Every vacuum delivery is composed of five basic steps, hence the Vacca 5-Steps Technique.

DOCTOR ALDO VACCAThe late Dr Aldo Vacca (OAM, MBBS, FRCOG,

FRANZCOG, Grad Dip Ed) was renowned as a tireless

advocate and educator of vacuum-assisted delivery

(VAD), both in Australia and internationally. His efforts

are estimated to have avoided 100,000 cases of maternal

trauma in Australia alone*.

Dr Vacca identified and refined the five steps common to

all vacuum deliveries and developed the Handbook of Vacuum Delivery in Obstetric Practice.

He continued to research VAD issues throughout his life, producing many peer-reviewed

journal articles. However, Aldo was also driven to share his knowledge in a more practical sense

and so he produced a suite of teaching and training resources (which form the foundation of

the Vacca Academy) and travelled extensively to train operators in correct vacuum delivery

procedure around the world. As part of his training, Dr Vacca commissioned the development

of ‘Lucy’s Mum’, a maternal manikin that can simulate a vacuum delivery to a high degree of

realism. He also co-developed the Kiwi OmniCup vacuum device (including a reusable device for low

resource countries).

EDUCATION AND TRAINING COORDINATOR BELINDA VACCABelinda is a master trainer in vacuum-assisted delivery

(VAD) and a practicing midwife of 23 years. As the

daughter of the late Dr Aldo Vacca, Belinda has benefited

from decades of observing, conversing with, and learning

from the foremost expert in VAD during his lifetime.

Vacuum-assisted delivery has been a strong feature

throughout Belinda’s life and she is proud to carry on her father’s legacy by continuing to

deliver his high quality 5-Steps training program both in Australia and abroad through the

Vacca Academy.

SO WHAT IS VAD?

WHAT IS VACUUM-ASSISTED DELIVERY?The vacuum extractor, sometimes called by the French name ventouse, is a device which, like

the obstetric forceps, may be used to assist a woman with the delivery of her baby. Essentially,

all vacuum extractors consist of a cup, which can be attached to the baby’s scalp by suction, a

traction system and a vacuum pump that provides the suction for attachment of the cup that

allows the operator to assist the mother with the birth.

THE AIM OF VACUUM-ASSISTED DELIVERY Where the second stage of labour is delayed or there is reason to expedite the second stage,

the aim of vacuum-assisted delivery is to replicate the normal mechanism of labour, assisting

the mother’s efforts with gentle traction and flexion correction. Vacuum-assisted delivery is not

designed to overcome resistance to descent with excessive traction.

It is essential for operators to acquire an appreciation of the obstetric circumstances in which the vacuum extractor may or may not be used safely.

* THE FLEXION POINT IS A CRITICAL LANDMARK FOR VACUUM EXTRACTION; WHEN THE FLEXION POINT IS ALIGNED WITH THE AXIS OF THE PELVIS THE OPTIMAL DIAMETERS OF THE FETAL HEAD WILL BE PRESENTING FOR DELIVERY.

VACUUM-ASSISTED DELIVERY - IT ’S IN YOUR HANDS!Individuals should learn to grade instrumental procedures

according to the technical skill that is required and attempt

only those vacuum deliveries that fall within their own

level of expertise. Operators who are not familiar with the

vacuum extractor, whatever their seniority or skill with the

forceps, should begin with a program of straightforward cases such as outlet and non-rotational

VADs before attempting the more complex mid pelvic and rotational procedures. Outcomes

will be suboptimal and confidence in the technique will not be acquired if operators begin with

the more challenging procedures prematurely.

women’s health, the Royal Australian and New Zealand

College of Obstetricians and Gynaecologists (RANZCOG)

has established the Aldo Vacca Award.

DR VACCA TRIBUTEAldo has left a great legacy to the world of safer delivery

for moms and babies. As you know, his lifelong work has

been dedicated to the welfare of mothers and babies of which he has made an extensive and

long-lasting contribution.

His Handbook of Vacuum Extraction in Obstetric Practice, and his everyday hands-on

techniques for VE delivery are without question the world standard for proper vacuum delivery.

His colleagues, peers, students and friends throughout the world attest to this fact.

I feel that as his colleagues, friends, students and associates we all have a responsibility to

continue his work in educating obstetricians and clinicians to these valued techniques that he

has provided.

Aldo’s work and your support of this work have and will continue to make a significant

difference in better practical operative obstetrics. As a memorial to him, I feel that we have the

responsibility to carry forward his work and teachings.
