SUMMARY OF ELEVATORS

The US Bank complex at 777 Wisconsin Avenue, Milwaukee, Wisconsin, was inspected by Thomas

Branham and Steve Rains of Elevator Consulting Services, Inc. on May 18-20, 2016. Tom is certified

by NAESA and his QEI number is 1339. Steve is certified by QEITP and his number is E-000470

NAESA is the National Association of Elevator Safety Authorities. We have identified corrective

items that in our opinion need to be addressed by the Elevator Contractor. They are free to offer an

explanation or dispute any individual item within the report but we can offer a code, specification or

reasonable reason for each and every negative evaluation. They are evaluated on and for the ASME

17.2 Code for Inspectors Manual and industry standards for acceptable installation and maintenance

procedures.

The following are deferred repairs, preventive type maintenance and some testing that is behind. It is

normal for the service vendor to have 30 days to complete the punch list. When it comes to Code and

State mandated testing that is behind, it should be shortened up to a as soon as possible scenario.

However there are several handrails on the escalators, governor rope and some major repairs that may

take some additional time.

102. Blow out generator, regulator and exciter of all carbon build out.


103. Perform annual brake maintenance. This includes pulling the cores, cleaning and

lubricating, dropping the shoes and checking the lining for wear and lubing all the pins.

Clean off all filings from the core head. Provide a picture of core and shoes removed during

teardown. Date, tag and complete documentation.


104. Seal up machine. Put a bolt with a hole in it to vent the gear box or the OEM filter plug. Clean

up all oil on and around the machine.


105. Blow out motor of all carbon build up.


106. Replace C6 capacitor in the controller.


107. Clean the entrance door tracks of all fuzz and dirt.


108. Adjust the cab stabilizer. Too much lateral movement.

111. Put a bolt with a hole or the Westinghouse filter plug in the open inspection cover plate to stop the

oil spray from entering the machine room.


112. Replace GR3 relay. Has broken mature spring.


113. Properly mounter timer/counter on back of controller.


114. Clean the entrance door tracks of all fuzz and dirt.


115. Replace the top left (OLI) post roller on the counterweight.


116. Clean and adjust 2nd

floor door lock. Lock assembly is hanging up. Replace leading washer on

pick up assembly.

168. The balustrade trip piece at the bottom left curve is protruding out and is a catch hazard. Redo the

trim to eliminate catch point.
