FeeS There are usually four separate fees for cosmetic surgery: the surgeon’s fee, operating room

cost, anesthesia, and implant costs (if applicable). If the surgery is performed in the doctor’s own facility, the

operating room cost may be included. Generally, payment is made prior to surgery. Fees vary according to the

complexity of the operation, the location at which the surgery is performed and the type of anesthetic administered.

Choose your plastic surgeon based on qualifications, not price.

INSurANce As a rule, cosmetic plastic surgery is not covered by insurance. But when a cosmetic

procedure corrects a functional problem, there may be an exception. For example, a breast reduction can correct

a chronic backache, or an upper eye lift can improve vision impaired by eyelid hooding. In such cases, contact

your insurance company prior to surgery to obtain a predetermination of benefits.

SurgIcAl FAcIlITy Cosmetic surgery can be safely performed in a hospital, a

surgicenter or an accredited office-based surgical facility. If you select a member of the American Society

for Aesthetic Plastic Surgery, you can be sure that surgeon is operating in an accredited, state-licensed or

Medicare-certified facility. Even when you undergo a nonsurgical procedure, be sure you are in a facility

equipped for emergencies.

Before granting privileges, hospital review committees evaluate training and competency for specific

procedures. Even if you are having surgery in your doctor’s office, hospital privileges to perform the

procedure you will undergo is an assurance that you are in expert hands.

ANeSTHeSIA Some procedures can be performed with a local anesthetic to numb the treated area,

together with a sedative that induces relaxation. Occasionally a sedative may also be used to induce relaxation.

Other surgical procedures may require general anesthesia, providing sleep throughout the procedure. If general

anesthesia is used, it must be administered by a Board-certified anesthesiologist or certified registered

nurse anesthetist.

Surgery rISkS Cosmetic plastic surgery molds and reshapes living tissue, and the results are

not absolutely predictable. No surgeon can offer risk-free surgery or guarantee a perfect result.

Pre-AND POST-TreATMeNT INSTrucTIONS Your surgeon may request

medical clearance from your internist, heart doctor or anyone else treating you for a serious medical condition. Once

you’re cleared, your plastic surgeon will provide instructions that prepare you for a safe surgery and recovery. You will

probably be asked to avoid smoking, aspirin and certain other medications. After the procedure, you can expect activity

restrictions from several days to several weeks. Because it takes time for visible signs of healing to subside, plan significant

business and social activities after you’ve had sufficient time to heal.

This certification guarantees that

your surgeon has completed a

minimum of five years of surgical

training following medical school,

including an approved residency

program in plastic surgery, and has

passed comprehensive oral and

written exams. ABPS certification

qualifies your surgeon to perform

reconstructive and cosmetic plastic

surgery of the face and entire body.

Do not be confused by other

official-sounding boards and

certifications. The ABPS is

recognized by the American Board

of Medical Specialties (ABMS),

which has approved medical

specialty boards since 1934.

There is no ABMS-recognized

certifying board with “cosmetic

surgery” in its name.

*If you live outside the United States, check your surgeon’s affiliation with a member society of the International Confederation for Plastic, Reconstructive and Aesthetic Surgery (IPRAS), or the International Society of Aesthetic Plastic Surgery (ISAPS). Plastic surgeons in Canada are certified by the Royal College of Physicians and Surgeons of Canada.

The single most important factor

in your cosmetic outcome is the

surgeon you select. Choosing an

aesthetic plastic surgeon is the

smart choice. Members of the

YOUR skin 17

microdErmABrASioN iNjEcTABlESIn the last ten years there’s been a huge increase in FDA-approved products

available for office treatment of facial changes due to aging. In the right

hands, improvement can be dramatic. Your plastic surgeon is now equipped

with treatments to quickly soften creases, restore more youthful contours,

and freshen your appearance. These treatments are no substitute for the

longer-lasting more comprehensive corrections possible with surgery, but

are frequently used to complement surgical results and, in many situations,

are a reasonable alternative. Although less invasive than surgery, their

effects are generally temporary. Your result will be determined by the

product used and the training, skill, judgment, and experience of the

professional administering your treatment. Two categories of anti-aging

injectables are neuromodulators and soft tissue fillers.

NeurOMODulATOrSThese products target ‘dynamic’ creases, those wrinkles that occur with

different facial expressions. By relaxing the specific small muscles you use in

creating facial expressions, neuromodulators, such as Botox®, stop you from

wrinkling the overlying skin, which in turn prevents you from deepening

facial creases in the treated area. Neuromodulators can help soften wrinkle

areas such as ‘crow’s feet’ or forehead creases. Effects last for three months

or more. Brands introduced in the United States are variations of the

botulinum toxin A molecule and include Botox®, Dysport®, and Xeomin®.

16 YOUR skin

Microdermabrasion is superficially abrasive skin exfoliation that gently removes dead cells from the outside of your skin to stimulate cell turnover and increase blood flow to the skin surface. The microdermabrasion machines use either a spray of micro-crystals or a crystal-free hand-held device to polish the skin, sloughing off the top layer and leaving the complexion with a healthy glow. Microdermabrasion repairs sun damage and improves skin texture on the face, neck, décolletage, and hands.

Microdermabrasion is usually performed in a series of treatments to increase benefits and is frequently combined with a light chemical peel to further enhance results. Although your skin may be slightly pink following treatment, you will be able to immediately return to work and social activities.

AT-HOMe SkIN TreATMeNTSThere are home skin treatments that improve skin texture and decrease sun damage, fine lines, wrinkles, and also fight acne.

Tretinoin, a topical form of vitamin A, marketed as retin-A® or Renova®, is frequently prescribed to treat acne. It also helps to exfoliate the skin, unclog pores and stimulate cell turnover, thickening the dermis and promoting collagen production. It is also beneficial to use tretinoin to prep the skin before having a peel or microdermabrasion.

Hydroquinone, a lightening agent, improves mild to moderate hyperpigmentation. The effects of hydroquinone may be enhanced when used with a tretinoin regimen.

Sunscreens, especially those with zinc oxide or titanium dioxide, are important in maintaining healthy skin and preventing future damage from ultraviolet rays. Use sun protection daily, rain or shine, not just when you know you will be exposed.

1. A glycolic acid peel is a light treatment that gently penetrates the uppermost layers

of your skin to reduce the visible effects of aging and sun damage.

2. Microdermabrasion is a popular technique that uses a stream of micro-crystals

and suction to gently “polish” your skin, leaving your complexion healthy and glowing.

YOUR FACE 5

Today’s facelift techniques are designed to tighten deep tissues, firm up excess loose skin and address volume changes that occur with aging. The goal of a facelift is to deliver your most youthful and natural look. Heredity, skin texture and the underlying bone structure of your face all play a role in how many years your facelift will remove and, to some extent, how long these changes will last.

Facelifts are not one size fits all; the procedure is dictated by your facial characteristics. In some, the deeper facial tissues may be repositioned or tightened to restore a more youthful contour and shape. In others, removal or addition of fat or other soft-tissue fillers may be necessary to achieve optimal results.

Basic to all facelifts is that excess skin is removed to yield a smoother, firmer facial appearance. One important goal of every facelift is to hide incisions, so that following surgery you can easily camouflage them with hair or makeup. The placement and length of incisions vary, depending on the facelift technique that best suits you. A facelift incision typically starts in the front of the ear, wrapping around the earlobe, with variations going up into the hairline of the temple, or behind the ear. Your doctor will help you determine the best technique to meet your individual needs.

Facelifts can also improve your neck, with your surgeon treating cord-like neck bands through a small incision placed under your chin, and through the same incision remove excess fat and tighten neck muscles. Excess neck fat can also be removed through a very small incision (or incisions) using liposuction.

After your facelift, you may feel tightness or numbness in your face and neck. Some skin discoloration is common and you can expect some swelling, which will resolve shortly. Your skin will be sensitive, so you will need to be extra careful to apply sun block products daily. You can apply makeup soon after surgery and return to work

within a few weeks.

FAcEliFT / rHyTIDecTOMy

1. A facelift corrects visible signs of aging such as deep cheek folds, jowls and loose skin on the

front and sides of your neck.

2. Facelift incisions may be placed within the hairline and within natural contours in front of

and behind the ears, as shown.

3. Skin is then pulled back and the incision closed.

4. Modified incisions include variations of the ‘short scar,’ with shorter incisions around the ear.

5. After surgery, the skin on your face and neck will look smoother, firmer and fresher.

Cosmetic medical procedures are a blend of both art and science. The American Society for Aesthetic Plastic Surgery has a number of member surgeons who are weekend painters and sculptors, focusing on the human face and form. Plastic surgeons as a whole have a strong feeling for beauty. Their goal is to enhance the harmony of your facial features while preserving your unique facial character. Facial plastic surgery can lift your brow, open up your eyes, reshape your nose, reduce prominent ears, contour your cheeks, shape your chin and firm up your neck and jawline. Sometimes enhancing a single facial feature brings your whole face in balance, improving overall appearance, making you appear more youthful and increasing your self-confidence.

Your face is the first part of you that people notice, and a pleasing countenance will help you feel confident and assured.

PROCEDURAL STEPSSTEP 1 / ANESTHESIA Medications are administered for your comfort during the surgical procedure. Your surgeon will recommend whether general anesthesia or intravenous sedation is the best option for you.

STEP 2 / THE TECHNIQUES Most facelift techniques focus on the lower facial areas, such as the jawline, jowls, and cheeks. A facelift can also focus on the midface or the fore-head. In some patients, deeper facial tissues may be repositioned or tightened to restore a more youthful contour. In others, removal or addition of fat or other soft tissue fillers may be necessary to achieve the best results. Today, many different techniques exist with outcomes that are consistently reliable and safe and produce durable results, which your surgeon will discuss with you. Your incisions will depend on the area of the face that is targeted and the amount of change you want.

A facelift corrects visible signs of aging such as deep cheek folds, jowls, and loose skin on the front and sides of your neck.

Back Cover

PREPARING FOR SURGERYPreparation for surgery begins after your plastic surgeon examines you and discusses the details of the procedure. Your plastic surgeon will provide thorough preoperative instructions and may recommend that you achieve certain weight goals before surgery or make other lifestyle changes to help improve your results and minimize possible complications. Regardless of the type of surgery to be performed, drinking lots of water before and after is very important for safe recovery and optimal outcomes.

Before your procedure, your surgeon will discuss with you:

• The impact that smoking and tobacco products have on your surgery. Your surgeon may recommend that you be tobacco and tobacco product free, including nicotine patches, gum, and cigarettes.

• The importance of avoiding taking aspirin, nonsteroidal anti-inflammatory medications (NSAIDS: Advil, Motrin, Aleve), and vitamins or homeopathic regimens prior to the surgery, since these can increase bleeding.

• Recent facial procedures you may have had, such as lasers, injectables, peels, and topical treatments, because these can effect the evaluation and timing of your surgery.

Your facelift may be performed in a hospital or an outpatient surgical facility. If it is done on an outpatient basis, arrange for someone to drive you home after surgery and stay with you for at least the first night following your procedure—unless you and your surgeon have decided on other postoperative recovery options.

YOU... THE WAY YOU WANT TO BEWhether you’ve always wanted to enhance or improve certain features, or are considering aesthetic plastic surgery to help your outside match the way you feel inside, a board certified aesthetic plastic surgeon can help. Through a personal consultation, an aesthetic plastic surgeon can learn about your goals and provide insight and options for achieving the results you desire. With the help of a board certified aesthetic plastic surgeon, you can be the best YOU possible.

BREAST ENHANCEMENTBody image can be important for many women, but heredity, pregnancy, weight gain or loss, and even aging can result in changes to the size, shape and overall appearance of your breasts. These changes can affect your body image and your self-confidence.

Breasts are mainly composed of soft tissue, not muscle. Therefore, fitness cannot change the shape of your breasts. Only breast enhancement can permanently reshape your breasts. Breast enhancement procedures can dramatically improve the size and shape of your breasts, bringing your body into better proportion.

Among the results for many women are a more positive body image and enhanced self-confidence.

WHEN TO CONSIDER BREAST ENHANCEMENT

• If you want a better proportioned or more appealing figure

• If you wish your clothes fit better

• When pregnancy, weight loss or aging has affected the size and shape of your breasts

• To restore symmetry if one of your breasts is smaller than the other

TYPES OF ENHANCEMENTSBREAST AUGMENTATION, also known as augmentation mammaplasty, breast enlargement or breast implants, increases or restores breast size using silicone gel implants, saline implants or in some cases, fat transfer. One of the most popular and frequently performed aesthetic surgery procedures, breast augmentation has a long and successful track record in satisfying women who wish to enhance, regain or restore balance to their figures.

BREAST LIFT, also known as mastopexy and breast lift surgery, a breast lift addresses sagging and uneven breasts, decreased breast volume and drooping nipples and stretched areolas (the darker area surrounding the nipples), recreating a youthful shape and lift. If there is too little or too much breast volume, a breast augmentation or breast reduction might be recommended.

BREAST REDUCTION, also known as reduction mammoplasty, reduces the size of your breasts and reshapes them so that they are proportionate to the rest of your body and are no longer a source of physical discomfort.

BREAST RECONSTRUCTION, also known as reconstructive mammaplasty or postmastectomy surgery, includes a variety of procedures performed to restore the form and shape of the breast, following mastectomy or lumpectomy surgery. Discussing your cancer surgery with a plastic surgeon before undergoing mastectomy is crucial, because the proposed cancer removal surgery may significantly affect the choices and the results of any type of breast reconstruction.

BREAST REVISION, also known as breast implant replacement, involves removal and/or replacement of saline or silicone breast implants, is performed to change the size or type of your implants and to correct any irregularities from your previous breast operation(s). The overall goal is to restore a youthful breast contour and appearance.

TYPES OF BREAST IMPLANTSSALINE FILLED BREAST IMPLANTS are filled with sterile salt water. They may be pre-filled to a predetermined size, or they may be filled at the time of surgery to allow for minor modifications in implant size.

SILICONE FILLED BREAST IMPLANTS are filled with soft, elastic gel and can come in a variety of shapes. All silicone breast implants are pre-filled and may require a longer incision for implant placement.

COHESIVE GEL BREAST IMPLANTS, also known as “gummy bear” or “form stable” implants, are filled with a cohesive gel, made of crosslinked molecules of silicone, which makes it a bit thicker and firmer than traditional silicone implants and enables it to maintain its shape. Typically, these implants are inserted through an inframammary incision, just under the breast.

FAT TRANSFER removes fat from an area of your body where there are abundant fat cells (thighs, abdomen, and hip) by liposuction, and then through a process of careful preparation and refinement, the fat cells are re-injected into the breast. This procedure is currently considered experimental, due to a limited history of data, but the process has shown promise. Your plastic surgeon can tell you more about its benefits and limitations.

THE DOCTOR YOU CHOOSEMake certain your surgeon in the United States is certified by the American Board of Plastic Surgery (ABPS).* This certification guarantees that your surgeon has completed a minimum of five years of surgical training following medical school, including an approved residency program in plastic surgery, and has passed comprehensive oral and written exams. ABPS certification qualifies your surgeon to perform reconstructive and cosmetic plastic surgery of the face and entire body.

Do not be confused by other official sounding boards and certifications.

The ABPS is recognized by the American Board of Medical Specialties (ABMS), which has approved medical specialty boards since 1934. There is no ABMS-recognized certifying board with “cosmetic surgery” in its name.

By choosing a member of the American Society for Aesthetic Plastic Surgery or the American Society of Plastic Surgeons, you can be assured that your surgeon is certified by the ABPS.

SURGERY, RECOVERY & RESULTSYou may be asked to undergo a medical evaluation or to have lab tests prior to surgery. In some instances your plastic surgeon may recommend that you lose weight, engage in exercises to strengthen your abdominal muscles, or make other lifestyle changes before surgery to help improve your results and minimize possible complications. Your surgeon will discuss with you the impact that smoking and tobacco products have on your surgery, as well as the importance of avoiding aspirin, nonsteroidal anti-inflammatory medications (NSAIDS: Advil, Motrin, Aleve), and vitamins or homeopathic regimens before surgery, since these products can increase bleeding.

Drinking lots of water before and after the procedure is essential for safe recovery and optimal outcomes. The type of anesthesia recommended for you will be explained prior to your procedure.

Breast enhancement surgery is usually performed on an outpatient basis or with a brief hospital stay. You should arrange for someone to drive you home and to stay with you for the next day or two.

Your surgeon will provide complete instructions on preoperative and postoperative care. Read the instructions carefully to ensure that you fully understand them and know what to expect before, during, and after your procedure.

Scars are a normal outcome of surgery. In many cases, scars can be placed inconspicuously within natural contours and creases of the breast or where a modest bra can conceal the scars.

The results of breast enhancement can be long- lasting. Make sure to keep in contact with your plastic surgeon on a yearly basis.

* If you live outside the United States, check your surgeon’s affiliation with a member society of the International Confederation for Plastic, Reconstructive and Aesthetic Surgery (IPRAS), or the International Society of Aesthetic Plastic Surgery (ISAPS). Plastic surgeons in Canada are certified by the Royal College of Physicians and Surgeons of Canada. Policies and Procedures

PRE-SURGERY INSTRUCTIONS: LIPOSUCTION

Patient Name Date

Surgical Facility Surgery Date

Arrival Time

A successful surgery requires a partnership between you and ,MD

The following instructions are essential to a safe experience and good outcome. Use this as a checklist as you approach your surgery date. If you are unable to comply with these instructions, you must notify our office as soon as possible. As a result, your surgery may have to be postponed or delayed, at the judgment of , MDThis is essential to you health and safety.

THREE WEEKS OR MORE BEFORE SURGERY

There may be several weeks between your decision to have surgery and your actual surgical date. During that time, there are several important considerations:

Practice proper fitness: You need not engage in an aggressive or new fitness routine; however practicing good fitness habits is an important factor in your overall health and well-being. Stretching exercises and low-weight strength training now, can help to enhance your posture and your strength in the weeks following surgery.

Good nutrition. Eat well during the weeks prior to surgery. Crash dieting, over-eating or high alcohol intake can greatly affect your overall health and well-being. A healthy, balanced diet is essential. Also, begin taking the following supplements daily:

Stop smoking. Smoking can greatly impair your ability to heal. You must be nicotine and smoke-free for at least 4 weeks prior to surgery. You must also be free of any nicotine patch or nicotine-based products for a minimum of 4 weeks prior to surgery.

Lead a healthy lifestyle. In the weeks prior to surgery maintain the best of health and hygiene. A lingering cold, virus or other illness can result in your surgery being rescheduled. Make certain to address any illness immediately, and advise our office of any serious illness or change in your health.

Prepare and plan. Schedule any time off of work, and any support you will need at home in the days following surgery, including housework, childcare, shopping and driving. Make certain a responsible adult is enlisted and confirmed to drive you to and from surgery, and that one is confirmed available to stay with you around the clock for 24 hours, at least, following surgery.

Pre-operative testing. Make certain to schedule all of the pre-operative testing and clearance you have been given. Refer to the Pre-surgical Lab and Testing Orders form.

Make certain all test results are received by Dr. as required

Relax and enjoy life. Stress and anxiety over life’s daily events, and even your planned surgery can affect you. While some anxiety is common, any serious stress, or distress over the thought of surgery is something you must discuss with our office. We are here to support you and answer all of your questions. We want your decision to be one made with confidence.

Please see following categories for specific descriptions of benefits

EDUCATIONThe Aesthetic Meeting. Long considered the “gold standard’ in aesthetic education, early bird registration discounts for The Aesthetic Meeting result in substantial cost-saving for this premier educational event. (Discounts for Active Members and Candidates for Membership, complimentary registration for Resident Program.)

Aesthetic Surgery Journal. Indexed, peer-reviewed, and with an Impact Factor of 2.034, ASJ is essential for keeping up-to-date on the latest aesthetic techniques and research. While a Journal subscription costs approximately $300 a year, access to ASJ is complimentary for those engaged with The Aesthetic Society (Active Members and Candidates for Membership receive both print and online access; those enrolled in the Resident Program receive online access.)

RADAR Resource. With the RADAR Resource medical library and private professional netwwork, access is available to all issues of Aesthetic Surgery Journal from 1995-present. Annotate articles with text or voice notes, add links and videos, and create your own customizable aesthetic resource library. Engage in discussions and share content with fellow surgeons, upload and annotate your personal PDF documents, and even earn CME Search credits, simply by using RADAR for research (CME Search subscriptions sold separately.) Through RADAR you can also access Aesthetic Society News, practice management resources, selected Aesthetic Meeting videos, Shaped Implant and Premiere Global Hot Topics education, and much, much more. (Complimentary to Active Members, Candidates for Membership, and for those enrolled in the Resident Program.)

Webinars. Informative webinars are offered regularly, free of charge to Active Members, Candidates for Membership, and for those enrolled in the Resident Program, on a variety of timely subjects.

Symposia. The Aesthetic Society is continually creating new and differentiated learning modules, including The Aesthetic Cruise and the ASAPS Las Vegas Aesthetic Symposium. Early bird discounts for those associated with The Aesthetic Society apply.

MARKETINGSmart Beauty Guide. Developed exclusively for Active Members of The Aesthetic Society, this new consumer education and marketing campaign was designed to ensure your practice thrives by attracting new patients to your practice. In addition to the new, interactive website, SmartBeautyGuide.com, there are also complimentary ad templates for print and web advertising, complimentary procedural videos, which the surgeon can use as his or her own, and beautiful new brochures and marketing materials, available for purchase.

Referral Sources. Exclusively for Aesthetic Society Active Members, membership includes access to three potential Referral Sources: Select-a-Surgeon, Enhanced Practice Profile (EPP) Web Pages, and Ask-a-Surgeon, located on www.SmartBeautyGuide.com; cumulatively these produce approximately 40,000 referrals per month.

ASAPS Enhanced Practice Profile (EPP) with Video. Exclusively for Active Members for an additional fee, EPPs are a fully optimized service for impactful search engine results. EPPs consistently get first page rankings on Google, and in many cases, are listed above and/or along with your practice website. With an Enhanced Practice Profile, simply by linking to their EPPs, Active Members can become a Featured Local Surgeon on the website, with the Select-a-Surgeon feature removed.

Aesthetic Society Product Discounts. From our Smart Beauty Guide and accompanying procedure brochures, exclusively for members, to our other marketing and educational materials, those engaged with The Aesthetic Society receive substantial discounts on products.
