Executive Summary

To assess the handwashing and toilet facilities at 100 schools of Kabul city and to get a snapshot

of overall hygiene conditions an assessment was undertaken by Health and Youth Welfare

Department of Minisrty of Education supported by UNICEF. The gatherd information will be

used for planning interventions in the future and for the evaluation of works done under the

WASH activities in the targeted schools during the last four years. Data were collected using

observations of handwashing and toilet facilities at schools and asking questions from the boy

and girl students regarding the handwashing practice and other hygiene conditions. A semi-

structured questionnaire especially designed for 100 school handwash and toilet survey available

online was used for data collection on school hygiene conditions. Double data entry was done

using EpiData software and the analyses were done using SPSS software version 16.

As the total number of schools surveyed were almost 100, so the number of schools with a

specific attribute are equal to the percentage of schools with that attribute (number equal to

percentage as the denominator is almost 100) and hence the percentage are not necessary to

calculate so not reported frequenly in this report. The results are presented more in a way to use

for practical decision making rather for publication purposes. Almost all (98 %) of the schools

had toilets regardless of the type of the toilet. No toilet was present at all in two of the schools.

According to the teachers and students they use either toilets of neighboring houses or open area

for urination and defecation. Teachers and students of three schools in Kabul city use portable

toilets. Traditional toilets (latrines) were in use in around 75% of schools. Most of the schools

had flushing or pour-flush toilets but problems in water sources, storage and supply were the

main issues for not using of the flushing and pour-flush toilets in the schools. Reportedly, in 26

schools (26.8%) there were no separate toilets so the boys and girls use same toilet for urination

and defecation. From the total of the 97 schools, only 19 had the hand-washing facilities

(19.59%). Water cintaner with tap is usually used as a facility of hand-washing. Soaps were

available only in four schools for hand-washing purposes. The facility was rated as “very good”

for only six schools and as “need improvement” for 13 others.

When asked the students about hand-washing after the use of toilets, reportedly 67 % students

answered “Yes, I always wash my hands after toilet”. Around 19 % of the students reported

washing hands “sometimes” while 13.4 % students never washed hands after using toilets. The

practice of washing hands was not significantly different between boys and girls with Chi-square

value of 0.085 and p-value of 0.958. Washing hands of toilet use was not significantly associate

d with school level (high and secondary). Most of the students (45.4%) reported sometimes use

of soap, 27.8 % participants reported always use of soap while 26.8 % reported never use of soap

during hand-washing. When students were asked about the availability of the water and soap for

hand-washing in the toilets, majority of the students (72%) answered “No, Never”, 19.2 %

answered “sometimes” and 8.6% answered “yes, always”. It means in the most of the schools,

the facility of water with soap is not available for washing hands after using toilets.

Introduction

Evidence shows that handwashing with soap is the single most effective WASH intervention for

reducing the diarrhea, the second leading cause of death amongst children under the age of five

years. Good handwashing practices have also been shown to prevent and reduce the incidence of

other devastating diseases amolng all ages. So it is important to establish handwashing as a

routine practice performed in communities, homes and schools to save lives. Mainly it is

recommended to wash hands after using toilets, before eating, when hands are visibly soiled

(dirty) and when hands are visibly contaminated with blood or body fluids. It is evident that

raising awareness is the key to practice handwashing. The Global Handwashing Day, a global

awareness raising initiative on handwashing, involves especially the participation of the school

children and uses their participation as an opportunity for the awareness rising of the community.

Global Handwashing Day is now an annual event for awareness rising in many countries around

the globe including Afghanistan.

On the other hand, inadequate water supply and sanitation facilities in schools, not only are

health hazards, but also affect school attendance, retention and educational performance. Data

from countries in Africa and Asia show that less than one third of the schools have adequate

water supply and hygiene facilities as is the case in Afghanistan. International community and

government of Afghanistan are trying to improve the WASH activities in schools. Namely

UNICEF supports the WASH activities in schools around the globe including Afghanistan with

providing water and/or sanitation facilities. UNICEF supports WASH is schools at various

levels, local, national and global. At local level, UNICEF supports through provision and

maintenance of facilities and trainings of teachers. Developing design standards for water supply

and sanitation systems, hygiene education curriculum development and national policy

development are the mian tasks of UNICEF at national level to support WASH in schools. It is

important while providing handwashing and toilet facilities to take into account the

characteristics of “locally appropriate and child friendly” with incorporation of privacy and

security. The sustainability is another very core criterion that is important to take into

consideration during the dedsign of such facilities.

Taking into consideration the importance of handwashing and appropriate toilets in schools, an

assessment was conducted by ministry of education supported by UNICEF to explore the

handwashing and sanitary situation of the schools in Kabul city. The results of this assessment

will help the decision makers to prioritize the interventions and areas for a better sanitary

situation including sanitary toilets in the schools with positive effects on the learning ability of

the students.

Rationale

In Kabul, as around the globe, 100 school surveys will be carrying out to gather data and

information on the state of handwashing and hygiene at schools. The information obtained fom

this snapshot survey will be used for different purposes as below but not limited to;

Advocate for improved conditions in schools before, during and after Global

Handwashing Day by lobbying local and/or national government to make schools

handwashing friendly.

Use the facts to make plans for the school year for continued handwashing and hygiene in

school programs.

Obtain official pledges on Global Handwashing Day from government, donors,

companies and other partners to carry out the year-long handwashing and sanitation

improvement plans.

Objectives

To assess the handwashing and toilet facilities at 100 schools of Kabul city

To get a snapshot of overall hygiene conditions in 100 schools of Kabul city and use the

data for advocacy and for action planning

Methodology

Study design

A cross sectional school survey was conducted to assess the handwashing and toilet state and get

a snapshot of the overall hygiene conditions of the 100 schools in Kabul. Handwashing and toilet

facilities at schools were observed and students were also asked about.

Study setting

The survey was carried out in Kabul city, the capital and largest city of the country with more

than five million populations and around 350 schools. Currently, 11 primary schools, 93

secondary schools, 208 high schools and 43 vocational and religious schools are open for the

students in Kabul city. Secondary and high schools and their students were surveyed in this

survey, only.

Selection of the Schools

100 schools were selected for the survey in Kabul city that were involved in Global

Handwashing Day and other hygiene education programs by the Health and Youth Welfare

Department (HYWD) of the ministry of education (MoE) since four years. These hygiene

programs were supported and coordinated by UNICEF, MoPH, MoE and MRRD. Required

permissions from the officials of the schools were taken before survying.

Data collection methods

To achieve the objectives of the survey and get a snapshot, the data for this survey were collected

using observations of handwashing and toilet facilities at schools by surveyers and asking

questions from the boy and girl students regarding the handwashing practice and other hygiene

conditions.

Data collection tools

A simple and semi-structured questionnaire especially designed for 100 school handwash and

toilet survey available online was used for data collection on school hygiene conditions. The

questionnaire has three main parts; i) Basic school information ii) Observing handwashing and

toilet facilities and iii) Asking questions from students.

Data collection team

Two teams of two members each (1 male and 1 female) were recruited for data collection by

MoE in close coordination with the UNICEF. The data collectors were having at least graduation

level of education from grade 12. All of the team members were familiar with the area and the

local languages. One day orientation on the data collection tool was given to the data collectors

by UNICEF and the Health and Youth Welfare Department of the ministry of education.

Before taking the information, well informed consent was taken from the students and the

importance of the survey and its purpose was conveyed to them. The Data collection was

completed over a time period of 25 working days.

Monitoring

To assure the quality of the data during the data collection process, the representatives from the

Health and Youth Welfare Department of the ministry of education had monitoring visits to the

schools. Randomly selected schools were re assessd and questionnaires were re-administered to

insure the quality.

Database development and data entry

A user friendly database was carefully developed in EpiData software version 3.1 keeping in

mind the capacity and complexity of the work. The EpiData is a free, downloadable, suite of

applications that facilitates data entry, manipulation, editing and tabulation released by “The

EpiData Association” Odense, Denmark (In Danish: EpiData foreningen). The database was

tested and rectified using dummy data and final approved database was given to the data entry

operators. Though the software is simple to use and little training is required for data entry

operators, value was added by not only training participants in performing data entry, but also

instructing the participants on how to create EpiData data entry systems. Specifically, the

database training curriculum included:

overview of the EpiData potential uses and its suite of applications

creating a simple data entry application from a questionnaire

performing data entry

exporting data from EpiData to Microsoft Excel and SPSS

simple analysis of exported data

The data were entered into the database by two separate data entry teams of two operators, each,

in two separate files. Codebook for the questions with translation to English language was

developed and was carefully used during data entry. The consistency between these two separate

files was checked through running “validate duplicate file” and “check consistency” commands

in EpiData. The inconsistent data were then validated through hard copy of specific

questionnaire.

Data management and data cleaning

Randomly selected hard copies of the questionnaires were cross checked with related records in

soft dataset for accuracy and correctness, verified by the data manager. And as mentioned earlier

two sets of data were made and compared with differences being reconciled and final correct

dataset was used for analyses. Data were also checked for missing and outlier values and

scientific steps were taken where appropriate. The data cleaning efforts took place in ODWA

office, Kabul. The data was exported from EpiData and analyzed using SPSS statistical software

(Version 16).

Analyses

As the study was an assessment of the selected schools so we focused on the descriptive analyses

of the data. No inferential analyses were done so far. For continuous data, median and range

were calculated and reported because of the skewness of the data e.g. age of the students at

schools. Proportions were calculated and reported for describing categorical data with

frequencies. However, only frequencies were also reported because of the nature of the data and

assessment requirement. Frequencies and/or proportions were presented in the tables and graphs

for easy analyses and understanding. Bar and pie charts were used to present the categorical data

while histograms were used for continous data. To test the association (and its significance)

between two categorical variables, Chi-square tests with p-valuse were reported where

appropriate. The analyses plan we used can answer the questions easily that we need to know.

All analyses were carried out using SPSS statistical software version 16.

Results

In this assessment, an overall 100 schools in Kabul city were selected for assessment of

handwashing and sanitary situation and data were collected. The questionnaires of three schools

were missing so the results for 97 schools are presented in this report. Out of total 97 schools, 55

were high schools and the remaining 42 were secondary schools.

Observation of handwashing and toilet facilities at school

Almost all (98 %) of the schools had toilets regardless of the type of the toilet. No toilet was

present at all in two of the schools. No toilet was found in Mohammad Anwar Bismel and

Khwaja Bughra schools. According to the teachers and students they use either toilets of

neighboring houses or open area for urination and defecation. Teachers and students of three

schools in Kabul city use portable toilets. Portable toilets are used in Sitara secondary school,

Khwaja Rawash high school and Saaduddin Ansari secondary school. Six typical local toilets

(latrines) are also used in Saaduddin Ansari located in Char qala wazir abad with 2997 students.

The number of total toilets, number of in-use toilets and type of toilets in each school are listed

in the tables below. Traditional toilets (latrines) were in use in around 75% of schools. Most of

the schools had flushing or pour-flush toilets but problems in water sources, storage and supply

were the main issues for not using of the flushing and pour-flush toilets in the schools. The

percentages of the schools with different types of the toilets are presented in figure 2. However,

some of the schools have all or two types of the toilts. Approximately 10–20 litres of water per

person per day for flushing toilets and 1.5–3.0 litres per person per day for pour-flush toilets are

required (World Health Organization, 2009). Please use the number of students and in-use toilets

to calculate the ratio of students per toilet facility and compare with a national norm if any.

In 56 schools, there were separate toilets for boys and girls and in 26 schools reportedly there

were no separate toilets. The boys and girls use same toilet for urination and defecation. The

answer for the remaining school was either missing or not applicable. The percentage of the

loilets separately for boys and girls are ptresentaed in figure 3. Table 6 details the schools where

no separate toilets were reported.

A toilet is not complete without a hand-washing point with soap and water. All toilet designs

should include convenient hand-washing facilities so that hand-washing after using the toilet can

become a routine activity for students and teachers.

From the total of the 97 schools, only 19 had the hand-washing facilities (19.59%). Water

cintaner with tap is usually used as a facility of hand-washing. The types of hand-washing

facilities are detailed in figure 5 and table 8. Soaps were available only in four schools for hand-

washing purposes. The facility was rated as “very good” for only six schools and as “need

improvement” for 13 others. The names of the schools with very goog hand-washing facility are

given in table 9.

Questions from students

The median age for the interviewed students was 16 years with range [7-19] years. The

frequency and valid percentages of each age is detailed in Table 11. More than sixty (60.8)

percent of the interviewed students were girls while 39.2 percent boys were interviewed in this

study (figure 6).

Both, boys and girls were studying in majority of the selected schools 77 (80.4%). And there

were 11 schools (10.3%) with only girl students while 9 (9.3%) schools were with only boy

students (figure 7). The names of the schools with only boys and only girls are given in the tables

below. The rest of the schools had both, male and female students (see table 21).

When asked the students about hand-washing after the use of toilets, reportedly 67 % students

answered “Yes, I always wash my hands after toilet”. Around 19 % of the students reported

washing hands “sometimes” while 13.4 % students never washed hands after using toilets. The

practice of washing hands was not significantly different between boys and girls with Chi-square

value of 0.085 and p-value of 0.958. Washing hands of toilet use was not significantly associate

d with school level (high and secondary).

Approximately 79.5 % of the students reported hand-washing always before eating food, 14.4%

reported washing hands sometimes and 6 % never washed hands for food. Here again no

significant difference was observed between boys and girls having the Chi-square value of 0.167

and p-value of 0.920. School level is significantly associated with hand-washing before eating

food (Chi-square value of 6.2 and p-value of 0.045). Higher percentage of the high school

students are washing hands as compared to secondary school students (56% versus 43.8%).

Most of the students (45.4%) reported sometimes use of soap, 27.8 % participants reported

always use of soap while 26.8 % reported never use of soap during hand-washing.

When students were asked about the availability of the water and soap for hand-washing in the

toilets, majority of the students (72%) answered “No, Never”, 19.2 % answered “sometimes” and

8.6% answered “yes, always”. It means in the most of the schools, the facility of water with soap

is not available for washing hands after using toilets. Refer to the figure 9 for percentages and to

table 14 and 15 for the schools where water and soap are always or sometimes available in

toilets.

According to the students, detergents for cleaning the toilets are available “always” in 5.2 % of

Students of 61 schools (63.5%) reported that overall the toilets of their schools are clean and of

28 schools (around 30%) reported that the toilets are not clean. Students from remaining seven
