Calderdale’s Vanguard – the Care Closer
to Home Programme Simplifying the language - talking to people,
communities and stakeholders
Debbie Graham – Head of Service Improvement, Calderdale CCG
Soo Nevison – Chief Officer, Voluntary Action Calderdale
Why? • We had clinical models, specifications, PPI data, diagrams
etc
• We needed to have a meaningful conversation
• So that we could build support and enable our stakeholders to respond
• But it was really difficult for us to tell our story in a way that we truly accessible
• So, we used animated stories; to show the model as
whole and focus on individuals

of Health and Social Care

Individual Stories • Stories describe the impact CC2H has, and will have on people
• We used real patient stories from Calderdale residents
• The stories focus on areas where our work will have the biggest benefit
• We have several now and intend to develop more
• They take time to generate, but have played a powerful role in our
programme
• These can be found at www.youtube.com/watch
• We end with Andrew:
