2. Confidentiality Statement:

It is recognised that students will have access to sensitive information about people’s lives during their practice, and that they will need to abide by placement agency and university policies in relation to confidentiality and professional ethics. It is also recognised that students will need to draw on material relating to their practice with service users, family members, carers and agency staff in order to evidence their competence. All identifying features of service users must be anonymised in the portfolio; this includes names, addresses, dates of birth and any individual features that because of their unusual characteristics may lead to the service user being identified. Students must seek the agreement of the Practice Educator before presenting any evidence based on agency documentation. When referring to professionals within this document, please use generic professional terminology, rather than specific names (with the exception of the Practice Learning Agreement and where professional signatures are required). It is good practice to seek the consent of service users before using details of their lives for written assignments. Any exceptions to this policy will be clearly stated within the documentation.

Portfolios that are considered to breach confidentiality may be marked as a fail. Please complete the form below to indicate confirmation that confidentiality has been adhered to:

Please note that the Practice Educator will need sight of your completed portfolio to enable them to sign this statement.

Confidentiality Statement I have removed all information that may enable service users to be identified. I am aware that I may incur a reduced mark or a fail, should I have breached confidentiality (see Programme Handbook for further details).

Student signature: Date: To my knowledge the student has abided by agency policy in relation to confidentiality in compiling their portfolio material. I have read the material presented in the portfolio and can confirm that no service user can be identified.

7 National Occupational Standards: This is part of the required work for Component 1

of the Summative Assessment. This will be assessed pass/fail by the Practice Educator.

Information and Guidance

The National Occupational Standards are used as the basis for the assessment of the student’s practice competence. The Key Roles and Units are detailed in the handbook on the forthcoming pages. Full details of National Occupational Standards, underpinning knowledge requirements and Performance Criteria can be found at the following website: www.skillsforcare.org.uk.

It is anticipated that students will cover all National Occupational Standards across both modules. These standards build on those addressed at Level 4 and will prepare students for demonstrating professional competence at Level 6.

It is expected that students achieve all these identified occupational standards during their practice experience. The student should negotiate with the Practice Educator how to take advantage of the learning opportunities available within the setting in order to meet the standards and what kind of evidence will be needed.

Students should discuss progress in meeting the occupational standards with their Practice Educator and Placement Consultant and make sure they are aware of any difficulties or problems.

The student will need to consistently demonstrate each National Occupational Standard (NOS) for these to be considered as met.

Collecting Evidence of Competence Evidence may be drawn from a number of sources (suggested abbreviations for inclusion in the following proforma are indicated below)

O - Direct Observation.

W - Written Feedback from colleagues, other professionals, co-workers

SU - Feedback from service users. The latter may be in a variety of forms, for example some service users may prefer to have their views recorded, or to draw a picture.

V/A - Video and Audio Recordings, where consent is obtained. Please be mindful of ethical issues related to this practice.

PR - Process recordings of work with service users and carers meetings.

D - Team /individual discussions

S - Supervision Notes
