INVITATION & WELCOME from the 2015 Annual Session Co-Chairmen

At 211 degrees, water is HOT. At 212 degrees, it boils. And with boiling water comes steam, and steam can power a locomotive. Raising water’s temperature by one degree means the difference between some-thing that’s simply very hot and something that generates enough force to power a machine. Ideally, this metaphor should fuel our every endeavor, consistently pushing us to make the extra effort in every task we undertake. 212 degrees serves to remind us that seemingly small things can make tremendous differ-ences. This simple idea has a singular focus that is actionable, and action is necessary to produce your desired results. Bioesthetic Dentistry is the ONE EXTRA DEGREE in dentistry—transformative, powerful and actionable. OBI is an international bioesthetic dentistry foundation that teaches dental professionals that one extra de-gree. We are honored to host alumni, team members, guests and exceptional speakers from around the world. In keeping with our theme “THE EXTRA DEGREE” we met the challenge in planning an international meeting that will deliver the power to transform. We now challenge you, and we ask you to challenge yourself. Use 212 degrees as the motivation for everything you do in your office and in your lives. Push for that one extra degree, that additional effort—even if you don’t feel like it. Make a promise and do it because you said so. Start by signing up for the 2015 Annual Session. It’s time to turn up the heat!

The Academy’s purpose is to provide educational opportunities to its members and support for professional and personal growth in their endeavors to practice bioes-thetic dentistry effectively. Academy Members are dentists, specialists and techni-cians who have finished or are currently enrolled in Level III or Level IV classes. The completion of Level III or Level IV is not an end-point, but rather a beginning in our collective bioesthetic education. ABD is the needed vehicle for continual engagement in our journey to-wards understanding. The ultimate goal of the Academy is to increase the number of patients who receive the health benefits of bioesthetic dentistry.

PRO ARCH SOLUTION: Introduction to Fixed Full-Arch Implant RestorationsJohn Haupt, MDT BDT Brea, California

The process of treating edentulism with full-arch implant-supported restorations remains a significant priority for both dental clinicians and their patients. Most edentulous patients want a fixed restorative option, shorter healing time and immediate fixed solutions with minimal appointments. Surgical treatment of complete and single-arch edentulous cases require a team approach, and the treatment will vary based on factors such as anatomy, available bone and the patient’s desires. Learn how to identify and evaluate appropriate can-didates. Review the clinical documentation of this concept and techniques for other available options for full-arch replacement. We’ll also discuss proper case selection, diagnostic necessities for evaluation, and the impact of incorporating fully edentulous treatment options into a practice.

ACADEMY OF BIOESTHETIC DENTISTRY — Member Freebies & Discounts!Academy CE Day is FREE for ABD members. In addition, General Session fees are discounted—including the fees for their spouse, guests and team! If you are not already a member of the Academy of Bioesthetic Dentistry, and if you’re a graduate of Level III or Level IV, it is a great time to join! We encourage Academy members to bring a guest.

SESSION CO-CHAIRMEN INTRODUCE 212˚ THE EXTRA DEGREE John Basil, DDS Wellsburg, West Virginia Tim Dornin, DMD Moon Township, Pennsylvania

“At 211 degrees, water is hot. At 212 degrees, it boils. And with boiling water, comes steam. And with steam, you can power a train. Just one extra degree makes all the difference.”** Ideally, this metaphor should fuel our every endeavor, consistently pushing us to make the extra effort in every task we under-take. 212 degrees serves to remind us that seemingly small things can make tremendous differences. This simple idea has a singular focus that is actionable, and action is necessary to produce your desired re-sults. Bioesthetic Dentistry IS the one extra degree in dentistry—transformative, powerful and actionable. Use 212 degrees as the motivation for everything you do—in your office and in your lives. We’ll discuss how to push for that one extra degree and make that additional effort, even when you don’t feel like it. **From the book 212˚ the extra degree by Sam Parker. THE EFFECT OF SPECIALLY DESIGNED AND MANAGED OCCLUSAL DEVICES ON PATIENT SYMPTOMS AND PAIN: A COHORT STUDY

Thomas D. Dumont, DDS FICD FACD Ashland, Oregon

Wayne Sletten, DDS Albert Lea, Minnesota

L. Parnell Taylor, DDS FICD FACD Redlands, California

There is limited data regarding the benefit of using an occlusal device to help patients resolve a variety of symptoms involving temporomandibular disorder, as well as head, neck and shoulder pain. The purpose of this study was to evaluate the effect of a carefully adjusted occlusal device on 12 symptoms to deter-mine if there was enough evidence to justify a randomized control trial of this methodology. Splints were designed to ensure a stable, reproducible, mandibular position in a cohort of 157 dental patients with mixed histories of the following 12 symptoms: temporomandibular joint “pop,” “click” and lock; jaw, neck, shoulder, and mouth-opening pain; headache; earache; tinnitus; and clenching and grinding of teeth. The results showed significant improvement (P < 0.001) in 11 of the 12 symptoms.

BIOESTHETICS—TEETH TO TOES Part 1 & 2Jeff McClendon, DMD FAGD ABGD New York, NY

Mike Gunson, DDS MD Santa Barbara, California

Dara Chira, DMD Burlington, Vermont

Gayle Myers, MD Milton, Vermont

Holly Spence, PT PRC Williston, Vermont

To deliver complete, comprehensive, predictable care, it is imperative to look beyond the forms of the teeth, TMJs, face and airway when we evaluate and treat our patients with Bioesthetic Dentistry. This in depth re-view of the Bioesthetic principles and guidelines de-fined by Dr. Robert Lee examines how they apply, not only to the dentition, but as integral components in as-sessing and caring for our patients from head to toe. A wellness-based approach to diagnostics and treat-ments takes a team of dental and medical specialists along with adjunctive health care providers who share the same vision and goals. Dr. McClendon and his wellness team will share their processes and experi-ences in treating a wide range of patients with dental, health and wellness issues. Learn how each team member integrates the Bioes-thetic Principles, their professional training and treat-ment philosophies to achieve neutral posture, balance and alignment from head to toe in their mutual patients.

THE 3D EFFECT OF BODY POSTURE on Occlusion, TMJ, and Muscle Function of the Head & Neck Bill Esser, MS PT CCTT Medford, Oregon (USA)

This presentation brings together both research and clinical data to show how the position of the cranium and maxillary teeth in the sagittal, frontal, and transverse planes has a profound effect on occlusion, EMG activity of the head and neck, and the temporomandibular joint. Following this presentation dentists and or-thodontists will have a better understanding of how the head, neck, and teeth work in a dynamic balance on either side of the TMJ and why interdisciplinary treatment is vital in the treatment of patients with TMD.

•Dr. McClendon will provide a comprehensive overview of the Bioesthetic Dentistry principles and share his process and techniques for treating simple and complex restorative patients. •Oral Maxillofacial Surgeon, Dr. Gunson will review his Face, Airway, Bite principles and share how he systematically plans surgery to achieve treatment goals. •Orthodontist, Dr. Chira will share her process of diagnosing and caring for her patients by inte-grating her Roth/Williams principles and Bioes-thetic Dentistry with Complementary Alternative Medicine. •Integrative Holistic Medical Physician, Dr. Gayle Myers will introduce philosophy and principles of Biodynamic Osteopathy and share her personal and professional experience co-treating patients undergoing Bioesthetic rejuvenation. •Physical Therapist, Holly Spence will share her approach as a Postural Restoration Institute-trained physical therapist and demonstrate the value and effectiveness of PRI physical therapy in achieving neutral posture for our mutual Bioesthetic patients. From head to toe, this program will provide an understanding and appreciation for a team-based approach to delivering optimum dental health and overall wellness when caring for your patients.

THE JOY OF TEAMWORK—PRE-PROSTHETIC ORTHODONTICS Domingo Martin, DDS San Sebastian, SpainMaria Provedo, DDS San Sebastian, SpainOrthodontics today has become a purely cosmetic profession where everyone is concerned about align-ing teeth and obtaining that “beautiful smile”. However, when one looks beyond the surface, esthetics with-out function is of little value. Those of us trained in the Roth and OBI philosophies know this very well. In many instances, we realize we cannot achieve our most important goals by doing only orthodontics or only OBI. Because of tooth wear, missing teeth, periodontal disease and many other dental problems, the teamwork of Roth and OBI doctors is so important since both have the same functional occlusion goals, and we can work together to achieve them. In this presentation, we will share many of our cases to demon-strate our diagnostic and treatment protocol, and show you that working together with the same goals is a tremendous joy.

SITE PREPARATIONS FOR DENTAL IMPLANTS Mark Silberg, DMD McKees Rocks, Pennsylvania

The clinician today is faced with countless choices that need to be made prior to being able to place a dental implant, These choices range from when and how to extract a tooth, whether or not to graft a socket, how to graft the socket, what materials to use to graft a socket and how to predict whether or not a socket graft will work. If a tooth has previously been extracted and the residual ridge is insufficient for the placement of an endosseous implant, what choices are possible for augmenting the ridge? This presentation will focus on the regeneration of the atrophic alveolar ridge. A brief history of techniques will lead up to cases that have been treated with recombinant BMP and frozen human stem cells.

FROM SIMPLE TO COMPLEX! Facially Generated Treatment PlanningAlejandro James, DDS MSD FID León, Gto, Mexico

The purpose of this presentation is to share how different ideas and philosophies of work can all blend to-gether in order to achieve the principles and a super outcome! Topics such as adhesion, modern conser-vative tooth prepping, updates in composites and ceramics, and soft tissue management will be covered.

Nashville, Tennessee The talk in hygiene circles these days is about biofilms, inflammation, systemic disease impacting periodon-tal disease, and vice versa. Learn about a different model of hygiene practice that will allow you to fully partner with patients while providing the framework for their own engagement in the process of moving to-wards health. We know a healthy periodontal status leads to a healthier dentition leads to a healthier oc-clusion. It all begins with the foundation!

LIVING A LIFE OF ABUNDANCE: Smart Women Finish “Wealthy”Brenda Penwell, BS (President, Schuster’s Performance Coach, Inc.) Scottsdale, Arizona

While we don’t define “wealthy” or “abundance” ONLY in terms of finances, it’s certainly the primary crite-ria that comes to most people’s minds. Women have always been an integral part of our nation’s history. Their participation in the workforce is just one contribution to our country’s success. But, working women are busy women who also often continue as their families primary caregivers. Although women in the USA generally live longer than men, they often retire with smaller pensions and fewer assets. We all want to live with dignity and security in our later years, but that requires planning. Married or single, there are certain financial security issues that women face as they age. However old we are, there is plenty to do ahead.

DIET, DETOX AND TMJ: Addressing Inflammation to Promote HealingCarol Bennett, RD CCN Colorado Springs, CO Do you or your patients experience achy joints, headaches, irritability, brain fog, or fatigue? If so, the foods you eat every day may be to blame. Could the bagels you eat for breakfast bring on your afternoon head-ache? Could the milk in your coffee cause joint pain, brain fog or eczema? There is a powerful connec-tion between what you eat and how you feel. Foods can cause inflammation, which can aggravate many health problems, including TMJ. If you or your patient are suffering from inflammation or TMJ, come and learn more. We will discuss the tools for assessing overall nutrition needs and dietary triggers, how to re-move toxins that cause inflammation, and how to identify dietary and lifestyle choices to support health and wellness. This program will outline the nutritional protocols necessary for success in treating TMJ patients.
