1 SUMMARY OF ELEVATORS The US Bank complex at 777 Wisconsin Avenue, Milwaukee, Wisconsin, was inspected by Thomas Branham and Steve Rains of Elevator Consulting Services, Inc. on May 18-20, 2016. Tom is certified by NAESA and his QEI number is 1339. Steve is certified by QEITP and his number is E-000470 NAESA is the National Association of Elevator Safety Authorities. We have identified corrective items that in our opinion need to be addressed by the Elevator Contractor. They are free to offer an explanation or dispute any individual item within the report but we can offer a code, specification or reasonable reason for each and every negative evaluation. They are evaluated on and for the ASME 17.2 Code for Inspectors Manual and industry standards for acceptable installation and maintenance procedures. The following are deferred repairs, preventive type maintenance and some testing that is behind. It is normal for the service vendor to have 30 days to complete the punch list. SUMMARY OF ELEVATORS

The US Bank complex at 777 Wisconsin Avenue, Milwaukee, Wisconsin, was inspected by Thomas

Branham and Steve Rains of Elevator Consulting Services, Inc. on May 18-20, 2016. Tom is certified

by NAESA and his QEI number is 1339. Steve is certified by QEITP and his number is E-000470

NAESA is the National Association of Elevator Safety Authorities. We have identified corrective

items that in our opinion need to be addressed by the Elevator Contractor. They are free to offer an

explanation or dispute any individual item within the report but we can offer a code, specification or

reasonable reason for each and every negative evaluation. They are evaluated on and for the ASME

17.2 Code for Inspectors Manual and industry standards for acceptable installation and maintenance

procedures.

The following are deferred repairs, preventive type maintenance and some testing that is behind. It is

normal for the service vendor to have 30 days to complete the punch list. When it comes to Code and

State mandated testing that is behind, it should be shortened up to a as soon as possible scenario.

However there are several handrails on the escalators, governor rope and some major repairs that may

take some additional time.
