Welcome to King’s Medical Practice, previously known as Princess Street Surgery. This leaflet explains the services we offer. If you have any questions after reading the leaflet please do not hesitate to ask a member of our staff.

MISSION STATEMENT

King’s Medical Practice will strive for continuous quality improvement in patient care, a caring approach to staff, a business approach to practice management, and collaboration with others.

ACCESS TO THE SURGERY

The surgery has been designed with the disabled in mind. The entrance, appointments area, consulting rooms, and toilets are suitable for the access of wheelchairs.

PRACTICE AREA

The surgery treats patients who live in Normanton, Altofts, Whitwood, Ackton, Old Snydale, Warmfield, Kirkthorpe, and Heath. Below is a map of our practice area:

Dr Patricia Mooney (female) BSc MB ChB (Leeds 1986) DRCOG MRCGP (Partner)Joined the practice in May 1992. Dr Mooney is a GP with a special interest in Dermatology and undertakes one session in Dermatology at Pontefract General Infirmary and two sessions for the Primary Care Dermatology service at Castleford and Normanton District Hospital. She also has an interest in family planning.

Dr Jacinta M Walsh (female) MB BCh (Cardiff 1993) MRCGP DippallMed (Partner)Joined the practice in August 1999 after training in Cardiff. Dr Walsh is particularly interested in Palliative Care and is a Hospice Practitioner in Palliative Care at Pontefract Hospice where she does two sessions per week. She is also a GP Trainer and Training Programme Director for Dewsbury, Pontefract and Wakefield.

Doctors training for general practice

The practice is an approved training practice for doctors wishing to train for general practice. Our training doctors, known as GP registrars have an education programme when attached to the surgery and are supervised in their clinical work. Dr Jacinta Walsh, Dr Sarah Deeley, and Dr Rebecca Harding are approved GP Trainers.

Some consultations may be video recorded. The doctors use these videos as a means of improving their skills. Your permission will always be sought if the doctor is taping their surgery. You may request that your visit is not recorded. It will not affect your treatment in any way. The examination couch is outside the field of vision of the camera.

The wider nurse team comprises of practice nurses, district nurses, health visitors, and midwives based in the surgery and offers an ‘integrated’ approach to deliver a high quality service to patients both in the surgery and at home.

Practice nurses have specialist areas of expertise including minor illness management, health promotion (including family planning, cervical smears, weight management, and smoking cessation) and chronic disease management.

The District Nursing Team offers care, support, and advice for patients with acute, chronic, disabling and/ or life threatening illnesses.

Services Available

The practice runs the following dedicated disease management clinics:

Asthma Diabetes Hypertension Heart Disease screening and advice Immunisations and travel vaccinations COPD (chronic bronchitis) Health checks for new patients and patients between 40 and 74 Weight management

The practice offers the following services for women’s and child health:

Family planning: including contraceptive pills, injections, coils, implants, emergency contraception, and sexual health advice

Cervical smears: this can be taken by either a nurse or doctor Menopause advice Ante-natal care Baby clinic Immunisation clinic Child health surveillance: the doctors and health visitors take part in the local child

health surveillance programme.

Patients have the right to choose the healthcare professional they wish to see and the staff at our reception desk will assist you in making appointments to see a doctor or practice nurse, take requests for repeat prescriptions, and offer help and assistance to any queries you may have.

They are committed to making the best arrangement that suits you, bearing in mind the need of all our patients. It is a legal requirement that patient confidentiality is maintained at all times and all our staff are trained in maintaining this responsibility to the highest degree.

Patient Confidentiality and Information Management

We take our responsibility for keeping the confidentiality of data within your medical record very seriously. The practice has been fully computerised since 1988. It helps us to work more efficiently and safely. The information is held on a strictly confidential basis. Information is shared amongst the practice team on a need to know basis. We will only disclose information to clinicians (e.g. hospital doctors) who are not part of the practice team with your explicit consent and we only disclose information to third parties (e.g. insurance companies) with your written consent. A separate practice leaflet is available which provides detailed explanations of our Confidentiality and Information Management policies.

As part of the practice's contribution to medical research, we provide completely anonymous patient-level treatment information to certain reputable third party organisations with your consent. No individual is ever identified from the information and all patients stand to benefit from this work. If you would like more information about this please contact Dr Walsh.

Patients' Rights

You have the right to use all the services provided by the practice. You have the right to complain if you feel you are not receiving a satisfactory service

from the practice. You have the right to see a GP of your choice (you may not be able to see the GP of

your choice immediately).

Patients' Responsibilities

To treat all members of staff in a reasonable and courteous manner. To make every effort to be punctual for your appointment. When you have been given an appointment you are responsible for keeping it or giving

adequate notice of cancellation so that the appointment can be made available to someone else.

If possible, please avoid contacting the practice before 10:30am unless you require an URGENT appointment or home visit that day. We have several lines and the call handlers cannot always answer immediately. The call handlers deal with patients in a polite and professional manner and will answer your call as soon as possible.

To ring before 10:00am if you require the doctor to make a home visit. To inform us of any changes of address or contact details. Accompany children aged less than 14 years when they are consulting a health care

professional. Ensure your family is fully immunised.

Chaperones in the Practice

King’s Medical Practice is committed to protecting the privacy and dignity of all patients.

Some examinations and treatments, particularly those that involve intimate parts of the body and the need to undress, can make some patients feel vulnerable and uncomfortable; this is when a chaperone can help support a patient during such consultations.

Chaperones are specially trained members of staff at the practice. Family members are not allowed to formally witness or take part in the examination. However, you can request a family member or friend to be present. You will be offered a chaperone for any intimate examinations. However, you can request a chaperone for any consultation that you feel uncomfortable with. Your clinician will introduce you to a chaperone and outline their role during the procedure. You do have the right to refuse a chaperone. The member of staff will document your reason.We do our best to ensure there are plenty of trained chaperones. However, if in the unlikely event a chaperone cannot be offered, we will arrange an alternative appointment when a chaperone will be available.Please speak to a member of staff on reception or in the admin team if you require any more information.

Violent or abusive patients

In the interests of our staff and patients, the surgery upholds a policy of zero tolerance towards any patient who is threatening, violent, or abusive whilst at the surgery or when a health professional makes a home visit. Such patients may be reported to the police and/ or removed from the practice’s list. Patients removed from the practice list will be informed in writing of the reason(s) for removal and the action they need take to register at another surgery.

Keeping you up-to-date with what’s happening in the surgery

Information is available from notice board items and leaflets in the waiting room. Dr Walsh also produces articles on our web site almost weekly if not daily and his blog covers a wide range of healthcare topics including what is happening at the local hospitals. King’s Medical Practice is not a limited partnership, and is a practice contracted to NHS England, PO Box 16738, Redditch, B97 9PT. Telephone: 0300 311 22 33.

The members of the partnership are: Dr P Mooney, Dr J Walsh, Dr R Harding, Dr S Deeley and Mrs. C Sanderson. Wakefield CCG holds the responsibility for the out of hour’s service to our patients, which is currently provided by NHS Commissioning Board Area Teams.

WHAT TO DO WHEN YOU WANT TO…Register as a new patient

New patients wishing to join the surgery are requested to complete the ‘Family doctor services registration form’ GMS01 (known in the surgery as the purple form). You will then be offered the opportunity to attend a new patient clinic, which provides a basic health check and further information about the services we have to offer.

You are able to register on-line at www.kingsmedicalpractice.co.uk, but please be sure to complete all the required fields.

Make an appointment

Appointments can be made by telephoning (01924) 223 909 and choosing option1 or you can call in person at the surgery.

The practice appointments system aims to be able to provide urgent appointments available on the day but also appointments up to 6 weeks in advance if you know you will need to see a GP. We also have a number of routine and follow-up appointments throughout the week.

If you are unable to keep an appointment, please inform us a soon as possible as this will allow us to offer the appointment to a different patient.

Appointments can be made ‘on-line’ if you have registered to use this service. To register you need to request a SystmOne password in person at the practice. You must bring proof of your identity.

Offered a telephone consultation

The surgery offers telephone consultations with our doctors. When you ring for an appointment you may be offered (or you can request) a ‘telephone consultation’. The doctor will then ring you at the agreed time to discuss your problem.

Telephone consultations are shorter than surgery consultations and therefore are best for simple things, which can be easily dealt with over the telephone.

Please ensure you keep your telephone with you to hand to take the call, it will be displayed on caller ID as “Withheld”.

Sit & Wait Morning sessions

The surgery offers a Sit & Wait session for Not Urgent & Not Routine appointments; this is held Monday to Friday between 8.40 and 10.30. Please book in at reception, take a seat and wait to be seen in turn.

Request a home visit

To request a home visit please telephone before 10:00am for a same day visit. We only visit you if you are too ill to come to the surgery. Please give the member of staff taking your call as much information as possible to enable the surgery to prioritise home visits.

We restrict home visits to those who are too ill to come to the surgery, as several patients can be seen at the surgery in the time it takes to do one home visit.

How to contact a doctor when the surgery is closed

To contact a doctor when the surgery is closed you should telephone NHS 111. All telephone calls to the service are recorded.

Order a repeat prescription

If you are allowed a repeat prescription you can order one by:

Please note: At least 2 working days (excluding Saturday, Sunday, and bank holidays) notice is

required to have your prescription ready for you to collect. You can only get a repeat prescription if our computer system will allow you one to be

issued. Generally you will need to see the doctor or practice nurse every six months to have your prescription reviewed.

Get your test results

Telephone (01924) 223 909 choose option 4, available between 10:00am and 5:00pm. In order to maintain patient confidentiality we are not able to give results to anyone but the patient (except in the case of children).

Change your address or telephone number

Please inform the practice (in writing) immediately any changes are made. We do have a form available for collection from reception if you prefer.

Obtain a medical or fitness certificates

The surgery issues certificates (free under the NHS regulations) to enable you to claim benefits. Other certificates are not provided free under NHS regulations. The surgery will charge a fee for insurance reports and medicals, fitness to drive medicals and certificates required by employers. The fees we charge are available from reception.

Access for young people

We are aware that sometimes young people have things they want to discuss on a one to one basis with a Doctor or Nurse. Here at King’s Medical Practice all the Doctors and Nurses are happy to see young people on their own if they wish.

Make a complaint

Our aim is to provide you with the best service possible. If you have a complaint or concern about any aspect of the service you have received from any of the staff working in the surgery then please let us know. We operate a practice complaints procedure as part of the NHS system for dealing with complaints. You should contact Christine Sanderson, our practice managing partner in the first instance.

OUT OF AREA REGISTRATION

Out of area registration

New arrangements introduced from January 2015 give people greater choice when choosing a GP practice. Patients may approach any GP practice, even if they live outside the practice area, to see if they will be accepted on to the patient list.

GP practices have always had the ability to accept patients who live outside their practice area. Regardless of distance from the practice, the practice would still provide a home visit if clinically necessary.

The new arrangements mean GP practices now have the option to register patients who live outside the practice area but without any obligation to provide home visits.

Out of area registration (with or without home visits) is voluntary for GP practices meaning patients may be refused because they live out of area.

If your application is considered the GP practice will only register you without home visits if it is clinically appropriate and practice in your individual case. To do this we may:

Ask you or the practice you are currently registered with questions about your health and to help decide whether to register you in this way.

Ask you questions about why it is practice for you to attend this practice (for example, how many days during the week you would normally be able to attend)

If accepted, you will attend the practice and receive the full range of services provided as normal at the surgery. If you have an urgent care need and the surgery cannot help you at home we may ask you to call NHS 111 and they will put you in touch with a local service (this may be a face to face appointment with a local healthcare professional or a home visit where necessary).

We may decide that it is not in your best interests or practice for you to be registered in this way. In these circumstances we may offer you registration with home visits, for example, if you live just outside the practice area or we may not register you and advise you should seek to register (or remain registered) with a more local practice.

If accept but your health needs change we may review your registration to see if it would be more appropriate for you to be registered with a GP practice closer to your home.

ALLOW US TO POINT YOU IN THE RIGHT DIRECTION

Practice Nurse Clinics

The nurse team can offer advice and treatment (if necessary) for sore throats, hay fever, chickenpox, conjunctivitis (eye infections), cold sores, infected wounds, head lice, emergency contraception advice, threadworms, urine infections, thrush, colds. and flu’.

NHS 111 and online service www.nhs.uk

NHS 111 is a confidential NHS help line, which offers advice and information about health problems and services. It is available 24 hours a day at the cost of a local telephone call. If you are anxious about ill health or uncertain about what to do then NHS 111 trained nurses can give you a full range of advice from self-care at home, to arranging an out-of-hours consultation or sending for an ambulance.

NHS Walk in Centre

Wakefield has an NHS Walk-in Centre where you can walk and see either a nurse or, if appropriate, a GP. The centre is the Thornhill House, Thornhill Street in Wakefield and is open from 9:00am to 17.30pm Monday to Friday.

Accident and Emergency Department

Our local A&E departments are amongst the busiest in the country. Every day many people attend who could have gone to the surgery, chemist, or dentist. Think before you go and if you are unsure then ask NHS 111 for advice.

999 – Ambulance Services

The ambulance service is provided for patients who are extremely ill and need emergency treatment.

For emergencies such as loss of consciousness, severe chest pain, serious accident, or severe blood loss, telephone 999 and ask for an ambulance.

Practice Patient Participation Group

The practice does have a Patient Participation Group which has been active since 2005. The group meets on average four times per year and is very active in practice developments. If you want to join the Patient Participation Group please leave your contact details with reception and you will then be invited to the next meeting.

This service is open 7 days a week, every day of the year, Monday to Friday 18.00 to 22.00 hrs. Saturday, Sunday & Bank holiday 9.00 to 15.00 hrs.

ADVICE ON DEALING WITH COLDS AND ‘FLU’

Should you worry?

Not usually. Most people get better in five to seven days with no problems. As a virus usually causes colds and ‘flu’, antibiotics do not help. Generally, there is no need to see the doctor; there is nothing he or she can do that you cannot do yourself.

What can you do for yourself (or your child)?

Rest in a well-ventilated room Drink plenty of fluids Take paracetamol regularly to relieve pain and fever Steam inhalations and sips of warm juice an also help Salt water or aspirin gargles can relieve a sore throat Don’t smoke

See the doctor if:

Symptoms are rapidly worsening or persist for more than one week You develop ear or sinus pain You start coughing up green/yellow spit or blood You are passing very little or no urine or stop drinking

Contact the doctor immediately if any ill person develops a violet or brown rash, which does not fade under pressure. You should also do this if the ill person develops any confusion or disorientation or fails to react normally to their surroundings.

ADVICE ON DEALING WITH GASTROENTERITIS IN CHILDREN

Gastroenteritis is an infection of the guts (intestines). Many bacteria, viruses, and other microbes (bugs) can cause gastroenteritis. It can range from a mild stomach upset for a day or two with some mild diarrhoea, to severe vomiting and diarrhoea for several days or longer.

What can you do to help your child?

Give the child lots to drink. The aim is to avoid dehydration (low body fluid), which is the main possible complication. Even if the child vomits or feels sick you should still give frequent sips, as some fluid will still be absorbed. Ideally, give water as the main drink, but also give some fruit juice as this contains some sugar. However, any drink is better than none. If the child will only drink their favourite drink then that is fine. (Ice cubes and ice-lollies are useful extra sources of fluid.)

A doctor or nurse may advise rehydration drinks if they are concerned that the child is at risk of dehydration. You can give these instead of, or in addition to, normal drinks. Rehydration drinks are made from sachets which you can buy from pharmacies. You add the contents of the sachet to water. Rehydration drinks provide a perfect balance of water, salt, and sugar and are better than just drinking water alone. The small amount of sugar and salt helps the water to be absorbed better from the gut into the body. They do not stop or reduce diarrhoea, but are the best drinks to prevent or treat dehydration. Do not use homemade salt drinks as the quantity of salt has to be exact.

The child should eat as normally as possible. Do not 'starve' a child with gastroenteritis. This used to be advised but is now known to be wrong. However, if he or she does not want to eat, then that is fine. Drinks are the most important and food can wait until their appetite returns, however offer some food every now and then. Soups and food high in carbohydrate such as bread, crackers, and pasta are best to start with.

Breast fed babies should continue to breast feed if they will take it. This is in addition to extra rehydration drinks if they are advised.

Bottle fed babies should be fed with their normal full strength feeds if they will take it. Again, this is in addition to extra rehydration drinks if they are advised.

You should never give medicines to stop diarrhoea to children. They sound attractive remedies, but are unsafe to give to children due to possible complications.

Give paracetamol to ease fever, headache, or stomach pains.

See the doctor if you are concerned about

Dehydration. Symptoms include: passing little urine, a dry mouth and tongue, drowsiness

The surgery is open Monday to Friday from 8:00am to 6:30pm; however we open at 7:00am on Thursdays for routine appointments only and close at 8:00pm alternating Tuesdays and Thursdays. The surgery is also open for routine appointments only on Saturday mornings.Also Sit & Wait, Monday to Friday 8.40 to 10.30
