Program Accountable Care Organizations (ACOs) are groups of health care providers that work as a team to coordinate care for a group of patients, with the goals of providing high- quality, patient-centered care and reducing costs. Conference MaterialsTwo days before the conference, registrants will be emailed a link to a website where the conference materials, including handouts, agenda and evaluation, may be accessed and printed. No hard copy of handouts will be provided. Please provide a correct, individual email address when registering.

Certificate of Completion: To receive your certificate, you must attend 90 percent of the conference and complete an evaluation. A link to an electronic evaluation and attendance confirmation will be emailed to program attendees. You have two weeks to complete the program evaluation in order to receive a certificate. Evaluations submitted after the two-week period will not be honored.

Continuing Education CreditsAttendees will receive up to 6.25 contact hours with registration, check-in at registration desk, attestation of attendance, and submission of program evaluation within two weeks after the conference. No late requests for continuing education credit will be honored. Certificates will be emailed in four to six weeks.

University of Southern Indiana College of Nursing and Health Professions is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s Commission on Accreditation.

Contact hours earned will be accepted for mandatory re-licensure requirement for Kentucky and Illinois nurses.

As an accredited university, USI is an approved provider of continuing education for occupational therapists, dental professionals, social workers, marriage and family therapists and mental health counselors, as well as psychologists licensed in Indiana.
