Wilson Centre SPECIAL GUEST PRESENTATION Thursday, April 28, 2016 from 12:00-1:00 pm Toronto General Hospital, 200 Elizabeth St, Rm 1EN441, [Library area] Jessica Stokes-Parish - Simulation Coordinator Chameleon Simulation Centre, School of Medicine & Public Health, University of Newcastle Does authenticity in moulage matter? A review of the literature The environment in which the simulation is situated plays a large role in the degree of engagement. Moulage techniques are used in current-day simulation to mimic illnesses and wounds, acting as visual and tactile cues for the learner. Against this background of the use of moulage in simulation, we investigated how the authenticity of moulage has an impact on participant engagement using a systematic review. The resulting nine papers clearly outline an assumption that moulage is essential in simulation-based education for improved realism and subsequent learner engagement. However, there is no clear evidence

from the literature that this is the case, suggesting that further research to explore the impact of moulage on participant engagement is warranted. About Jessica: With a diverse background in health over the past 8 years, Jessica’s current role is diverse; including Simulated (Standardized) Patient Program management, simulation facilitation, design and development, and delivery of simulation pedagogy and training modules to clinical and academic educators. Chameleon Simulation Centre is located off-campus, at a large teaching hospital providing simulation-based education to undergraduate and postgraduate fields including Medicine, Nursing, Speech Pathology, Midwifery, and provides consultation to Nutrition/Dietetics and Physiotherapy. Training modules that Jessica co-writes/facilitates include simulation design and development, simulation centre logistics, debriefing and feedback skills, Simulated Patient training and program management, moulage, and video teaching adjuncts. Jessica is currently undertaking a PhD (Supervised by Prof. Brian Jolly and Dr. Robbert Duvivier) in the area of visual cues (moulage) and their impact on participant engagement in simulation, particularly the level of authenticity required for maximal outcomes in participant learning. She is active in professional organisations, with representation on the executive committee of the Australian Society for Simulation in Healthcare and the organization committees for SimHealth 2016 (September 2016). Her research examines the construction of legitimate knowledge and values in scientific and health professional practice and education. She has published papers in the fields of sociology, STS and health professional education and has edited the Handbook of the Sociology of Medical Education (Routledge, 2009, with Bryan Turner), Bourdieusian Prospects (Routledge, forthcoming, with Lisa Adkins and Steven Threadgold) and a themed issue of the journal Health Sociology Review on ‘Sociological perspectives on the politics of knowledge in health care’ (forthcoming 2016, with Emma Kirby). She currently leads the project ‘Complementary and Alternative Medicine Degrees: New Configurations of Knowledge, Professional Autonomy and the University’, funded by an Australian Research Council Discovery Early Career Researcher Award (2014-2016).

Degrees of Legitimacy: Emerging findings from a study of complementary medicine education in Australia and the UK In this brief presentation she will provide an overview and discuss some emerging findings from the current study she is conducting which explores the forms that complementary medicine teaching and research takes in university settings, and what factors influence this. The study builds on the research reported in her recent Sociology paper and involves interviews with key stakeholders and observation of teaching in three university-based complementary medicine disciplines in Australia and the UK. Lorraine is pursuing a part-time doctorate in medical education at the University of Aberdeen (UoA), under the supervision of Professor Jennifer Cleland and Dr Simon Kitto (University of Ottawa), while working as a Clinical Educator in the Institute of Education for Medical and Dental Sciences at the same institution. A Registered General Nurse by training, she worked in a number of clinical roles over 20 years, latterly working in a research-active intensive care unit where she was involved in clinical research. With reference to Whitehead and Hodges’ work on the discourses of the “new”, she is using theory to critically unpack the reproductive nature of curriculum reform, to make explicit the influence of local and national factors such as space and place on medical education and curriculum reform. The individual and organisational factors which influence medical trainees’ (residents) and students career decision making have been extensively studied but how they “trade off” different factors is under-researched. Drawing on data from 1500 trainees and nearly 1000 students from seven UK medical schools, I will take about how we are examining the relative strength of “push-pull” factors in medical careers decision making. Professor Cleland is John Simpson Chair of Medical Education Research, University of Aberdeen, Scotland, and Chair of the Association for the Study of Medical Education (ASME). She is Director of the Scottish Medical Education Research Consortium (SMERC), a collaboration between NHS Education for Scotland and the five Scottish medical schools. She is also Lead for the AMEE Research Committee. She has published over 100 peer-reviewed articles in healthcare education. She is the lead editor on “Researching Medical Education” (Wiley, 2015). Jen’s particular research interests are selection and assessment, and medical careers decision making, and her work spans the continuum of undergraduate, postgraduate and continuing medical education and training. She uses both qualitative and quantitative approaches – from large “number crunching” studies to context-dependent ethnographic research. In the past decade, the discipline of medical scholarship has been experiencing a paradigm shift away from the strict adherence to biomedical ideology and towards an understanding of the medical experience that acknowledges that medicine is both a science and an art. At the same time, humanities scholars have been drawing on medicine as a point of inspiration, using narrative, fine arts, and critical theory to examine the depths of the human experience and to complicate our understandings of what it means to be healed, to be ill, to feel pain. However, significant barriers still exist between these two exciting fields of research; moreover, even within the fields, there are limited possibilities for engagement between students and professionals, students and scholars, artists and academics. In the interest of promoting collaboration between these initiatives, the ‘Humanities in Medicine Interest Group’ from McMaster University’s Faculty of Medicine will host “A Palpable Thrill,” an interdisciplinary conference that seeks to promote inclusion of the humanities in medical education and, conversely, encourage arts students and scholars to find salience and inspiration in medicine. We will to work create sustainable cross-disciplinary exchange by identifying common goals, barriers, and methods. We intend to help break down the divide between medical students, humanities students, and the larger professional and cultural communities that inform each of these fields of study. We hope to generate new ideas and develop practical skills among attendants that can be applied to future work and education. The conference will be a two-day event from May 6-7, 2016 held primarily at McMaster University in Hamilton, Ontario. The event will feature contributions in many different formats; we welcome submissions ranging from formal academic papers to workshop proposals to visual art and creative writing. We look forward to reviewing your submissions and expanding our vision for the weekend as we receive them. We intend to provide logistical support for a wide variety of mediums; please notify us of any special requirements your project or presentation may have. Mary Seeman Humanities Award-2016 This award is intended to encourage creative and scholarly activity in the interface between the humanities and issues related to mental illness and emotional well-being. The award is open to students from Dentistry, Kinesiology and Physical Education, Medicine (including undergraduate, residents and fellows), Nursing, Medical Radiation Sciences, Occupational Science and Occupational Therapy, Pharmacy, Physical Therapy, Physician Assistant, Social Work, and Speech-Language Pathology at the University of Toronto. The award is based on documented evidence of completed work of artistic and/or scholarly merit that contributes to a better understanding of the human condition. Submissions will be judged on the basis of originality, creativity and relevance to mental health and illness. Please note only one submission per applicant will be accepted. The following guidelines are suggested to assist applicants: a) Contributions to scholarship in the area of humanities and mental illness; for example, a critical review

of the literature, an essay on a topic bridging the humanities and mental illness and emotional well-being, or material relating to the history of psychiatry.

b) Evidence of creative activity in the area of humanities and understandings of mental illness; for example, art or literature. c) A demonstration of the capacity to integrate interest in the interface of humanities and mental health

Changing a curriculum is like steering a cruise ship – it requires careful planning, good navigation maps, and it happens slowly. Luckily, the route is usually well known because others have gone before. Everyone is enthusiastic about the journey, and if one is lucky, the Dean has promised an excursion to a tropical island when the job is done. Changing the assessment system is somewhat like steering an oil tanker in the North Atlantic. There are many icebergs as well as foggy and stormy routes. And one is always mindful of where the lifeboats are located. But these are exciting times for those of us who are excited about assessment. All over the world, assessment is increasingly being aligned and reconciled with the larger vision for our education programs. We recognize that assessment can be scientific, rigorous, but can also provide value added experiences to our trainees. We at the University of Toronto (U of T) and the Wilson Centre, are doing our small part to make this shift in thinking more concrete. The UofT MD curriculum is going through a significant renewal and I’m proud to say that the Wilson Centre has been heavily involved in that change. The scientist group at the Wilson Centre, like our friends at Maastricht, are known for the heavy emphasis on excellence in research and theory building. But a group of us are collaborating with like-minded curriculum and teaching leaders at the U of T MD program. Medical education often speaks of bridging the gap between theory and practice. For us this is not a cliché! Our science has been informing the high level vision of the curriculum. The ideas that have been developed from the diverse, multi-disciplinary research programs at the Wilson Centre are now being used to design pre-clinical and clinical curricula. Concepts like cognitive integration, adaptive expertise, directed self-regulation, and identity construction are being incorporated in our pedagogical techniques and faculty development. Not only has this been a ‘fun’ exercise, it has also been an opportunity to design studies to test our theories in ‘the wild.’ As always when diverse groups of people meet and challenge one another, interesting and unique ideas emerge. We recognized early on that our hard work in the curriculum will be for nought if our assessment practices do not change. This is where I come in as the Assessment Scientist for the Wilson Centre and MD program. Along with our colleagues we have been thinking about how assessment for learning can be achieved within our system. We’ve committed to a programmatic assessment model for the pre-clinical Foundations curriculum and a long term roll out to the clinical training environment. We’ve been fortunate to have guidance from friends at Maastricht including Cees, Sylvia, Jimmie as well as from other institutions. It’s reassuring to have guidance from colleagues who have taken the same route. Our unique challenges are in adapting programmatic assessment models to the historical context of U of T without significantly expanding the time commitment for our generous faculty. Addressing these changes has involved a lot of creativity on the part of our faculty without compromising the central vision for programmatic assessment. For us, programmatic assessment is about changing the way students approach their learning. The bonus for our program is that we can systematize and enhance the feedback we give to students as well as our ability to measure their competencies in a rich and meaningful way.

With any major change, there is a lot of visioning, planning, and management issues to address. Often times, there are not enough hours in the day to do everything, not the least of which is to build enthusiasm and support for this new vision of assessment. Surprisingly, the ideas of holistic judgement, low stakes formative assessment, have been well received. Part of this reason is that our vision of programmatic assessment really systematizes what has already been happening on an informal and semi-structured way in many medical schools. Faculty have always made holistic decisions – based on data informed judgement – on student progress. And different course leaders have historically discussed the successes and struggles of students, trying to triangulate the areas of excellence or difficulty. Programmatic assessment tries to bring this ‘hidden’ assessment and use it more informatively. Moreover, it really brings our education back to the relationships that facilitate learning. Too often large class sizes and overwhelming content can make pre-clinical training impersonal. Our programmatic assessment model will be built around close contact with faculty and students. As a researcher in assessment, this is also an opportunity to test some of the ideas our group has been developing. One thesis is that assessment for learning means that assessment experiences must evoke and align appropriate cognitive processes in learners in relation to the cognitive processes of the outcomes desired for the students. For example, Dr. Nikki Woods has developed models of how basic science should be integrated and integration means changing the mental organization of basic and clinical concepts to support clinical reasoning. Dr. Maria Mylopoulos has extended this to show proper integration of basic science can assist in future learning of novel clinical concepts. What does it mean to have assessments that support these cognitive processes? We know what doesn’t work: traditional factual recall questions. We’re currently experimenting with assessment design and practices that may shed light on these questions. The wonderful thing about working in a medical school is that there are always participants for experiments! The new curriculum rolls out in August. There is much work and lots of learning that must happen before then. We hope to share our findings as well as report on the implementation of programmatic assessment at Toronto next year. We’re very excited as the Dean has promised us a trip to a Caribbean beach resort if we pull this off. Dr. Brian Hodges Brian D. Hodges MD, PhD, FRCPC is Professor in the Faculty of Medicine and Faculty of Education (OISE/UT) at the University of Toronto; the Richard and Elizabeth Currie Chair in Health Professions Education Research at the Wilson Centre for Research in Education and Executive Vice President Education at the University Health Network (Toronto General, Toronto Western Princess Margaret and Toronto Rehab Hospitals). He leads the AMS Phoenix Project: A Call to Caring, an initiative to rebalance the technical and compassionate dimensions of healthcare.

Ms. Judith John Judith John’s career spans all aspects of communications, marketing and public affairs, with an extensive background in arts, culture, not-for-profit, health and social services. Ms. John was Vice President, Marketing, Public Affairs and Communications at United Way of Greater Toronto, Mount Sinai Hospital/Foundation, and the Hospital for Sick Children. She volunteers with Toronto Western Hospital, Princess Margaret Cancer Centre, University Health Network, Michael Garron Hospital and Mount Sinai, concentrating on the patient experience, education, partnerships and communications. A UHN Partners in Care advisor, Ms. John also worked with Phoenix Project, Patients Canada, Community Care Access Centres, and The Change Foundation. She lectures for Health Quality Ontario, University of Toronto, Schulich School of Business and Ontario Brain Institute and will be keynote speaker for the South Central LIHN, Michener Institute commencement, and Acquired Brain Injury Society. Currently, she handles government and stakeholder relations for the Council of Academic Hospitals Ontario. The Citizens’ Advisory Committee Chair for the Quality Management Partnership, she sits on the Partnership’s Healthcare System Reference Group, senior leaders from organizations having a system level role in health care quality. Ms. John’s extensive education and community experience includes Ontario Trillium Foundation, Ryerson City Building Institute, and Rotman School of Management. She is a writer and communications / marketing consultant, executive coach, media coach and facilitator. As a public relations, communications, ethics and marketing instructor and lecturer, she has lectured at universities and colleges across Ontario and institutions worldwide, including England’s National Health Service.

Dr. Charles Prober Charles Prober, MD is the Senior Associate Dean for Medical Education at Stanford School of Medicine. He is a Professor of Pediatrics, Microbiology and Immunology and Co-Director of the Stanford Center for Clinical and Translational Education and Research. He is an expert in pediatric infectious diseases with an academic career focused on the epidemiology, pathophysiology, prevention, and treatment of infections in children. Much of his research has focused on viral infections, especially those caused by herpes simplex virus (HSV). He has conducted a number of seminal studies concerned with the epidemiology of HSV-2 infections in pregnant women, their partners, and neonates as well as investigations of the immunologic response to HSV infections. Dr. Prober has published extensively in peer-reviewed journals and is editor of Principles and Practice of Pediatric Infectious Diseases, one of the major textbooks in the field of pediatric infectious diseases. Dr. Prober has been involved in medical education throughout his career. He has directed a number of undergraduate and graduate student courses in the classroom and at the bedside, served as Associate Chair for Education for the Department of Pediatrics, and lectured locally, nationally, and internationally on infectious diseases and medical education. As Senior Associate Dean, he oversees undergraduate, graduate, and postgraduate medical education at Stanford Medicine.
