Welcome Back to School! District Principal Robert C. Sailer As a stawe are eager and excited for the start of another school year. Last year we began the roll out of PBIS (PosiƟve Behavioral IntervenƟon Supports). This is a program that focuses on “catching” students being good or if they are misbehaving, to focus on re-teaching the expected behavior. NaƟonwide the PBIS program averages 3-5 years for complete roll out and implementaƟon. We are entering year two of the program. As with any new program, some things rolled out very easily last year and some things we needed to tweak and change. During the 2014-2015 academic year, a school store was opened that not only provided miscellaneous school supplies, snacks, and knick-knacks but was also a place where students could turn in their PBIS Ɵckets. Welcome Back to School!

As a staff we are eager and excited for the start of another school year. Last year we began the roll out of PBIS (Posi ve Behavioral Interven on Supports). The sound system is receiving an upgrade that will not only help during athle cs, but during assemblies as well. The Hornet Club is working on crea ng new signage for the Hornets’ Nest to recognize previous outstanding athletes and a host of other things. The Doll Gym received a fresh coat of paint that really brightens up the interior. For li le cost, it will feel like walking into new gyms.

I could con nue wri ng about the good things that are happening in the District, but I would rather invite you to come and see for yourself. We have a host of ac vi es coming up this fall that I strongly encourage you to a end such as: the co-curricular code mee ng, registra on, open house, and many other events.

I hope as you read this ar cle you have no ced a theme - the theme that there is a direct and purposeful eff ort being made to improve facili es and educa onal opportuni es for our students. We want to focus on school climate and make this a place students want to come to everyday. If students are excited to be in school their learning and growth will grow substan ally.

Finally, I would ask you to welcome new staff to our District: Chris Kapinus, Building & Grounds/Transporta on Supervisor; Kirsten Reichmann, Agriculture teacher and FFA advisor; Kathy Oswald, Bus Driver; and Kimberly Jones, Food Service. We have a great staff that is going to work harder than ever to give our children the best educa on possible. Welcome back!

I want to thank each and every member of the community for your support and positive advocacy for the School District of Wauzeka-Steuben. While I have only been in the District for a few years – it has become a “home away from home.” It has truly been an honor and privilege to be able to serve the District and the community.

I have accepted another position with the Fall River School District (90+ miles closer to my home in Jefferson). I am excited to start a new venture and to have the opportunity to spend more time with my family, but will also miss working for Wauzeka-Steuben and the great students, families and community members of the District.

I officially begin my position there in August and so will be working to help provide smooth transitions for both districts over the next few weeks. The School District of Wauzeka-Steuben is a great district. Many things are headed in a very positive direction and there is much for which to be proud. While there is continued work to be done, I know the community and District are up to the challenge! Thank you!

Volleyball Camp was a “HIT”Wauzeka-Steuben played host to JK Elite Volleyball Training Camp for

local athletes July 6-8. 25 area athletes, grades 6 -12 registered for this 3-day opportunity to be coached by Jake Kujak and his assistant Kendall Wienkes.

Kendall is a 2011 graduate of Highland High School and signed a na onal le er of intent to play volleyball at Division 1 Indiana Purdue Fort Wayne (IPFW). She was the fi rst athlete from Highland to receive a Division 1 scholarship right out of high school.

Wienkes performed middle blocking du es for the Cardinals in their run to the Division 4 state tle. She was a 4-year starter, a 4-year all-conference member, was named fi rst team all state tournament, fi rst team all state, and a 4-year Wisconsin Power Volleyball Club player. Kendall successfully added a 4-year starter at IPFW’s 1,000 Dig Club College Player of the Week & other numerous awards, to the list of her accomplishments.

Coach Kujak began training players in western Wisconsin 5 years ago out of his love for the sport of volleyball, and his genuine passion to help our local athletes be successful at all levels of this awesome sport!

Jake Kujak has been involved in organized, compe ve volleyball for the last 25 years. He began his volleyball career as a junior’s club player in Minneapolis,

MN. He was named an All-American player at Club Na onals as a high school senior, and was a gold medalist represen ng Team USA as a Junior Olympian.

He began his college career with the University of Minnesota Golden Gophers as an OH/MB, playing club ball, and later was awarded

a full scholarship to Division I George Mason University in Fairfax, VA, as an OH/RH.

A er college, he con nued playing and serving the following programs:

• 2010 – Holmen High School varsity program• 2011-2012 WTC Cavaliers Women’s volleyball head coach• 2011-present; Viterbo V-hawk Xtreme Club coach, girls volleyball• 2011- present; Asst. coach Badger Region H.P. boys volleyball• 2012-2013; Lead Coach, Top Tier girls volleyball club• 2013 – present; Director and Lead Coach, JK Elite Volleyball• 2011- Gold medalist, USAV High Performance Championships• 2012- Bronze Medalist USAV High Performance Championships

Coach Kujak, along with his wife, daughter, and stepson live in the Coulee Region.

Lockers for Sale

Recently the elementary level was upgraded with new lockers. The former lockers were taken out and are wai ng for new homes. They would make good storage in garages, workshops, basements, playhouses, fi shing shacks, cabins, porches, mudrooms, and other areas as well. Store coats, rakes, shovels, other garden tools, fi shing equipment, mops, brooms, beverages for the lawnmower and rider (like extra bo les of water), those lawn ornaments you want to store over winter – be imagina ve! A aching shelves or pu ng crates in on their sides to construct dividers for stacking items can be very handy. There is a lot of use for old lockers if you want to be crea ve. Paint them to match or brighten the inside of your garage and your neighbors will be envious! Where else could you get extra metal cupboard space for $10 apiece or 6 for $50?

Summer School AdventuresNext year’s 4th-7th grade students built ra s using cardboard, milk jugs, and duct tape in June’s Summer School

Adventures! Then they took the ra s by bus to Boscobel and fl oated them in Sanders Creek. There were fi ve ra s and all fl oated at least halfway between two bridges. A fabulous me was had by all! Students also had the opportunity to golf at the Barn in Prairie du Chien and learned some prac cal Spanish. The class was off ered by Ms. Katarina Jungbluth, Mr. Dave Petrowitz, and Mrs. Karen Leonard. Students a ending the sessions were Carsen & Cohen Redman, Jaden & Morgan Berger, Lily & Blake Mitchell, Grace Zinkle, Jacob & LaVern Atkinson, Ethan Prine, Andrew & Lizzie Udelhoven, Jonah Klema, and Ryan Beers.

July’s group of summer adventurers was led by Ms. Katarina Jungbluth, Mr. Dave Petrowitz, and Mr. Christofer Smikrud. The students built ra s, did some computer programing using Scratch, golfed, and made movies with iMovie on iPads. Those par cipa ng in July’s session were Morgan Berger, Lindsay Krachey, Tamara Marfi lius, Mackenzie Mar n, Grace Zinkle, Jaden Berger, LaVern Atkinson, Jacob Atkinson, Cole Allert, Ethan Prine, and Bre Boylen.

The regular monthly mee ng for the Town of Wauzeka was called to order at 6:36 p.m. on Monday, July 13, 2015 in the Town Mee ng Room by Chairman Tom Jazdzewski.

Verifi ca on was made that the mee ng was properly posted at the Tn. Mtg. Rm., Bush Hollow Rd., Horseshoe Tavern, Oak Ln., and on Radio Sta on W.P.R.E.

Vaccines use the body’s natural defense system to help create immunity. Vaccines trigger the same immune response that a germ might trigger, but not cause illness. Vaccines are our best weapon against widespread illness. The more people in our school who receive their vaccinations the more likely we can curtail the spread of illness. Safe Sitter is a medically accurate course for young adolescents to teach them how to be safe, responsible, nurturing babysitters. The course is offered to 11-13 year olds in recognition of the fact that at this early age, young adolescents are routinely sought out as babysitters. The long term goal is to prepare young people for their future responsibilities as parents. There are many additional benefits to the adolescents including enhanced self-esteem, introductory employment skills, and a number of safe habits and life skills.

Wauzeka Labor Day Celebration 5K Walk-Run

Monday, September 7th, 2015

Come start your Labor Day off with a walk/run. This course will wind throughout the village. All ages are welcome to participate. Registration forms can be picked up at L&M’s BP, Krachey’s BP in Boscobel, all area People State Banks, or Stuckey’s Market in Wauzeka. Entry cost will be $10.00 per person and $5.00 for kids 14 and under. Shirts will be given to all those who pre-register by August 21, 2015; however, entries will be accepted until 7:15 a.m. on September 7th. The walk/run will start at the football field and wind throughout the village.

I. Jazdzewski called the meeting to order at 7:01 p.m. in the Library Media Center of the Wauzeka-Steuben School Building. Roll call was taken. Present were: Tom Jazdzewski, Bill Marfilius, Terri Acker, and Jessie Udelhoven.

Open Meeting Law Compliance: Jazdzewski announced that “On Wednesday the 10th of June, 2015 notification of this meeting - the agenda - was emailed to WPRE and KADR radio stations, the Courier Press, the Boscobel Dial, the District’s website manager, and to the Village Board of Steuben for posting. A copy of the agenda was also posted at L&M’s BP and People’s State Bank in Wauzeka, the faculty/staff lounge bulletin board, and on the North and South entrance of the Wauzeka-Steuben School Building.

II. Community Comments A. MiKayla Walters talked to the Board about dual sports participation. B. Mrs. Vicki Walters presented her concerns about the NHS Program. C. Mrs. Deanna Wallin-Sander declared her thoughts regarding the NHS Program and FFA – in particular, the Petting

Zoo at the Crawford County Fair.

C. After some discussion of the revisions to Policy 431 – Compulsory School Attendance, the Board decided to have the Policy Committee review it at their next meeting.

D. No action was taken on the 2015-2016 Co-Curricular Code of Conduct. It will be brought back to the July regular Board meeting.

E. Motion by Jazdzewski, seconded by Acker to approve a spending cap of $125,000 towards the purchase of a new bus for the 2015-2016 school year. Motion carried 4-0 by voice vote.

F. No action was taken on officially copyrighting the Wauzeka-Steuben Hornet. More information is needed on cost. This will be brought back to the July Board meeting.

VII. Principal Sailer gave his report A. Close of the 2014-2015 School Year – The school year ended strong. Many end-of-the-year activities took place

recognizing positive student success and putting closure on the year. Two years ago brought many changes from state initiatives to local changes in policy and procedures. The school year as a whole was much smoother than the previous school year as we were into the second year of these changes. The only large changes I would foresee for 2015-2016 is with regards to state testing. State testing is still very much up in the air as to what direction the state will be heading.

B. National Honor Society – This year we had a new advisor in the NHS position after Mrs. Luster left the district. This person ran NHS very similar to how she had run it previously in other districts. The overall process itself was acceptable, but the fine details and transparency of the procedures and process drew a lot of attention during the last month of school since it was different than years past. I made the decision to postpone the inaugural ceremony to the very beginning of fall. This will allow me to review and create clear and consistent messaging and expectations of the organization. At the beginning of the 2015-2016 school year we will repeat the application process with the new system and immediately hold the inaugural ceremony in fall. This should not negatively impact the students. In general, the Wauzeka-Steuben NHS does not do anything over summer so the students will not be missing out on anything. Lastly, historically NHS has never been part of the actual national organization. I recently found out we do not technically have an NHS chapter. Going into next year we will begin taking the steps to be a recognized NHS chapter.

C. 2014 – 2015 Seclusion and Restraint – We are required yearly to report this to the school board. This school year a total of two students were restrained.

D. Update on Student Testing – State testing is complete!!!! We met all of the deadlines and completed all of the state expected tasks. At this time we still do not have results for the Badger Exam. The last email I received from DPI stated that it may still be months before we see results. For next year the state is in flux and may not know until late summer or early fall as to what testing will take place. There are many ideas floating around the political spectrum. The good news is that most of the rhetoric is about reducing the amount of testing and providing more choice to schools.

E. Dual Sport Participation – Many students asked me about dual sport participation and I always directed them to Dr. Garrow and informed them of the meeting dates and times when this topic would be discussed. From my understanding, I believe only one student attended a meeting and that was coincidental because their mother was at school. Students started to voice concerns after the decision was made that they had to limit/choose between sports.

F. Summer School – We have excellent numbers at the 6-12 level for credit recovery. Only three students who were suggested to come this summer did not. Otherwise all other students are in attendance and are working hard. In general, our elementary numbers are a little lower than previous years at the K-2 level. I will be looking at and discussing this with staff to determine why. One theory is that summer school followed the week immediately after school was let out and some parents wanted to provide a break for their child. We actually discussed this concern when picking the dates. But with summer camps hosted here at school and a variety of other activities we picked the weeks with the least amount of conflicts. Numbers for second session are higher from the initial sign-up so we do expect an increase.

VIII. District Administrator’s Report A. Info/Thanks for DACs (District Assessment Coordinators) Work: The Office of Student Assessment has typically

relied on DACs to effectively communicate essential assessment information, train staff members, and coordinate resources. However, the demands of this implementation year have required a significant amount of effective planning, as well as superior management and leadership skills - in many cases, above and beyond the call of duty, and in addition to the primary role you fulfill. To that end, we recognize that you have been charged with the task of coordinating and facilitating a lengthy checklist related to the implementation of multiple tests, including WKCE, WAA-SwD, PALS, Badger Exam, Dynamic Learning Maps, ACT Aspire™ Early High School, ACT® Plus Writing, ACT® WorkKeys®, ACCESS for ELLs®, and NAEP.

B. We are receiving $1,094,367 in aid payment from the state (we will actually have this by the time of this meeting) - $884,964 in Equalization Aid and $245,403 in Open Enrollment Tuition. After all billing for June 2014 has been completed and regular and summer payrolls have been taken out, we will be in good financial standing for year end. I should be able to provide a closing balance in July, at which point we should be able to make any final decisions for expenditures on any outstanding/upcoming projects.

C. Board Candidate Update: At this point we have 3 available candidates willing to fill the position left by Jaden McCullick. We will need to set a special meeting to interview them after June 19th. We can talk about a date at the meeting.

SCHOOL DISTRICT OF WAUZEKA-STEUBEN

COMPULSORY DISTRICT ATTENDANCE

State Stat. 118.16 (1)(a) has provided a legal definition of "habitual truant" as a "pupil" who is absent from school without an acceptable excuse... for part or all of five (5) or more days on which school is held during a school semester." Subsequent Crawford County Ordinance 9.05 directs school boards and their agents to identify habitual truants and initiate legal procedures prescribed in the Statute and the Ordinance.

The School District of Wauzeka-Steuben staff and parents bear mutual responsibility for the accountability of minor children during an official school day.

The Board of Education adopts the Crawford County Truancy Plan as the foundation for dealing with truancy in the District. This plan shall be reviewed at least once every two (2) years by the Board, and if appropriate, revised according to State Statute 118.162. The Crawford County Truancy Task Force - organized by the Crawford County Truancy Officer, shall convene at least once every four (4) years according to Statute 118.162 (1). The plan includes: procedures to be followed for notifying the parent(s)/guardian(s) of unexcused absences of habitual truants, plans and procedures for identifying truant students of all ages and returning them to school, methods to increase and maintain public awareness of and involvement in responding to truancy within the District, types of truancy cases to be referred to the district attorney, the immediate responses to be made by District personnel when a truant student is returned to school, plans and procedures to coordinate the responses to the problems of habitual truants with public and private social service agencies, and methods to involve the truant student's parent(s)/guardian(s) in dealing with and solving the student's truancy problems. A student is considered to be "simple truant" if absent from school without an acceptable excuse for part or all of a single school day. According to statute, a student may be cited by law enforcement for simple truancy. The principal will determine whether or not to refer a student from 12 years of age until s/he reaches 18 years of age to the County Sheriff's Department for simple truancy. The administration shall notify parent(s)/guardian(s) as soon as practicable if a law enforcement agency has requested a copy of a student's attendance record for purposes of a truancy investigation.

According to State Statute 118.15 (3) (c), "any child excused in writing by his or her parent or guardian before the absence" is excused from school attendance. "The school board shall require a child excused under this paragraph to complete any course work missed during the absence. A child may not be excused for more than 10 days in a school year under this paragraph." The Board requires parent(s)/guardian(s) to state a specific reason for absence in the written excuse.

If a student is late in arriving to school during the first ten (10) five (5) minutes at the beginning of the school day the late arrival will be recorded as a Class 1 Tardy. After the first 10 5 minutes, the late arrival will be recorded as an unexcused absence without an acceptable reason, for a minimum of a half day.

If a student is late arriving to school after lunch, the first five (5) minutes shall be recorded as a Class 1 Tardy. After five (5) minutes, the late arrival after lunch will be recorded as an unexcused absence for a minimum of a half day. (Tardiness from class to class is classified as a Class 2 Tardy and will be dealt with by individual teachers and the District discipline plans.) Consequences may include after school detentions, suspensions, and citations.

In addition to the truancy law, tardiness is subject to the District discipline plan. A truancy citation may be issued for unexcused absences. All unexcused absences will affect co-curricular participation. The administration and Dean of Students will develop guidelines, printed in student handbooks, assigning consequences for tardiness and unexcused absences.

The interpretation of illness and crisis, and the degree of verification are at the discretion of the District Attendance Officer, but must be within the guidelines of statute and Board policy. The Wauzeka-Steuben School District Attendance Policy includes the following elements: 1. In accordance with Statute 118.16 (1)(a) parent(s)/guardian(s) of minor students of compulsory attendance

age/grade level (age 6-17, 18, or grade 1 to grade 12) who have accumulated five (5) or more unexcused absences in a semester, shall be notified by certified or registered mail that the parent(s)/guardian(s) and child have a legal obligation to confer with District administration and related staff (teachers, counselor, psychologist, attendance clerk, and may include the Crawford County Truancy Officer, etc.) within five (5) school days after the habitually truant notice is sent, unless the parent requests an extension of the meeting time. The extension cannot exceed an additional five (5) days. The conference may include a juvenile court worker, but it is not mandated.

2. The conference will address the possible need for curriculum modification, ability assessment, social adjustment, counseling, attention of the court, and reasons for absences.

3. If the parent(s)/guardian(s) refuses to meet with District personnel within 10 school days after the habitually truant notice is mailed, the court system may take action.

4. The Board's definition of excusable absence is verified illness, family crisis, or medical appointments and is subject to the following guidelines: a. Medical condition and illness must be verified by a telephone call or e-mail to the attendance clerk the

morning of an absence, no later than 9:00 a.m. If the District has not received contact about a student’s absence by 9:00 a.m., the District will contact a parent/guardian by 10:00 a.m. that day. Medical conditions and illness in excess of three (3) consecutive (partial or full) days may require written verification from a health care provider. This requirement shall be at the discretion of the administration and only if the absence is in question.

b. A child is allowed ten (10) excused absences. Any absences for which medical documentation is provided do not count towards the ten (10) excused absences. Any absence after ten (10) excused days will require medical documentation or will be considered unexcused. Parents are encouraged to contact the attendance clerk prior to a nonemergency medical appointment.

c. A death or serious illness in the family may result in an excusable absence since a student might be incapable of attending to his/her scholastic duties. A request for funeral or serious illness in the family leave must be presented in writing in advance (except in the case of emergency) to the principal or attendance clerk. If a student needs to be absent for a funeral or serious illness in the family, such absence(s) will be excused for a reasonable length of time. The principal may require verification if the absence is in question.

d. Per state statute 118.16 (c) (1m), the Board shall consider all suspensions to be neither excused or unexcused absences.

e. Students must be in attendance the entire school day to participate in a contest(s) or event(s) scheduled for that day. Students may not be more than five (5) ten (10) minutes late (unexcused) for any class on a contest or event day. Students are allowed to keep medical and/or dental appointments, college visits, or attend funerals on a contest or event day. Written verification from the medical or dental office must be turned in to the attendance office before the student may participate. The appointment excuse is only for length of appointment and traveling time unless otherwise indicated. (*See Certificate of Appointment Form). Parental excuses are not an excuse that allows a student to play. The Principal or Dean of Students may approve individual exceptions prior to a student’s absence. It is not an approved exception for students to leave school to retrieve forgotten uniforms, equipment, or other items. If a student leaves school “ill” they are not allowed to return for practice that night.

Note: the time = (T) (under ordinary circumstances) to be gone for any appointment should be no longer than: double the time it takes to drive there and back + the length of the appointment. Example: to PdC 20 miles = 25 minutes x 2 = 50 minutes (round trip) x 2 (double time) = 100 minutes + 45 minute appointment = 145 Minutes Excused

5. Attendance expectations for students whose age/grade level is above or below compulsory attendance age:

a. Parents of a student(s) younger than compulsory attendance age/grade level have an ethical obligation to send their children to school regularly and to follow notification procedures for absences if enrolled in early childhood, prekindergarten, or kindergarten programs.

b. Students who reach their 18th birthday no longer have a legal obligation to attend school but continue to have an obligation to obey school rules on attendance procedures, follow attendance procedures, and are responsible for any consequences and/or disciplinary actions (detentions) administered by the school.

c. Students of 18 years of age, as legal adults, may be suspended for a period of one (1) to five (5) days for patterns of attendance which would be defined as truant for minors, and/or failure to follow attendance procedures, failure to be held responsible for any consequences and/or disciplinary actions (detentions) administered by the school.

d. Students of 18 years of age, as legal adults, may be expelled from school after two (2) separate suspensions for truant-like behavior and/or violations of other school rules.

e. Students of 18 years of age, as legal adults, must meet all criteria put forth by the school before one graduates, including disciplinary actions.

Summer School

Any student who attends summer school at any time will be added to the summer school attendance roster.

If a student is absent for more than five (5) consecutive school days s/he will be withdrawn from the class.

FREQUENTLY ASKED QUESTIONS ABOUT FREE AND REDUCED PRICE SCHOOL MEALS

Dear Parent/Guardian:

Children need healthy meals to learn. The School District of Wauzeka-Steuben offers healthy meals every school day. Breakfast costs $1.40/day (gr. Pk-5) and $1.90/day (gr. 6-12; lunch costs $1.90/day (gr. Pk-5) and $2.40/day (gr. 6-12). Your children may qualify for free meals or for reduced price meals. Reduced price is 30¢/day for breakfast and 40¢/day for lunch. This packet includes an application for free or reduced price meal benefits, and a set of detailed instructions. Below are some common questions and answers to help you with the application process.

1. WHO CAN GET FREE OR REDUCED PRICE MEALS? All children in households receiving benefits from FoodShare, the Food Distribution Program on Indian Reservations (FDPIR), or W-2 Cash Benefits are eligible for free meals. Foster children that are under the legal responsibility of a foster care agency or court are eligible for free meals. Children participating in their school’s Head Start program are eligible for free meals. Children who meet the definition of homeless, runaway, or migrant are eligible for free meals. Children may receive free or reduced price meals if your household’s income is within the limits on the Federal Income Eligibility Guidelines. Your children may qualify for free or reduced price meals if your household income falls at or below the limits on this chart.

6. MY CHILD’S APPLICATION WAS APPROVED LAST YEAR. DO I NEED TO FILL OUT A NEW ONE? Yes. Your child’s application is only good for that school year and for the first few days of this school year. You must send in a new application unless the school told you that your child is eligible for the new school year.

7. I GET WIC. CAN MY CHILDREN GET FREE MEALS? Children in households participating in WIC may be eligible for free or reduced price meals. Please send in an application.

8. WILL THE INFORMATION I GIVE BE CHECKED? Yes. We may also ask you to send written proof of the household income you report.

9. IF I DON’T QUALIFY NOW, MAY I APPLY LATER? Yes, you may apply at any time during the school year. For example, children with a parent or guardian who becomes unemployed may become eligible for free and reduced price meals if the household income drops below the income limit.

10. WHAT IF I DISAGREE WITH THE SCHOOL’S DECISION ABOUT MY APPLICATION? You should talk to school officials. 11. MAY I APPLY IF SOMEONE IN MY HOUSEHOLD IS NOT A U.S. CITIZEN? Yes. You, your children, or other household members do not have to be U.S. citizens to apply for free or reduced price meals.

12. WHAT IF MY INCOME IS NOT ALWAYS THE SAME? List the amount that you normally receive. For example, if you normally make $1000 each month, but you missed some work last month and only made $900, put down that you made $1000 per month. If you normally get overtime, include it, but do not include it if you only work overtime sometimes. If you have lost a job or had your hours or wages reduced, use your current income.

13. WHAT IF SOME HOUSEHOLD MEMBERS HAVE NO INCOME TO REPORT? Household members may not receive some types of income we ask you to report on the application, or may not receive income at all. Whenever this happens, please write a 0 in the field. However, if any income fields are left empty or blank, those will also be counted as zeroes. Please be careful when leaving income fields blank, as we will assume you meant to do so.

14. WE ARE IN THE MILITARY. DO WE REPORT OUR INCOME DIFFERENTLY? Your basic pay and cash bonuses must be reported as income. If you get any cash value allowances for off-base housing, food, or clothing, or receive Family Subsistence Supplemental Allowance payments, it must also be included as income. However, if your housing is part of the Military Housing Privatization Initiative, do not include your housing allowance as income. Any additional combat pay resulting from deployment is also excluded from income.

15. WHAT IF THERE ISN’T ENOUGH SPACE ON THE APPLICATION FOR MY FAMILY? List any additional household members on a separate piece of paper, and attach it to your application. HOW TO APPLY FOR FREE AND REDUCED PRICE SCHOOL MEALS

Please use these instructions to help you fill out the application for free or reduced price school meals. You only need to submit one application per household, even if your children attend more than one school in [School District]. The application must be filled out completely to certify your children for free or reduced price school meals. Please follow these instructions in order! Each step of the instructions is the same as the steps on your application. If at any time you are not sure what to do next, please contact [School District of Wauzeka-Steuben, 608-875-5311 x 228; [email protected]]. PLEASE USE A PEN (NOT A PENCIL) WHEN FILLING OUT THE APPLICATION AND DO YOUR BEST TO PRINT CLEARLY.

STEP 1: LIST ALL INFANTS, CHILDREN, AND STUDENTS UP TO AND INCLUDING GRADE 12 WHO ARE HOUSEHOLD MEMBERS Tell us how many infants, children, and school students live in your household. They do NOT have to be related to you to be a part of your household.

A) List each child’s name. For each child, print their first name, middle initial and last name. Use one line of the application for each child. When writing names, print one letter in each box. Stop if you run out of space. If there are more children present than lines on the application, attach a second piece of paper with all required information for the additional children. B) List the name of the school the child attends or N/A if the child does not attend school. C) Do you have any foster children? If any children listed are foster children, mark the “Foster Child” box next to the child’s name. Foster children who live with you may count as members of your household and should be listed on your application. If you are only applying for foster children, after completing STEP 1, skip to STEP 4 of the application and these instructions. D) Are any children homeless, migrant, or runaway? If you believe any child listed in this section may meet this description, please mark the “Homeless, Migrant, Runaway” box next to the child’s name and complete all steps of the application. E) Are any children in Head Start? If any child is enrolled in a Head Start Program, check the Head Start box after the child’s name and complete all steps of the application.

When filling out this section, please include all members in your household who are: Children age 18 or under and are supported with the household’s income; In your care under a foster arrangement, or qualify as homeless, migrant, runaway youth, or Head Start; Students attending School District of Wauzeka-Steuben], regardless of age.

Circle ‘NO’ and skip to STEP 3 on these instructions and STEP 3 on your application.

Leave STEP 2 blank. B) IF ANYONE IN YOUR HOUSEHOLD PARTICIPATES IN ANY OF THE ABOVE LISTED PROGRAMS:

Circle ‘YES’ and provide a case number for FoodShare, W-2 Cash Benefits, or FDPIR. You only need to write one case number. If you participate in one of these programs and do not know your case number, contact your case worker. You must provide a case number on your application if you circled “YES”. Please note: A BadgerCare case number cannot be used on this application, only the programs listed above.

STEP 3: REPORT INCOME FOR ALL HOUSEHOLD MEMBERS A) Report all income earned by children. Refer to the chart titled “Sources of Income for Children” in these instructions and report the combined gross income for ALL children listed in Step 1 in your household in the box marked “Child Income.” Only count foster children’s income if you are applying for them together with the rest of your household. It is optional for the household to list foster children living with them as part of the household.

A child is blind or disabled and receives Social Security benefits. A parent is disabled, retired, or deceased, and their child receives social security benefits.

Income from any other source A child receives income from a private pension fund, annuity, or trust.

What is Child Income?

Child income is money received from outside your household that is paid directly to your children. Many households do not have any child income. Use the chart below to determine if your household has child income to report.

If anyone in your household participates in the assistance programs listed below, your children are eligible for free school meals:

FOR EACH ADULT HOUSEHOLD MEMBER:

B) List Adult Household member’s name. Print the name of each household member in the boxes marked “Names of Adult Household Members (First and Last).” Do not list any household members you listed in STEP 1. If a child listed in STEP 1 has income, follow the instructions in STEP 3, part A. C) Report earnings from work. Refer to the chart titled “Sources of Income for Adults” in these instructions and report all income from work in the “Earnings from Work” field on the application. This is usually the money received from working at jobs. If you are a self-employed business or farm owner, you will report your net income.

D) Report income from Public Assistance/Child Support/Alimony/SSI/VA Benefits. Refer to the chart titled “Sources of Income for Adults” in these instructions and report all income that applies in the “Public Assistance/Child Support/Alimony/SSI/VA Benefits” field on the application. Do not report the value of any cash value public assistance benefits NOT listed on the chart. If income is received from child support or alimony, only court-ordered payments should be reported here. Informal but regular payments should be reported as “other” income in the next part.

What if I am self-employed?

If you are self-employed, report income from that work as a net amount. This is calculated by subtracting the total operating expenses of your business from its gross receipts or revenue.

Who should I list here?

When filling out this section, please include all members in your household who are: Living with you and share income and expenses, even if not related and even if they do not receive income of their own.

Do not include people who: Live with you but are not supported by your household’s income and do not contribute income to your household. Children and students already listed in Step 1.

How do I fill in the income amount and source?

FOR EACH TYPE OF INCOME: Use the charts in this section to determine if your household has income to report. Report all amounts in gross income ONLY. Report all income in whole dollars. Do not include cents.

o Gross income is the total income received before taxes or deductions. o Many people think of income as the amount they “take home” and not the total,

“gross” amount. Make sure that the income you report on this application has NOT been reduced to pay for taxes, insurance premiums, or any other amounts taken from your pay.

Write a “0” in any fields where there is no income to report. Any income fields left empty or blank will be counted as zeroes. If you write ‘0’ or leave any fields blank, you are certifying (promising) that there is no income to report. If local officials have known or available information that your household income was reported incorrectly, your application will be verified for cause. Mark how often each type of income is received using the check boxes to the right of each field.

E) Report income from Pensions/Retirement/Social Security/All other income. Refer to the chart titled “Sources of Income for Adults” in these instructions and report all income that applies in the “Pensions/Retirement/Social Security, Other Income” field on the application. F) Special Situations. For seasonal workers and others whose income fluctuates and usually earn more money in some months than others. In these situations, project the annual rate of income and report that. This includes workers with annual employment contracts but may choose to have salaries paid over a shorter period of time. This includes school employees. G) Report total household size. Enter the total number of household members in the field “Total Household Members (Children and Adults).” This number MUST be equal to the number of household members listed in STEP 1 and STEP 3. If there are any members of your household that you have not listed on the application, go back and add them. It is very important to list all household members, as the size of your household determines your income cutoff for free and reduced price meals.

H) Provide the last four digits of your Social Security Number. The household’s primary wage earner or another adult household member must enter the last four digits of their Social Security Number in the space provided. You are eligible to apply for benefits even if you do not have a Social Security Number. If no adult household members have a Social Security Number, leave this space blank and mark the box to the right labeled “Check if no SSN.”

STEP 4: CONTACT INFORMATION AND ADULT SIGNATURE All applications must be signed by an adult member of the household. By signing the application, that household member is promising that all information has been truthfully and completely reported. Before completing this section, please also make sure you have read the privacy and civil rights statements on the back of the application. A) Provide your contact information. Write your current address in the fields provided if this information is available. If you have no permanent address, this does not make your children ineligible for free or reduced price school meals. Sharing a phone number, email address, or both is optional, but helps us reach you quickly if we need to contact you. B) Sign and print your name. Print your name in the box “Printed name of adult completing the form.” And sign your name in the box “Signature of adult completing the form.” C) Write Today’s Date. In the space provided, write today’s date in the box. D) Share children’s Racial and Ethnic Identities (optional). On the back of the application, we ask you to share information about your children’s race and ethnicity. This field is optional and does not affect your children’s eligibility for free or reduced price school meals.

Once the lower doors lock at noon everyone must enter through the upper doors by the District Offi ce. Those doors will be open from 7:45 a.m. - 4:00 p.m. Monday - Thursday and 7:45 a.m. un l noon on Friday.
