ENROLLMENT INFORMATION REGULAR SUMMER SESSION

JUNE 19, 1995 at the Joyce Center

Continuing Notre Dame Students and non-Notre Dame summer students who received notification of their acceptance must report to the Joyce Center (Supplemental instructions for enrollment for Summer Institutes, mini-courses, and workshops will be made available by the appropriate program directors.)

Non-Notre Dame students who have not receive notification that all of their records (including transcripts) were completely processed, must report to the office of the director of the Summer Session, Room 310,

At the Joyce Center in exchange for the completed enrollment form, you will receive your Summer Session "Student Class Schedule", and your summer ID. (Continuing students can use their permanent ID).

You may fill out a Transcript Request form in the Registrar's Office-'or send a letter to:

A request shall include your full name, dates of attendance, social security number, address of person, school or company where the transcript is to be sent, your return address, and your signature. There is a charge of $2.00 for each transcript. A transcript may not be ordered by telephone.You

may send the request by FAX machine instead of mail. However, the transcript itself will be mailed. Our FAX number is (219) 631-5872. Transcripts are normally mailed, but can be picked up in the Registrar's Office, two or three days after receipt of the request. Transcripts given directly to students are stamped ISSUED TO STUDENT. The University issues only transcripts of academic work taken at Notre Dame. Requests for transcripts of work taken at other institutions should be directed to those institutions. Requests for transcripts will not be honored if you are financially indebted to the University.

FAMILY EDUCATIONAL RIGHTS AND PRIVACY ACT ANNUAL NOTIFICATION

Each year the University of Notre Dame is required to give notice of the various rights accorded to students pursuant to the Family Educational Rights and Privacy -Act (FERPA). Students have, a right to be notified and informed. In accordance with FERPA, you are notified of the following:

1). RIGHT T O INSPECT: You have the right to review and inspect substantially all of your education records maintained by or at this institution.

2) RIGHT T O PREVENT DISCLOSURES: You have the right to prevent disclosure of personally identifiable information contained in your education records to third parties with certain exceptions allowed by Federal regulations. This includes: (1) full name, (2) address, (3) telephone number, (4) date and place of birth, (5) college, major or level, (6) participation in officially recognized activities and sports, (7) weight and height of members of athletic teams, (8) dates of attendance, (9) full or part-time status, (10) degrees and awards received, (11) the most recent previous educational agency or institution attended by the student, and (12) other similar information such as a photograph.

Directory information may be disclosed by this institution for any purpose, without the prior consent of a student, unless the student has forbidden its disclosure in writing.

Students wishing to prevent disclosure of the designated directory information must file written notification to this effect with the Registrar's Office.

In the event that such written notification is not filed, the University assumes that the student does hot object to the release of the directory information.

To enter DART dial 219-634-3003 (4-3003 from on-campus phones) from any touchtone telephone or come to the Office of the Registrar.

Your registration session will be cut off 7 . 5 minutes after you start the session. If you prepare your worksheet in advance, it should take less than 5 minutes to register for classes. However, if you need more time, you can re-enter DART immediately.

DART will allow you to add, drop, exchange, audit a course (graduate students only), review your schedule, or change the number of credits of a variable credit course.

GLOSSARY

COURSE ID DAYS

Call No. A unique 4-digit identification number. Use this number to request the course and section in DART.

Dept. A three or four letter department or discipline abbreviation. See pages 2 and 3 for a complete listing.

No. Each course is identified by a three digit number and, in some cases, with a one letter suffix. The suffixes "A", "B", and "C" are used to distinguish different sections of a course when the approach, content or credit hours differ. "D" indicates a drill or demonstration course; "L" indicates a lab course; "R" is for research courses; "S" is for studio courses; "T" is for tutorial courses.

Start-End Time

The starting and ending times of the section.

Instructor

The name(s) of the instructors) teaching the section. If no name appears, the instructor will be announced later.

SEC

Each section of a course has a unique two-digit section number.

Credit Hours

The number of credit hours associated with the course. "V" indicates a variable credit course. "0.0" is used for non credit courses receiving grades or for courses which are part of another course and for which no separate grade is to be given.

EXAM
