Practice TransformationTOOLKIT TO ENGAGE HIGH-RISK PATIENTS IN SAFE TRANSITIONS ACROSS AMBULATORY SETTINGSSource: HHS, Agency for Healthcare Research and Quality

This toolkit is designed to help staff actively en-gage patients and their care partners to prevent errors during transitions of care. Toolkit materials were field tested in two facilities, one rural and one urban setting. Qualitative analysis of interviews with staff members was completed to better under-stand how materials were implemented, the effective-ness of support materials, barriers to and facilitators of implementation, and lessons learned from implementation. Toolkit components include:

� Guide for Safe Patient Transitions to New Ap-pointments: instructions on how to use the toolkit, which also includes useful resources

� Narrated PowerPoint slide deck for team training

� Pre-Intervention Assessment: step-by-step exercise in an Excel format for team members to identify safety gaps or areas for improvement

� Checklist to Prepare Patients for New Appointments: best practices team members should follow with every patient

� Appointment Aide: a tool for pa-tients to refer to and use at their current appointment, and take with them to their follow-up appointments

The purpose of this webinar is to enable participants to increase their understanding of IPE and how it can be applied to improve the HIV care continuum. This IPE Toolkit contains free-source activities and methods to actively en-gage educators in learning and applying various instructional methods to teach IPE competencies. The IPE compe-tencies in this toolkit are designed by the Interprofessional Education Collaborative (IPEC). They fall into four domains: Values/Ethics for Interprofe-sional Practice, Roles/Responsibilities, Interprofessional Communication, and Teams and Teamwork. As an extension of HRSA HAB’s SMAIF Jurisdictional Approach to Curing Hepatitis C among HIV/HCV Co-infected People of Color – Jurisdictional Sites project, 2 patient engagement tools were developed with assistance from the AETC NCRC-facilitated HIV/HCV Co-infection Community of Practice and Learning workgroup to help keep HCV co-infected PLWH educated and engaged in their care, and encourage their progress. Passport to Cure is an interactive engagement tool to help clients track clinic visits and monitor their HCV treatment progress to achieve a sustained viral response. It folds down to a discreet size that can be carried always – in a wallet, or a handbag or pocket. Patients have found it useful to have on hand during emergency room visits or to help inform providers outside of their HIV/HCV care team about their current HIV and HCV medications. The Hep C Free postcard is designed to celebrate and acknowledge patient success when HCV viral suppression is achieved. It also serves as a reminder of the risk factors that can lead to HCV re-infection and reinforces steps to follow to remain HCV free. Abstracts can be structured as scientific research findings, programmatic summaries, or storytelling sessions. Scientific or programmatic abstracts can be presented as oral presentations, panel presentations, or poster presentations.
