Critical Care Nursing Course

Overview

A Postgraduate Certificate (PG Cert) in Nursing is jointly offered by Dunedin Hospital and Massey University in 2012. The PG Cert comprises two papers concurrently run at Dunedin Hospital from February to November 2012 (Pending approval).

o Critical Care Nursing – endorsed in Intensive Care or Acute Care

o Non Prescribing Practicum Each paper is a 30 point paper.

The Critical Care Course is a patient and family-centred clinically based course in which postgraduate students advance knowledge to provide nursing care that effectively responds to the needs of acutely ill, critically ill & deteriorating patients. Students are assessed on evidence of ability to meet the course outcomes.

Application Process

Registered nurses are advised to discuss their professional development plan and intention to apply with their Charge Nurse Manager (CNM). Complete the HWNZ Application Form for Postgraduate Training Funds for Registered Nurses - MIDAS 57109 Critical Care Course Supplementary Form – MIDAS 57111. Applications can also be downloaded from the Southern DHB website www.southerndhb.govt.nz/hwnzpgnursing, or contact Isobel O’Brien Course Coordinator or Leanne Gardner, Administrator – Postgraduate Nursing Education, Knowledge Centre, Dunedin Hospital. Applicants must also meet the entry requirements for Massey University.

Applications close, Friday, 14th October 2011. Requirements for application for the Critical Care Course

HWNZ Specific Requirements:

o Be registered as a Nurse under the Health Practitioners Competence Assurance Act (2003)

o Have a current annual practising certificate

o Be currently employed as a Registered Nurse by a health service that is funded by the District Health Board or Ministry of Health from Vote Health check this

o Be a New Zealand citizen or hold a New Zealand residency permit as conferred by the New Zealand Immigration Service

o Have a written career plan within PDRP

o Meet the entry requirements of the tertiary provider

Course Specific Entry Requirements:

o Practicing FTE is negotiable

o Professional Development Recognition Portfolio (PDRP) assessed at level 2. Portfolio to include a performance appraisal within in last six months and a professional development plan

Learning Outcomes: Critical Care Nursing

At completion of this paper the student will be able to:

Demonstrate advancing nursing practice through evidence of meeting NCNZ competencies for the nurse practitioner scope of practice domain 1-3 (excluding Domain 4 prescribing practice). Cover content and associated skills recommended in the Critical Care Nurses’ Section (CCNS) New Zealand standards in critical care nursing education Professional Responsibility and Leadership (Domain 1)

o Describe a framework to guide advancement in nursing practice

o Demonstrate developing ability to articulate clinical practice and ongoing advancement

o Demonstrate critique of and accountability for practice

o Demonstrates nursing leadership that positively influences health outcomes of patients and nursing

Management of Nursing Care (Domain 2)

o Employ physiological frameworks & models in clinical practice

o Evaluate health history and patient assessment skills across a variety of clinical settings

Learning outcomes – Non-Prescribing Practicum This individualised practicum paper provides students the opportunity to advance their learning and practice within areas of nursing acute/critically ill and deteriorating patients. At the completion of this paper, in a variety of clinical areas, students will be able to:

o undertake comprehensive systematic head to toe physical health assessments supported by diagnostic tests and evidence of pathophysiological knowledge

o use clinical decision making processes and critically reflective frameworks in planning interventions and evaluating patient responses

o use scholarly inquiry and current theoretical knowledge to inform evidence based patient management and decision making

o demonstrate integration of NCNZ competencies for the nurse practitioner scope of practice (2008) (excluding Domain 4)

o demonstrate integration of Critical Care Nurses’ Section (CCNS) New Zealand standards in critical care nursing education (2010)

o have met learning outcome guidelines provided for each preceptored practicum experience

Preparation: Postgraduate Certificate

The Course Coordinator will arrange a meeting with the applicant and their CNM (or delegate) prior to the start of the course to ensure that expectations and practical details of the course are clear. It is essential that consideration is given to the RN’s ability to meet the commitments of the course. Course Content & Structure (Postgraduate Certificate)

Non-prescribing practicum content will provide students the opportunity to advance their learning and practice through preceptored clinical experience. The non-prescribing practicum paper is guided by academic and clinical staff from Massey University and Dunedin Hospital. Critical care content is compiled with input from key stakeholders, and is guided by national and international practice and education guideline documents. This is to ensure that current standards for acute & critical care education are met. Critical Care Course stakeholders include Charge Nurse Managers from participating students’ areas, Clinical Nurse Specialist & CNM (ICU) and Knowledge Centre Manager. Theoretical content will fulfil the requirements of the Critical Care Nurses’ Section NZNO standards in critical care, specialist knowledge from acute care areas and will include the following and enable students to meet NCNZ competencies for the Nurse Practitioner scope (excluding Domain 4 prescribing)

Content includes:

Frameworks, skills and knowledge to plan, manage/intervene, document, communicate and critically evaluate the care of the acutely ill, critically ill & deteriorating patients

Selected principles of anatomy, physiology, pathophysiology related to:

Cell metabolism; respiratory system; neurological system; cardiovascular system; gastrointestinal system; renal system; haematological system & fluid electrolyte balance; recognition of the deteriorating patient; shock, SIRS & sepsis; trauma; core aspects of ‘Care of Children’

Learning and Teaching Strategies

Course content that supports learning and practice development will be delivered through a variety of learning and teaching strategies that include the following:

o Supported online web based learning

o Formal lectures delivered by clinical experts in study days

o A variety of strategies including case studies, presentations, discussions, critical evaluation, journal reviews, quality improvement methods and evidence based practice

o Emphasis is placed on students’ experiential learning in work areas and the way in which this supports advancing nursing knowledge, clinical practice and patient experience

o Preceptored clinical practicum placements

Formative and Summative Assessment for the Critical Care Nursing Paper

Description and weighting of assessment activities:

1. Cell Metabolism 2500 word assignment (submit on the first study day of the course)

2. Self-assessment and CNM assessment of clinical practice (x2) (submit at beginning and end of course)

3. Written episodes of practice (submit episodes on-line for peer discussion on the 1st of each month)

4. Regular tests, worksheets & scenarios (submit on-line)

5. ACLS Level 6. (New Zealand Resus Council) (pass/fail)

6. Oral Structured Clinical Examinations (OSCE) (pass/fail)

7. Skills and procedures acquisition (documented)

8. Assignment Critical Evaluation of Professional Nursing Care of Complex Patients 3000 words graded - (50% weighting)

9. Written Examination – (50% weighting) 65% or greater required to pass the exam. Specific requirements for passing the Critical Care Nursing paper

Assessment activities: 1-7 are formative assessments (not graded) activities. Assessment activities 8 & 9 are summative assessments and constitute the final grade. Non-Prescribing Practicum paper Description of assessment activities:

1. Practicum proposal (1000-1500 words) (submit)

2. 2 x written case studies (pass/fail)

3. An oral examination (pass/fail)

4. Demonstrated advancing practice through the learning outcomes guidelines based on NCNZ competencies for the nurse practitioner scope of practice (2008) excluding Domain 4 (submit)

Specific requirements for passing the non-prescribing practicum paper:

The practicum will be assessed on a pass/fail basis. In order to gain a pass in the practicum you are required to:

1. submit all assessments activities by due date

2. pass required assessment activities

3. actively participate in on – line discussion activities

Course Work File (Requirement to pass the course)

Student workload

As a guide a 60 credit value post graduate certificate requires approximately 25 hours of combined study and clinical application per week across the 26 week academic year. This time will be spent in critical reflection on clinical practice, accessing literature, reading, discussion with colleagues and preparing assessment activities. This is in addition to the clinical placements required for the postgraduate certificate (course).

Presentation of work (including referencing)

In general work should be in word processed form.

Assignments are expected to conform to the APA referencing style and academic standards of postgraduate study accessible via Massey website.

N.B. Course students are required to keep secure back up and hard copies of all submitted work.

Extensions

Extensions are negotiated with the Course Coordinator before the due date of submission. Written documentation of your extension must be attached to the work when you submit. If an extension has not been approved, a penalty of 10% of the assignment mark will be incurred. Plagiarism

The Critical Care Course conforms to the policy outlined in the tertiary partner Massey University. Audit

For audit purposes the Course Coordinator may copy aspects of the course work.

Preceptors

Critical Care Course Students are required to identify suitable preceptors within practice areas. Preceptors will:

o supervise the student’s clinical practice with regard to clinical decision-making

o provide guidance in the use of assessment framework, interpreting diagnostic tests, and diagnoses within clinical decision-making

o assess the student’s clinical skills

o provide feedback to the student and their academic supervisor regarding progress

Non-Prescribing Practicum

Students will work with a preceptor, clinical supervisor and academic supervisor; details to be arranged.

The Course Coordinator will also provide ongoing critique and review.

Graduation

In order to graduate with a Postgraduate Certificate, students must pass all requirements as set out for both papers (Critical Care Nursing and Non-Prescribing Practicum)

Critical Care Course Administration, Assessment and Moderation Team

Kathryn Brookes, Linda Grady and Isobel O’Brien are specifically responsible for moderation of participants work. This team deals with administration and student issues. Assessment and marking

Isobel O’Brien, Course Coordinator, Linda Grady, CNS and Kathryn Brookes, CNM will mark & assess the majority of course work.

The following people may be consulted & contribute to assessment and marking.

Preceptors, Nurse Educators, CNMs, Clinical Nurse Specialists, suitably qualified and experienced RN’s Impaired Performance

Where a participant has been prevented by illness or injury or personal bereavement from sitting an assessment, or when they believe their performance has been impaired by these factors they must immediately notify the Course Coordinator. This should be submitted in writing as soon as possible with an attached medical certificate if appropriate. The Course Coordinator will consult with relevant members of the marking and moderation team and relevant members of the Critical Care Course Advisory Group. Failure to Meet Acceptable Standards

The Administration, Assessment and Moderation team will assist participants to meet course outcomes.

o In the event of a participant not obtaining a pass mark or not meeting requirements to graduate in any specific pieces of work, the Course Coordinator will consult with Administration, Assessment and Moderation Team to identify preparation required to

repeat the assessment within an agreed timeframe. Repeat assessment may include resubmission, re-sitting, or oral examination. Relevant members of the Critical Care Course Advisory Group will be consulted where necessary. Failure of resubmission will result in failure of the course.

Opportunity for resubmission or re-sit

o An opportunity for resubmission of the written assignment will be offered if a course participant fails this assessment. Where a participant resubmits the assignment, the maximum mark for this piece of work is C- grade.

o An opportunity for resit will be offered to students who receive a grade less than the pass grade of 65% for the examination. Failure of resit will result in failure of the course. Where a participant re-sits a special examination the maximum mark for this piece of work is 65%.

o An opportunity for resit will be offered to students who do not receive a pass grade in the OSCE. Where a participant re-sits the OSCE a “pass” as per the OSCE specific marking criteria is required. Failure of re-sit will result in failure of the course.

This process also applies in the event that a participant’s clinical practice does not meet minimum standards on assessment by Charge Nurse Manager, or designated assessor i.e. Educator or CNS. Appeal Process

If the course participant has concerns about the assessment they have received they should discuss this with the Course Coordinator within a week (7 days) of receiving the results. If they do not achieve a satisfactory resolution they should formally outline their concerns in a letter to the Course Coordinator and copy this to the Knowledge Centre Manager. Appeals will be taken to relevant members of the Critical Care Course Advisory Group for review and decision of outcome. The decision of the advisory group is final.

Confidentiality

Patients:

Under the Health Information Privacy Code (1994) course students and coordinator have a responsibility to maintain privacy, confidentiality and anonymity of all health and personal information i.e. when writing about episodes of practice, references that could identify the patient must be excluded.

Course students must obtain and record verbal consent from patients or their next of kin before they write about them in a case study.

Course students:

All information relating to the course student will be used for the purposes of the course and reporting demographic date to the HWNZ only. This information will be kept confidential and secure.

Responsibilities: course students

Students are responsible for completing all course requirements.

Course expectations

o Concerns or issues relating to the course to be raised as soon as possible with the course Coordinator

o Organise your ward roster to facilitate other rostered clinical placements

o Take responsibility for entering your clinical placements and study days into your ward roster

o Meet clinical charge nurse in placement areas at least one month before placement to confirm shifts and preceptors

o Annual Leave by negotiation with Course Coordinator and Charge Nurse Manager

o Contact Course Coordinator if you are sick on a study day or clinical placement

o Course participants are required to attend all Study Days (as per the year planner). In extenuating circumstances, participants’ will negotiate an exemption with Course Coordinator

o Southern DHB Course participants are required to be on hospital e-mail and use this as the main avenue for exchange of information with the Course Co-ordinator. Please ensure you have your Southern DHB logon prior to commencement of the course

o Participants are expected to use Internet. Facilities are available in the Dunedin Hospital Staff Library – Southern DHB participants, please familiarise yourself with the Dunedin Hospital Staff Library before the course starts. Christine Bone, Librarian, is available for assistance

o Cell phones on silent or meeting mode in class

o Please have ID on person during study days

o In exceptional clinical need circumstances course participants will be required to provide assistance in the clinical work areas

Otago is the lead site for the Coordination and Administration of HWNZ Postgraduate Nursing Education Funding. Juliet Manning is the Acting Knowledge Centre Manager and Coordinator - Postgraduate Nursing Education. s Coordinator Juliet has oversight of the HWNZ funded programmes.
