Pre-Council Executive Board Meeting

Session 1: Welcome, approval of meeting agenda
President Janko Kersnik welcomed all. New member Martine Granek-Katarivas, Israel was introduced. The Meeting Agenda was approved. Jachym Bednar, as host, gave some organisational announcements. The members then split into 3 small groups as directed by Janko in a brain-storming exercise related to the 2016 EURACT Medical Education Conference.

All members continued to work in the 4 committees. Short reports were then presented on the work done:

CME/CPD: Jachym: Discussed organising workshops in Istanbul, ePortfolios for teachers, proposing workshops for Copenhagen
BME: Natalia: Developing and finalising current research projects; Francesco: involvement in Dublin conference (through topics of mapping and core curriculum)
ST: Katarina: Worked on document being prepared on importance of research training during ST programmes (why, how, what)

Session 5a: Business meeting – part 1
1. Janko welcomed Council members and presented the Agenda
2. The report of the October 2014 EURACT Council Meeting in Porto prepared by the
Honorary Secretary Mario R Sammut (available on the EURACT website) was
taken as read and, with no feedback forthcoming from the council members
present, was approved.
3. The President’s Report (Appendix 4) was taken as read. The report was approved.
4. Approval of new members: 67 applicants, all endorsed by their respective country
representatives, were unanimously approved. The full list may be seen in Appendix
5.
5. General meeting
a. The 1st General Meeting for 2015 was opened by President Janko Kersnik,
and the set agenda was approved by the Council.
b. As 31 out of 38 Council Members were present (as confirmed by Hon.
Secretary Mario Sammut), the required quorum was reached.
c. Financial report 2014 (Appendix 6) was presented by Esra:
- New membership fees for 2015
- Income for 2014
- Details of membership fees paid in 2014
- Expenditure
Following a query by Dimitrios, Mario clarified that, according to the Articles of
Association No. 7.4, if a member does not pay his/her membership fee by the 1st
December despite repeated reminders, s/he should be issued with a notice of
termination of membership with 4 weeks’ notice.
The report was approved unanimously.

d. Audit report for 2014 (Appendix 6) was presented by Esra: only one report
from Filipe Gomes was received with a query which was clarified by the
secretariat. The other auditor did not submit a report. After a call for
volunteers, Razvan accepted to be the second auditor. The 2014 audit report
therefore will be presented for approval in the Sarajevo Council Meeting next
October following receipt of Razvan’s report.
e. Membership fees: already discussed and approved under item (c).
f. Predicted budget for 2015 (Appendix 6): this was presented by Esra and
unanimously approved.
g. Election of Honorary Treasurer (Mario): one nomination from Esra was
received by the closing date of 1 month before the election (as per point I of
the EURACT guidelines approved in Prague in 2006). According to Article
15.3 of the Articles of Association, as none of the Council members desired a
poll, the President proposed that she be elected by acclamation and the
nomination was approved with a round of applause.
h. Other issues
- Belarus Council member Dr. Ljudmila Bohush has not attended this
meeting despite receiving a letter of warning sent to Council members
who have not attended for 2 or more meetings. As per Article 9.6 of
the EURACT Articles of Association, the Council regretfully dismissed
her as Council member.
- Council members Dr. Givi Javashvili, Georgia, and Prof. Ljiljana
Cvejanov Kezunovic, Montenegro have not attended a second
Council meeting in succession. As per Article 9.6 of the EURACT
Articles of Association, the Council decided to send them a notice of a
letter of warning.
6. Activities reports
a. Leonardo da Vinci project “Continuing educational development and
harmonisation of expert teachers in General Practice/Family Medicine in
Europe through a systematic process of quality improvement”. Adam
presented an update on the project, which is on time and as planned. The
ePortfolio is nearly ready for use, and the handbook is being considered. If
translations are required for countries, representatives will need to do this
themselves by writing to Jo Buchannan as chair of the GP Teacher Appraisal
Task Group. Two meetings of the project are left till November, and a
workshop will be presented in the next WONCA Europe conference.
b. EURACT website: to be reported in Session 4b.
7. Approval of the ‘EURACT Statement on Assessment in Specialty Training for
Family Medicine’ (Francesco): approved by EB in January 2015, and distributed
twice by Francesco since then. Only one suggestion was received by Stefan
regarding a reference to EUPA which has been implemented. The statement was
unanimously approved. The statement may be viewed on
http://www.euract.eu/publications/others.
8. Future EURACT courses

a. Level 1 Course, Tbilisi, Georgia, 15-17 October 2015. Esra noted that
eighteen nominations for sponsored participation have been received. These
nominations will be vetted according to the approved criteria by a committee
appointed by the EB. Janko appealed to council members to publicise the
course for paying participants.
b. Level 2 Course, Tartu, Estonia, 6-8 October 2016. Ruth gave an update of
the plans.
c. EURACT educational conference, Dublin, Ireland, 9-10 September 2016. In
absence of Darach, Janko presented an overview of the current plans
(Appendix 7 - which he emphasised was a working document) to be
discussed in the next session.
9. Future Council Meetings
a. Autumn meeting 8-11 October 2015, Sarajevo, Bosnia and Herzegovina:
Zaim gave a presentation inviting participants to attend.
b. Spring meeting 7-9 April 2016, Adana, Turkey. Esra confimed the dates.
c. Autumn meeting 7-8 September 2016, Dublin, Ireland (see Session 6)
d. Spring meeting 2017: offer from Israel (Martine) accepted by Council.
10. WONCA Europe/World Conferences
a. WONCA Europe Istanbul Conference 21-25 October 2015: workshops,
information session, keynote, booth, etc. To be organised as usual: members
are invited to volunteer to be present at the stand.
b. WONCA Europe Copenhagen Conference June 15-18 2016: Janko invited
members to start thinking of workshops for this meeting
c. WONCA World Conference, Rio de Janeiro, Brasil, 2-6 November 2016: a
number of members are already interested in attending.
11. Action points not covered elsewhere: none.
12. Any other business
a. The website regarding next September’s Bled Course is to be updated soon
and members will be informed by Barbara when it is ready.
Session 6: EURACT educational conference Dublin 2016
The Council split into a number of groups and made proposals as follows:
Mario’s group: additions made to the plan prepared by Darach and added to by the EB in January
Jan’s group: proposed list of topics for keynotes and workshops
Ruth’s group: proposed list of topics for workshops. Abstracts are needed.
Janko’s group: some general proposals were made and questions were asked. Janko asked the moderators to send in their proposals and questions for incorporation into the plan.

Session 4b: Task groups session – cont.
The task groups submitted presentations of their work. The full reports of the Task Groups may be seen in Appendix 8. Educational Research task group: Nynke gave a verbal presentation. The Council Meeting unanimously agreed to the group’s request that a research questionnaire be sent to council members regarding selection of GP Trainees. GP Teacher Appraisal task group: Jo gave a slide presentation. Applications for translation should be submitted by October to Jo. Website task group: Mario gave a report using Word. The Council unanimously agreed with the group’s recommendation that the EURACT website be transferred to the WONCA Europe website.
Session 8: Educational research / Future of EURACT (Presentations by 3 Council members to Czech trainers in concurrent session)

A discussion followed Jan’s presentation, with the following being a summary of the suggestions and comments:
Jan’s proposals should be discussed during the next Council Meeting.
Agreement was expressed with a mission statement be drawn up for EURACT.
Young GP Teachers need a stage where they present their work.
Requests for funding could be made through EURACT, and that departments from different countries could collaborate together in projects.
Prospective representatives could be allowed to accompany outgoing representatives at Council meetings.
It was augured that young VdGM doctors from the host country will, as in the past, continue to participate in Council Meetings.
There should be regular turnover of country representatives, with new blood coming in.
A sub-group of EURACT be formed for young members, allowing them to meet in parallel to the Council meeting. Also special interest sub-groups could be formed.
The importance of taking the opportunity of Council meetings to organise visits to local GPs in their practices.
EURACT provides the opportunity for networking.
Council members must work in between meetings as well.
Council membership should be neither a lifetime mandate nor a short rotation.
We need to find a way to involve all national members, not just the young trainees.
Council Members should be able to present their works and their research: this is a vital point for involvement.
The national representative's role should involve less bureaucracy and more time to be a proactive pioneer
Mario stated that the EB will discuss and plan the best way to tackle Jan’s proposals and the suggestions made by members during the next Council meeting. (In a concurrent session, 3 Council members gave the following presentations to Czech trainers:
CME-CPD and re-certification of GPs in Europe - Ruth Kalda (Estonia)
Framework for continuing educational development of trainers in GP-FM in Europe - Adam Windak (Poland)

Session 9: Joint Meeting with Czech EURACT members
EURACT Council member Dr Jachym Bednar gave a presentation entitled ‘Czech Republic – heart of Europe’, after which Dr Josef Stolfa, leader of Czech ST trainers, talked about Specialist Training in the Czech Republic.

Farewell to Jana Bendova, interim election to be held in Slovakia
Roar Maagaard reported on development of “Recognition of Professional Qualifications in EU/Document on professional standards”. The Council supports Roar to continue with the process. A discussion for the next meeting, a topic for agenda.
Martine Granek invited the Council to the 1st International Congress on Controversies in Primary Care and Outpatient Care (COPOC) in Tel Aviv, May 20 - 22.
Session 10: Review of meeting.

Group work in trios, discussing future topics and giving comments on the Prague meeting (journal club and appraisal work were great - more work, less business; excellent organization of the Prague meeting, excellent Jachym’s presentation – a proposal that every country prepare a PP presentation of a national GP history to be put on the website… We have to send the meeting topics and papers soon enough so the Council members can prepare).
At the end of the meeting, Janko thanked Jachym and the Czech College.

Tbilisi course: agreement to be signed, deadline for approved sponsorship, names to be known by May 5th
Elections in Belgium and Slovakia, letters to be sent out by May 10
Dublin conference: regular contacts to be established, skype meetings for important items, PCO is probably needed. Website with preliminary programme and key note speakers, fees is urgently needed. External visibility is needed, promotion materials for Istanbul (leaflet with keynotes and topics). Janko will write to Darach by May 5th to suggest a skype meeting (Darach, Janko and Mario). Proposals for keynotes: Mette Brekke, 1 Irish… 6 key notes is too much, 3 – 4 is enough. We go for up to 200 participants and intense promotion after this summer. We go for abstracts and posters. Should we invite other networks and WESIGs – How to teach special topics? Social programme to be organized to attract people. Perhaps an Irish style of the opening ceremony in the pub?
Journal club after Jan: how can we keep it? Nynke will probably take over but that will be decided next time. Perhaps we can include it in the programme of the Sarajevo meeting?

Appraisal workshop in Sarajevo: we plan 1 hour and a half. Just for a demonstration? Task force members to be educated first?
Institutional membership: 4 members at least, application form to be modified with introduction text (Adam). We have to change the text on the website. Bruce and Dimitrios to be informed. A list of these organizations to be published on the website.

Peterburg, Russia, F. Carelli, Milano, Italy. • Background. A 2007 study from the UK showed that approximately 80% of patients
aged 50 years and older in the primary care setting have multiple health problems. Despite the increasing numbers of patients with multimorbidity, the delivery of care is usually built around one single disease. Guidelines are derived from studies that often exclude patients with multimorbidities. The studies highlight difficulties of managing multimorbidity in family practice. The amount of educational literature about patients with multimorbidities is very limited. However, findings indicate that recognition of the significance of multimorbidity in family practice is growing. Clinicians need a management approach that considers the challenges particular to each individual, including patient preferences, goals and prognosis.
• Aim of the workshop.

• Identifying the most challenging areas of clinical practice in the case of patients with multimorbidity.
• Methods of the workshop. The workshop will start with a small presentation about clarification of the terms and importance of multimorbidity in primary care setting. After this participants will be divided into small groups (5-6 people) to discuss on a case-vignette on a preestablished structured way. The groups are organised so that in each there are some younger and some more experienced FDs. These groups will be asked to identify the most problematic areas in working with patients with multiple diseases as well as components of good practice to overcome these problems.
• Results and Conclusion. At the end of the workshop the participants are more aware about the complexity of the multimorbidity and know more about their personal learning needs related to this challenging topic.

• Learning needs for cultural competence in family medicine, so to develop a teaching curriculum in culturally responsive healthcare for medical education in Europe.
• Background: • Cultural competence is a set of behaviors, attitudes, and policies that come together
in a system and among professionals that enables the system or the professionals to work effectively in cross–cultural situations. It is an important skill because it reduces disparities in health services and increases detection of culture-specific diseases. Adoption of cultural competent behaviour is a lifelong continual process starting from sensitivity and awareness leading to knowledge and skillful practice. The problem of racial and ethnic health disparities and specific barriers affecting the provision of this care are documented and well known. At the present time, in Europe, no universal professional training and curriculum in cultural competence exists. It is dealt differently and with significant differences across the European Union.
• Aim of the workshop: We let participants to be aware of what cultural competence is to think over and summarize in 1,5 hour workshop their learning needs in GP, so to work out a future model as teaching curriculum in culturally responsive healthcare .
• Methods of the workshop: Participants will be split into three groups and each group will be asked to make up learning needs for one of three target groups they choose (group at BME, ST, CPD/CME level) describing besides the learning objective of the WS (according to adoption of cultural competence in level of awareness knowledge or skill - they choose the level too: also methods, tools, used materials and resources,). The groups will present the results to the others.
• Results and conclusion: After the workshop participants are more aware of their learning needs and as well as colleagues´ ones and through learning cultural competence they are more interested to be involved also in teaching and developing of the model of culturally responsive healthcare in their countries. Together with WONCA SIG - Migrant Care EURACT group would like to develop teaching curriculum in culturally responsive healthcare for medical education in Europe.

Natalia, Ivana, Ilse, Mira, Markku, Diurdica, Francesco Absent: Stefan, Givi On Thursday, Natalia presented the development in our long and really difficult research on "Activities to enforce General Practice / Family Medicine around Europe - a Delphi method". This took near all the first hour at disposal. The matter was going forward really well. Also, on Saturday, Natalia and Stefan (arrived on Friday evening) discussed on Saturday morning, the best methodology to interpret, analyze and congregate the collected data for a paper of research to be published as the Committee used to do. On Friday (half an hour at disposal) we agreed to present a WS at the EURACT Educational Conference in Dublin, September 2016. We decided to focus on our last publications: Core Curriculum and Mapping of Undergraduate in Europe, to be managed as a WS. Ilse will manage in large as timetable during the period going to September 2017 and as speakers duties and as abstract. Valentina and Francesco will develop the matter also as a questionnaire concerning the Mapping. research should be part of the specialization, and different experiences to be given

Appendix 4: President’s Report Since the last meeting in Porto I attended Continuing Educational Development and Harmonisation of Expert Teachers in General Practice/Family Medicine in Europe through a systematic process of quality improvement (CEDinGPII, No. 2013-1-PL1-LEO05-37537) project meeting in Ljubljana November 21-22. Jo Buchanan replaced Justin Allen because he was not able to attend. There were also EURACT invited experts on assessment Renzo de Gabriele and Francesco Carelli taking part. Next project meeting I took part was in Vienna 20-21 February. Jo Buchanan also took part as member of the EURACT staff on behalf of missing Justin Allen. There were also EURACT invited participants Jachym Bednar and Jana Bendova, who were asked to fill in electronic portfolio; Renzo de Gabriele attended at his own expenses. More details on the progress of the project will be presented during other sessions. I attended winter EB meeting in Vienna January 9-11. We reviewed EURACT businesses and reached some decisions including the proposal on the draft criteria for organisations in collaboration which shall be finalised by MSC and approved by EURACT Council. Membership fees were adjusted to the GDP. More details in the attached EB report. As Honorary Secretary I attended two WONCA Europe EB meetings in Prague in November 7-9 and in Istanbul April 9-12. The composition of the WONCA Europe EB will not change drastically if WONCA Europe Council accepts our most recent proposal, i.e. 4 officers proposed by MO, 3 nominated by EURACT, EGPRN and EQuiP and one by VdGM. WONCA Europe Conference: 16 – 18 June 2016, Copenhagen, Denmark: All networks will be represented – streams related to key topics (education, research, quality, VdGM). EB will propose to the Council that EJGP becomes open access. On behalf of WONCA Europe I attended the XIV. congress of CSAKOSZ, a Hungarian member of WONA Europe in Hajduszoboszlo near Debrecen in Hungary February 26-28. I used the opportunity to present EURACT. As a result of good collaboration with EURIPA (I sit in EURIPA EB as a representative of EURACT and I am liaison for EURIPA in WONCA Europe EB) I was invited speaker at the 13th Wonca World Rural Health Conference 2015, Dubrovnik April 15-18. I co-authored a few WS and the conclusion of one was to work on a statement what rural practice teaching can add to undergraduate education of future doctors. EURACT and EURIPA shall join in this task. Similar collaborative efforts will lead me to Timisoara to EGPRN conference, where we will discuss teaching research skills. This would enable us to take the decision what needs to be archived and what needs to be kept for the new website. This work will be shared between members of the editorial board according to a timetable. Articles would be rated as follows: essential, useful, to be archived, to be omitted. Regarding the rebuilding/transfer option, rebuilding the website in Ireland would cost 3-4000 Euros. As regards transferring it, contact has been made with WONCA Europe who has offered to host the EURACT website on www.woncaeurope.org. The advantages of the latter option are:

Educational Research task group
Nynke Sherpbier de Hann, rapporteur We started with critical assessment of a research paper: A competency based selection procedure for Dutch postgraduate GP training, a pilot study on validity and reliability - Margit Vermeulen et al. We discussed the psychometrics of a test. We had a discussion on the goal of selection procedures: predicting poor performance of selecting the best candidates? We decided to propose the council to perform a survey on selection procedures, to answer the following research questions:

• How many countries have a selection procedure?
• What is the purpose of the selection?
• What kind of selection procedure is used?
• Who is performing the selection procedure?
• Who defines the number of vacancies for primary care specialty training?

many candidates are not admitted?
• Is there an entrance limitation for medical school?
• How many doctors qualify every year from medical school?
• How many dotors that were trained abroad apply for primary care specialty training?
• Who is financing the training program and the salary of the trainees?
Council members agreed with a survey, which will be held before the meeting in Autumn 2015.

Adam summarized the current position of the project which is on schedule and will be ready to be
passed on to EURACT by December 2015. The e-portfolio is reaching completion having been
trialled at the last partner’s meeting. The electronic platform is straightforward to use and the
process of assessment is simple and not time consuming. The group considered how EURACT
should take this project forward to the implementation stage.

Administration – there needs to be one administrator for the electronic platform – this could be
part of the EURACT secretariat or contracted elsewhere. The role does not require a high level of
technical expertise.
Council will need to have an oversight of the process and it was thought that this would best be
done by a permanent task group.
The question of funding was discussed, it was thought this should be a benefit of membership for
assessment at the competent level. It was thought, reasonable to charge a fee for the assessment
at expert level to cover administrative costs.
Assessment - at the competent level this will be done at a country level by one appraiser,
although it was suggested initially that there should be an option for this to be done out with an
applicant’s country. The competent level will be able to be completed in the national language and
there is a facility for making translation straightforward, the expense of translation will need to be
met by individual countries. The expert level will be assessed internationally and in English.
The selection and training of appraisers: It is envisaged that council members and authors of
the project would be expected to take on this role and implement the system in their country.
Training will be provided in a two-stage process at the council meetings in October 2015 and April
2016.
An observation was made about the risk of problems that may occur when an appraisee is known
to an appraiser as will occur commonly and the potential difficulties where judgements are made
on areas where cultural bias may be relevant. These issues will need to be dealt with in training.
The appraiser will not be paid even at the expert level. It was felt that this could be seen to be part
of our academic role and it is not expected to be onerous. The assessment will generally take
about 30 minutes once the appraiser will be
Promotion – The national representative will promote the process at a national level. Workshops
at international conferences such as WONCA will be used to disseminate information and training
about the project. The project provides an opportunity to increase EURACT’s profile and expand
the membership.
