What is The Skilled Nurse International?The Skilled Nurse was founded by Australian Nurse Educators who know how difficult it can be to meet our continuing education requirements, especially if you work in a small facility, or in a rural area.

Case Study - Your initial observationsGerald appears orientated and alert although weak - He is very cold to touch - His clothes are damp and he smells strongly of stale urine - His left leg is shortened and externally rotated.

As you assist the paramedics in transferring Gerald on to the stretcher in your emergency department the ambulance officers relate the initial history. At around 10 p.m. last night Gerald went into the kitchen to make himself a cup of tea and tripped on a piece of loose lino near his stove. He immediately felt severe pain in his left hip and was unable to rise.

Consequently he spent the night exposed on the cold kitchen floor. When he was found this morning, he was extremely cold, difficult to rouse, had been incontinent of urine and in severe pain from his left hip.

When you ask Gerald about his past history, he tells you that he is normally fit and well, although he gets a little bit dizzy sometimes. He takes medication for hypertension, but is proud of his general good health. He tells you he lives alone, since his wife's death last year.

You make a quick call to the doctor requesting his attendance.

Gerald, would be a category 2. He has a query fractured left neck of femur, he is hypothermic and in severe pain. His mechanism of injury also suggests that he may have other as yet undiscovered injuries.
