Subject: Turnover of Responsibilities.Dear gents,I am leaving on 8th November 2015, and I turn over following inspection responsibilities to Mr. Keshav Moorthy/ Mr. Naga Suryanarayan/ Mr, S. Ganesh- Static & Rotary Equipment Inspector as instructed by Mr. Siva (UQL.)

Responsibilities:Daily On-going inspection as per RFI /surveillance activities, LBE’s & NCR , Proactive Notification & follow up for open LBE’s, weekly reporting.

1. Status of Log book entries & NCR:

Violation /Proactive Notification : refer to excel sheet log attached

Standard violation NCR : refer to excel sheet log attached Inspection status : refer to excel sheet log attached

2. Ongoing Activities in Static & Rotary Equipment

Pre-installation, installation & alignment inspection of safety showers in chlorine area.

Inspection of internals of towers, vessels, closure inspection.

Pre commissioning activities of rotating and static equipments at site as per schedule.

Final alignment of rotating equipments in chlorine area as per schedule.

Final acceptance of the system (rotating & static equipments)

Surveillance inspection at site and monitoring contractor personal performance in field.

3. Highlighted Issues:

1. SK- 3600 Wet Hydrogen Vent Tower: on hold

02 nos. of stiffener plate of grade -14462 (duplex) welded with bubble cap tray at bottom, these stiffeners need to be reflected in revised drawing.

Bubble cap are expanded type where as drawing show’s seal welded type, drawing need to be revised as agreed by vendor/Jacobs engg.

Bubble cap tray leak test performed and witnessed by vendor, 03 nos. of bubble caps found mild leakage, vendors/Jacobs engg clarification required for acceptance.

Stiffener welding need to witnessed by welding inspector and welding pitch to reduced (welding at more location) as advised by vendor closure report.

EPDP gasket for top body flange installed in 04 pieces where as in drawing 03 piece mentioned, gasket installation as per STQ-10027-E-3004 suggested for scarf joint but in actual this parallal cut joint, which may affect the sealing capability of the joint. Jacobs engg. clarifications required.

2. Agitators A-9100/9101/9102 Area -3 and A-2008/2010/2060: on hold

NSH raised RFI for installation, leveling and orientation of Agitators A-9100, A-9101, A9102 in area-3 chlorine and A-2008, A-2010 and A-2060 in area-27 chlorine. Following concerns were observed, this is for your information and further corrective action.

All these agitators are installed by vendor on mounting flange, vendor accepted and confirm the leveling is ok, now this is not in practical to check the level because agitator is already installed on mounting flange.

Run out at mechanical seal and impeller not offered for inspection hence not witnessed, however this was performed by the vendor for all agitators.

The gap between the bottom impeller blade and tank bottom is 1145mm for A-9100, 1150 mm for A-9101 and 1130 mm for A-9102 in actual, but as per drawing this is 1181 mm. vendors/engg clarification required.

Please provide hydrostatic test report of mechanical seals at site, as per RT175-327300 for all agitators.

Please provide the torque value for the bolt tightening. Agitators A-2008, A-2010 and A-2060 motor orientation is west as per drawing, in actual this is in

between west & north ( 30 degree rolling towards north)

3. Final Inspection and acceptance of Pumps: on hold

MP-3765 A/B:

Mechanical seal cooling line not yet completed, the flange connection joint near solenoid valve is not torque tightened, in MP-3765 A temporary gasket blind was installed and P-3765 B no gasket observed and joint is loose, please ensure bolt torque of this joint with correct gasket.

Please provide flushing report of mechanical seal tubing, as discussed with NSH mech. QC he said it was done by Instrument but as I confirmed with Velosi Inspector it was not done.

Oil need to be changed with new oil recommended by vendor. Motor and pump skid cleaning required, pump shaft found rusted- cleaning required, Unwanted

plastic strap tied with the pump must be removed. Mechanical seal covers were not installed and fouling with tubing.

MP-3406 A/B:

Mechanical seal cooling line not yet completed, one of tubing is not connected with pump , the flange connection joint near solenoid valve is not torque tightened, please ensure bolt torque of this joint with correct gasket.

Please provide flushing report of mechanical seal tubing, as discussed with NSH mech. QC he said it was done by Instrument but as I confirmed with Velosi Inspector it was not done.
