Our Purpose:- The MyST service has been established through a partnership between Blaenau Gwent and Monmouthshire County Councils. MyST provides a range of support to the most vulnerable looked after children and young people with the most complex needs who tend to experience multiple foster care placement and other service breakdowns. Their risk taking and challenging behaviours, as well as their significant mental health needs, often exceed the typical resources of Local Authority services. MyST has been established to provide intensive support to such children including the recruitment, training and specialised support of therapeutic foster carers. The Purpose of this Role:- MyST is a therapeutic service for looked-after children with complex needs, who are at risk of being placed in out-of-area residential or institutional care due to their emotional and mental health needs. This is an exciting opportunity to be part of a multidisciplinary service, ensuring the provision of a responsive, high quality, customer and outcome focused service to children, young people, their families and partner agencies across Blaenau Gwent and Monmouthshire. Expectation and Outcomes of this Role:-

You will be expected to work as part of a multidisciplinary team, providing therapeutic intervention and support to children, young people, their carers and families by agreeing and directly delivering interventions. With appropriate support and direction, you will work as part of a team to enable positive and agreed outcomes to be achieved, in accordance with organisational policies, procedures and regulatory requirements. Your responsibilities are to:-

Providing a high-quality service to children, young people and their families, based on an assessment of need and identified outcomes

Delivering interventions and working collaboratively with children, young people or families on a 1:1 basis or in a group setting, with appropriate direction

Involving children and young people in their care and enabling them to be active participants in service development

Developing and maintaining effective working relationships and excellent communication with colleagues and with children, young people and families and where necessary encouraging and promoting their involvement to achieve positive outcomes

Achieve the highest standards of safeguarding, whether through direct care provision or support activities to the children and young people or being alert to child protection and safeguarding issues and informing appropriate line managers of any concerns in line with safeguarding policies and procedures

Attending and actively participating in relevant multi-agency meetings

Being responsible for organising achievement of work objectives on a daily basis, including forward planning

Ensuring accurate and contemporaneous records, using appropriate IT systems to capture case details, outputs and outcomes and to produce and present reports as required

Guiding or directing colleagues and volunteers where necessary

Ensuring familiarity and compliance with relevant policies and procedures

Engage in therapeutic sessions, team meetings, reflective practice sessions and individual and peer/group supervision as required to work within the ethos of continuous professional development within the service

Demonstrate a commitment to the service values

Promote Equality and Diversity including linking with local equality and diversity frameworks to ensure promotion of best practice and action plans

Demonstrate willingness and ability to operate within a constantly changing environment and targets

Undertake such other duties that occasionally arise, which ensure that the service delivers what’s needed, and what works for children

DIOGELU: Mae Diogelu ac Amddiffyn Plant ac Oedolion yn flaenoriaethau allweddol i'r Cyngor. Ein nod yw cefnogi plant ac oedolion sydd mewn perygl i fod mor ddiogel ag y gallant ac i gyflawni eu potensial. Ein Pwrpas:- Mae’r gwasanaeth MyST wedi ei sefydlu drwy bartneriaeth rhwng Cynghorau Sir Blaenau Gwent a Sir Fynwy. Mae MyST yn darparu ystod o gymorth i’r plant a’r bobl ifanc mwyaf bregus ac sydd yn meddu ar yr anghenion mwyaf cymhleth ac yn tueddu i gael eu gosod mewn sawl sefydliad gofal maeth a’n profi methiannau eraill o ran gwasanaeth. Mae’r duedd sydd ganddynt o gymryd risgiau ac ymddwyn yn heriol, yn aml yn golygu bod angen mwy o adnoddau nag sydd gan yr Awdurdod Lleol i’w cynnig. Mae MyST wedi ei sefydlu er mwyn darparu cymorth dwys i’r fath blant gan gynnwys recriwtio, hyfforddi a rhoi cymorth arbenigol i ofalwyr maeth therapiwtig.

Pwrpas y Rôl hon:- Mae MyST yn wasanaeth therapiwtig i blant sydd yn derbyn gofal ag anghenion cymhleth, sydd mewn risg o gael eu gosod mewn sefydliad gofal preswyl neu sefydliadol sydd y tu hwnt i’r ardal, a hynny yn sgil eu hanghenion emosiynol ac iechyd meddwl. Dyma gyfle cyffrous i fod yn rhan o dîm amlddisgyblaethol, yn darparu ymyriadau therapiwtig a chymorth i blant, pobl ifanc, eu teuluoedd ac asainaethau partner ar draws Blaenau Gwent a Sir Fynwy. Disgwyliadau a Chanlyniadau’r Rôl hon:- Bydd disgwyl i chi weithio fel rhan o dîm amlddisgyblaethol, yn sicrhau darpariaeth gwasanaeth ymatebol, safon uchel, yn ffocysu ar gwsmeriaid a chanlyniadau i blant, pobl ifanc, eu gofalwyr a’u teuluoedd drwy gytuno a darparu ymyriadau yn uniongyrchol. Gyda chymorth a chyfarwyddyd priodol, byddwch yn gweithio fel rhan o dîm er mwyn sicrhau bod modd sicrhau positif a chytunedig, yn unol gyda pholisïau, gweithdrefnau ac anghenion rheoleiddio'r mudiad. Bydd eich cyfrifoldebau yn cynnwys:-

Darparu gwasanaeth safon uchel i blant, pobl ifanc a’u teuluoedd, yn seiliedig ar asesiad o’u hanghenion a’r canlyniadau sydd wedi eu hadnabod.

Darparu ymyriadau a chydweithio yn agos gyda phlant, pobl ifanc neu deuluoedd ar sail 1:1 neu mewn grŵp, gyda chyfarwyddyd priodol

Cynnwys plant a phobl ifanc yn eu gofal, a’u caniatáu hwy i fod yn gyfranogwyr yn y broses o ddatblygu’r gwasanaeth

Datblygu a chynnal perthnasau gwaith effeithiol gyda chydweithwyr a gyda phlant, pobl ifanc a’u teuluoedd, a lle bod angen, annog a hyrwyddo’r broses o’u cynnwys er mwyn sicrhau canlyniadau positif.

Yn cyrraedd y safonau uchaf o ran diogelu, p’un ai bod hyn drwy ddarpariaeth gofal uniongyrchol neu weithgareddau cymorth ar gyfer y plant a phobl ifanc neu’n sylwi ar faterion diogelu plant ac yn rhoi gwybod i’r rheolwyr llinell priodol am unrhyw bryderon yn unol gyda’r polisïau a’r gweithdrefnau diogelu.
