APPLICATIONS Setting Goals and Empowering Lifestyle Modification T here are many attributes and characteristics that are important to obtaining success in a dietary change program, whether it is for participants in a research study, a group of new moms in WIC (the Special Supplemental Nutrition Program for Women, Infants, and Children), or patients getting ready to go home after a coronary artery bypass graft. It is important to distin- guish which characteristics promote the best control along with assuring that, through dietary adherence to a specific program, overall nutrition quality is maintained. Winters and associates showed that strict adherence to a reduced fat program in the Women’s Intervention Nutri- tion Study (WINS) was obtained by following the five principles of the WINS Low-Fat Eating Plan while still maintaining nutrition integrity (1). Winters and colleagues set out with a group of 163 postmenopausal women treated for early stages of breast cancer. They decided to assess the food choices of those who achieved the fat gram goal of 15% of energy from fat, which equaled approximately 20 to 30 g fat per day. These women were deemed strictly adherent to the as- signed dietary intervention. These participants achieved this adherence by reducing intake of subgroups of the food guide pyramid such as sweet breads and pastries, cheese, beef, pork, and lamb. The participants accom- plished this without compromising micronutrient intake. The most important strategies utilized by the partici- pants included the elimination of high-fat foods, reduced portion sizes, and a decrease in fat grams per serving. Additionally, self-efficacy played a very important role. The authors found that in this group the strictly adherent reported having less difficulty in following the diet, along with reporting more satisfaction with their lives. This adds to the body of evidence that individuals must per- ceive lifestyle modification as accomplishable. This research reinforces the concept of individualizing broad nutrition plans. It is important to understand all aspects of the patient to provide them with the tools to obtain success. The primary goal is to tailor the interven- tion to become achievable and encourage the motivator for adherence. According to Borra and colleagues, nutri- tion messages must engender a sense of empowerment (2). They should also build the patient’s confidence in making healthy choices. To make a goal achievable, encourage the patient to create specific and realistic goals. This process can be done by working with the patient to determine what he or she considers an obtainable goal. The patient can then decide, with the dietitian as a guide, how to take steps to achieve dietary adherence. Allowing the patient to set these goals encourages the sense of empowerment. It is important for both the patient and the counselor to un- derstand why the patient chose these goals. Working with the patient in this process allows the dietitian to under- stand the resources the patient has available to him or her and gives the dietitian insight on the motivator for adherence. Armed with this knowledge, the dietitian can follow up with the patient and provide guidance specific to the patient’s circumstances. The bottom line for dietitians is that we need to provide clear information, encourage specific strategies, and em- power our patients on how to obtain success with dietary intervention programs. This research supports this ap- proach in addition to clearly showing how success can be achieved within a free-living population. References 1.
