LC NEWSLETTER Articles. A healthy, lively discussiontook place at our recent Club Meeting as to whether

PLC should be including articles about Derriford Hospital inits Newsletter. The consensus was that we should. AsEditor, my personal view is that Derriford Hospital is veryclose to the hearts of all members of PLC. And as such, we

should all wish to know all that we can about the building, the staff, its management, and of course the grassrootsmedical and nursing aspects. Not only that... but we would wish to know the bad news as well as the good! But thats just my personal view. What are YOURS? Please do tell!

LC NEWSLETTER Editor. Its an accepted rule that the editorship of any Newsltter should be rotated and

handed over now and again so as to create a fresh imageand prevent it from becoming stale. Its alway good toofor an Editor to be reminded that the PLC is not all abouthim... as Geoff was so aptly and quite rightly reminded at the Club Meeting in May 2011! If anyone would like totake over the post of Editor do please submit your namesto Ted BURNETT or Geoff READ ( Secretary ).

    GREETINGS!

EST Wishes to the Chair of PLC, Ted BURNETT, for hisbirthday on Friday, 10 th June 2011. Have a goodun Ted

and thanks for all the hard work that youve put into our Club!

ANY Happy Returns to Mr. Peter STOCK for hisBirthday on Friday, 24 th June 2011. Peter is a new

    LETTERS TO THE EDITOR

From: Isa [email protected] To: Geoffrey READ [email protected] Sent: Tuesday, 19 th April 2011, 0:08Subject: THE PLYMOUTH LARYNGECTOMY CLUBNEWSLETTER FOR APRIL - MAY 2011

    Dear Geoff,

I would like to say that your candidness and opennessabout your terrible ordeal has reached across the Atlantic.I am a smoker myself and never really thought aboutquitting. But I have finally come to the conclusion... it takesme a long time and damn it is hard... to decide to quit!No amount of warnings nor thousands of dollars each yeargoing up in smoke, pun and no pun intended, convinced mebefore. Real life accounts did!I am awaiting with eagerness my kit of e-cigarettes orderedfrom the US ( not available in Canada ) and withdetermination... hopefully... Thank you and all my best!

    IsaxxxxxxxxxxxxCanada

P.S. I allow this email to be published in the PLC Newsletter if it can help or encourage others.

    Dear Isabelle,

Thank you for your email! It does make one think doesn't it! I wish I could turn theclocks back, knowing then what I know now! However, although a lifetime of excessive smoking anddrinking almost certainly caused my cancer, it should not be

assumed that this is always the case. Some sufferers havenever taken drink nor smoked.Nonetheless, thanks for your email and I'll most certainlyinclude in the next edition of our PLC Newsletter.Well done for quitting thats the biggest step done!

Best wishes,Geoff Secretary The Plymouth Laryngectomy Club

    Hello Geoffrey,

Ive just listened with interest to the video of Dr. ItzhakBROOK. This gent who holds a prominent position wherehe has to constantly speak must be an inspiration to many

people whore rather afraid, or embarrassed to use theirnew sounding voice in public. Im a bit of a chatterbox andtalk to anyone wholl listen! However, Ive had a hands -freeunit in my drawer upstairs for some weeks now andalthough I promise myself to start practising, I keep puttingit off because I know itll take many hours before I learnhow to use properly. Starting from to-day I ll commence mypractise and hopefully, before my next holiday in July, willbe confident in public to use it.Thank-you for sending video to me.

    Best wishes,

HEAD & NECK CANCER PRIORITY SETTINGPARTNERSHIP

HE National Association of Laryngectomee Clubs (NALC )has been involved in Head & Neck Cancer research

    Projects for many years, through

having Members serve on the NationalCancer Research Institutes Head & NeckCancer Clinical Studies Group. The textbelow was forwarded to us by NALCfrom what is on The Chesterfield Clubs website.

HEAD AND NECK CANCER PRIORITY SETTINGPARTNERSHIP (ENTUK and Head & Neck 5000 in association with the James Lind Alliance )

    To Laryngectomee Clubs:

HEAD AND NECK CANCER RESEARCH PRIORITY SETTINGPARTNERSHIP - INTRODUCTION

I am contacting you to ask for the help of your club and itsmembers in a project that has just been launched. The aim of the project is to produce a list of uncertainties or unansweredquestions about the treatment of head and neck cancer thatcan be resolved by research. The aim is then to produce apriority list and the results will be passed to researchcommissioning bodies to be considered for funding. What is unique about the project is that it is a collaborationbetween clinicians and patients on equal terms and the views

of both groups will have equal weight in arriving at decisionsabout the priority list. People may suffer unnecessarily becauseuncertainties about the effects of treatments have not beenaddressed in research. Patients and doctors may have verydifferent ideas about what important questions need to beaddressed compared to those raised by the drugmanufacturers or medical technology companies. The aim is tobring our ideas to the fore.

    How you can help

Your club or individual members of the club can contribute tothe project by completing the form and submitting one ormore examples of a treatment uncertainty or unansweredquestion. Could you please arrange a discussion of the projectat one of your meetings or pass the information to individualmembers who you think may be interested? The address towhich forms can be returned is included on the form and thedeadline for completion is Friday, 10 th June 2011.

The form is available for download, so that it can be completedand sent in by email rather than by post. The Chesterfield Clubwebsite www.hnchelp.org.uk has a link which can be found byclicking on the Priority Setting Partnership item on the homepage menu.It does not matter if the ideas that patients andcarers submit are real uncertainties or not, once they arecollected they will be checked to find out if they have beenthoroughly researched or not.

Please consider treatment in its widest sense to includeanything that contributes to a successful outcome in treatinghead and neck cancer and improves the quality of life of

    patients.

    Further Guidance

Please use the Contacts page in "About Us" on this website if you need further information or help.

Thank you for your time and any assistance you can give to thisproject.

RITISH Stiff Upper Lip for Her Break Down 'She JustBottled It All Up': Michael DOUGLAS Blames Catherine

ZETA-JONES. This article was published in The Mail By

Daniel Bates on Wednesday, 27 th

April 2011.CancerIt was announced on 16 th August, 2010, that DOUGLASwas suffering from throat cancer and will undergochemotherapy and radiation treatment. On 31 st August,2010 DOUGLAS appeared on Late Show with David LETTERMAN. and confirmed that the cancer was at anadvanced stage IV . DOUGLAS attributed the cancer to acombination of stress, his previous alcohol abuse, andyears of heavy smoking. In November 2010, DOUGLAS was put on a specialweight gain diet by his doctors due to the excessive

weight loss leaving him weak. On 11 th January, 2011, hesaid in an interview that the tumour was gone. Headmitted that the illness and aggressive treatment hadcaused him to lose 32 lb in weight. He will have to havemonthly screenings because there is a very high chancethat the cancer could return over the course of the nexttwo to three years. Although DOUGLAS has described thecancer as throat cancer , many doctors believe he wasactually diagnosed with oropharyngeal cancer .

Michael DOUGLAS has claimed that Catherine ZETA-JONES broke down under the stress of his cancer battle because of

her British stiff upper lip.Talking to Oprah WINFREY, the actor said his wifes Welshbackground meant that she bottled everything up and wasovercome with emotion when he was given the all-clear.Whilst he just got on with it ZETA-JONES remained stoicthroughout and just stored up problems for herself later on.

Left: Opening up: Michael Douglas spoke to Oprah in acandid interview about his cancer battleand how his wife Catherine Zeta-Jonesstruggled to cope with it.'I just got on with it': But Michael saidCatherine bottled it all up before beingovercome with emotion when he wasgiven the all clear.

The Swansea-born actress was admittedto a US rehabilitation clinic for five daysearlier this month where she was

diagnosed with bipolar II disorder brought on by the strain of DOUGLASbattle with throat cancer.DOUGLAS, 66, was given the all-clear in January. But Zeta-Jones, who has two children, son Dylan, 10, anddaughter Carys, eight, with Michael was still experiencing

the effects of depression.DOUGLASagreed that his health problems had exacerbatedZETA-JONES plight.

When youve got cancer youve got to take care of business. They tell you have seven weeks of chemo, radiation, you kindof dive in and do it, he said. Im the kind of person when theyre sick, I just curl up, Idont like a lot of fuss and no attention. I just like to get on with it. So I got on with it.I think that part of Catherines whole background, shes Welshand the Welsh its all stiff upper lip, not let your feelings show.

Right: Stressed out: Catherine, pictured last week onlocation for her new moviePlaying The Field in Louisiana, wastreated for bipolar II at a clinicearlier this month.

DOUGLAS said that the last 18months had been harrowing andhe had suffered a string of personal disasters that putpressure on his wife.In my last year a nd a half myoldest son is in federal prison, myex-wife is suing me, and I gotcancer. It's kind of hard for thewife to say, "I'm depressed".'He joked: 'You're depressed, huh?How about some cancer, youwant to get depressed?'That played a big part of it, shesgot to be stoic and deal with allthe stuff Im going through. Probably the time Im off the hook you finally relax. Douglas added that he was so proud of his wife for seekingtreatment for her condition.Shes great now, shes shooting a movie and shes relived shehas a much better understanding because this was notsomething where she was clearly aware of what was going on. Speaking in a slightly raspy voice, DOUGLAS became emotionalwhen talking about how cancer had changed his attitude tolife. He said that his priorities were completely different from yearsgone by when he was driven to succeed in his career at theexpense of spending time with his family.Asked he was asked if he was a different man having beatencancer DOUGLAS choked up... and tears were clearly visible inhis eyes.Wow. I think, I know.. . um ... I know that Im much , muchcloser to my friends and family, I have a much deeperappreciation of family and friends, he said. I was truly overwhelmed by the support. 'For somebody who has no formal religious education theamount of prayers and support I received worldwide trulydid have an influence and a help. DOUGLAS next project is to play flamboyant entertainerLIBERACE in a biopic alongside Matt DAMON , who will playhis younger lover.ZETA-JONES is currently filming comedy Playing the Field inLouisiana.

    Head-and-neck cancer

Around 9,000 people in theUK are diagnosed with head-

and-neck cancer each year.Cancers include those of theeye, mouth, voice box andfood i e.

ANCER breath tests a step closer. This article wasbroadcast by the BBC on Thursday, 21 st April 2011.

Could a breath test detect cancer? A breath test that cansniff-out cancer is a stepcloser to reality, according toa preliminary study.Researchers found anelectronic nose was able to

identify chemical signals of cancer in the breath of patients with lung or head and neckcancer.A cancer charity said it would take years of research to seeif the breath test could be used in the clinic.About 80 volunteers took part in the Israeli research, published in the British Journal of Cancer . Of these 22 had various head-and-neck cancers, 24 had lungcancer and 36 were healthy. The prototype breath testuses a chemical method tospot markers of cancerpresent in the breath. The hope is that one daysuch a test could be used ina GP's surgery to give aninstant diagnosis.'Urgent need' Researchers at the Technion (Israel Institute of Technology ) are working on a device called the nano artificial nose. They looked at head-and-neck cancer, which is oftendiagnosed late, making it more difficult to treatsuccessfully.Lead researcher, Professor Hossam HAICK , said: "There's anurgent need to develop new ways to detect head-and-neckcancer because diagnosis of the disease is complicated, requiring specialist examinations."We've shown that a simple 'breath test' can spot thepatterns of molecules which are found in head-and-neckpatients in a small, early study."We now need to test these results in larger studies to findif this could lead to a potential screening method for thedisease."Dr. Lesley WALKER, of Cancer Research UK , said it wasincredibly important to spot the disease as soon as possiblewhen it was easier to treat successfully. She added: "These interesting initial results show promise for

the development of a breath test to detect head-and-neckcancers which are often diagnosed at an advanced stage.

"But it's important to be clear that this is a small study, at avery early stage, so many more years of research with patientswill be needed to see if a breath test could be used in theclinic."

    2011.

Cancer Research UK fundraisingtops 500m. Income of UK'sbiggest fundraising charityincreased by 3.4% in the year toMarch 2010.

The annual income of Cancer Research UK , Britain's biggestfundraising charity, passed 500m for the first time last year.It increased by 3.4% from 498m to 515m in the financialyear ending March 2010.In its annual report and accounts , the charity says growthoccurred "principally because our fundraising held up strongly"during the recession."It was an excellent year for recruiting new donors, increasingthe level of donations, encouraging people to give in differentways and winning back supporters who had stopped giving,"the report says.Income was boosted by a single legacy worth 10m, the largest

ever received by the charity, and a 9m VAT rebate. Reservesincreased by 34m.The charity issued a press release when the report was filed inSeptember last year saying it had spent 334m on researchduring the year, but it did not mention the sum it hadgenerated.Lynne ROBB, Chief Financial Officer at CRUK, said the impact of the economic downturn had so far "not been as bad as feared"but that this could change if the economy worsened. She saidshe expected income to be "fairly flat" in the 2010/11accounts.Formed by a merger in 2002, Cancer Research UKs income has

grown by 134m over the past four years."It shows we made the right decision because people gotbehind us," said Robb.

VERCOMING Depression After Laryngectomy: Anaccount by a physician as a cancer-of-the-neck patient.

Depression is one of the most difficult issues a patient that hadbeen diagnosed with cancer has to face. I am a physician whohad been diagnosed with throat cancer several years ago andam writing this manuscript to share mypersonal experiences as a cancersurvivor and a clinician. I am doing sohoping that my perspectives will assistother individuals who had undergonesimilar experiences cope better withtheir situation and hopefully gain insightinto their lives. It is also presented tohealth care professionals who care forpatients with this condition in the hopethat by gain insight into the patients world they would be ableto better assist them in their plight.Coping with and overcoming depression is very importantnot only for the well being of the patient but may actuallyfacilitate their recovery and may even increase their chancefor longer survival and perhaps even ultimate cure. There is

growing evidence in many scientific studies that there isconnection between mind and body. Even though much of these connections are not yet understood it is well knownby experienced practitioners that individuals who aremotivated to get better and exhibit a positive attituderecover faster from serious illnesses, live longer andsometimes survive immense odds. There are so many reasons to become depressed afterlearning about cancer and living with it. It is a devastatingillness to the patients and their families and medicine hasnot yet found a cure for most types of cancer. By the timecancer had been discovered it is too late for prevention and

if the cancer had been discovered at an advanced stage therisk of dissemination and ultimate cure is significantlydecreased. Many emotions go through the patients mind after learningabout the bad news. Why me? and Can it be true? Afterfirst becoming stunned by the news, feelings of denials, whichare followed by anger, with ultimate acceptance of the newreality. It is believed that depression is a form of anger. Perhapsanger at the new devastating reality. The patient has to face their ultimate mortality, sometimes forthe first time of their life and have to deal with all theimmediate and long term consequence consequences on them

and their loved ones. Paradoxically feeling depressed afterlearning about the diagnosis allows the patient to accept thenew reality. By not caring any more it is easier to live with theuncertain future. I don t care anymore what happens feelingmakes it easier for a while. However, this coping mechanismcarries a heavy prize because it can actually interfere withgetting appropriate medical and surgical care and can actuallylead to rapid decline in the quality of life.Becoming a laryngectomee after losing the vocal cords addsadditional stress and difficulties. The inability or difficulty tospeak creates a sense of isolation. Difficulties in verbalisingbuilt-in emotions can create anger and frustration that maylead to depression. Recognising these challenges by the spouseand care givers can alleviate much of the stress.

This is why individual and group counselling and therapy forboth patient and spouse are very important. The patientsspouse and caregivers often experience emotional strain thatalso need to be addressed.Hopefully, a patient can find strength within themselves tofight the depression. In my case after I returned home from theextensive surgery I had to remove my cancer which includedalso the excision away of my vocal cords, I was overwhelmedby the daily tasks I had to perform and the new realities I had

to accept. I was mourning the many losses I had experienced, which included my voice, my wellbeing, and the need toaccept many permanent deficits such as not having a normalvoice anymore and having to speak using a weak and rustyvoice generated by a prosthesis that connects my trachea andoesophagus. I felt that I had to make an early choice betweensuccumbing to the creeping depression and let it consume meor become proactive and fight back and return to life. I chosethe latter because deep inside I had a very strong desire to getbetter and overcome my handicaps. I also realised that mystruggle is not over and will be with me for a long time andthat I will need to conquer the downhill slope again and again. The driving force on many occasions to become proactive andresist depression is my wish to set an example for my childrenthat one should not give in the face of adversity. I knew thatthey may and unfortunately would also have to face difficult inthe future and did not want to leave them the legacy that theirfather gave up and did not do his best to get back to his feet. This was and still is a driving force that I resort to whenever Ifeel down.

Support by family members and friends is very important.Feeling that ones continuous presence in the world and actualinvolvement in their lives is very helpful in re-igniting the will togo on. The involvement and contribution to others lives can beinvigorating. As a parent or grandparent one can draw strengthand will to go on by seeing and enjoying the interaction and

the impact one has on them. What I found out to be helpful was to become re-involved inactivities I liked before and find a continuous purpose for mylife. I started to return to the hospital to participate and teachin medical rounds and listen to medical lectures. What wasmost encouraging and rewarding was that my contributionsimpacted and improved individual patient care. I felt that I wasmaking a difference again. This made me want to return to thehospital and teach even more. In the process of helping others,I was also helping myself.I was also able to gradually return to many of my otherroutines. I started with simple challenges such as reading

medical literature again, accepting invitation to review articlessubmitted to medical journals, learn how to take a showerwithout aspirating water, and even simply walking more.

I gradually became able to ride a bicycle and even climb to thetop of a mountain with my family. Even though I realised thatthe quality of my voice is not the same as before, one of mygreatest comebacks was to be able to teach and lecture againwith the help of a microphone. Each of these small steps mademe feel better and stronger. I was discovering the world again, quite like an infant learning to walk.Soon after my return home, I started to attend the monthlymeetings of the local Laryngectomee Club, as well as their

monthly speech therapy sessions. I cherished the support andadvice I received from the other club members and especiallyfrom the club president, who was extremely dedicated anddeeply caring and helpful toward the members. I kept comingto the club even when my needs were no longer intense andbecame its Secretary the following year.Seeking the help of a mental health professional can be veryhelpful. I was fortunate to be assisted throughout my illnessand recovery process by an excellent, compassionate andskillful social worker. We discussed various issues relating tomy recovery process, as well as my anxieties and frustrations.Having a caring and competent physician and speech andlanguage pathologist that can provide continuous follow-up.Their involvement can help deal with any emerging medicaland speech problems are veryimportant for the sense of well being.I found ways by which I can use the setback in my life in a positive way. Irealised that I have much to contributeto others. By lecturing and writingabout my experiences and sharingthem with other laryngectomee andhealth care providers, others can learnand benefit from my experience.
