Tips for creating a good questionnaire:

Tips for increasing response rates:

questions beforehandO Stress anonymity of the questionnaireO Make the e-mail request personal. Genuine request – do

2. Installing Survey Software:

O Requirements to install the survey (MySQL, PHP, 60 MB web space)

3. Creating the First Survey:
