• Conducts OPD and indoor activities for diagnosis, staging, treatment and monitoring disease progress.

• Offers unique experimental and research-oriented diagnosis and treatment services to test new forms of diagnosis and therapy in order to improve quality of life and expected survival of those afflicted with cancer.

Chairman’sMessage
Communitiesworldwidearegrapplingwiththescourgeofcancer. Slowlyandgraduallythismenacehasbecomeahealthhazardcreepinginquietlyanddevastatingourlives.
Morethan14millionnewcasesofcancerarereportedeveryyearacrosstheglobeandthenumberofnewcases is expected to rise by 70 percent in the next two decades, as estimated by theWorld HealthOrganisation.InIndia,cancerisamajorcauseofmorbidityandmortality.Onanaveragenearly1500peoplebattlethisdiseaseeverydayandithasbecomeoneoftheleadingcausesofdeathinthecountry.Itisalso increasingly impacting the quality of life with the Lung, Liver, Colorectal, Stomach and Breastemergingasthemostcommoncausesofcancer.
Havingsaidthat,Ibelievethatamidstthisgrowingmenaceisalsoarayofhope.Itisestimatedthatabout30%to50%ofcancerscanbepreventedbycontrollingthemodi�iableriskfactorssuchastobaccouse,alcoholuse,unhealthydietandphysicalinactivity,amongstothers.Inaddition,asigni�icantproportionofcancerscanbecured,bysurgery,radiotherapyorchemotherapy,especiallyiftheyaredetectedearly.
Inmanyurbanandruralhouseholds,cancertreatmentisdisruptedduetolackofawareness,denialanddelayeddiagnosisandtreatment.Earlydiagnosisandtreatmentofcancercantrulymakeadifference.Thisiswhatde�inesthemissionoftheGujaratCancer&ResearchInstitute(GCRI),reachingouttothepatientssufferingfromcancerespeciallyfromthe�inanciallydisadvantagedsectionswithbest-in-classservicesinprevention,detection,treatmentandpalliationinthe�ieldofcancer.
Sinceitsinceptionin1972,GCRIhasbeenleadingthe�ightagainstcancerprovidingworld-classfacilitiesforcancerdiagnosis,treatment,care,research,rehabilitationandeducation.Overthelast�ivedecades,ithasestablisheditselfasthemostaffordablecentreforbonemarrowtransplantationinmalignantandnon-malignantconditions.
Toprovideanoverviewontheyeargoneby,30,163newcancerpatientsregisteredand3,94,712patientsattended OPD during the year. 1,20,616 radiological, 1,54,759 histologic and cytologic and 7,69,297laboratoryinvestigationswereperformedattheInstitute.Nearly6023majorsurgeriesand15667minorsurgeriesincludingIVTCwereperformed,9180patientsunderwentradiotherapy,55390patientsweretreatedwithchemotherapyand42patientsunderwentbonemarrowtransplantation.
Painmanagementiscriticalforcancerpatients,especiallyinpatientssufferingfromadvancedstagesofcancer.Thisisaddressedbythepalliativemedicinedepartmentoftheinstitute.Duringtheyear,13768patientswereofferedpalliativecareforpainmanagement,563patientsweregivenHomeCareServicesand298patientsattheHospiceCentre.

GCRIisapartofthecancerregistryprogrammeensuringgoodqualitydatanecessaryforplanningofhealth service and carrying out cancer control programmes. The Hematology, Flow Cymetry andMicrobiologyLaboratoriesofGCRIareNABLaccreditedsince2011.
TheGujaratCancer&Research Institutehasbeenat the forefront in the �ightagainstcancer throughawareness and prevention programmes, preventive health check-ups, setting up super specialities incancercareandotherareaslikephysiotherapy,hospicecare,stomaclinic,speechtherapy,prosthesisandrehabilitation.
Today,GCRIspecialisesinsurgicaloncology,medicaloncology,radiationoncology,preventiveoncology,paediatriconcology,neuro-oncology,gynaec-oncology,radiodiagnosisandnuclearmedicine,pathologyandmicrobiologyandlaboratorymedicineledbyateamofhighlyquali�ieddoctorsandsurgeons.
Placinganimpetusoninnovation,GCRIhasalsobeenpromotingbasicresearchatits�ivewell-equippedresearchlaboratoriesonseveralaspectsofcancerbiologyforearlydiagnosis,monitoring,prognosticationandcancermanagement.TheinstitutealsopromotesacademiccourseslikeDM,MCh,MD,DMRE,DA,DCP,DGOinoncologyaswellasM.Sc.inCancerBiologyandPhDinLifescience.
Whilewehavemadeabeginninginwagingawaragainstthisdeadlydisease,thereisstillalongwaytogointhisbattle.Iamcon�identthatwewillmakeheadwayasGCRIisblessedwithateamofdedicatedandhighlycommitted doctors, physicians, surgeons, academicians, researchers and administrators who willcontinuetoleadthis�ightagainstcancerwiththeirknowledge,skills,careandcompassion.

Gujarat Cancer&Research Institute (GCRI) established in 1972 and in a journey of 46 yearsmetamorphosedas StateCancer Institute.The Institutehas comprehensive facilities forpatient care,teachingandresearch.GCRIisinstrumentalinthedevelopmentoftertiarycancercenters(TCC)inthestate.GCRIhasdevelopedSiddhpurCancerCenterandestablishingnewTCCsfunctionalatRajkot.
GCRI focuses on four major areas: 1) providing state-of-the-art diagnostic and therapeuticmanagementbymultidisciplinaryteam,2)treatingpatientswithcancerpainandwithterminalstage,3)reducingcancerburdeninGujaratStatebyconductingcommunityout-reachprograms,earlydetectioncamps,regularhealthcheck-upprograms,and4)offeringmedicaleducationandresearch.
ComprehensiveCancerCare:
GCRIofferscutting-edgecancertreatmentoptionsathighlevelsofcareandconvenience,andhelpimprove cancer survival rates. Institutemanages adult andpaediatric cancerpatientswith650bedsfacility.Twenty-oneclinicaldepartmentsincludingsuperspecialtywithothersupportservicesinvolvedintreatmentofalltypesofcancers.Thisyear29,952newcancerpatientsregisteredand3,58,499patientsattendedOPD.Themostcommonthreeleadingsitesofcancerobservedinmenareoralcavityexcepttongue(21.92%),tongue(12.18),lung(10%)andinwomenarebreast(21.45%),cervixuteri(17.18%),mouth (6.77%). In a year, 1,18,983 radiological, 1,84,913 histologic and cytologic, and 17,54,904laboratoryinvestigationswereperformed.Further,4,372majorsurgeriesand16,520minorsurgeriesincludingIVTCwasperformed,10,226patientstreatedwithRadiotherapy,49,605patientstreatedwithchemotherapyand28patientswithBoneMarrowTransplantation.TheHematology,FlowCytometryandMicrobiologyLaboratoriesareNABLAccreditedsince2011.
GCRIcaterscancercaretothepoorestofpoorwithqualityservicetothecancerpatientsofGujarat,Rajasthan,MadhyaPradesh,Maharashtraandotherstates.MAYojana,MAVatsalyaYojana,LowIncomeGroupScheme,SchoolHealthProgram,S.C.,S.T.andotherGovernmentSchemeshelptoservelowsocio-economicgroupwithbettercancercare.
Palliative&HospiceCare
Painisacommonsymptomforcancerpatients,especiallypatientswithadvancedstageofcancers.Cancerpain and treatmentof terminally ill patients aremanagedbypalliativemedicinedepartment,HospiceCentreandHomeCareServices.Thisyear18,854patientswithcancerpainaremanagedattheinstitute,114patientsbyHomeCareServicesand225patientsatHospiceCentre.

Thecampaignaimstoprovideawarenessoncancer,earlydetectionandtipsforpreventionforvarious cancers thereby to reducemorbidity andmortality through cancer awareness and detectioncamps.Thisyear5,218peoplevisitedin22cancereducationcamps,and265suspiciouscancercaseswerereferredtoGCRIthrough78CancerDetectionandSanjeevaniRathCamps.
Education&Research
GCRI provides medical education and training to future health professionals. The medicaleducationoftheinstitute isaf�iliatedtoBJMedicalCollegeforconductingMD,D.M./M.Ch.coursesinoncologyand145medicalstudentsareenrolled.Ph.D.inLifeSciencesandMicrobiologyaf�iliatedwithGujaratUniversityarealsoconductedattheinstitute.ScientistsofCancerBiologydepartmentmentor8PhDstudentsand35MScCancerBiologystudents.Thereare57researchpublications,2International,32Nationaland67StateConferencepresentationswith28ongoingresearchprojectswith18clinicaltrials.Thisyear13staffmemberssupportedfortrainingandscienti�icworkshops.
IncomingyearGCRIwillmoveinnewbuildinganddiligenttogetNABHAccreditation.Wecontinuetostrivetowardsourmissiontopowerfulandinnovativetherapiestocancerpatients,improvequalityoflifeandrestorehope.Wewishtoemergeasgloballeaderincancercontrolandtoacceleratethepaceofgroundbreakingtranslationalresearchthatcangetlifesavingnewtherapies.

ClinicalServices:Thedepartmentprovidesservicesof anesthesia for oncosurgical patients. Anesthesiaservices areprovidedduringperi-operativeperiod,which includes pre-anesthetic checkup andanesthesia fitness, intra-operativemanagementandpost-operativecareofthepatients.Ourdepartmentprovidesanesthesiaormonitoredanesthesiacareasper the requirement of the patient and surgeons.Different super specialties like Plastic surgery,Musculo-skeleton surgeries, Urology surgeries,Neuro Oncology; Gynecological surgeries etc. areinvolvedintreatmentofcancer.Treatmentandcareofcritically ill patients in ICUs (Surgical, Medical andNeuroOncology)ispartofanesthesiaservices.Theseservicesareprovidedforallmajorandminorsurgical
procedures, gynecological surgeries, neurologicalsurgeries and day stay surgeries. Anesthesiadepartment also gives services for eitherprovidinganesthesia or providing only monitoring care forradiotherapy patients, patients undergoingradiologicalprocedures,invasiveproceduresinIVTCorCTscan,PETScanandMRI.Ourdepartmenttakeslead in peripheral venous access aswell as centralvenousaccessasperrequirementofthepatient.Allemergency calls for resuscitation, monitoredanesthesia and venous access are attended by thedepartmental staff. The department providesemergency services in all the wards for central /peripheral venous access of the patient as well aspulmonaryfunctiontestsmeasurement.

HPV Vaccine Project: The Gujarat Cancer andResearchInstituteishavingacollaborativeresearchagreementwithInternationalAgencyforResearchonCancer(IARC),Lyon,Franceandworkingonaprojectnamed‘Implementationplanforextendedfollow-upof theparticipantsof IARC- IndianHPVvaccinationstudy to evaluate the effectiveness of one, two andthree doses of Quadrivalent HPV vaccine inpreventingcervicalneoplasias’.

1. Rural Cancer Registry Ahmedabad DistrictProject:Ithasbeenfunctioningsincetheyear2004with the objective of assessing the magnitude andtypes of cancers in Ahmedabad rural areas and tocalculateestimateofcancerincidenceinAhmedabaddistrict.
Activities: Work of cancer data collection fromvarious sources for the year 2017 and 2018 is inprogress.
2.PopulationBasedCancerRegistryAhmedabadUrbanAgglomerationArea:Ithasbeenfunctioningsincetheyear2007withtheobjectiveofassessingthemagnitudeand typesofcancers inAhmedabadcityand to calculate estimate of cancer incidence inAhmedabadurbanarea.

Hospital Based Cancer Registry: The hospitalbasedcancerregistryhasbeenfunctioningsincelongas per the standards and norms prescribed by theNationalCancerRegistryProgramme(NCRP)oftheIndian Council of Medical Research (ICMR). FromJanuary 2014, the National Cancer RegistryProgramme (NCRP) - National Centre for DiseaseInformaticsandResearch(NCDIR)-IndianCouncilofMedical Research (ICMR) sanctioned the project‘HospitalBasedCancerRegistryandPatternsofCareandSurvivalStudiesonCancerBreast,CancerCervixand Head and Neck Cancers’ at our Institute. Thebroadobjectivesofthisstudyistoassessqualityofhospitalcareandcancerservicesincoveredareaandtodescribelengthandqualityofsurvivalinrelationtoanatomicalsite,clinicalstageandaspectsoftypesof

ClinicalServices:Inpatientservice:Departmentofpalliativemedicinehasbeenallotted10dedicatedbedsand20sharedinpatientbeds.BedOccupancyRate(BOR)is80%.In-patients fromother specialty are referred tous forpain and symptom control. On rounds,wemanagetheirsymptomsandpsychosocialissueswiththehelpofcounselorasneeded.
Outpatient service:Wesee about80-120patientsperday inourOPD.Weprovidepainandsymptommanagement. Counseling of all patients is done forpsychosoc ia l and sp i r i tua l i s sues . We docommunicationwithpatientsforbreakingbadnews,issuesofcollusion,counselingfordiseasestatusandprognosis,nursingcare,dietaryadvice,oralhygiene,etc.
Homecareservice:Wedo2-3home-visitsperdayfor5daysaweek, forhomeboundcancerpatientsresidingwithincityofAhmedabadandGandhinagar.
Hospice service: We visit our hospice centrefrequently as per requirement, to see admittedpatients who are terminally ill, requiring palliativecareandfortrainingofparamedicalstaffs.Thereare16inpatient’sbedsatHospicecentre.BORofHospiceis80%.

The new development in the department isDiagnostic Molecular Microbiology and it helps toknow complication occurring in the patients bydifferentinfectiousagentswhicharenotdiagnosedbyconventionalmethods.ThistechniquecalledRT-PCRgives the results of quantitation of the infectiousagents (number of copies) present in the patientssystem which complicates and deteriorates thetreatment.

In 2008 when rules and guidelines for biomedicalwaste had become stringent, the then Director, Dr.Pankaj Shah, had taken keen interest and activitiesrelated to this were formed and followed. Thecommitteeformonitoringactivitieswasformedandtherulesandregulationsarestrictly followedsincethen. Daily rounds are taken to check propersegregationofwaste.Therearemeetingsandtrainingwhich happen throughout the year for smoothrunningoftheactivitiesofwastemanagement.ThenewbiomedicalwastemanagementruleshavebeengivenbyandareasperstandardsofNABHandGPCBwhich are in the process of been followed by ourinstitute.
HIVcounsellingandtestingserviceswerestartedinGCRIinMarch2015.It isarequirementofahealthcare facility tohaveone integrated counsellingandtestingcentreforallgroupsofpeople.ThereispreandposttestingcounselingforHIVtestingbecausewearetesting HIV for all our admitted patients (medical,surgicalandgynaecpatients). FICTC-GCRI is run by qualified and trainednursing staff that provides HIV counselling.Laboratory technician is also trained under theregionalNACOcentreofCivilhospital.ThisFICTCsissupportedbytheNationalAIDSControlOrganization(NACO).

They were classified into Device related(CA-UTI,CA-BSI&VAP) and post-operative surgicalsiteinfections(SSI).InSSItherewasriseofinfectionby1.3%.Allthedatawerewithintherangeascomparedwiththedataoftheotherhealthcareinstitutes.Averycautiousvigilanceisbeenkeptonthequalityindicators of infections occurring in hospital likeVentilator associated pneumonias, Catheterassociated urinary tract infections, central lineassociatedbloodstreaminfectionsandsurgicalsiteinfections. It is clear from the data that stringentmeasures are taken to control infections, startingfromhandhygienetosterilizationanddisinfectioninthehospital.

knownin61responders.115patientswereawareofsexualtransmission.31patientshadgiventhehistoryuseof re-usableneedles and29patientshadgivenhistory of blood transfusion. There were only 2patients who had given history of pre-nataltransmission. Most of the patients were from Gujarat (210),fromRajasthan(26)andfromMadhyaPradesh(24). Itwasalsoobservedthat47patientswerenewlydiagnosed forHIV infection, and187patientswerealreadyHIVPositivefromoutsideoreitherwereonAntiRetroviralTherapy.Therewere28patientswhounderwentrepeatedHIVtesting.Around234patientswere referred to ART Centre for counseling andtreatmentasperNACOprotocol.

Major initiatives taken up by this department isimplementingComputerizedGrievanceandFeedbackManagement Portal to resolve patients and theirrelatives'complaintsaswellasinternalmaintenanceissues of the staff for engineering and computerservices.SystemisinteractiveandcomplainantgetsimmediateresponsethroughSMSaswellasconcerndepartment'sHead/OfficerordoctororMatronsalsogetSMSalertaboutthecomplaint.Onredressalofthe
complaints,thesameisbeingupdatedinthesystemandthewholeprocessisbeingcloselymonitoredbytheGeneralAdministrationforeffectiveuse.

ClinicalServices:CytogeneticsLabisequippedwiththestateofartsetupforresearchintheareaofclinicalcytogenetics. The division is actively engaged inresearch on various aspects of chromosomalabnormalities.Majorclinicalservicesprovidedbythedivision are for diagnosis and prognostication ofpatients with various hematological malignancies.TheservicesareprovidedtotheGCRIpatientsaswellasothercasesreferredfromvariousotherplaces.
ThedepartmentisequippedwithMetafer,AutomaticMetaphase and Interphase Scanning and CapturingSystem for Karyotyping and FISH (Fluorescence InSitu Hybridization) assays. It has a fullymotorizedepi-fluorescencemicroscope(Imager.Z2)fromZeiss,Germany with special filter sets for FISH andmulticolor FISH. It has IKAROS software for
Karyotyping and ISIS software for FISH fromMetasystems,Germany.Thesystemalsohasfacilityofautomatic spot counting for FISH analysis forhematological malignancies and solid tumor usingMetacytesoftware.Ithas4reviewstations.Moreover,the division also has 4 other systems fromMetasystems, Germany. In addition to Karyotyping,FISH (Fluorescence in situ hybridization), M-FISH(multicolor FISH) and generation of custom madeFISH probes from Bacterial artificial chromosomesalso provides precise chromosomal breakpointmapping in leukemia as well as in solid tumorpatients. For FISH automation, hybridization anddenaturation chamber (Thermobrite, Vysis, USA)helps in rapid and consistent FISH assays.DepartmenthasalsostartedsolidtumorFISHforHer-2neuforclinicaldiagnosticwork.

Medical laboratory technology is oneof the largestandmost importantprofessions thatprovidealliedhealthcare.TheTechniciansareskilledscientistwhoperformtestthataidinthediagnosisandtreatmentofdiseases.Asvitalmemberofthehealthcareteam,theyassist physician during diagnosis and treatment ofpatients.
The main objective of the course is to traintechnologists and help them to get ampleopportunities in a variety of laboratory setting. Asthere isahealthydemandofwellqualifiedmedicallaboratory technologist, they cangetplacements inhospitals and laboratory settings, public healthfacilities, industrial laboratory and in colleges.Incharge Director Dr. Shashank Pandya is theChairperson, and Dr. Priti Trivedi, Professor ofPathology is the Coordinator of the course. All thedoctors of the Pathology, Microbiology and BloodBankareinvolvedintheteachingactivities.Alljuniorandseniortechnologistsareinvolvedinthepracticaltrainingofthestudents.
Itisoneyearfulltimecourse.BScMicrobiologyandBiochemistry students are eligible for the course.Total fiftystudentsareselectedon thebasisofBScresultsandentrancetestresults.
MLT course of training is divided in to two parts:TheoryandPracticals.Total80lecturesaretakenbytheteachingstaffdoctorstocoverthesubject.Sofarpracticals are concerned, each students has twomonthsrotationineachsubspecialtylikeBloodbank,Haematology, Biochemistry, Microbiology (clinicalmicrobiology and immunology), Cytology andHistology. Each student has to work with atechnologistoneachtableinthecourseofeachsinglerotation where they are given adequate technicalknowledge.Studentsarealsoexpectedtocompleteallthepractical’sandprepareajournalwhichattheendoftherotationmustbesignedbyaseniordoctor.Theyhave to submit feedback formwhich includes theirworkefficiency,sincerityandregularity.Thisformisfilledupbytechnologistandinchargedoctoratendofeachrotation.Therearetwotypesofassessmentofthe students, day to day assessment and regularassessmentattheendoftheyearinformoftheoryandpracticalexamination.
