SUMMARY OF FINDINGS The following items were noted:

1. The Sanitary Sewer Overflow dated April 16, 2014, at 277 N. Davis states that the overflow was 500 gallons and only impacted land. However, there were two storm drains observed behind 277 N. Davis. One was located approximately 10-15 feet to the west of the manhole. The second storm drain was approximately 75 feet south of the manhole. Thus, the report needs to be updated to address the storm sewer system/ditch north of Marvin’s Grocery Store.

2. There was an opening observed within the manhole next to the sewer line that needs to be repaired. See photograph DSCN2342.

GENERAL COMMENTS On April 22, 2014, Matt Holden, District 1 Field Inspector, and I conducted a reconnaissance inspection at 277 N. Davis Road in response to a complaint. The complaint alledged that an SSO occurred behind her residence. Clean-up efforts had been performed in the area where the discharge occurred by the evidence of lime that had been distributed by the manhole. Ken Hayes, Pea Ridge Utilities Manager, confirmed that the overflow had occurred and stated that it had been reported as required to the Department. The report was observed on the Department’s SSO database. According to the report, the overflow occurred on April 16, 2014. It consisted of 500 gallons and only impacted land. However, there were two storm drains observed behind 277 N. Davis. One was located approximately 10-15 feet to the west of the manhole. The second storm drain was approximately 75 feet south of the manhole. Mr. Hayes stated that there are grease issues on this sewer line from a commercial area. Three restaraunts are using a grease interceptor. Mr. Hayes stated that Pizza Hut and Marvins Grocery Store do not currently have an adequate grease interceptor. Mr. Hayes stated that the line is jetted every Thursday on Canon St. The City of Pea Ridge has scheduled to camera the line that runs north and south. Mr. Hayes stated that the manhole behind 277 N. Davis Road will be replaced within 30 days. This manhole is an old brick manhole in an older area of town.

May 13, 2014 Ms. Joy Alison West Arkansas Department of Environmental Quality Water Division Inspection Branch 5301 Northshore Drive North Little Rock, AR 72118-5317 RE: Reconnaissance Inspection AFIN: 04-00152 Permit Number: AR0020672 Dear Ms. West. This letter is The Utilities response to the “Summary of Findings” from a reconnaissance inspection that took place on April 22, 2014. The two storm drains that were referenced in the findings are tied together, the flow going from north to south. Both are about four feet deep and are designed with percolation drains in the bottom. In other words, they have four inch holes in the concrete bottom that allows retained water to absorb into the ground. The drainage basin that is adjacent to the manhole has a retention capacity of over 400 gallons. Some water from the SSO reached this basin, but not the one located downstream. This water would have absorbed into the ground given a little time.

Storm water from the ditch north of Marvin’s Grocery Store flows into this basin. Therefore the area was not affected. All sewer lines in this area were jetted and camerad and no bad lines were found. I will continue to work with the two remaining businesses that do not have grease interceptors. They have both been in operation for a long period of time and were already in operation when we began requiring new businesses to install grease interceptors. Both facilities have interceptors but they are much too small to be effective. The damaged manhole was removed on May 5th. A new one was poured on the following morning.
