Risk Assessment Matrix The risk assessment data sheet is a tool that provides a standard approach to the qualitative assessment of risk. It is used to identify a suitable course of action to manage the assessed risk The quantitative assessment of risk is carried out for four separate categories (people assets, Environment, Reputation). In order that the priority for further action can be established and appropriate remedial action can be identified and implemented. Trivial (1)No actions required but documentary records need to be kept

Tolerable (2)No additional controls are required. Consideration may be to the most cot effective solution. Monitoring is required to ensure the controls are maintained

Moderate (3)Efforts should be made to reduce the risk. Risk reduction measures should be implemented with in a defined time of period. Need for improved control measure

Substantial (4)Work should not started until the risk has been reduced considerably resources may have to allocated to reduce the risk. Where the risk involves work in progress.

Intolerable (5)Emergency action should be taken with in the short time

How to use the risk assessment Matrix

1. Assess the potential consequences of the hazard for each categories

2. Assess the probability of these potential consequences occurring for each of the four categories

3. Classify the risk on the matrix for each of the four categories (people , asset, environment and reputation) from the intersection of the chosen row of(potential Consequence) with the chosen column of Probability
