Special Needs Placement - Background Information Dear Student Welcome to the Special Needs Physiotherapy Service of STHCT. The borough of South Tyneside includes South Shields, Whitburn, Cleadon, Boldon, Hebburn and Jarrow. The service is a community paediatrics service, providing for children of all ages. Most of the team are based at Palmer Community Hospital in Jarrow, but the service is provided across a number of locations throughout the borough. Referrals are received from a variety of sources including patients, carers, other health professionals, school and college staff, GPs and consultants. We have a holistic approach within the special needs service. With the children and adults we see, many aspects of development have to be taken into consideration, in addition to the physical elements in which we specialise. South Tyneside is also an area of economic and social deprivation, which can have a significant impact on the physical health (and potential health outcomes) of many of the patients in our care. The overall management of many of the conditions/physical problems we encounter requires a multi-disciplinary (and often multi-agency) approach. Consequently there is more of an overlap into other therapy/support roles than in other areas of physiotherapy practice. Our service has a reputation for friendliness, flexibility and adaptability. This approach enables the team to work successfully across a wide area, at a number of locations, with individuals who respond positively to a more informal approach to their care. Some of the placement will be spent at the Special Schools in the borough for children with physical disabilities and children with severe learning disabilities. Conditions may include cerebral palsy, muscular dystrophy, downs syndrome, and retts syndrome. Physiotherapy is arranged, where possible, around the academic timetable. The physiotherapy team encourage an integrated approach to the management of physical problems, where emphasis is placed on the transfer of skills developed through physiotherapy, to the classroom and home situation. You will be involved in orthotic and wheelchair clinics. There may also be opportunities to attend a variety of local and regional clinics including those for children with development co-ordination disorder, developmental delay, and for children with gait anomalies. Children’s outpatient physiotherapy at Palmer Community Hospital involves the assessment and treatment of children in co-operation with parents and carers. Conditions may include rheumatoid arthritis, perthes, torticolis, talipes, erbs palsy, developmental delay, DCD, and gait problems. We are a supportive team, and although we cannot guarantee an exceptional educational experience for every student, we will ensure you leave with some good experiences and a working knowledge of the specialist area of Special Needs. Special Needs Placement – Learning Opportunities There are opportunities for:- * Work with a wide range of clinical conditions including cerebral palsy, muscular dystrophy, developmental co-ordination disorder (DCD), developmental delay, erbs palsy, juvenile rheumatoid arthritis, cystic fibrosis etc * Developing orthopaedic, neurological and biomechanical assessment and treatment skills. *Familiarisation with a variety of assessment tools including the Gross Motor Function Measure, Tinnetti, Chailey Levels of Ability, Selective Lower Limb Examination, the Ashworth Scale, the Alberta Infant Motor Scale etc. * Work across the whole age spectrum including pre-school children, school children, children in transition and adults * Work across a wide variety of community settings including Palmers Community Hospital, Special Schools, Adult Education Colleges, patients homes, Residential & Nursing Homes, Community Centres and Respite Homes * Participation in assessment/ treatment sessions with an Extended Scope Practitioner – Specialist in gait analysis and work with pre-school children with developmental delay

* Developing skills in hydrotherapy in special school settings. Includes handling and Haliwick skills, developing training programmes etc. * Developing IT skills, including accessing ‘Centricity’ the Trusts Digital X-Ray site, and the opportunity to work with physiotherapy soft ware packages including ‘Physio Tools’ etc. * Participation in Specialist Clinics, including

Wheelchairs. Assessments/Reviews with the Wheelchair Therapist and Engineers, including provision of specialist support systems, matrix, moulds, and foam insert seating systems. Hospital based and limited referrals to Disability Centre Clinic at Freeman Hospital;, Newcastle

Orthotics. Community and hospital based. Assessment and provision work with a specialist senior orthotist. Provides further opportunities to develop gait and biomechanic assessment skills

Gait Analysis. Orthopaedic Consultant and Extended Scope Practitioner led Regional Service based at Newcastle General Hospital. Utilises force platforms, vector and video analysis.

Spasticity. Attendance at Childrens Disability Clinics with patients with complex neurological problems

The Special Needs Team. A joint physiotherapy/occupational therapy childrens community service. Collaborative assessment/treatments in a seamless care service.

The Private & Voluntary Sector. Working alongside independent sector nursing and care staff. Opportunities to develop health-care training skills

* Developing Communication and Inter-Personal Skills, with

Interactions with patients with mild, to severe, communication and comprehension difficulties

Presentations. At case conferences, MDT referral and review meetings, children’s school reviews etc.

Information packages. For long-term management of chronic disability, postural management, training and rehabilitation programmes for children with DCD, developmental delay etc, using a variety of media.

Advice. Advising patients, their carers and care organisations in the use of specialist equipment, orthotics etc.

Teaching. In 1 to 1 and group sessions with care/support staff, with carer organisations, educational staff, the background and problems associated with relevant conditions and encompassing basic anatomy, physiology, and biomechanics

Liason. Communicating between carers and a variety of service providers of health & social care

Statementing. Providing reports and review information for childrens Statement of Educational Needs

* Developing skills in the management of chronic physical disability, with

24 hour Postural Management. Includes assessment/advice for positioning profiles for spasticity management including wheelchairs, special seating and sleep- systems

Moving & Handling. Assessments, equipment provision, teaching and risk assessment

Special Needs Placement – Aims and Objectives Overall Aim

The main aim of the placement is to facilitate student’s experience with the assessment, treatment and long term management of children with special needs. General Objectives

By the end of the placement you will be able to:

Demonstrate assessment skills for a variety of common conditions encountered in the clinical area of Special Needs

Demonstrate skills in goal setting and the formulation of treatment plans (using standardised outcome measures where appropriate)

Carry out effective treatment programmes with patients of a variety of ages and abilities

Demonstrate an understanding of patient pathways for referral and treatment

Demonstrate good communication skills with patients of a variety of ages and cognitive abilities

Show an appropriate use of initiative in professional development by responsible use of study space, having access to a computer with intranet and internet access, and use of the Journal and Book Libraries

Show an understanding of the role of the physiotherapist in Special Needs and the variety of settings in which work is undertaken

Show a working knowledge of the other agencies involved in the management of individuals with Special Needs eg. Social Care & Health, Education, Voluntary Agencies etc.

Demonstrate an ability to work within a multi-disciplinary (and multi-agency) team effectively

Show an understanding of professional working practices, boundaries and ethics as they affect the delivery of Physiotherapy Services within Special Needs
