Section 1.0 - Instructions to Proposers
1.1 Background
The City of Dade City (City) occupies 3.4 square miles, was founded in 1889 and has an approximate
population of 6,500. The City is also the county seat of Pasco County.
The legislative branch of the city government is composed of five (5) Commissioners elected at-large
including a Mayor who is elected by the City Commission. City Commissioners are elected for staggered
four (4)-year terms of office. Under the provisions of the adopted City Charter, Dade City operates under
the City Commission-City Manager form of government.
The City of Dade City hereby solicits sealed written proposals for ongoing computer support for the
existing City network and related equipment in accordance with the terms conditions and specifications
outlined in this Request for Proposal (RFP). 1.2 Scope of Work
City anticipates a well-planned proposal that supports the continued maintenance and operation of the
current City-wide computer system as outlined in Section 2. The primary objectives of the City are to
obtain computer support services that will (1) provide maintenance and repair of computers and related
equipment, (2) ensure data security and integrity, (3) provide technical troubleshooting and computer
problem solving, and (4) provide timely, reliable, competent, and cost effective services. 1.5 Critical Assumptions
The selected vendor will be held contractually to all information provided in their proposal and any
subsequent documentation and demonstration material(s). The response to this RFP will become an
attachment to the services contract.

1.6 Proposal's Due Date
Sealed proposals must be received on or before July 7, 2016 at 2:00pm at the Office of the City
Manager, 38020 Meridian Avenue Dade City, FL 33526-1355, 8:00AM to 5:00PM, Monday through Friday
except City holidays. Any proposal received after the specified time will not be considered. The Proposer
is solely responsible for ensuring the proposal is received on or before the deadline.

All proposals that are not submitted in the required format or missing required forms will be subject to rejection. It is agreed upon by the Proposer that the signing and delivery of the Proposal represents the
Proposers acceptance of the terms and conditions of the specifications.
1.7 Submission of Proposals
1.7.1 How Proposals Are To Be Submitted
All proposals must be submitted in a sealed envelope, addressed, and mailed or delivered as stated in
the previous section. Any Proposer may withdraw his proposal either personally, or by electronic or
written communication at any time prior to opening of the proposals. All proposals must be in the
possession of the City Manager’s office at the time of the proposal opening. Proposals submitted after
the time specified shall not be considered and shall be returned unopened. It is the sole responsibility of
the Proposer to ensure that his or her proposal reaches the City Manager’s Office on or before the
closing date and time. Proposals submitted by telephone, e-mail or facsimile will not be accepted. If the
proposal is sent overnight courier, (i.e., Federal Express, UPS, etc.), the proposal must be enclosed in an
envelope marked as instructed above.
1.7.2 Incurred Expenses
The City is not responsible for any expenses the Proposer may incur in preparing and submitting
proposals for this Request for Proposal.
1.7.3 Interviews
The City reserves the right to conduct personal interviews of any or all Proposers prior to selection. The
City will not be liable for any costs incurred by the Proposer in connection with such interviews or
presentations (i.e., travel accommodations, etc.)

1.7.4 Request for Modification
The City reserves the right to request that the Proposer modify his proposal to fully meet the needs of the
City.
1.7.5 Request of Additional Information
The Proposer shall furnish such additional information as the City of Dade City may reasonably require.
This includes information that indicates financial resources as well as ability to provide and maintain the
system and/or services. The City reserves the right to make investigations of the qualifications of the
Proposer as it deems appropriate, including but not limited to a background investigation. The City also
reserves the right to make such investigations, as it may deem necessary to establish the competency
and financial ability of any Proposer to perform the work.
1.7.6 Acceptance/Rejection/Modification to Proposals
The City of Dade City reserves the right to reject any or all proposals, in whole or in part, with or without
cause, to waive any informalities and technicalities, and to award the contract on such coverage and
terms it deems will best serve the interest of the City. Criteria utilized by the City for determining the most
responsive Proposer includes, but is not limited to, cost, whether Proposer meets the City's published
requirements, the Proposers experience, skill, ability, financial capacity, and any other factors which could
reasonably be asserted as being relevant to successful performance.
1.7.7 Proposals Binding
All proposals submitted shall be binding for 120 calendar days following opening.
1.7.8 Addendum and Amendment to Request for Proposal
If it becomes necessary to revise or amend any part of this Request for Proposal, the City Manager's
Office will furnish the revision by written Addendum to all prospective Proposers.
1.7.9 Economy of Preparation
Proposals should be prepared simply and economically, providing a straight forward and concise
description of the Proposers ability to fulfill the requirements of this Request for Proposal.
1.7.10 Proprietary lnformation
In accordance with Chapter 119 of the Florida Statutes (Public Records Law) and, except as may be
provided by other applicable State and Federal Law, all Proposers should be aware that Request for
Proposals and the responses thereto are in the public domain. However, the Proposers are requested to
identify specifically any information contained in their proposals which they consider confidential and/or

proprietary and which they believe to be exempt from disclosure, citing specifically the applicable
exempting law.
1.7.11 Proposals Will Not Be Returned
All proposals received from Proposers in response to this Request for Proposal will become the property
of the City of Dade City and will not be returned to the Proposers. In the event of contract award, all
documentation produced as part of the contract will become the exclusive property of the City.
1.7.12 Proposers Certification
By submitting a proposal, the Proposer certifies that he or she has fully read and understands the
proposal method, meets the requirements and has full knowledge of the scope, nature, and quality of
work to be performed.
1.8 Format for Response
1.8.1 Format and Availability of RFP
The proposal will follow the format and sections as shown in Section 1.24 of this document. In order to
ensure a uniform review process and to obtain the maximum degree of comparability, it is required that
the requirements are met and proposals be organized in the manner specified.
1.8.2 Number of Copies and electronic format
Proposer shall submit 5 paper copies of the proposal in the prescribed format along with an electronic
copy {PDF). All paper copies must be plainly marked by the Proposer with the vendor name, date and
RFP title. The electronic copy of the proposal, as well as concerns and issues should be directed to
Robert Shaw, by e-mail at [email protected].
1.8.3 Additional Comments
Any deviation from the specifications must be explained in detail; otherwise, it will be considered that the
proposal is in strict compliance with the specifications and the successful Proposer will be held
responsible for meeting the specifications. Any exceptions or clarifications to any section of the
specifications shall be clearly indicated on a separate sheet{s) attached to the proposal form and shall
specifically refer to the applicable specification paragraph number and page.
1.8.4 Proposers Responsibility to be Informed
Proposers are expected to fully inform themselves as to the requirements of the specifications and failure
to do so will be at their risk. A Proposer shall not expect to secure relief on the plea of error.

1.9 Evaluation of Proposal and Procedures
1.9.1 Evaluation Process
A Selection Committee (Committee) has been established by the City to review and evaluate all
proposals. The Committee will review all proposals for completeness of information submitted,
experience, installed base, references, and ability to meet technical requirements, functionality and cost;
with special emphasis placed on the technical and functionality requirements. The Committee may
request clarification or may, in its sole discretion, eliminate any proposals from consideration, which are
incomplete and/or do not comply with the requirements herein. The Committee shall conduct an
evaluation of all proposals on the basis of the information provided and other evaluation criteria as set
forth in this RFP or as reasonably determined by the Committee.
1.9.2 Evaluation Criteria
Proposers will be evaluated according to the following criteria:
• The ability, capacity and skill of the Proposer to perform the City-wide computer and
network support in a timely basis. (Based on Section 2)
• Current clients' level of satisfaction with Proposer services.
• Experience of staff relative to City computer network environment.
• Costs including travel, hourly rates, and other applicable fees.
1.9.3 Review Procedure
The Selection Committee will evaluate all proposals submitted in response to this RFP. The Committee
will first review each proposal for compliance with the requirements of the RFP. Failure to comply with
any requirements may disqualify a proposal immediately. Proposals meeting the requirements shall be
further evaluated against the evaluation criteria.
The City reserves the right to reject any and all proposals and to waive minor irregularities. The City
further reserves the right to seek new proposals when such a procedure is reasonably in the best interest
of the City to do so.
1.10 Questions Regarding Specifics or Proposal Processes
To ensure fair consideration for all Proposers, the City prohibits communication to/or with any department,
division or employee during the submission process, except as provided below. Any communication
between Proposer and the City in order to obtain information or clarification needed to develop a proper,
accurate evaluation of the proposal will be conducted through the City Manager's office.
Communications initiated by a Proposer to anyone other than the City Manager's office may be

grounds for disqualifying the offending Proposer from consideration for award of the proposal
and/or any future proposal. Proposer may receive a waiver from City Manager's office to
communicate with any involved parties if needed.

1.11 General Legal Requirements
Federal, State, County and local laws, ordinances, rules and regulations that in any manner affect the
items covered herein shall be deemed to apply. Lack of knowledge by the Proposer will in no way be a
cause for relief from responsibility.
• Vendors doing business with the City are prohibited from discriminating against any
employee, applicant for employment or client because of race, creed, color, ancestry,
religion, national origin, sex, or age with regard to, but not limited to the following:
employment practices, rates of pay or other compensation methods and training
selection. The Proposer further certifies compliance with the non-discrimination clause
contained in Section 202, Executive Order 11246, as amended by Executive Order
11375, relative to equal employment for all persons without regard to race, color, religion,
sex or national origin.
• A person or affiliate, as defined in §287.133 of the Florida Statutes, who has been placed
on the convicted vendor list following a conviction for a public entity crime may not submit
a bid on a contract to provide any goods or services to a public entity, may not submit a
bid on a contract with a public entity for the construction or repair of a public building or
public work, may not submit bids on leases of real property to a public entity, may not be
awarded or perform work as a contractor, supplier, subcontractor, or consultant under a
contract with any public entity, and may not transact business with any public entity in
excess of the threshold amount provided in §287.01 of the Florida Statues for
CATEGORY TWO, for a period of 36 months from the date of being placed on the
convicted vendor list.
• The obligations of the City under this award are subject to the availability of funds lawfully
appropriated for this purpose.
• This Request for Proposal shall be included and incorporated into the final award and
contracts.

1.12 Applicable Law and Venue
This RFP, responses from Proposers and contracts resulting from this RFP shall be governed by the laws
of the State of Florida.
1.13 Conditions of Proposals
1.13.1Late Proposals
Proposals received by the City after the time specified for receipt will not be considered. Proposers shall
assume full responsibility for timely delivery of the proposal to the location designated for receipt of
proposals.
1.13.2 Completeness
All information required by the Request for Proposal must be supplied to constitute a legitimate proposal.
1.13.3 Award Presentation
The Office of the City Manager will present to the City Commission for acceptance and final award, the
top three (3) ranked proposals, or will reject all proposals, within one ninety (90) calendar days from the
date of opening of proposals.
1.14 Terms and Conditions of Agreement
All sections of the City's specifications and all supplementary documents are essential parts of the City's
contract and requirements occurring in one area are deemed as though occurring in all. The response to
the RFP will be attached as an addendum to the final negotiated contract.
1.15 Discussions with Responsible Proposers
Discussions may be conducted with responsible Proposers who submit proposals determined to be
reasonably acceptable of being selected for award for the purpose of clarification and to assure full
understanding of, and responsiveness to, the solicitation requirements. Proposers shall be accorded fair
and equal treatment with respect to any opportunity for discussion and revision of proposals and such
revisions may be permitted after submission prior to award for the purpose of obtaining best and final
offers. In conducting discussions, there shall be no disclosure of any information derived from proposals
submitted by competing Proposers except as may be required by the Florida Public Records Law,
Chapter 119, Florida Statutes.

1.16 Execution of Agreement
The successful Proposer shall, within 30 calendar days after the City of Dade City issues Notice of Award,
negotiate and enter into a contract with the City for the proposed services. Failure to comply with the
established deadline for submittal of required documents may be grounds for cancellation of the award. 1.17 Conflict of Interest
The award hereunder is subject to the provision of State of Florida Statutes and City of Dade City
Ordinances.
The Proposer, by affixing his signature to the RFP, declares that the proposal is made without any
previous understanding, agreement, or connections with any City official or persons, firms, or
corporations making a proposal on the same items and without any outside control, collusion or fraud. By
signing the proposal, the Proposer further declares that no City Commission member, other City officer, or
City employee directly or indirectly owns more than five (5) percent of the total assets or capital stock of
the Proposer entity, nor will directly or indirectly benefit by more than five (5) percent from the profits or
emoluments of this contract. 1.18 Fiscal Year Funding Appropriations
1.18.1 Specified Period
Unless otherwise provided by law, a contract for supplies or services may be entered into for any period
of time deemed to be in the best interest of the City, provided the term of the contract and conditions of
renewal or extension, if any, are included in the solicitation and funds are available for the first fiscal
period at the time of contract. Payment and performance obligations for succeeding fiscal periods shall
be subject to appropriation by City Council for funds.
1.18.2 Cancellation Due to Unavailability of Funds in Succeeding Fiscal Periods
When funds are not appropriated or otherwise made available to support continuation of performance in a
subsequent fiscal period, the contract may be cancelled. 1.19 Public Entity Crimes
A person or affiliate, as defined in §287.133 of the Florida Statutes, who has been placed on the
convicted vendor list following a conviction for a public entity crime may not submit a bid on a contract to
provide any goods or services to a public entity, may not submit a bid on a contract with a public entity for
the construction or repair of a public building or public work, may not submit bids on leases of real
property to a public entity, may not be awarded or perform work as a contractor, supplier, subcontractor,
or consultant under a contract with any public entity, and may not transact business with any public entity

in excess of the threshold amount provided in §287.01 of the Florida Statues for CATEGORY TWO, for a
period of 36 months from the date of being placed on the convicted vendor list.
1.20 Florida Prompt Payment Act
For purposes of billing submission and payment procedures, a "proper invoice" by a contractor, vendor or
other invoicing party shall consist of at least all of the following:
• A description (including quantity) of the goods and/or services provided to the City (or a
party on behalf of the City) reasonably sufficient to identify it (or them);
• The amount due, applicable discount(s), and the terms thereof; The full name of the
vendor, contractor or other party who is supplying the goods and/or services including a
mailing address in case of a dispute and a mailing address for payment purposes (if they
are different) and a telephone number.
• A description (including quantity) of the goods and/or services provided to the City (or a
party on behalf of the City) reasonably sufficient to identify it {or them);
1.21 Insurance Requirements
Proposers must submit with their proposal, Proof of Insurance, meeting or exceeding the following
requirements or a Letter of Intent to provide the following requirements if awarded the contract:
• Worker's Compensation Insurance - as required by law.
• Employer's Liability Insurance- $100,000 per occurrence.
• General Liability Insurance- $500,000 per person and $250,000 per accident for bodily
injury.
• Automobile Liability Insurance - $250,000 per occurrence, $250,000 per accident for
bodily injury and $250,000 per accident for property damage.
The required insurance coverage shall be issued by an insurance company authorized and licensed to do
business in the State of Florida, with the minimum rating of B+ or better, in accordance with the latest
edition of A.M. Best's Insurance Guide.
The successful Proposer must submit, no later than 10 days after award and prior to commencement of
any work, a Certificate of Insurance naming the City of Dade City as additional insured.

1.22 Code of Ethics
With respect to this proposal, if any Proposer violates or is a party to a violation of the Code of Ethics of
the State of Florida per Florida Statutes, Chapter 112, Part Ill, Code of Ethics Officers and Employees,
such Proposer may be disqualified from performing the work described in this proposal or from furnishing
the goods or services for which the proposal is submitted and shall be further disqualified from submitting
any further proposals for work or for goods or services for the city.
1.23 Patents, Royalties and Copyright
The Proposer, without exception, shall indemnify and hold harmless the City of Dade City, Florida and its
employees from liability of any nature or kind, including cost and expenses for, or on account of, any
copyrighted, patented, or unpatented invention, process, or article manufactured or used in the
performance of the contract, including its use by the City of Dade City, Florida.
If the Proposer uses any design, device or materials covered by letters, patent, or copyright, it is mutually
understood and agreed, without exception, that the prices shall include all royalties or cost arising from
the use of such design, device, or materials in any way involved in the work. 1.24 Proposal Format
In order to facilitate easy comparison of each proposal, Proposers must prepare their proposals in the
following format. Failure to fully and accurately complete the required forms may result in rejection of the

1.24.1Cover Letter
This section should be limited to two pages and should include the name of the firm, address, and
telephone number, name of contact person, date, and the subject - RFP Dade City Computer

1.24.2 Forms
Proposers must complete the attached forms and/or provide the information below. These forms include:
• Sworn Statement Pursuant to Section 287.133 (3) (a), Florida Statutes, on Public Entity
Crimes. (pp 15-18)
• Supporting documentation that vendor can do business in Dade City or have
documentation from originating County of issuance. If vendor does not have Dade City
Business Tax License, vendor will have to submit a Business Tax Application plus
appropriate fee with the City after final vendor selection.
• Required Reference Information (p21)
• Sample vendor contract normally used for similar scope of service.
• NCIC/FCIC certification (if attained). This certification will need to be maintained for work
in Police Department.

1.24.5 Cost Structure

Mission Critical Availability The City has technical support requirements for mission critical applications and infrastructure. The following is a breakdown of these requirements.
Public Safety - Within 2 hours of notification by City personnel, adequate technical support personnel need to be on-site to begin remedy of the situation 7 am EST to 7pm EST.
