Certified Home Health Agency (CHHA), is a trusted resource for children
and young adults with disabilities and medically complex conditions. St.
Mary’s Home Care provides short-term skilled care for patients from
newborn to 44 years of age. Our goal is to provide extraordinary care
and support services in the home to help patients regain independence
and achieve a better quality of life.

We understand the important role family plays in a patient’s recovery.
At St. Mary’s Home Care, we use a family-centered, team approach to
care. Our compassionate team of clinicians works closely with discharge
planners before the patient arrives home to ensure a smooth transition.
We continue to coordinate care with the patient’s physician, family, and
other health professionals during the course of treatment. Additionally,
we are there to assist patients and families overcome any fears or
concerns about caring for a loved one at home.

Pediatric Day Healthcare Program
Therapeutic After School, Weekend and Summer Camp Program
St. Mary’s Healthcare System for Children, a national leader in pediatric
post-acute care, offers dynamic after-school, weekend, and summer
camp programs for children ages 6-18 years and young adults from 18-25
years of age who have medical, physical, and developmental disabilities.
Located on the park-like grounds of St. Mary’s flagship facility in Bayside,
NY, St. Mary’s Pediatric Day Healthcare Program (PDHC) provides
therapeutic, rehabilitative, and recreational activities to help children
and young adults with special healthcare needs reach their full potential.
Children receive state-of-the-art rehabilitative and medical care while
participating in a variety of supervised indoor and outdoor play, sports,
arts & crafts, field trips, and other fun therapeutic recreation and social
activities. The program also includes onsite homework assistance, a hot
meal, and round-trip transportation.
The Young Adult program includes a focus on vocational skills. St. Mary’s
counselors work with participants to promote their skills, confidence, and
independence in a nurturing and fun environment. Family members are
also educated on the critical roles they play in the child’s success.
Eligibility
The program is open to children 6-18 years of age and young adults from
18-25 years of age who have a medical diagnosis and a physical and/or
developmental delay. The child must be eligible for Medicaid or Managed
Medicaid and require medical services or therapy.

Care at Home
Specialists at Caring for Children with Medical Fragility at Home
St. Mary’s Care at Home Program allows children with medical fragility
and complex conditions to be cared for at home, keeping the family
together and the child involved in family life. The program is designed for
children from birth to 18 years of age. Services include but are not limited
to comprehensive case management, home health aides, nursing care,
and the evaluation and coordination of a child’s therapeutic, medical,
educational and psychosocial needs. Home adaptation and vehicle
modification are also available to improve movement and/or safety for the
child at home and to improve access from home to the community.
There are five levels of the Care at Home Program. The first two (Levels I
and II) fall under the Department of Social Services (DSS); the remainder
are overseen by the New York State Office for People with Developmen-
tal Disabilities (OPWDD).

Medicaid Service Coordination
Caring for Children and Adults with Developmental Disabilities
St. Mary’s provides case management services to children and young
adults with developmental disabilities, many of whom have complex
medical conditions, through Medicaid Service Coordination (MSC), a
program administered through the Office for People with Developmental
Disabilities.
MSC is provided by qualified service coordinators and uses a person-
centered planning process in developing, implementing, and maintaining
an Individualized Service Plan. The Medicaid Service Coordinator
assists eligible individuals and their families in gaining access to services
and supports appropriate to their needs.
MSC promotes the concepts of choice, individualized services and
supports, and consumer satisfaction. Medicaid Service Coordinators
are experts on community services, resources, and programs. MSC is
provided to people with developmental disabilities, including intellectual
disabilities, autism spectrum disorders, cerebral palsy, epilepsy, and
other neurological impairments.
Focus
The focus of MSC is to assist individuals in achieving their unique goals
such as having a home, a job, a circle of friends, enjoyable leisure,
activities, and obtaining various services and supports.
Eligibility
In order to be eligible to receive MSC, a person must:
Have a documented diagnosis of developmental disabilities
Be enrolled in Medicaid
Have a need for ongoing and comprehensive service coordination
Live outside of a Medicaid-funded setting that provides service
coordination

St. Mary’s Hospital for Children is New York City’s only pediatric post-
acute facility, caring for children with special healthcare needs and
medically complex conditions. Children and their families come to St.

is a home away from home for our young patients. The Children’s
Pavilion includes four patient neighborhoods that house generously-
sized patient rooms, Great Rooms for play and dining, floor to ceiling
views of Little Neck Bay, and a personal entertainment and education
console at each bed. From the colorful windows in the main lobby to
leading edge respiratory and rehabilitation technology, every detail
plays an integral role in a child’s healing.
St. Mary’s broad array of programs and services are designed to
provide the most goal-oriented medical care that addresses all aspects
of the healing process, while encouraging growth and independence.
Our family-centered philosophy ensures that parents and caregivers
are partners in their child’s care. Multi-disciplinary teams of pediatric
specialists in medicine, nursing, rehabilitation, behavioral health, social
work, and child development work with parents and caregivers to
coordinate each child’s individualized care plan to ensure the best
outcomes.
In addition to receiving medical care and rehabilitation therapies,
children attend school, participate in onsite and off-campus recreational
activities, and are encouraged to socialize, play, and just be kids.

Center for Pediatric Feeding Disorders
It has been reported that 25% of all children have some form of feeding
disorder. This number increases in children with developmental delays.
Feeding disorders occur when a child has difficulty consuming nutrition
by mouth to promote physical and cognitive growth.
The Cindy and Tod Johnson Center for Pediatric Feeding Disorders at
St. Mary’s Hospital for Children in Bayside, NY, offers an interdisciplinary
evaluation service and the only intensive day patient program in New
York State to help diagnose and treat feeding disorders.
The program uses a team approach, which includes the disciplines of
medicine, nutrition, behavioral psychology, and speech language
pathology to identify the factors contributing to each child’s feeding
difficulties and develop a plan to address their needs.
The team focuses on the whole child, addressing clinical and behavioral
aspects of the child’s feeding disorder. Designed with a family-centered
behavioral approach, our intensive program identifies strategies to assist
children who present with eating difficulties such as:

St. Mary’s Early Education programs serve preschool children with
special needs and medically complex conditions. Our programs address
the academic, social, emotional, and therapeutic needs of our children,
helping them to gain independence and confidence.
St. Mary’s is a New York State approved provider of Evaluations,
Preschool, and Special Education Itinerant Teacher (SEIT) Services.
We offer a preschool for children with medical fragility on our Bayside
campus, as well as evaluation and SEIT services at home and in the
community. It is our goal to serve the specific needs of every child and
his/her family.
Preschool Classrooms
St. Mary’s Preschool in the Queens Borough President Helen Marshall
Early Education Center is the only preschool in NYC dedicated to caring
for children with the highest levels of medical complexity. It is located on
the grounds of St. Mary’s Hospital for Children in Bayside, NY. Our staff
specialize in the unique needs of children who are medically fragile and
their families.
Evaluation Team
St. Mary’s Evaluation Team offers comprehensive English, bilingual,
and diagnostic (including Autism Spectrum Disorders), multi-disciplinary
evaluations. Our team of education professionals uses various assess-
ment tools to determine eligibility for the Committee on Preschool
Special Education Program (CPSE). Evaluations are conducted on
site, at our Bayside campus, or in the community.
SEIT Services
A Special Education Itinerant Teacher or SEIT is a licensed special
education teacher who works with young children with disabilities in
early childhood programs, as well as young children who are medically
fragile at home or in a residential facility. SEITs assist in the designing
of learning environments and activities that promote each child's devel-
opment in the least restrictive environment.
