1. Purpose & Scope The purpose of the Plant Emergency Action Plan is to ensure the safety of all employees and reduce property damage in the event of emergencies, such as fire, power failure, chemical spills, severe weather, bomb threat or active shooter. Proper planning and training are necessary to prevent panic and confusion. OSHA's Emergency Action Plan requirements, found at 29 CFR 1910.38, requires STR to have a written Emergency Action Plan (EAP). This plan applies to all operations in our company where employees may encounter an emergency.
The plan includes:
• Emergency escape procedures, escape route assignments, and meeting places.
• Procedures and responsibilities of members of the Plant Emergency Organization team. (Listed in App. A)
• The means of reporting fires or other emergencies to the Public Fire Department and/or other agencies.
• Procedures to account for all employees after emergency evacuation.
• List of employees trained in first aid and CPR.
• Method of notifying employees of emergency situations.

Command Structure
Personnel assigned duties under this plan and their contact information are listed in
3.1 Emergency Coordinator (Located in Appendix A) The Emergency Coordinator (EC) must be familiar with all operations throughout the facility and fire protection equipment.
The Emergency Coordinator must know the tasks and assignments of all members of the Plant Emergency Organization.
In the event of an emergency, the emergency coordinator must exhaust all efforts to resolve the situation.
The EC will relinquish authority only to a lead responding agency or a company official.
The Emergency Coordinator:
• Will assess the situation and determine if emergency procedures are required.
• Instructs the Notifier to alert outside agencies such as fire department, ambulance, and other members of the PEO team. The Coordinator will designate a representative to meet the public agency at the main entrance and direct them to the location of the emergency.
• Directs all efforts, including evacuation of employees, minimizing property loss, shut down of electricity and gas.
• Directs shut down of plant operations when necessary.
• Assists in training members of the PEO team.
3.2 Discovery - Need of Evacuation After the activation of the emergency alarm, the Department Manager or Shift Supervisor will instruct employees to follow their primary or secondary evacuation route.
The Department Manager and the Supervisor will check areas such as restrooms, offices and other enclosed spaces located within their area of responsibility.
It is the responsibility of the Department Manager and Supervisor to account for all employees.

3.3 The Notifier (Located in Appendix A)
Emergency Notifications
The Notifier must be reliable and constantly available and have immediate access to outside phone lines.
Their duties and responsibilities are:
• Has immediate access to the current contact list so to notify members of the Plant Emergency Organization.
• Notify Fire Department, and other agencies of an emergency, (usually 911). • The Notifier or designated appointee should stand by at the main entrance to
guide responders to the appropriate location
• Evacuation Routes are highlighted on a Site Map. These maps will be posted at strategic areas within the facility and is also presented as Appendix D.
3.4 Emergency Evacuation Notification and Procedures During an emergency, should evacuation of the plant and/or office become necessary, the Notifier or designated appointee will activate the emergency alarm by announcing over the intercom.
After picking up the phone receiver this screen will appear.
To announce over the intercom.
• Press the button under the ExPg and then press the number 1. This will allow you to page out through all the plant.
• You would also need to press InPg and then press the number 1 This will allow you to page in all office areas
The Notifier or designated appointee will use the paging system to announce “May I have your attention; all employees evacuate the building in an orderly manner. Report to your Rally point.”

All Department Managers and/or Supervisors will instruct employees to follow primary or secondary evacuation routes. It is the responsibility of the Department Managers and/or Supervisors to account for all employees in the event of emergency evacuation. To accomplish this, Department Managers and Supervisors have the accessibility of using an application on their phones to access their employee roster. If the phone application isn’t used, then the Supervisor will run an employee on premise head count report from the payroll system at the beginning of each shift. Employees will be counted immediately when reaching the assigned rally point. Fire Department Officials should be notified of missing employees.

• All Riser valves are in the OPEN position.
• After the emergency is resolved and only at the direction of the Fire Department, the Sprinkler Valve Operator and the Plant Maintenance Team will restore normal sprinkler protection.
3.6 Electricians Duties
The Electrician must be familiar with electrical and ventilation systems and have a working knowledge of shut down and start up procedures. The Electricians must know the location of main disconnect panels and ventilation controls.
• The Electricians will make sure that all exhaust fans are shut off in the facility.
• After the electrician has completed the electrical and ventilation shutdown, they must proceed to evacuate the building and to meet at the rally point with their group.

3.7 Salvage Squad
All members of the Maintenance personnel and members of Management will make up the Salvage Squad, under the direction of the Emergency Coordinator. The Salvage Squad will be responsible for salvaging equipment and/or stock. The Salvage Squad will assist as needed with the clean-up work necessary to resume operations.
3.8 Pipe Fitters The Pipe Fitter must understand the hazards associated with flammable liquids and gases at the facility. The PF must know the layout of the distribution system and the location of all primary and the secondary shut off valves.
• The Pipe Fitter shuts off all potential fuel supplies, such as natural gas, L. P. gas, and any system that may interfere with firefighting such as steam. The Pipe Fitter will see that systems remain shut down until directed to reopen by STR Emergency Coordinator
• After the Pipe Fitter has completed the shutdown, they must proceed to evacuate the building and to meet at the rally point with their group.
• After the emergency has passed the Pipe Fitter works with the Sprinkler Valve Operator and with the fire protection company to replace sprinkler heads to ensure continued fire protection.

Evacuation Route(s)
Evacuation routes are highlighted on the map. Maps are posted throughout the facility. During the evacuation, team members will be directed to their assigned primary or secondary route by Evacuators and Supervisors.
Some reminders during an evacuation:
• In case of fire, stay low below the smoke.
• Stay upwind or cross wind from smoke, fumes, or vapors.
• Stay calm.
• Walk briskly, avoid running (unless it is absolutely necessary).
• If there is a visitor’s log, assign personnel to bring it with them.
• If there are contractors or other facility tenants, they must also be made aware of the procedures and that an evacuation is in progress.
4.0 Severe Weather Emergency
Tornado Watch and Tornado Warning
Tornado Watch: Be Prepared A tornado watch is issued when tornadoes are possible in and near the watch area. Be ready to act quickly if a warning is issued. It does not mean that they will occur. It only means they are possible.
Tornado Warning: TAKE ACTION When a Tornado Warning is issued, seek safe shelter immediately. A Tornado Warning is issued when a tornado is imminent. A tornado has been sighted or indicated by the weather radar. During a severe weather emergency of the plant and/or offices, the Notifier or designated appointee will activate the emergency.
• This facility is equipped with weather radios.
• In the event of severe weather; the supervisor will notify the Emergency Coordinator(s).
• The Emergency Coordinator(s) will alert the employees to take shelter in their designated place of safety. The assembly areas are listed and are highlighted on the map in Appendix D

Announcement:
During severe weather, should taking shelter inside the plant become necessary, the Notifier or designated appointee will activate the emergency alarm by announcing over the intercom.

We are currently under a tornado warning. Everyone MUST stay in your designated area until the warning has been lifted and it’s announced over the paging system that it is safe to return to work.

To accomplish this, Department Managers and Supervisors have the accessibility of using an application on their phones to access the on-premise employee roster. Employees will be counted immediately when reaching the assigned shelter. Notification of missing employees should be made as soon as possible to other department managers or supervisors to see if the employee has taken shelter in another location.

4.3 Severe Weather Route(s)
Severe Weather shelters routes are highlighted on the map. Maps are posted throughout the facility along with designated shelters labeled outside at the location. During severe weather, the team members will be directed to their assigned shelter by Department Manager and Supervisors.
Some reminders during Severe Weather:
• Stay calm
• Walk briskly, avoid running (unless it is absolutely necessary).
• If there is a visitor’s log, assign personnel to bring it with them.
• If there are contractors or other facility tenants, they must also be made aware of the procedures and an evacuation in progress.
5 Active Shooter Emergency Response
Active shooter, is defined as an employee or an unknown individual who would come to the premises and utilize a firearms or explosive devices in an attempt to kill or harm employees and other persons who may be at the workplace. In anticipation of such an event, STR has developed an active shooter emergency response plan.
For the first employee to identify an active shooter, these are the steps of how to react until law enforcement arrives.
As soon as the active shooter has been discovered it should be announced over the paging system.

Then the notifier should contact 911 to alert the law enforcement of the situation. if you cannot speak, leave the line open and allow the 911 dispatcher to listen. If possible, details should be given
• Facility address
• Description of suspect (s) and possible location inside of facility
• Number of shooters
• Number and type of weapons
• Direction the shooter was headed
• Location and condition of any victims
Evacuate, Only if it is safe to do so. Remember try to remain calm
▪ Know all evacuation routes for your area ▪ Evacuate regardless of whether others agree to follow ▪ Leave your belongings behind ▪ Help others escape, if possible ▪ Do not attempt to move wounded people ▪ Prevent individuals from entering an area where the active shooter may be ▪ Once outside the building, hide behind retaining walls, parked vehicles, etc. ▪ Do not leave in your vehicle. If no one knows you have left the premises the
police may try to go back inside the plant to find you. ▪ Call 911 when you are safe, if you cannot speak, leave the line open and allow
the 911 dispatcher to listen. ▪ Always keep your hands visible, once outside because the Police officers will be
on high alert and this will let them know you are not the shooter. ▪ Follow the instructions of any police officers
Hide out If evacuation is not possible, find a place to hide where the active shooter is less likely to find you, with these recommendations
▪ Be inconspicuous, try to stay calm ▪ Be out of the active shooters view ▪ Provide physical protection if shots are fired in your direction, stay as low to the
floor as possible, remaining quiet and motionless ▪ Don’t trap yourself or restrict your options for movement ▪ Lock the door, try to prevent the shooter from entering the area ▪ Turn the lights off ▪ Blockade the door with heavy furniture ▪ Silence cell phones and/or pagers ▪ Remain quiet and motionless
Self defense STR would only suggest self-defense as the last resource and only if it is not possible to evacuate or hide, only if or when your life is in imminent danger should you make a personal choice to attempt to negotiate with or overpower the shooter.
▪ Acting as aggressively as possible against him/her ▪ Throwing items and improvising weapons ▪ Yelling ▪ You must commit yourself to defensive physical actions if self-defense is the
only choice you have.

6. Medical Emergencies The employee who is the first to discover anyone with a medical emergency must notify the nearest coworker to contact the supervisor to assess the situation so he or she can decide if they need to call 911.

7. Bomb Threat The purpose of this section is to outline procedures to be followed in the event of a bomb threat.
Prevention: All employees should notify their Department Manager or Supervisor immediately making them aware of any unauthorized people on STR Property. The Department Manager or Supervisor will investigate to see who the person is. Bomb Threat call: The person who receives the call should:
• Keep the caller on the line as long as possible and make detailed notes of the conversation.
• Pay attention to background noise.
• Ask for the location of the bomb and the time it is set to go off.
• Remind the caller that innocent people could be killed.
• Try to get the caller to talk to the Plant General Manager or other company official.
• Notify the Plant General Manager and police immediately.
Evacuation
• Emergency Evacuation procedures shall be followed.
• The Emergency Coordinator or Safety Manager is responsible for initiating evacuation.

Search
• STR personnel will not search the building after a bomb threat without the cooperation of the local police department.
8. Power Outage In the event of a power outage:
• Stay put: Do not wander around. Emergency lighting may not provide enough light to assure safe movement.
• Supervisor: Will gather everyone in a safe area, Do a head count and direct the employees on staying in the area until further instructions are given.
• Lift Truck operators: Stop your truck immediately and do not drive until lights in the PLANT are restored. If equipped with lights, turn the lights on to mark your location and provide lighting in the event that evacuation is initiated.

9. Chemical Spill
• Response Procedure
Because of the degree of containment and the generally conventional nature of material handled at this facility, it is not anticipated that a spill response of any great magnitude or complexity will be required.
However, as protection against such an occurrence, various environmental services are available and are listed in Appendix A External Contacts list
A Spill Response Quick Guide is in Section 11.
In the event of a spill, the following procedure is to be followed by all plant personnel: ➢ The person detecting the spill should immediately stop the source of the spill and
prevent flow to the surrounding soil, if this can be done safely. Stopping the spill may involve turning off pumps, closing valves, righting a barrel or other appropriate action.
➢ The Department Manager and the Safety Manager will be summoned to the scene and informed of the source and nature of the spill and of mitigation activities which have been carried out up to that time.
➢ The Department Manager and the Safety Manager will then assume responsibility for directing evacuation and/ or cleanup activities. He / she will summon to the scene the manpower and equipment which is needed to respond to the incident and will direct their activities for the duration of the response.
➢ The Department Manager and the Safety Manager will contact, or have a designated person contact, any support groups whose assistance is needed in the response effort, such as the police or fire department. Notification of regulatory agencies, should it be required, will be handled by the Plant’s General Manager or a member of the Company Management Group.
➢ Once the spill has been isolated from possible sources of outside discharge and the source of the spill has been eliminated, efforts will be directed toward containment of the spilled material within the smallest area possible. This should help minimize the amount of area which must be cleaned up and decontaminated, making the response effort as efficient as possible.
➢ After the bulk of the spilled material has been removed, final cleanup of the area will be conducted. This will include decontamination of the area and equipment used for the cleanup in a manner which will return the affected area to its previous level of usefulness and safety.
Whenever possible, cleanup will be conducted in a manner which will permit reuse of the material, either in processing operations directly or for some related purpose.
Where this is not possible, disposal of material resulting from the cleanup will be in accordance with all applicable regulations. Always follow the safety data sheets (SDS) for instructions.

10. External Emergencies There may be external emergencies from nearby companies, such as events like fires, explosions, chemical leaks, train derailment or other incidents that could have the potential to impact our people. The Emergency Coordinator, Safety Manager and Facility Management will assess each situation as it happens. Imminent danger may result in the need to evacuate the facility or shelter in place until directed otherwise by responding agencies. Things to consider during the assessment:
• Proximity of incident to the facility.
• Is there smoke or fumes from fire that is entering the facility? If so, it is not likely that it will be safe to “shelter in place”.
• If smoke or fumes are visible, are all the dock doors and entrance doors closed?
• Does evacuation routes or assembly areas place personnel unnecessarily close to the off-site incident?
• Wind speed and direction: Once personnel are assembled, monitor the wind speed and direction for changes, have alternate assembly and evacuation routes planned.
• Are parts of the facility or employee vehicles contaminated from the incident? Employee Exposures
• Has there been employee exposures from the incident? o Are those exposures life threatening or otherwise dangerous to health? o Call 911 immediately o Do not wait to decontaminate employee if there has been a chemical

Acid

Your contaminated clothing should be disposed of as a hazardous waste. Neutralize residual acid with sodium carbonate (soda ash) and enough water to make a slurry. Check slurry frequently with Ph paper to determine neutralization. Acid is considered neutralized if Ph is between 4 and 11. Prior to neutralization, remove any combustible materials from area as the neutralization process can generate enough heat to ignite some materials. Use absorbent to pick up neutralized acid for disposal. Diluted solution of sodium hydroxide will accomplish the same neutralization process.
Compounds are not flammable by themselves but can intensify adjacent fires or cause spontaneous ignition upon contact with organic materials. The proper type of fire extinguisher depends on the type of adjacent burning material.

12. Training The Safety and Training Manager, Supervisors/ Leads and members of Management review the Emergency Action Plan at the following times with each employee covered by the plan:

13. Media and Public Announcements STR Corporate Management will designate a representative who will respond to any media request for information in the event any reportable emergency should happen. The STR Corporate representative will carefully consider the nature of any requests to avoid disclosing information about any situation that is confidential or could be interfering with any ongoing police or company investigations. Employees are discouraged not to make any contact with the media or public with any comments until a full investigation of the situation has been completed by the members of management and / or local law enforcement.
14. OSHA In the event that there is a fatality, amputation, loss of an eye or one or more employees are hospitalized for treatment during an emergency situation, OSHA will be notified. The Human Resource Director and Safety Manager will contact OSHA within eight (8) hours of any death and within twenty- four hours (24) of any hospitalization, amputation or loss of an eye.
