WHAT IS CAMP ELEVATE? Camp Elevate is a fun-packed spiritual retreat for children (grades 1-5/6) and youth (grades 6-12.) This year Teen Camp is July 13-16, staying three nights, and Kid Camp is July 16-18, staying two nights. Our goal at camp is to help children and youth grow in their relaonship with God and with each other. Each year we choose a different teaching theme and focus verse. We explore that theme in our large group worship, small group teaching and cabin devoons. As we study and spend me with each other we are drawn closer to God and learn the importance of developing Chrisan life skills and doing life together in a community of like-minded peers. And...we just plain have fun! Camp is located at Eagle Sky Camp, Piedmont, MO. Some of the things we will enjoy: Swimming, tubing, sliding, climbing, human foosball, giant jenga, field games, water blobs, water/sand volleyball and basketball; tubing, gaga ball, giant connect 4, and more! Kayak, canoes and paddle boards! There is a ropes for teens for an addional fee. What are the accommodaons like? We will be staying in large dorms. Each dorm sleeps around 45 (with social distancing) and is self-contained with bathrooms, showers, spaces to hang out, and a cafeteria and dining area. Each bunk is a twin-sized memory foam maress. Each dorm has outdoor paos overlooking the lake with an island and large deck. Social Distancing: Worship, Cafeteria, and bunk areas WILL accommo- date social distancing. Extra hand-wash staons will be set up and USED. Activity Permission and Authorization to Consent to Treatment of Child

As the parent or legal guardian of the child named above, I authorize participation of my child in the activity listed

above. I authorize staff members and authorized volunteers to consent to any examination, anesthetic, medical or sur-

gical treatment and hospital care for my child which is deemed advisable by a physician or surgeon licensed under the

1. The risk of injury from the activities involved in this program is significant. While particular rules, equipment, and personal

discipline may reduce this risk, the risk of serious injury does exist;

2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, including that which arises FROM THE

NEGLIGENCE OF THE Participant, Eagle Sky, or others, and assume full responsibility for my actions;

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any

unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the

attention of the nearest Eagle Sky official immediately and;

4. I, the Participant, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE,

INDEMNIFY, AND HOLD HARMLESS EAGLE SKY, their officers, officials, agents and/or employees, other participants, sponsoring

agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used to conduct the event WITH RESPECT TO ANY

AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF Eagle

Sky or their agents and employees or similar, to the fullest extent permitted by law; and

5. I understand that photographs and video footage taken as a result of participation in the event may be used in Eagle Sky

materials, publications and/or posted to the Internet. By signature below, I hereby grant world-wide, perpetual license of my image

and agree to waive any right to inspect or to approve the materials that Eagle Sky may choose to publish in any manner.

MEDICATION DISPENSING FORM Medication will be administered to students during camp stay only when such medication is needed by the student to remain in camp and administration is required. NO MEDICATION WILL BE ADMINISTERED TO ANY STUDENT WITHOUT PROPER COMPLETION OF THIS FORM. The form should also be used for non-prescription drugs, such as aspirin, when prescribed by a physician. Any medication to be administered by the camp nurse must be delivered in the ORIGINAL AND PROPERLY LABELED CONTAINER to the camp nurse, along with this dispensing form. Prescription medication will be kept with the nurse. In the absence of the camp nurse, the youth pastor/counselor will administer the medication. In cases where the Medication Dispensing Form is not available and administration of the medication is necessary, the nurse may obtain verbal orders from the PARENT/GUARDIAN by phone. Such verbal orders must be documented by the camp nurse on the Medication Dispensing Form. In order for the medication to be administered the following day, a signed Medication Dispensing Form must be received from the parent or guardian. Failure of the parent/guardian to provide documentation will require the parent/guardian to be present at the camp to administer the medication personally.
