RMA REQUEST FORM
To help expedite Return Material Authorization (RMA) requests, please provide the following information to our customer service department at the time of request.
All return requests are subject to pre-approval by Advanced Interconnections. Upon approval, an RMA number and shipping instructions will be provided.

a) Customer name, AIC Sales Order number (with line #), AIC Part ID:

a) Repair (no credit issued)
b) Replacement (credit and new invoice issued for RMA quantity)
c) Credit (credit/refund issued – subject to AIC’s terms and conditions)
4) Reason for return:
Our customer service department will review your request with our Quality department, and if approved, will issue an RMA number. In some cases, we will request 1 to 2 pieces of the product (or photos) to evaluate prior to issuing an RMA. Instructions for returning additional pieces or the full RMA quantity will be communicated along with the RMA number. Please contact customer service if you have any questions.
