NATIONALITY 02 The annual contribution for the plan is RM350. All contributions (if applicable) will be subjected to relevant charges or taxes as deemed necessary by the Malaysian tax authorities. It is important to keep any receipts that the Person Covered has received as proof of payment of contribution. ELIGIBILITY 01 Savings, Security, Serenity Hasanah Benefits of Hasanah PAYABLE BENEFIT RM75,000 Upon the Person Covered’s death or TPD, the Person Covered’s nominee or Person Covered will receive death or TPD benefit. DEATH OR TPD (TOTAL AND PERMANENT DISABILITY) Basic Benefit PAYABLE BENEFIT RM150,000 Double Takaful coverage on accidental death and TPD. DOUBLE ACCIDENTAL BENEFIT Basic Benefit PAYABLE BENEFIT RM3,000 In the event of death or TPD before the Person Covered has had the opportunity to perform Hajj, Etiqa will help the Person Covered’s obligation to be fulfiled by paying cash contribution to the Person Covered’s nominee to arrange for Hajj. BADAL HAJJ Additional Benefit PAYABLE BENEFIT RM1,000 A lump sum benefit will be paid to your appointed nominee upon the Person Covered’s death for the purpose of performing good deeds in the Person Covered’s name for the interest of religious or general needs of the society. WAQF Additional Benefit In the event of the Person Covered’s death, your family will receive a lump sum payment as compassionate benefit. FUNERAL EXPENSES Compassionate Benefit PAYABLE BENEFIT RM1,500 Note : The benefits received from Hasanah are non-taxable and contribution paid may qualify for tax relief subject to Malaysia’s Inland Revenue Board decision. Performing Hajj is an obligatory duty for every physically and financially able Muslim and it constitutes one of the 5 pillars of Islam. It is a lifetime ambition for every Muslim to make this pilgrimage to Makkah and make all possible efforts to fulfil it. MAJOR EXCLUSIONS Some of the exclusions under this plan shall include: a) Pre-existing conditions for death that occur due to any direct or indirect internal illness and malfunction of the body that existed 12 months prior the effective date. This exclusion is applicable to all benefits except Badal Hajj, Waqf and Funeral Expenses benefits. b) Participation in any criminal acts, riots, civil commotion, insurrection, war, revolution, terrorism, chemical warfare, dangerous or hazardous sports, any form of aviation or aerial sports, self-inflicted injuries or suicide or attempted suicide while sane or insane, drug addiction, under the influence of alcohol, HIV infection, Acquired Immune Deficiency Syndrome (AIDS) and committing or trying to commit any illegal acts. IMPORTANT NOTES 1. Shariah Concept Tabarru’ This product applies the Tabarru’ concept, whereby the Person Covered agrees to pay Contributions to the Takaful fund for the purpose of mutual aid and assistance to any other Persons Covered in case of need. The Person Covered agrees to participate in this Takaful scheme based on the principles of Takaful and to pay the Contribution on the basis of Tabarru’ (donation) for the purpose of helping other Persons Covered who have suffered tragedy and with this Contribution, the Person Covered is entitled to the Takaful cover as expressed in the terms and conditions of this Takaful contract. All Tabarru’ charges, other charges and fees (if applicable) will be subjected to relevant charges or taxes as deemed necessary by Malaysian tax authorities. It is important to keep any receipts that you received as proof of payment of contribution. Wakalah This product also applies the Wakalah concept, whereby the Person Covered nominates the Takaful operator to act on their behalf to invest and manage the Takaful fund. As an agent, the Takaful operator is entitled to receive a “Wakalah Fee” to meet the management expenses and direct distribution. The Person Covered further agrees that the Person Covered’s Contribution will be credited into the Takaful fund (Fund) and to appoint Etiqa Family Takaful Berhad (Etiqa) to invest and manage the Fund according to the principles of Shariah. The Person Covered also permits Etiqa to be paid a Wakalah Fee based on the following rates: Etiqa is appointed as the Takaful operator, under a Wakalah concept. A Wakalah concept is an agency relationship between two parties. The Wakalah fee is paid to Etiqa to cover the costs of distributing and managing the plan. The contribution, minus the Wakalah fee, will be paid into the Participants’ Risk Fund (PRF). 50% of any distributed surplus (if any) will be shared amongst the Persons Covered with in-force certificates, who have not made any claim within the financial year. The remaining 50% will be paid to Us as a performance fee for operating and managing the PRF. If the sum payable to a claimant is less than Ringgit Malaysia Ten (RM10), the amount will be paid to a charitable fund on behalf of a Person Covered, and utilised as Amal Jariah. Note: For more details on Shariah concept, please refer to the Takaful Certificate. 2. This plan is a Group Term Takaful product where the coverage is on annual basis. 3. The brochure should not be construed as a contract or commitment. The information contained in this brochure may be changed without prior notice. 4. Etiqa has the right to amend the Tabarru’ fees and charges under this certificate by giving ninety (90) days prior notice and such amendments will be applicable from the next anniversary date of the certificate. The rates will be increased and may be revised for other adverse experiences. 5. Before participating in this plan, You should satisfy yourself that this certificate will best serve your needs and that the contribution payable is an amount that You can afford. 6. You may cancel Your certificate by returning the certificate within fifteen (15) days after the certificate has been received by You. The contributions that You have paid (less any medical fee incurred) will be refunded to You. Sumbangan tahunan bagi pelan ini adalah sebanyak RM350.Segala sumbangan (jika berkenaan) akan tertakluk kepada caj atau cukai yang dianggap perlu oleh pihak berkuasa cukai Malaysia. Ia adalah penting untuk memastikan anda menyimpan resit sebagai bukti pembayaran sumbangan.

PENGECUALIAN UTAMA Beberapa pengecualian di bawah pelan ini termasuk:-a) Kematian akibat keadaan sedia ada yang berlaku disebabkan

oleh penyakit dalaman secara langsung atau tidak langsung dan kerosakan badan yang sedia wujud dalam tempoh 12 bulan sebelum tempoh perlindungan berkuatkuasa. Pengecualian ini untuk semua manfaat kecuali manfaat Badal Hajj, Waqf dan Perbelanjaan Pengebumian.

b) Penglibatan di dalam sebarang perbuatan jenayah, rusuhan, kekecohan awam, pemberontakan, perang, revolusi, keganasan, peperangan kimia, sukan berbahaya, dalam apa-apa bentuk penerbangan atau sukan udara, kecederaan diri yang disengajakan atau membunuh diri atau cuba membunuh diri ketika waras atau tidak waras, ketagihan dadah, di bawah pengaruh alkohol, jangkitan HIV, Sindrom Kurang Daya Tahan Penyakit (AIDS) dan melakukan atau cuba untuk melakukan apa-apa perbuatan yang menyalahi undang-undang.

NOTA PENTING1. Konsep Shariah

Tabarru’Produk ini menggunakan konsep Tabarru‘, di mana para individu yang dilindungi bersetuju untuk membayar Sumbangan mereka kepada Dana Takaful bagi tujuan saling membantu mana-mana individu yang dilindungi lain yang memerlukan bantuan.Individu yang dilindungi bersetuju untuk mengambil bahagian dalam skim Takaful ini berdasarkan prinsip Takaful dan membayar Sumbangan atas dasar Tabarru’ (derma) bagi tujuan membantu setiap individu yang dilindungi lain yang telah mengalami musibah dan dengan Sumbangan ini, individu yang dilindungi berhak terhadap perlindungan Takaful seperti yang dinyatakan dalam terma-terma dan syarat-syarat Kontrak ini.

Semua caj Tabarru’, lain-lain caj dan yuran (jika terpakai) akan tertakluk kepada caj atau cukai yang berkenaan sebagaimana yang dianggap perlu oleh pihak berkuasa cukai Malaysia. Ia adalah penting untuk menyimpan setiap resit yang anda terima sebagai bukti pembayaran sumbangan.

WakalahProduk ini juga menggunakan konsep Wakalah, di mana para individu yang dilindungi melantik pengendali Takaful untuk bertindak bagi pihak mereka untuk melabur dan menguruskan Dana Takaful. Sebagai ejen, pengendali Takaful berhak menerima “Yuran Wakalah” sebagai caj perkhidmatan.

Individu yang dilindungi seterusnya bersetuju bahawa Sumbangan individu yang dilindungi dimasukkan ke dalam Dana Takaful dan melantik Etiqa Takaful Keluarga Berhad (Etiqa) untuk melabur dan menguruskan Dana tersebut mengikut prinsip-prinsip Shariah. Individu yang dilindungi juga membenarkan Etiqa untuk membuat pembayaran tuntutan/manfaat Takaful, Retakaful, peruntukan dan mewujudkan rizab berdasarkan garis panduan dan dasar yang ditetapkan oleh pihak berkuasa, dan ETB perlu dibayar yuran Wakalah berdasarkan kadar berikut:

Etiqa dilantik sebagai pengendali Takaful, di bawah konsep Wakalah. Konsep Wakalah adalah sebuah hubungan agensi di antara dua pihak. Yuran Wakalah dibayar kepada Etiqa untuk menampung kos mengagih dan mengurus pelan itu.

Sumbangan yang telah ditolak yuran Wakalah akan dibayar kepada Dana Risiko Peserta (PRF), 50% daripada lebihan agihan (jika ada) akan dikongsi bersama dengan individu yang dilindungi dengan sijil-sijil yang dikuatkuasakan, yang belum membuat sebarang tuntutan dalam tahun kewangan. Baki 50% yang akan dibayar kepada Kami adalah sebagai yuran perkhidmatan untuk mengoperasi dan mengurus PRF.

Sekiranya jumlah muka perlu dibayar kepada pihak menuntut adalah kurang dari Sepuluh Ringgit Malaysia (RM10), jumlah akan dibayar kepada tabung kebajikan di atas nama individu yang dilindungi, dan digunakan sebagai Amal Jariah.

2. Pelan ini adalah produk Takaful Berkelompok Bertempoh di mana perlindungan diperbaharui setiap tahun.

3. Brosur ini tidak boleh dianggap sebagai kontrak atau komitmen. Maklumat yang terkandung dalam brosur ini mungkin akan dipinda tanpa diberitahu terlebih dahulu.

4. Etiqa mempunyai hak untuk meminda yuran Tabarru’ dan caj untuk sijil ini dengan memberi 90 hari notis awal dan pindaan tersebut akan berkuat kuasa dari tarikh ulang tahun sijil ini. Kadarnya akan meningkat dan mungkin akan berubah mengikut keadaan.

5. Sebelum menyertai pelan ini, anda harus memastikan bahawa sijil ini akan memenuhi keperluan anda sebaiknya dan sumbangan yang perlu dibayar adalah mengikut kemampuan anda.

6. Anda boleh membatalkan sijil dengan mengembalikan sijil tersebut dalam tempoh lima belas (15) hari selepas anda menerima sijil anda. Sumbangan yang telah dibayar (selepas sebarang bayaran perubatan yang dikenakan ditolak) akan dibayar balik kepada anda. Anda dimaklumkan bahawa bagi maksud menentukan tempoh lima belas (15) hari, Sijil ini akan dianggap dikembalikan kepada Etiqa pada tarikh Etiqa menerima Sijil ini dihantar secara peribadi atau pada tarikh dihantar melalui pos berdaftar atau pada tarikh penghantaran jika Sijil ini dihantar secara elektronik.

7. Anda diberi tempoh enam puluh (60) hari dari tarikh perlu membuat bayaran sumbangan. Kegagalan untuk berbuat demikian boleh menamatkan sijil anda.

8. Sekiranya anda memerlukan maklumat lanjut mengenai Takaful Keluarga, sila rujuk kepada buku panduan infoinsurans mengenai Takaful keluarga, yang boleh didapati di semua cawangan MBSB Bank atau anda boleh mendapat Salinan dari Penasihat Kewangan Peribadi (PFA) MBSB Bank atau layari laman sesawang www.insuranceinfo.com.my.

Adakah sumbangan saya bertambah apabi-la umur saya meningkat?Jumlah sumbangan akan datang adalah tidak dijamin dan mungkin akan dikaji semula pada setiap tahun.

Bagaimana cara untuk saya membayar sumbangan saya?Pembayaran sumbangan sama ada secara tunai atau melalui cek di mana-mana cawangan MBSB Bank yang berdekatan dengan anda.
