A systematic process to maintain and improve the quality, effectiveness and safety of casualty care within Industry.

Governance should form a fundamental part of the safety, risk management and quality improvement plans for the organisation. This document summarises the levels of Clinical Governance provided by The ATACC Group.

Clinical Governance

INTRODUCTION

Effective Clinical Governance is an active safety & quality improvement process which is an essential part of risk management for any organisation. The ATACC Group has extensive experience in establishing such processes within a diverse range of industries and emergency services, as well as the NHS.

Clinical Governance is embedded in all aspects of first aid and casualty care and ensures that appropriate risk assessments are made, training needs are defined, relevant and delivered to the required standards with equipment that is fit for purpose.

It also involves ongoing audit processes to monitor and review the safety and effectiveness of policies, training and equipment. This includes a review of every casualty intervention to help define further training needs and any changes needed in the future strategic direction of the company.

Clinical Governance also provides a proactive and essential internal route for reviewing accidents, incidents or even deaths, that are likely to involve HSE investigation. Identified learning can then be fed back through the process to the relevant parties or back to senior management teams for consideration of any recommended changes.

Implemented correctly, Clinical Governance can positively impact on all areas of an organisation not only achieving its aim of continued improvement in casualty care, but also by greatly increasing the confidence of your non-medical professionals attending a casualty with injuries or life-threatening problems.

For Industry, effective Clinical Governance can bring improvements in staff engagement, morale and even success in securing new contracts. It improves public perception of the organisation, as a responsible and caring employer within a community that is demonstrating the required due diligence.

In summary Clinical Governance should not be viewed as restrictive or binding, but more as a process that empowers the organisation to develop and grow on a robust foundation that ultimately benefits colleagues, communities and customers they serve.

ADOPTING CLINICAL GOVERNANCE WITH THE ATACC GROUP

The first stage is to assess your current position and case of need. We can do this prior to signing contracts, but this can incur a charge, which is included in the annual fee once the contract is signed.

We will provide you with a carefully structured questionnaire which will allow us to assess your current Clinical Governance status. Many processes may already be in place, but under a different title. For some very large or high risk organisations it may be necessary to bring in one of our specialists to conduct a more extensive assessment.

We base our assessments on a standardised framework, much like other quality assessors e.g. HSE, Care Quality Commission (CQC) etc but it will be tailored to meet the specific needs of each individual industry and any specific risks that you face. Many Clinical Governance standards in the NHS are not directly applicable and relevant to industry, whereas others are essential and we will consider these factors on your behalf.

Based on this review The ATACC Group will produce a clinical governance report and a working plan to guide the organisation over the following 12 months. This plan will be constantly updated and reviewed every 12 months as a minimum.

Our Standard level Clinical Governance is designed for organisations providing casualty care or medical interventions on a regular basis.

Standard Clinical Governance includes the following:

REPORT ON CURRENT POSITION OF ORGANISATION’S CLINICAL GOVERNANCE SYSTEMS

The first priority once we take on a new customer is to assess your current position and case of need. This status assessment will be conducted through a combination of a Governance questionnaire and direct questioning by our Governance Lead. From this a short report will be produced by the Lead in conjunction with the Medical Director.

The questionnaire and process is based on a standardised framework, much like other quality assessors use e.g HSE, Care Quality Commission etc but it can be tailored to meet your specific needs, as many standards in the NHS and industry are not applicable, whereas others are essential.

Based on this review The ATACC Group governance report will define a working plan to guide the organisation over the following months.

This will include a training log, incident report and risk register specific for clinical work and will all be discussed with the customer at the first regular Governance meetings.

DEDICATED MEDICAL ADVISOR FOR THE ORGANISATION

In medical terms the most senior doctors within the NHS and healthcare are consultants or specialists. These will typically have at least 10 years experience and will have specialised in key areas of medicine. We have a large pool of such individuals, who all work within the fields of medicine relevant to emergency care e.g Emergency medicine, critical care, pre-hospital care or anaesthetics. They would be considered ‘experts’ within their fields and typically have a national or international reputation.

The ATACC Group will assign such an individual to your organisation as a dedicated Medical Advisor, but you will also have access to The ATACC Group Medical Director and Medical Advisory Group if required.

DEDICATED CLINICAL GOVERNANCE ADVISOR

We will provide a dedicated Clinical Governance Lead by email contact. For all governance and clinical queries we aim to provide a response email back within definitive guidance, or a holding measure within 48hrs.

ADVICE ON MEDICAL LIABILITY

This service will provide guidance and advice on any risks to your organisation in terms of medical liability with your current practice, that you may not be aware of e.g administration of drugs without licence or training e.g oxygen, epipen, entonox etc.

Alternatively, for more advance trained staff this could include drug storage policies, lack of report forms or inappropriate care etc and the risks that this presents. Many of these can be placed on the risk register and managed over following months, whereas others must be stopped immediately until addressed.

URGENT CLINICAL ADVICE

On a day to day basis, advice will be available through the Account Manager or Governance lead. This is a named individual that provides reliable contact for your service within normal working hours.

Many queries we will be able to answer immediately, but for anything that requires medical expertise, further consideration or research we will RAG rate the enquiry and respond within the given times below:

If our response is going to be delayed or require further research or investigation then we will send a holding update message within the RAG rated timescale.

RED: within 48hrs

AMBER: within a week

GREEN: within a month

Clinical Governance - Standard

Standard Clinical Governance includes the following:

QUARTERLY MEDICAL ADVISOR CLINICAL GOVERNANCE MEETINGS:

Whilst there will be regular contact with your Account Manager and you will always have day to day access to medical advice and guidance, the formal quarterly meetings are designed to bring everything together in series of face to face reporting about recent issues, incidents and how the Clinical Governance progress is progressing.

At these meetings the customer will bring the necessary data, prepared under the guidance of the account manager, so that best use can be made of the time, to continue the process of improvement and oversight of the service.

Key issues that may arise include: reviews of clinical incidents, serious injuries, equipment failures, identified gaps in governance and the current progress in addressing them. Patient report forms will be reviewed and checked and identified equipment of training needs will be reported with an action plan for improvement. Finally, other issues such as data protection, new potential risks or business or changing staff responsibilities can all be discussed.

ACCESS TO SUPPORT FROM OUR MULTI-DISCIPLINARY MEDICAL ADVISORY GROUP

We include in the Governance package a named Medical Adviser for regular guidance and advice. However this individual is also part of our International Medical Advisory Group, which is a carefully selected expert group of senior clinicians, that can be drawn upon for more challenging decisions or guidance. This provides organisations with a much broader base of expert knowledge.

PROVIDE WRITTEN OR ORAL STATEMENT TO COURT IN THE EVENT OF CRIMINAL OR CIVIL PROSECUTION

As above, we can provide an official high level medical statement or comment regarding any potential litigation or clinical error that has occurred. Further medico-legal services are available on request by separate arrangement.

PROVIDE ADVICE AND SUPPORT IN CASES OF MEDICAL NEGLIGENCE OR COMPLAINTS

We do not offer an inclusive medical defence. However, we do include comment and advice on any pending litigation or legal question. Based on any further legal proceedings we can offer a comprehensive range of health care professionals as potential expert witnesses for the organisation to consider using for their representation.

EMERGENCY POINT OF CONTACT FOR OPERATIONAL MEDICAL ADVICE

This will always go through the Account Manager/Clinical Governance Lead who will aim to get them a response to their question within 48hrs. However, for more serious or urgent issues, we can typically get an initial response within 24hrs.

Clinical Governance - Enhanced

Enhanced Clinical Governance includes the following:

We would recommend our Enhanced support for high risk organisations where a greater level of medical oversight and input will be required on a frequent basis, either through visits, direct telephone advice or modifications to training.

Our Clinical Governance Lead and a named Medical Director, who will be a senior clinician, with relevant clinical and operational experience, will be in regular contact with your team on a monthly basis.

Patient Report Forms (PRFs), incidents, feedback and equipment provision will be reviewed monthly and this will all be fed back to the Clinical Governance meetings.

Clinical Governance meetings will be SIX times per year (typically every other month) and will be attended by both the Medical Director and our Clinical Governance Lead.

This Level is not rigidly defined, but maintains a degree of flexibility, which is ideal for organisations that are significantly developing their casualty care provision or considering ultimately achieving Care Quality Commission (CQC) accreditation or similar professional standards.

Our Enhanced level of Clinical Governance is a supplemented version of the standard package, with increased contact time, scrutiny and support.

Clinical Governance - Bespoke

Bedspoke Clinical Governance includes the following:

This package is typically reserved for organisations that have made a commitment to achieving or are already accredited by the Care Quality Commission (CQC) or equivalent bodies.

This will typically involve a dedicated Medical Director who will work within your organisation on a regular basis, alongside a Governance Lead to evaluate your specific organisational requirements. Based on their assessment and any relevant national standards we will produce a detailed work plan for the implementation of all levels of clinical governance, which may even include medicines management policies, appraisal plans and all relevant procedures.

The Medical Director will provide day to day advice and direct communication with the care providers, trainers and managers. The support may include interaction and representation in meetings with other agencies such as hospitals and ambulance services.

Within this package we can potentially provide a diverse range of services including occupational health screening, nursing, therapist, specialist rescue and pharmaceutical advice and support.

Other services include support for procurement of equipment, safety and risk assessments and safe storage of medicines and medical equipment.

Some agencies will require provision of 24hr emergency contact and even potentially emergency call out and Medical Response by specialist rescue experts, paramedics or one of our doctors.

In summary, the bespoke Clinical Governance package offers a complete and comprehensive range of services that will suit organisations requiring advanced levels of provision typically seen in ambulance services, hospitals, specialist health services and casualty care leading organisations.

Our Bespoke Clinical Governance option is our most comprehensive level of provision.
