 plus years of strong experience in data management, statistical analyses, quality and accreditation in health care and patient safety. I have also provided advice on healthcare quality and systems design, implementation, assessment and improvement in different regions of the Kingdom of Saudi Arabia. I have assisted different health organizations, coordinate projects and plan for quality initiatives and designs performance improvement projects. During these past years, I had served as a consultant in health sector reform projects, system assessments, surveyor for hospital accreditation and quality in health care, accreditation, gap analyses and performance measurements. My expertise also includes helping many healthcare facilities for certification by ISO 9001, 14001 and 18001 and establishment of Integrated Quality Management Systems (IQMS) that serve national and international licensing, certification and Accreditation standards. I served as a Consultant for JCI Accreditation and Re-Accreditation projects for many hospitals in the Kingdom of Saudi Arabia

and GCC according to JCI 2nd, 3rd, 4th & 5th editions standards; leading more than ten Actual accreditation & re-accreditation survey and 15 Gap analysis and JCI mock surveys. I am a certified CBAHI (Central Board of Accreditation for Healthcare Institutions) and NCAAA (National Commission for Academic Accreditation & Assessment) Surveyor. I had served as a consultant in health sector reform projects, system assessments, and quality in healthcare, accreditation, gap analyzes and performance measurements. I am also a certified Hospital Organization and Higher Education surveyor and trainer on different topics of performance improvement models and techniques, accreditation by JCI Accreditation standards, and NCAAA for Higher Education, as well as leadership and professional development. As a certified surveyor of NCAAA, I provide assistance and recommend continuous development and maintenance of accreditation methodologies processes and tools. I had a very significant role as the leader during the accreditation survey of the National Commission for Academic Accreditation and Assessment last April 2015. Modesty speaking I played a very significant role as the leader and the University of Dammam was successfully granted a full accreditation by the NCAAA. I also contribute to the development and maintenance of educational programs at the University of Dammam. I am presently working as an Academician, Researcher, Associate Professor, Supervisor General for the Deanship of Quality and Academic Accreditation at University of Dammam, Saudi Arabia and the Directorate of Quality and Safety at King Fahd Hospital of the University (KFHU). The Directorate of Quality & Safety is composed of eight (8) units namely: Accreditation Unit, Performance Improvement Unit, Training Unit, Environmental & Safety Unit, Risk Management Unit, Decision Support Unit, Documents Control Unit and last but, not the least Credentialing & Privileging Unit. In my capacity as Director of the Directorate of Quality and Safety (KFHU), I provide guidance and education to healthcare professionals and administrators of KFHU on different aspects of healthcare quality management, systems international accreditation, quality improvement, data management and health information systems. Also I am an active and valuable member of the JCIA Steering Committee of King Fahd Hospital of the University (KFHU) that is responsible for monitoring the progress of accreditation process and insure compliance with JCIA standards and requirements. As the Director of the Directorate of Quality & Safety (DQS) of the King Fahd Hospital of the University (KFHU), I was responsible for the Joint Commission International Accreditation (JCIA) preparation and successful accreditation of KFHU as an Academic Medical Center Hospital last 17th of September 2015 with a final score of 98%. Being a founder of the Directorate of Academic Affairs and Training (formerly known as Directorate of Medical Education) at King Fahd Hospital of the University, it had contributed to my experience in organizing conferences and symposia in the Eastern Province of Saudi Arabia. I also had developed various workshops and training programs in quality improvement & tools, academic and hospital accreditation and risk management. I also did several speaking engagement and was invited to be key note speaker on quality assurance, quality management, and risk management and accreditation process. I am certified by the American Institute for Healthcare Quality (AIHQ) as a Hospital Surveyor and had completed mock surveys, and assisted in the administration of one final survey for the Joint Commission International. I am an experienced leader in establishing many organizations and establishments of programs in various Healthcare organizations and Educational institutions.

Presently, I am a Trustee and Editorial Board Member of the International Association of Health Care Professionals (IAHCP). I have also been appointed as the Regional President of the IAHCP for the Middle East Regional Office. I am a Trustee/director of the International Academy of Educational Services (IAES), with its collaborative community health and education projects. As a researcher and public speaker, I had published three textbooks and over 20 scientific papers. I had also presented lectures, seminars and workshops on Quality, Accreditation systems, Risk Management and Patient Safety, Professional Development and Leadership.

B (2) Special Interest Skills:

A. Quality Management:

As an expert in Quality Management both education and healthcare services, I have taught and

implemented hospital policies, protocols, regulations, standards and accreditation measures, using

examples from US and UK hospital settings and applying these to Saudi Arabia. I have been involved

in the continuous evaluation of healthcare services, conducting surveys that measure patient and

employee satisfaction as well as service accessibility. I also created a CQI background in various

university hospitals in Saudi Arabia, and have implemented many quality improvement programs,

advising faculty members and hospital employees on a consultancy basis, facilitating communication

processes, recruiting, supervising and training staff, and designing and documenting processes, policies

and procedures in order to bring departments into full regulatory compliance with the Joint Commission

of Health Organization regulations and standards on Quality Healthcare. My work included the

invention and implementation of two hospital management courses as well as courses in ‘Leadership’,

‘An Introduction to Quality Management’ and ‘Health Information Management’ at the University of

Dammam.

As the Director of the Directorate of Quality & Safety at King Fahd Hospital of the University (KFHU):

Policies & Procedures: In collaboration with all the departments’ chairpersons, directors and

appropriate responsible personnel, I initiated the development, writing and extensively editing the

Risk Management: I initiated the establishing of the Risk Management Unit at KFHU. Under my

leadership, all occurrence variances are encourage to be reported at KFHU and as an organization, I

established a common understanding of what constitutes an untoward incident. An Occurrence

Variance Reporting (OVR) System was introduced to provide a systematic, standardize hospital-wide

mechanism to identify and/or develop prevention/improvement programs which have direct or indirect

adverse effect on patient care; and which represent a potential hazard to patients, visitors, employees

or the facility as quality improvement approach.

Key Performance Indicator (KPI): In collaboration with the departments’ of KFHU, I have established and implemented KPIs for all areas with measurable performance. In 2013, the total KPI that was identified was 21 and 16 were implemented. This number was increased in 2014 as 97 KPIs were identified and 34 were implemented. B. Medical Education and Healthcare:

As a faculty member and expert consultant on the design, implementation, assessment and evaluation

of various curriculum developments in medical and healthcare education, my work has involved

research and collaboration with university departments in order to maintain high standards of quality in

medical and professional development. I am especially interested in problem-based learning and other

innovative teaching and learning methods; using various research methods, I have worked with faculty

members at several medical education institutions in order to tailor courses to both students and faculty,

and to provide adequate training in both teaching and study skills. Being concerned to provide

education programs that are practical and not based solely on lectures, I invented a ‘Learning Skills’

course that was implemented by several universities and enables students to gain important laboratory

skills. I have also developed instructional materials in healthcare for both undergraduate and

postgraduate education, and provide advice to both students and faculty regarding these and other

learning resources. Finally, I have been involved in the ongoing evaluation of education at various

medical, dental and health science schools, and have written various academic grant proposals for

universities wishing to carry out research.

C. Performance Analysis:

I also had special interest in medical biostatistics utilizing powerful statistical software such as SPSS,

MINITAB and NVIVO to analyze results from mixed methodology research, I have been able to analyze

quantitative and qualitative data using statistical methods that include regression, t-test, non-parametric

test, one-way, two-way and three-way ANOVA, factorial analysis, etc.

I also have the abilities to lead, establish and manage various Deanships, Administrative Departments,

Units, and Centers at University of Dammam. With my proven experience and leadership skills, I re-

FORM D: ADDITIONAL CONTRIBUTION TO UNIVERSITY OF DAMMAM

D (1) Strategic Plan

Participate in the development (all phases) of an Institution-wide Strategic plan. This includes the design, editing and compilation of the final plan.

D (2) Self-Study Document 2007 - 2014

In relation to Academic Accreditation, I am part of the Principal Committee and was actively involved throughout all phases of this demanding and searching project for the whole institution. Four of its colleges were also involved, namely: Architecture, Dentistry, Medicine and Nursing.

D (3) Self-Study Document -2014 Institutional

The University of Dammam has undertaken its second Self Study Project in search for Academic Accreditation. I serve as the de facto secretary of the Principal Committee, actively involved in all phases of this searching project for the whole institution. All of its 26 colleges are now involved. However, my focus remains on Institutional Accreditation.

I am scheduled to lead the Programs and Colleges in the University of Dammam which are ready to embark on the voyage to Academic Accreditation. The first will be the College of Engineering.

G. (1) College of Dentistry, University of Dammam – 2009 to present

(a) Undergraduate:

1. Communication Skills: (2011 to present)

I t is a three-hour course taught for students of the Preparatory Year- Health Cluster in the second semester. The aim of this course is to help the student develop the skills which will enable them to communicate effectively and sensitively with patients and their relatives and with their colleagues.

2. Learning Skills: (2007 to 2012)

It was a one-hour course taught for students of Dentistry in the second semester. The course was aimed to enable dental students to acquire basic principles of life-long learning methods and to use them effectively.

3. Learning, search and thinking skills (2011 to present) It is a three- hour course taught for students of the Preparatory Year- Health Cluster in the first semester. This course aims to help the students enhance their life-long learning, thinking and search abilities.

G. (2) College of Applied Medical Sciences, King Faisal University and University of Dammam – 1996 to 2012

(b) Undergraduate:

1. Health Information Management System: (2001 to 2009)

This course provides students of Applied Medical Sciences with knowledge and skills regarding the methods, and techniques of information management in order to prepare them in the design, development and management of health information.

2. Management in Healthcare Organization: (September 2002 to 2009)

This course enables students to gain a complete understanding of the structure and operation of the health care delivery systems with a special focus on the hospital. This course is given as an advanced course for the Respiratory Therapy students and an intermediate level course for Medical Secretary and Transcription Diploma students.

3. Physician Practice Management: (2010 to 2012)

This course presents an overview of physician practice management, which incorporates the basic functions of health care administration and applies them to the physician practice setting with a strong emphasis on managing care delivery systems.

G. (3) College of Medicine, King Faisal University – 1996 to 2009

(c) Postgraduate Teaching:

1. Basic Course on Quality Management

Has initiated, planned, developed and administered the first course in Basic Quality Management in King Fahd Hospital of the University (KFHU). It provides hands-on experience of the basic aspects of the implementation of QA/QI/QM in a hospital setting. During my teaching of this course, eight (8) leaders of the hospital has registered and participated and presented difference research topics. Since then, several courses have been held where my involvement was mainly with the mini research component.

2. Advanced Course on Quality Management An advanced course was also established and initiated. This provides a comprehensive and in-depth exposure to Quality Management to a standard required by the Certified Professional in Health Quality (CPHQ) examination. The course includes lectures on research methodology, data collection, computer data analysis and preparing the material for presentation. Each participant requires to be rehearsed three or four times in order to be prepared to make a public presentation.

3. Medical Education Methodology I (824) Credit course offered to the Resident of Family and Community Medicine Fellowship Programme.

4. Medical Education Methodology II (837) Credit course offered to the Resident of Family and Community Medicine Fellowship Programme.

1. Principles of Curriculum Development: (Post Graduate Training Course)

Identifying appropriate educational strategies for curriculum development, with emphasis on the ability to identify the aims and objectives of the course or curriculum. I have particular interest in the use of new and innovative curricula to achieve the objectives.

2. Principles of Assessments: The course aims to provide the participant knowledge on how to construct clear instructional objectives, differentiate facts, procedures, concepts and principles in the context of assessing student learning; distinguish between non-referenced assessment and criterion referenced assessment; describe the differences between formative and summative assessment; describe the characteristics of content, predictive, concurrent and construct validity; and determine test reliability via three prescribed methods.
