Memory Care Services
Living with memory loss and dementia or caring for a loved one suffering from these conditions in challenging and stressful. Lexington can provide relief and comfort through our Memory Care Program.
Our program is based on a relationship-based model of care, allowing us to know our residents beyond their diagnoses and care for them as individuals like a family member.
Through family-like relationships, we create days that go beyond predictable clinical outcomes, inspiring better quality of lives.
Our vision is to create a long-term care setting where people living with memory loss can contribute while connecting with others in meaningful ways each day.
We work with families to create personalized spaces for loved ones with pictures, mementos, and souvenirs as reminders of special people, proud accomplishments, and memorable times.

Safety and Support Our memory care units are secure and supportive environments with special healthcare staff trained in comprehensive Alzheimer’s and related dementia courses.
Lexington’s training was developed by the Alzheimer’s Association-Greater Illinois Chapter in collaboration with the Illinois Department of Public Health.
Our memory care staff can address the symptoms and concerns that arise with the various stages of memory loss and dementia, such as:
• Restlessness, anxiety, and agitation • Wandering and waking during the night • Resistance to activities of daily living • Sleep quality, bathroom pattern,
toileting and bathing
