Label Printing Instructions

Return Packaging Instructions

1. The Debit Memo (DM), and Return Authorization (RA) if applicable, must be placed in the container with the return product. Stericycle recommends making a photocopy of the documents for your records.

2. Only product(s) on the corresponding DM may be placed in the shipping container.

3. If you have multiple returns (DMs) requiring multiple shipping labels, the shipper must ensure each return is packaged with the corresponding Debit Memo in a separate container (i.e. individual plastic storage bag) and labeled with the corresponding shipping label within the shipping carton.

4. If you have a single return (DM) with multiple manufacturers requiring multiple shipping labels, the shipper MUST ensure each manufacturer’s product is packaged with a copy of the Debit Memo in a separate container (i.e. individual plastic storage bag) and labeled with the corresponding shipping label within the shipping carton.

5. Shipping labels are specific to each return and may not be re-used for future returns.

6. A bar coded shipping label generated must be placed on the exterior of the shipping carton next to the carrier shipping label.

7. If you have more than one shipping carton for one return, the shipper must request multiple labels at the time of shipping label generation.

a. Place a copy of the DM in each carton

b. Label each carton with a bar coded shipping label

c. Labels will specify 1 of #, 2 of #, etc.

Returned product must be segregated within the shipping carton according to Debit Memorandum and Manufacturer. Deviations from these shipping procedures may prohibit processing and credit issuance.

NOTE: When printing the PDF document, set scaling to “None” in the Adobe Reader Print Dialog box.

Return Shipping Instructions

1. Stericycle recommends using a carrier with package tracking capabilities for all shipments.

2. Ship all returns using the shipping label(s) generated and appropriate postage to:

Manufacturer’s Return Policy

Stericycle manages returns for the following Manufacturers per the individual Manufacturer’s Policy. Please make note of individual Manufacturer requirements and submit returns accordingly.

ALL CII Scheduled Controlled Substance Returns must have a Stericycle RA and 222 Form. Documentation can be obtained by submitting Debit Memo information to:
