RISK MANAGEMENT

FREQUENT (A) Occurs very often, continuously experienced

- Single item occurs very often in service life. Expected to occur several times over duration of a specific mission or

operation. Always occurs.

- Fleet or inventory of items occur continuously during a specific mission or operation, or over a service life.

- Individual soldier occur very often in career. Expected to occur several times during mission or operation. Always occurs.

- All soldiers exposed occur continuously during a specific mission or operation.

LIKELY (B) Occurs several times

- Single items occur several times in service life. Expected to occur during a specific mission or operation.

- Fleet or inventory of items occur at a high rate, but experienced intermittently (regular intervals, generally often,).

- Individual soldier occur several times in career. Expected to occur during a specific mission or operation.

- All soldiers exposed occurs at a high rate, but expected intermittently

OCCASIONAL (C) Occurs sporadically

- Single item occurs some time in service life. May occur about as often as not during a specific mission or operation.

- Fleet or inventory of items occurs several times in service life.

- Individual soldier occurs some time in career. May occur during a specific mission or operation, but not often.

- All soldiers exposed occur sporadically (irregularly, sparsely, or sometimes).

SELDOM (D) Remotely possible, could occur at some time

- Single item occurs in service life, but only remotely possible. Not expected to occur during a specific mission or

operation.

- Fleet or inventory of items occurs as isolated incident. Possible to occur sometime in service life, but rarely. Usually does not

occur.

- Individual soldier occurs as isolated incident during a career. Remotely possible, but not expected to occur during a specific

mission or operation.

- All soldiers’ exposed occur rarely within exposed population as isolated incidents.

UNLIKELY (E) Can assume will not occur, but not impossible

- Single item occurrence not impossible, but can assume will almost never occur in service life. Can assume will not

occur during a specific mission or operation.

- Fleet or inventory of items occurs very rarely (almost never or improbable). Incidents may occur over service life.

- Individual soldier occurrence not impossible, but may assume will not occur in career or during a specific mission or

operation.

- All soldiers exposed occurs very rarely, but not impossible.

CATASTROPHIC (I)

- Loss of ability to accomplish the mission or mission failure. Death or permanent total disability (accident risk).

Loss of major or mission-critical system or equipment. Major property (facility) damage. Severe environmental

damage. Mission-critical security failure. Unacceptable collateral damage.

CRITICAL (II)

- Significantly (severely) degraded mission capability or unit readiness. Permanent partial disability, temporary total

disability exceeding 3 months time (accident risk). Extensive (major) damage to equipment or systems. Significant

damage to property or the environment. Security failure. Significant collateral damage.

MARGINAL (III)

- Degraded mission capability or unit readiness. Minor damage to equipment or systems, property, or the

environment. Lost day due to injury or illness not exceeding 3 months (accident risk).

NEGLIGIBLE (IV)

- Little or no adverse impact on mission capability. First aid or minor medical treatment (accident risk).Slight

equipment or system damage, but fully functional and serviceable. Little or no property or environmental damage.

RISK ASSESSMENT

Risk Assessment Code Matrix

ACCIDENT PROBABILITY

HAZARD SEVERITY

Catastrophic: Involving a catastrophe

Critical: A state of crisis or emergency

Marginal: A lower risk

Negligible: Not significant amount of risk

Risk Level Definitions

E - Extremely High

- Loss of ability to accomplish the mission if hazards occur during the mission. A frequent or likely probability of catastrophic

loss (IA or IB) or frequent probability of critical loss (IIA) exists.

H - High

- Significant degradation of mission capabilities in terms of the required mission standard, inability to accomplish all parts of the

mission, or inability to complete the mission to standard if hazards occur during the mission. Occasional to seldom probability of

catastrophic loss (IC or ID) exists. A likely to occasional probability exists of a critical loss (IIB or IIC) occurring. Frequent

probability of marginal losses (IIIA) exists.

M - Moderate

- Expected degraded mission capabilities in terms of the required mission standard will have a reduced mission capability if

hazards occur during mission. An unlikely probability of catastrophic loss (IE) exists. The probability of a critical loss is seldom

(IID). Marginal losses occur with a likely or occasional probability (IIIB or IIIC). A frequent probability of negligible (IVA)

losses exists.

L - Low

- Expected losses have little or no impact on accomplishing the mission. The probability of a critical loss is unlikely (IIE), while

that of a negligible loss is likely or less (IVB through IVE).

Risk Level/Risk Decision Approval

* Extremely High Risk - Division

* High Risk - Brigade

* Moderate Risk - Squadron

* Low Risk - Troop/Platoon

NOTES:

1. * Before risk is mitigated by use of administrative controls.

2. Risk decisions will be made two levels up.

SPECIAL APPROVALS: Missions that must be approved by the Commanding General or the designated ADC.

- All helicopter missions over water.

- All flight missions out of corridor beyond the "No Fly" Line.

- River crossing operations

- Non-standard live fire operations (SOF, etc.)

- * Squadron deep attack

- * Artillery operations requiring a SDZ waiver

- * CALFEX (TF or mix of direct, indirect, and aerial gunnery operations)

- * Troop+ force-on-force (mix of dismounts and wheel and tracked vehicles)

- * Squadron+ motor march

HOT WEATHER TRAINING

Heat Injury

(1) Table 3-5 below should be applied as guidance for the prevention of heat injuries in acclimatized individuals doing moderate

levels of work.

(2) In situations where work is heavy or prolonged, the risk of heat injury exists at approximately Wet Bulb Globe Temperature

(WBGT) = (0.8 WGT) + (0.2 Dry Bulb Temp). For additional information refer to FMs 21-10 and 21-10-1. 3ID 15TH ASOS

(DSN 870-475-3031) maintains status of local Heat Categories for all Division camps while in Garrison.

(3) Commanders and leaders at all levels must be knowledgeable and enforce measures for preventing heat injuries as outlined in

FM 4-21.11.

Table 3- 5: HEAT CATEGORY/WATER INTAKE GUIDE

Notes:

*Mission Oriented Protective Posture (MOPP) gear or body armor adds at least 10 degrees Fahrenheit to the WBGT index.

**Suspend physical training and strenuous activity. If operational (non-training) mission requires strenuous activity, enforce

water intake to minimize expected heat injuries.

(4) During and after physical activity:

(a) Supply adequate water and enforce its intake before, during, and after physical activity.

(b) Adjust the work or training activities to correlate with the environmental heat stress conditions.

(c) Provide adequate rest periods for physically active personnel in a hot environment.

(5) Commanders should be aware of the following concepts to counter heat injuries:

(a) Provide Soldiers chilled water.

(b) Allow acclimatization. In general acclimatization applies to all new personnel, but it can also apply to new increased levels

of activity, such as foot marching.

(c) Ensure Soldiers consume at least two meals per day. Unless directed by medical personnel, salt supplementation in water

supplies should not be taken as a prevention or treatment measure.

(d) Don't be fooled by a "safe" WBGT index, as most heat injuries occur during early morning training (especially physical

training), when the air temperature is at its lowest point and the relative humidity is highest. This condition results in a low

WBGT, but does not allow for adequate cooling by the evaporation of sweat. Individuals exercising at this time are less likely to

drink adequate volumes of fluid prior to training. Ensure your troops are well hydrated and rested before conducting strenuous

physical training regardless of the WBGT index.

(6) Take measurements in the field and garrison using the standard Wet Bulb Globe Temperature (WBGT) kit.

(7) All heat casualties requiring hospital admission and/or clusters of heat injuries must be reported promptly (within five days)

8-STEP TRAINING MODEL

Training Event / Date:_______________________________________________________

STEP 1 - PLAN THE TRAINING (T-8 TO T-7)

Assigned platoon and commander develop a training plan based on input from subordinate leaders. Commander

allocates the training resources required to sustain or improve proficiency of Soldier, leader and collective tasks. Commander

ensures that subordinates know and train to the standard.

____Training Objective(s) or Tasks

____ID Resources required (land/facility, transport, ammo, trainers, TAS-C, medics)

____Coordination/Submit Resource Request

STEP 2 - TRAIN AND CERTIFY LEADERS (T-7 TO T-6)

Ensure that trainers are trained before deploying to the field or training site. Set up trainers for success by providing

them the discrete preparation time, resources, and knowledge required to successfully execute the training event.

____OIC/RSO

____Trainers/NCOIC identified (by name)

STEP 3 - RECON THE TRAINING SITE (T-6 TO T-5)

Ensure the training site meets the conditions outlined in each of your training objectives. Also, use the recon to perform

an on-site risk assessment and identify necessary risk reduction measures.

____Site Reconnaissance

____Trainers Identified and trained/certified (as needed)

____Back brief CDR the initial plan (verbal) for OPORD issue

STEP 4 - ISSUE THE PLAN (T-4 TO T-2)

The training plan identifies soldier, leader, and collective tasks to be trained.

____OPORD/FRAGO (Mission Statement)

____Timeline

____CO Order of March/participating personnel identified

____Details assigned (guard, briefer, police call, etc)

____Resources Confirmed (land/facility, transport, ammo, trainers, TAS-C, medics)

____Risk Assessment

STEP 5 - REHEARSE (T-1)

Rehearsals are conducted in order to identify weaknesses in the training plan as well as teaching effective training

techniques. A good rehearsal will ensure the trainer is comfortable with the task to be trained and all safety considerations have

been met.

____PMI/PCC/PCI

____Rehearse

STEP 6 - EXECUTE (T WEEK)

Successful execution of training is conducted by tactically and technically proficient leaders who actively participate in

the training event.

STEP 7 - CONDUCT AAR (T+1)

After each training event, the Troop will conduct an AAR to identify what happened, why it happened, and how to

correct any shortcomings. Written format will follow will Issue, Discussion, Recommendation.
