Dear fRIENDS,

Forty-seven years ago Summit County Developmental Disabilities Board, or Summit DD, was created to support adults and children with disabilities in their own community. In its early years, Summit DD chose to operate a school for more than 600 children who were being educated in church basements and other makeshift facilities because the public schools were not yet offering education services to children with disabilities. The County Board also opened sheltered workshops that offered vocational training for adults who would otherwise be institutionalized or at home sitting idle without any programming.

Summit DD’s role has evolved since those monumental beginnings. During the past four decades, we have been a driving force behind a great deal of positive change for those living with developmental disabilities in our community. And as a result, Summit DD and those individuals we support, are more connected to our communities than ever. Today, all children are educated in their home school districts, more adults are working in community jobs, and people live in homes of their choice in nearly every community in Summit County. Summit County residents see individuals with disabilities as fellow citizens who have the same right to achieve their personal vision of happiness and success. Our commit-ment, to continuing as a leader in the effort to build accepting communities that embrace and include everyone, is stronger than ever. We know there is still work to do and Summit DD accepts this responsibility with excitement and optimism.

As its role evolves, Summit DD sees itself becoming more of a connector of our region’s business, religious, education, advocacy and government leaders to engage in conversations that help our communities flourish for all. In this annual report, we invite you meet some of your neighbors who, with the right support, are redefining what is possible for themselves. Meet some of our partnering agencies who work to support individuals throughout their lifetime. Read about our innovative programs for adults and children that work to build an inclusive community. Know also, however, that in the end, every citizen in Summit County plays a role in creating a welcoming culture where everyone has the opportunity to accomplish great things. Your contribution is essential.

Our Early Intervention services are based on the principle that children learn

best through everyday experiences with people they know; making par-

ents their child’s best teacher. Summit DD connects families to a team of

experts that help create individualized strategies that make daily routines

easier and help the child meet essential milestones. Our family-centered,

evidence-based consultative approach to Early Intervention services gives

families a single point of contact for all of their needs.

Community Partnership for Inclusion Connecting children to their community by supporting local child care centers.

Kids are kids, and we believe that all kids should have access to inclusive programs that

enrich their early childhood experience such as story times, arts and crafts, sports, or any

other community program. We are the experts in inclusion strategies and connect with

existing community programs, like area YMCAs, libraries, and the zoo, among others, to

ensure there are inclusive opportunities for all children and families, regardless of ability.

School-Age Services Connecting 1,218 children and families to their schools and communities.

Summit DD has Service and Support Administrators (SSAs)

available specifically for school-age children to help families

navigate their child’s individualized education plan. We can

connect families with inclusive community resources and

activities, plus assist with finding adaptive equipment for

the child. Summit DD can even help fund summer camp

opportunities when the school year ends. Our SSAs are also

there to help teens during the transitional period between

high school graduation and adulthood.

School-Age Services Connecting 1,218 children and families to their schools and communities.

Supported 246 campers in 69 different community camps,

such as Rotary Camp, YMCA camps, Akron Zoo Camp. Summit DD’s Camp Service Coordinator

helps create a Camp Service Plan for each family that ensures each camper has a safe and fun time at summer camp.

The best support sometimes comes naturally through friends and family.

Service Coordination Creating individualized service plans for more than 2,000 adults by listening to their goals and aspirations.

Summit DD Service and Support Administrators, or SSAs, listen to those we support, their

families, caregivers, and other important people in their lives to assess challenges, goals,

and opportunities for each individual. Our SSAs develop individual service plans, or ISPs,

that connect citizens with disabilities and their families to providers within the community

that can help people achieve their vision of a happy, satisfying life.

team of caregivers to ensure each individual received customized support, with a

95% satisfaction rating with individuals

and 87% satisfaction rating with parents.

Residential Services Connecting 1,665 people to their communities with residential support.

Whether an individual is looking for the perfect place to call home or is in need of extra

support in their current residence, Summit DD Service and Support Administrators (SSAs)

can help discover the right-fit options. Residential supports are provided by a network

of quality private providers. From independent and shared settings, to foster care and

family living, Summit DD residential services help individuals find their home sweet home.

Summit DD and Summit County Jobs and Family Services

teamed up to streamline the application process for those who may be eligible for food assistance. Food assistance provides individuals more choices and participation in grocery shopping.

Residential Services Connecting 1,665 people to their communities with residential support.

Summit DD coordinates an array of services for adults with disabilities to help people

reach for their dreams. Day services and employment services are two of the many

supports available for adults that offer people tools for success. Transportation services

get adults to and from their jobs or center-based day programs. Individuals choose from

a large network of quality providers to meet their daily needs, and Summit DD is here to

provide the connections that create opportunities for people achieve their personal vision

of health, happiness and success.

Summit DD has teamed up with seven providers to implement a Discovery process, which helps individuals determine

where they fall on their path to community employment. In 2014, six individuals completed the Discovery process.

with a 94% satisfaction rate.

1,911 individuals used transportation providers to get to work

with a 95% satisfaction rate.

RecreationConnecting 2,830 individuals to activities that enhance their quality of life.

Summit DD makes it a priority to connect individuals to a variety of programs that

foster community involvement based on what interests them most. Options range from

socially-oriented clubs like Project Inclusion, to sports such as Special Olympics, and

travel clubs that explore the world, to senior activities for older citizens. Depending on

a person’s individual interests, there are many community activities and recreation

opportunities available to adults with developmental disabilities in Summit County.

Summit DD develops and maintains win-win relationships with partnering providers,

area employers, educational institutions, community leaders, and local organizations

to transform perceptions and build welcoming communities. Summit DD empowers

more than 4,400 children and adults with disabilities to reach for their dreams and

more fully participate in their communities. Our goal is to cultivate a strong, diverse

network that offers support and opportunity to people of every ability.

As Summit DD employees, we are passionate about those we support. We continue to

innovate through supports like our Community Partnership for Inclusion (CPI) program,

where we support families in their own neighborhoods; our person-centered service

plans, where we listen to individual’s goals and create the personalized plans to help

achieve them; and our new website, which is filled with events, success stories, and

helpful information about available services and supports. It’s our goal to never stop

moving forward!
