Suicide, even though suspected of being understated in the cause-of-death statistics, has been among the 12 leading causes of death in theUnited States for the past 10 years. In this report, the trends for sui-cide are reviewed and the rates for age, color, sex, marital status, andother gYowps are examined.

Firearms are more frequently used for committi~ suicide than any

other means, Hanging and strangulation rank second. The memts usedto commit suicide have changed over the years. Analgesic and sopo-Yi@c substances and motor vehicle exhaust gas aye used more fre-quently, while gases in domestic use fov cooking, heating, or light-ing aye now used less. The method varies by sex and, to a lesser ex-tent, by coloY.

Suicides are more j?equent in the spring than in any other season.April nearly always has the highest daily average number and DecembeYthe lowest.

Suicides are much more frequent among males than among females and

more frequent in the white population than in the nonzohite. The ratesfov white males geneyally incyease with successive age groups upthrough 85 yeavs and over, while for white females they reach. a peakin the 45-54 ov 55-64 age g’rowp. Rates foy nonwhite persons show dif-ferent and less decided pattenzs.

As for marital status, the suicide rate (age-adjusted) is lowest for

married persons. Single pevsons Yank next, followed by widowed and,last, divorced persons.

The swicide Yates (using the 1959-61 average) foy the southevn States,

with the exceptions of Virginia and Florida, were among the lowest inthe United States, while those for the uieste?’n States were among thehighest. In 1960 theye zoeye six standayd metropolitan statistical ayeaswith a suicide Yate of moye than 15 suicides foy eveyy 100,000 people.

Five of these six areas were in two States—California and Florida.

The age-adjusted suicide rate was the same in 1964 as in 1950 but, ingeneyal, the Yates weye a little lower fo?’ the 1950rs than for the 1960ts(thvowgh 1964). The age-specific Yates show upward trends for theyoungey and middle ages, with a downwayd tyend foy the older ages.

Cause-of-death statistics for 1964 show anaverage of 56 suicides committed daily in theUnited States. The 20,588 reported suicidesrepresented about 1 percent of all deaths occur-ring in the United States during the year. Therate was 10.8 per 100,000 population. Since1900, the suicide rate has ranged from a high

of 17,4 in 1932 to a low of 9.8 in 1957. It has notexceeded 12.0 per 100,000 since 1941 (fig. 1).

Suicide has been among the 12 leadingcauses of death for the past 10 years. It hasbeen even more prominent among teenagersand young adults. It ranked fourth in 1964 forage groups 15-24 and 25-44 years. (Of course,this high rank for these age groups reflects thesmal Ier number of deaths from most othercauses, as well as the actual number of suicides.)Although suicide was of less importance as acause of death in the upper age groups, wheredeaths from other causes are more numerous,it remained one of the 10leading causes throughthe age group 45-64 years.

Suicide is suspected of being grossly under-stated in the statistics based on cquse-of-deathcertification. A general reluctance to admit sui -cidul intent of friends and relatives and difficul-ties in recognizing it under certain circumstancesundoubtedly result in underre~rting. Variousestimates have been made of the actual occurrenceof suicide, some indicating that it is as much as

twice the reported figure. In addition, many at-tempts at suicide are unsuccessful. Althoughthere are no national statistics on this topic, atleast one study (Farberow and Shneidman, The

Cry for Help) places, the ratio of unsuccessfulattempts at suicide to committed suicide in theneighborhood of 8 to 1.

Detailed statistics on which this report isbased are presented in tables 1-7. They show thetrends of suicide by age, color, and sex for theyears 1950-64 and data for the 3 years 1959-61by marital status and geographic area.

MEANS OF INJURY

Firearms and explosives are more frequentlyused for committing suicide than any other m cansof injury. This category, comprised principallyof deaths attributed to self-inflicted gunshotwounds, accounted for 48 percent of all suicidesin 1964. Hanging and strangulation was the nextmost frequent means and accounted for about15 percent of the total suicides. About 12 percentwere from poisoning by analgesic and soporificsubstances. Nearly three-fourths of these in-volved the use of barbituric acid and its deriva-tives. The only other category which accountedfor more than 10 percent of the suicides in 1964was “poisoning by other gases. ” Of these, 9 outof 10 involved motor vehicle exhaust gas.

The means selected for committing suicidehave changed,over the years. The direction ofthis change is indicated in tableA, which showsthe percentage distributionof deathsfromsuicideby means of injury for 1950, 1960, and 1964.While the total suicide rate was about the samefor these 3years(11.4 per 100,000 in1950, 10.6

in 1960, and 10.8 in 1964), decided differenceswere observed in the methods used.

The classification “suicide by firearm andexplosive” ranked far above all others for eachof the3 years shown. It accounted for 43 percentof all suicides in 1950 and 48 percent in1964.Hanging and strangulation ranked second for the3 years but declined from 21 percentof the totalin 1950 to 15 percent in 1964. Poisoning byanalgesic and soporific substances increased be-tween 1950 and 1964, from about 5 percent oftotal suicides to 12 percent. Poisoning by other

gases (predominantly motorvehicle exhaust gas)also increased, accounting for about 6 percentof total suicides in 1950 and 11 percentin 1964.On the other hand, suicides committed by poi-soning by gases in domestic use accounted forover 6 percent of all suicides in 1950 and lessthan one-halfof lpercentin 1964.

Thechoice ofa way to commitsuicide variesby sex. For instance, analgesic and soporificsubstances were used by only 6percent of malescommitting suicide, compared with 30 percentof females. Except for suicides from poisoningby other gases (which includes motor vehicleexhaust gases), poisonous substances were usedby a greater percentage of females than ofmales.Also, a greater percentage of females drownedthemselves. Firearms and explosives accountedfor 56 percent of all suicides for males, but only25 percent of those for females. Other meansof

negligible.

Methods differ according tocolor aswell assex, but the differencesbetweenthewhiteandnon-white populations are not so great as those be-tween males and females, As shown in table B,

a greater percentage of white than of nonwhite

persons poisoned themselves with analgesic andsoporific substances (12.7 compared with 5.3)and with “other’’gases (ll.3and2.8). Conversely,

nonwhite persons were more likely than white

persons to use other solid and liquid substances(such as denatured alcohol, rat poison, or lye)

as a poison (5,3 compared with 3.2) and to killthemselves by jumping from high places (8.6versus 3.4)0

SEASONAL VARIATION

Suicides are more frequent in the springthan in any other season. When the number ofsuicides per day by each month is computed (toeliminate length of month as a factor influencing

the number of suicides for agivenmonth) ,March,

April, and May have the highest averages. Aprilhad the highest daily average in 10 of the 15years from 1950 through 1964, Mayand June the

highest for 2 years, and March forlyear. The

months with the lowest number of suicides perday are thewinter months-November ,Decemberand January. An average for the 15 years showsthe number of suicides ranging from 46 per dayfor December to 54 per day for April (fig. 2).

Suicide rates for the United States increase

with age at least untilthe 65-74-year age group

is reached. This was true for each year from

1950 through 1964. For some of the years therates fluctuated slightlyat the 65-74 and sub-sequent age groups(75-84 and85andover) .Deathsof children under 8ybars of age arenot tabulatedas due to suicide, regardless of the informationentered on the death certificate.

Age-specific rates exhibit different patternsby color and sex (see fig. 3). In1964, the suicide

rates for white males increased with each suc-cessive age group, starting with a rate of O.5per 100,000 at ages 5-14 years and continuing

to 65.1 per 100,000 for the 85-years-and-overage group. A similar relationship was observed

throughout the 1950-64 period. The suicide ratesfor white females present a different pattern,They reached a peak in the 45-54 or55-64 agegroup and decreased thereafter. For nonwhitemales, suicide rates showed much less uni-formity. In 1964 the rate was highest for theage group 25-34 years. For other years it

reached a peak among persons in their 60’s,70’s, or 80’s, but the rate for the 25-34-year

group was generally as high as or a littlehigher thanthe rates forthemiddle-aged groups.The age-specific rates for nonwhite femalesfollowed no set pattern, but in 1964 all rateswere under 5per 100,000.

Suicides occur much more frequently amongmales than among females. The rate for maleswas almost three times that for femalesin 1964.Seventy-three percent of the persons committingsuicide in the United States in that year weremales. In 1950 this percentage was 78 percent.There is evidence, however, that a much largerproportion of the unsuccessful attempts at sui-cide are made by women. In a study by Farberowand Shneidman (reported in The CYY for Help),based on statistics for Los Angeles County in1957, 70 percent of the suicides were committedby males while almost 70 percent of the personsunsuccessfully attempting suicide were females.Other studies support these percentages.

Suicide also occurs more frequently in thewhite population t-in the nonwhite. In 1964,the ratio between rates for white and nonwhitepersons was 2 to 1.

In 1960, the nonwhite population made up11 percent of the total population, yet committed

only 5 percent of the suicides. However, whenNegroes were excluded, the remaining nonwhitepopulation had suicide rates which were almostequivalent to the white suicide rates (table C). Infact, the nonwhite female population, excludingNegroes, had a higher suicide rate than the whitefemale population. The rate for nonwhite males,excluding Negroes, was a little less than that forwhite males.

GEOGRAPHIC VARIATION

Of the States, Nevada hadthehighest averageannual suicide rate for the 3-year period of

1959-61. The other western States also had highrates. The suicide rates for the southern States,

with the exceptions of Virginia and Florida, wereamong the lowest in the United States. This is

also true when the rates are adjusted for age

(fig. 4).Between the 3-year periods 1949-51 and

1959-61, the average annual suicide rates for

the New England, Middle Atlantic, East North

greatest decline (15 percent). The rate for theother geographic divisions increased. with thelargest increase (13 percent) for the East %uth

Central.The suicide rate (1959-61j fornonmetrc-spoli-

tan counties was slightly higher than the rate k>rmetropolitan counties (fig. 5). By contrastin~

race and sex differentials, it is found that this

higher rate is entirely a result of 013 percent

higher rate for white males; the suicide ratesfor white females, nonwhite males, and non-white females in metropolitan counties were

all higher than their, respective nonmetropolitiln

For the standard metropolitan statisticalareas in 1960 with a population of 50(J)(M) ormore, suicide rates ranged from 6.7 for Provi-

dence, Rhode Island, to 1~.3 for Tampa-St.

Petersburg, Florida. ThercweresixSMSA’s withrates higher than 15 suicides per 100,WO andallbut one are in California and Florida. Mostof thestatistical areas with low suicide rates are lo-

cated in the northeastern ”and southern sections ofthe country.

MARITAL STATUSThe age-specl(w sui~. ide rates (19.59-61)

for married persons were lower than those for

the single. widowed, cm divorced (fig. 6). ‘Ibiswas true of all age groups except for the J .5-19.SO-84, and the 85-years-and-over groups. Di-vorced persons showed thehighest suicide rates-

from three to five times the rates for marriedpersons under 65 and from two to three timesg;-eater in age groups over 65. The rates forwidowed persons were eight times those for

married persons at ages 20-24, but this ratio<Tradually declined with age and the rates W3-e.-almost identical after reaching age 60. Suicide

rates for single persons in most age groups. ranged from l!i to 22 times those for marriedpersons.

Total suicide rates, for persons 15 years ofage and over, which are not adjusted for ageshow single persons with the lowest rate. Thisresults from the disproportionate number ofsingle persons (as contrasted with the married,widowed, and divorced persons) in the populationat the younger ages where suicide rates are

lowest. When the rates are adjusted for age.they show that married persons do indeed havethe lowest suicide rate, with the single, widowed,and divorced ranking next, in that order. Theage-adjusted rates for persons 15 years and overare 11.9 for married, 20,9 for single, 23.8 forwidowed, and 39.9 for divorced persons. (How-ever, of the four color-sex groups, only whitefemales showed a higher rate for the divorcedthan for the widowed.)

The suicide rates (age-adjusted) for malesvaried from three to four tim,es the rates for fe-males, with the exception of the widowed wherethe rate for males was over six times that forfemales. The suicide rates for white personswere approximately twice as large as the ratesfor nonwhite persons, with the exception ofdivorced personspersons was over

for whom the rate for whitefour times that for nonwhite.

TRENDS

The age-adjusted suicide rate was the samein 1964 as it had been in 1950. It was slightlylower for white males and slightly higher forwhite females, nonwhite males, and nonwhitefemales. However, review of the age-adjustedrates for the intervening years shows that, ingeneral, rates were a little lower for the 1950’sthan for the 1960’s (through 1964) for all fourcolor-sex groups (fig. 7).

The sex differential narrowed between 1950and 1964, The ratio between suicide rates forwhite males and white females declined from 3!ito 1 in 1950 to a little less than 3 to 1 in 1964.A similar decline occurred among the nonwhitepopulation. The suicide rate for nonwhite maleswas four times the rate for nonwhite females in1950, declining to three and one-third times in1964.

The age-specific rates for the 1950-64 periodshow opposite trends for the younger and the olderage groups. Figure 8 shows a slightly upwardtrend for the younger and middle ages, and aslightly downward trend for the older ages.

Suicide rates in 1964 for both white malesand white females were generally higher thanin 1950 at the younger ages and lower at theolder ages. The rate for nonwhite males washigher in 1964 for all age groups except the55-64, 65-74, and the 85-years-and-over agegroups. Likewise, the suicide rate for nonwhitefemales was higher in 1964 for all age groupswith the exception of the 5-14, 45-54, and the75-84 age groups.

QUALIFICATIONS OF DATA

The rates shown in this report are based ondeaths occurring in the United States, with Alaskaadded in 1959, and Hawaii in 1960. Alaska andHawaii are included in all of the 3-year-averagerates for 1959-61. Deaths among nonresidentaliens in the United States are included. Deaths

among Armsd Forces overseas and among U.S.citizens living abroad are excluded. Data pre-sented for States, geographic divisions, standardmetropolitan statistical areas, and metropolitan-ncmmetropolitan counties are by place of resi-dence, For nonresident aliens the place ofresi-dt?nce is considered the same as the place ofoccurrence.

Cause of Death

Since 1900 the causes of death have beenclassified according to seven different revisionsof the International List of Causes of Death.Each revision has produced some breatis in thecomparability ofcause-of-death statistics. How-ever, suicide is among the causes for which theclassifications are essentially comparable forall revisions.

The Sixth Revision of the International Listsof Diseases and Causes of Death, adopted by theWorld Health organization in July 1948, was usedfor mortality data in the United States from1Y.N through 1957, The Seventh Revision, adoptedin 1Q55, has been used since 1958.

In order to estimate the effect of the SeventhRevision on the comparability of mortality trendsfor various causes, a 10-percent sample ofdeaths in 195S was classified using both theSixth and the Seventh Revisions. Comparabilityratios were computed by comparing the numberof deaths assigned to a particular cause by theSixth Revision with the number of deaths assignedby the Seventh Revision. In the case of suicidefor 1958, there were 1,729 suicides assigned ac-cording to the Sixth Revision and 1,786 suicidesassigned by the Seventh Revision, giving a com-parability ratio of 1.03. (See Vital Statistics—Special Reports, Vol. 51, No. 4.)

The comparability ratios by age, sex, andcolor for the most part were very close to theoverall ratio of 1.03. The ratio was 1.03 for thewhite and 1.06 for the nonwhite population. Thecomparability ratios by age groups for all sui-cides ranged from 1.02 to 1.05. Ratios for meansof injury varied from 1.02 for suicides com-mitted with firearms and explosives to 1.55for those caused by jumping from high places.

These ratios reflect also a change in theinterpretation of “self-inflicted” injuries which

was made at the time the Seventh Revision wasput into use, Beginning with data for 19S8, deathsfrom “self-inflicted” injuries not designated aseither a suicide or an accident were classified assuicides. Formerly they had been classified asaccidents.

In both the Sixth and the Seventh Revisionsof the International Lists tl. e suicide categorynumbers were E963, and E970-E979. Exceptfor the average annual rates for 1959-61, ratespresented in this repmt were computed fromdeaths assigned to these categories. The 1959-61average annual rates were taken from tabulationswhich excluded category E963. However, duringall 3 years, only 15 deaths were assigned to thisnumber,

Ranking Causesl of Death

The ranking of suicide among the leadingcauses of death is based on the ranking of causeswhich are included in the table listing 60 selectedcauses of death, published annually in Vital Sta-tistics of the United States.

Race

The category “white” includes in addition topersons reported as white, those reported ashlexican, Puerto Rican, or Cuban and, until 1964,those with race “not stated. ” In 1964, deathswith race “not stated” were assigned to whiteor Negro on the assumption that the distributionof “not stated” events by race is the same as thatof deaths with race stated. No allocation of “notstated” was made to the other nonwhite groups.The category “nonwhite” consists of personsreported as Negro, American Indian, Chinese,and Japanese; other nonwhite groups; and personsof mixed white-nonwhite races.

Deaths by race were not available for NewJersey in 1962 and 1963. The item was omittedon the death certificate at the beginning of 1962and was not reinstated until the end of 1962. The1963 statistics were incomplete because of thecontinued use of the certificates omitting therace item. Consequently, rates for color for theUnited States exclude New Jersey in both 1962 and1963.

Marital Status

Marital status is tabulated as either single(never married), married, widowed, divorced, ornot stated. For the 3 years of 1959-61, maritalstatus was not stated for 1.2 percent of the per-sons committing suicide, Of the four color andsex groups, nonwhite males had the highest per-centage of “not stated” suicides (3.2 percent).

Geographic Classification

Standard metropolitan statistical areas.—The standard metropolitan statistical areas(SMSA’S) used in this report are basically thoseestablished by the Bureau of the Budget and usedby the Bureau of the Census. The only differencesoccur in the New England States. In New England,the metropolitan State economic area (MSEA),established by the Bureau of the Census and com-posed of county units, is used. The only SMSA’sfor which data are shown in this report are thosewith a population of 500,000 or more in 1960.

Metropolitan and nonmetropolitun coutities.-Counties which are included in SMSA’S or, inNew England, MSEA’S, are called metropolitancounties; all other counties are classified asnonmetropolitan.

Rates

All rates are shown per 100,000 population.In many cases, the rates are shown beyond thelast significant figure, merely for conveniencein computation and publication,

Rates based on fewer than 20 deaths areshown with an asterisk to indicate that thesefigures are subject to large fluctuations. Thenumber “20” is arbitrary and is not set forthas a critical point distinguishing statistic llyreliable rates from unreliable rates.

The age-adjusted rates presented in thisreport were computed by the direct method, thatis, by applying the age-specific death rates forsuicide to the standard population distributed byage. The total population as enumerated in 1940was selected as the standard. The rates for thetotal population and for each color-sex group

were adjusted separately, using the same standardpopulation. It is important not to compare age-adjustecl rates with the crude rates shown inother tables.

population Bases

Rates are computed on the bases of populationstatistics made available by the U.S. Bureau ofthe Census. They include populations present inthe specified areas, including the Armed Forcesstationed in the area, but exclude the ArmedForces overseas and persons living abroad.

Populations for 1950 and 1960 are enumeratedas of April 1. For all other years the populationestimated as of July 1 was used. The rates forthe years 1951 through 1959 are based on thefinal intercensal population estimates (P-25,No. 310) and, as a result, may differ slightlyfrom rates published earlier which were basedon provisional population estimates. Populationestimates by color for 1962 and 1963 excludeNew Jersey.

The 3-year average annual rates were com-puted using the number of deaths occurring forthe 3 years and the decennial enumerated popu-lations, multiplied by 3.

Some of the 1960 population data publishedby the Bureau of the Census was obtained on acomplete-count basis in the 100-percent enumer-ation and some on a 25-percent sample enumera-tion. There are differences between these twosets of published figures, with the greatest dif-ferences in the older age groups, For the totalUnited States, the largest percent differences forsex-color- and 5-year-age groups are for whiteand nonwhite males 85 years of age and over.

As a general rule, the population seriesbased on 100-percent enumeration are used incomputing vital statistics rates. Exceptions aremade where populations are required for charac-teristics which were not covered in the completecount, In this report, death rates for suicide bymarital status are exceptions; they are based on@pulations obtained from the 25-percent sample.As a result, the rates appearing in table 6 for allmarital status groups combined are slightly dif -ferent from those in table 5, The only rateswhich can be compared directly from these twotables are the total U.S. rates for the group85 years and over by color and sex. For each ofthe nine categories, the rate shown in table 6 ishigher (ranging from 6.1 to 8.6 percent) than thecorresponding rate shown in table 5.

Programs and Collection Procedures. -Reports which describe the general programs of the National

Center for Health Statistics and its offices and divisions, data collection methods used, definitions, andother materiaf necessary for understanding the data.

Data Evaluation and Methods Research .–Studies of new statistical methodology including experimentaltests of new survey methods, studies of vital statistics collection methods, new analytical techniques,objective evaluations of reliabilityy of collected data, contributions to statistical theory.

Analytical Studies. –Reports presenting analytical or interpretive studies based on vital and healthstatistics, carrying the analysis further than the expository types of reports in the other series.

Documents and Committee Reports. –Final reports of maior committees concerned with vital andhealth statistics, and documents such as recoin-mended model vital registration laws and revised bi~hand death certificates.

Series 10. Data from the Health Interview Survey. –Statistics on illness; accidental injuries; disability; use ofhospital, medical, dental, and other services; and other heafth-related topics, based on data collected ina continuing nationaf household interview survey.

Series 11. Data from the Health Examination Survey. –Data from direct examination, testing, and measurementof national samples of the civilian, noninstitutionalized population provide the basis for two types ofreports: (1) estimates of the medically defined prevalence of specific diseases in the United States andthe distributions of the population with respect to physical, physiological, and psychological charac-teristics; and (2) analysis of relationships among the various measurements without reference to anexplicit finite universe of persons.

Series 12. Data from the Institutionalized Population Surueys.-Discontinued effective 1975. Future reports fromthese surveys will be in Series 13.

Series 13. Data on Health Resources Utilization. –Statistics on the utilization of health manpower and facilities

providing long-term care, ambulatory care, hospital care, and family planning services.

Series 14. Data on Health Resources: Manpower and Facilities. -Statistics on the numbers, geographic distrib-

ution, and characteristics of health resources including physicians, dentists, nurses, other health occu-

pations, hospitals, nursing homes, and outpatient facilities.

Sers-es20. Data on Mortality. –Various statistics on mortality other than as included in regular annual or monthlyreports. Special analyses by cause of death, age, and other demographic variables; geographic and, timeseries analyses; and statistics on characteristics of deaths not available from the vital records, based onsample surveys of those records.

Sen”es 21. Data on Natality, Marriage, and Divorce. –Various statistics on nata]ity, marriage, and divorce other

than as included in regular annual or monthly reports. Special analyses by demographic variables;

geographic and time series analyses; studies of fertility; and statistics on characteristics of births notavailable from the vital records, based on sample surveys of those records.

Series 22. Data from the Nafi”onal Mortality and Natality Surveys. –Discontinued effective 1975. Future rep~rtsfrom these sample surveys based on vital records will be included in Series 20 and 21, respectively.

St+es 23. Data from the Nationcl Survey of Family Growth. –Statistics on fertility, family formation and disso-lution, family planning, and related maternal and infant health topics derived from a biennial survey ofa nationwide probability sample of ever-married women 1544 years of age.
