RESPONSIBILITY: Reporting to the Manager, Community Development and Housing, the Coordinator, Housing Case Management is responsible for the clinical oversight of community housing support programs and the activities of the Housing Case Management team.

QUALIFICATIONS:

Education: Completion of a bachelor’s degree in health, social services or related discipline. Consideration will be given to those who are working towards completion of a degree.

Professional Designation/Certification (one of the following are required):

i. Registration, in good standing, with a regulating professional body (e.g. OCSWSSW); or ii. Certification, in good standing, with a professional certifying organization* within health or

social service fields (e.g. CPRRP from PSR Canada)

*Organization must have a written code of ethics, core values and standards that are consistent with those of CMHA-S/M. Certification must require members to meet a core set of competencies, have a requirement for continuous learning to maintain certification and be approved by CMHA-S/M.

Certified Health Executive (CHE) an asset

Experience: Min. of 3 years’ experience working in mental health and substance use field

required. 2 years supervisory experience preferred. Experience with housing support or

working with marginalized populations is an asset.

Other: Advanced proficiency with Microsoft Excel required.

Knowledge and application of quality improvement methodology preferred.

Reliable vehicle required.

Language requirements: Fluency in English is essential. Advanced conversational French

is an asset. Priority will be given to candidates who are bilingual in French and English as

per our French Language Service Plan.

INCUMBENT: REPORTS TO: Manager, Housing and Community Development

POSITION SUMMARY:

Reporting to the Manager, Community Development and Housing, the Coordinator, Housing Case Management is responsible for the clinical oversight of community-based housing support programs and the activities of the Housing Case Management team. The Coordinator, Housing Case Management will be required to work week days as well as evenings or weekends as required.

ESSENTIAL JOB RESPONSIBILITIES:

Program Coordination o Oversee and direct the activities of the Housing Case Management programs. o Develop and maintain program standards, procedures and outcomes for urban and rural programs. o Oversee and manage reporting requirements for active partnerships. o Review clinical letters, documentation and care plans. Provide feedback and guidance as required. o Assist staff with service recipient issues and escalate to the Manager as required. o Review and update program tools, including forms, templates and care plans, as required. o Manage referrals to the program to minimize wait times for service. o Review program outcomes, performance metrics and statistics. o Assist with coordinating annual unit visits to rent supplement recipients. o Work collaboratively with community partners to identify community needs and implement support

programs that improve housing outcomes for individuals. o Submit regular program reports to Manager.

Team Coordination o Provide guidance, direction, and mentorship to staff. o Direct activities of the team by scheduling staff hours, approving time off requests, assigning and

monitoring work, gathering resources and implementing productivity standards. o Maintain and update staff schedule on file and/or portal. o Identify team and individual learning needs; coordinate training for staff. o Participate in interviews and provide feedback to the hiring committee. o Develop orientation plans and conduct new hire orientations. o Ensure regular communication with staff and Manager. o Conduct meetings with staff as part of regular supervisions, performance reviews, team meetings and to

address human resources issues as needed.

Other o Approve one-time purchases within the defined scope for the team and/or program through a purchase

This may include: being an active participant and/or lead

on committee(s), preparing meeting agendas and distributing meeting minutes; and implementing and following up on action items.

o Ensure monthly, annual and other required reporting documentation are completed. o Maintain the Agency’s values, vision and purpose by ensuring that clients’ rights are respected and that all

programs and activities meet legislative requirements, policies and procedures, funding body criteria, and professional and Branch standards.

POSITION: Coordinator, Housing Case Management

Program Description:

The Housing Case Management Program is part of CMHA-S/M’s supportive housing portfolio. The program provides short term, goal oriented support to assist individuals who are living in the community with maintaining their living environment and wellness to reduce the risk of homelessness, crisis or hospitalization.

Priority Population: Vulnerable individuals who have had difficulty securing and maintaining housing due to various challenges including health challenges, lack of adequate financial resources and social status. Individuals may be homeless, at risk of homeless or otherwise marginalized which can contribute to them being able to successfully maintain housing.

Please answer the following questions:
