PART 1 - THE SCHEDULE

DEFINITIONS

1 Act The Fire Safety and Safety of Places of Sports Act 1987.

2 Authorised Person Person authorised in accordance with Section 35 of the Act.

3 Holder The person determined by the Local Authority as being in a position to prevent contravention of the terms and conditions of this Certificate, referred to in the Safety of Sports Ground Act 1975 as a "qualified person", or their nominee for the time being approved in writing by the Local Authority.

4 Local Authority Oxfordshire County Council.

5 Chief Constable The Chief Constable of Thames Valley Police or such designated officer.

6 County Fire Officer The Chief Fire Officer of the County of Oxfordshire.7 Ambulance Service Chief Executive of the South Central Ambulance

Service NHS Trust.8 Responsible Person A person who is concerned in the management of the

Regulated Stands or the organisation of any activity taking place there at the time when an offence is alleged to have been committed. (Section 36 (3) of the Act).

9 Specified Activity The use of the Regulated Stands.a. During the playing of and the period one hourimmediately before and one hour immediately after following the end of Rugby Union and Rugby League Football Matches; orb. At any other time as may be approved, in writing, by the Local Authority after prior consultation with the Chief Constable.

10 The Code The "Guide to Safety at Sports Grounds" published by Her Majesty's Stationery Office on Direction of the Home Office or any replacement, addition or amendment to that document.

PART 2 - GENERAL CONDITIONS OF THIS CERTIFICATE

1. The responsibility for the safety of spectators in the Regulated Stands lies at all times with the Holder. The Holder, or a nominee approved in writing by the Local Authority, shall ensure compliance with the terms and conditions of this Certificate and shall be present whenever the Regulated Stand(s) are used for a specified activity.

2. Any responsible person (as defined in Section 36(3) of the Act) shall assist the Holder to ensure compliance with the terms and conditions of this Certificate and shall not obstruct the Holder in carrying out their obligations.

3. The Holder shall ensure that whilst the Regulated Stand(s) are being used for a specified activity, the maximum number of spectators admitted to the Regulated Stand(s) shall not exceed the capacities set out in Part 3 hereof.

4. Subject to the terms and conditions hereof and general statutory requirements, the Regulated Stand(s) may be used, ancillary to a Specified Activity for one or more of the following:-

a. the playing of music or singing;b. the provision of news or comment associated with the specified activity;c. the sale of tickets for forthcoming activities, programmes, news-sheets and

souvenirs;d. the sale and consumption of refreshments;e. public collections of any nature with the prior written approval of the local City/

District Council;f. any other activity ancillary to a Specified Activity which has the prior approval

in writing of the Local Authority.

5. Unless any provision of this Schedule requires otherwise, where prior written approval is required under the terms and conditions hereof, it shall be sought in writing and served on the appropriate officer at least twenty eight days before the occurrence in respect of which it is required. In relation to references to written approval from the local authority, the appropriate officer shall be the County Emergency Planning Officer.

6. The Holder shall make and keep the records referred in Part 11 hereto which shall be available at all reasonable times to any authorised person.

7. The Holder shall produce a contingency plan to deal with emergencies. The plan shall I cover all reasonably foreseeable contingencies and shall identify appropriate individuals and their respective tasks. A copy of the plan shall be given to all stewards and other staff employed. The Police, Fire and Ambulance Service and Local Authority will be consulted by the holder before the contingency plan is produced.

8. The Holder shall carry out such works or repair and maintenance to existing structures in the Regulated Stand(s) as may be required and confirmed in writing from time to time by the Local Authority.

9. Where a Club Inspection Report under paragraph 15 of Part 11 of this Schedule or the result of the crush barrier testing under paragraph 16 of Part 11 of this Schedule shows that in the opinion of an Authorised Person remedial works are necessary to prevent danger to spectators, such works shall be carried out before the next Specified Activity takes place and the area of the Regulated Stand which may be dangerous to spectators shall be fenced off and made inaccessible to spectators to the satisfaction of the Local

10. Authority and the maximum number of spectators permitted under Part 3 of this Schedule shall be reduced appropriately by the Local Authority.

11. The Holder shall comply with any Special Terms and Conditions specified in Part 12 of this Schedule.

12. No flammable gases or liquids shall be stored or used in any part of the Regulated Stand without the prior written approval of the Local Authority.

13. No accumulation of flammable rubbish, dirt, surplus material or goods shall be permitted to remain in any part of the Regulated Stand except in a manner, place and quantity approved in writing by the Local Authority.

14. No alteration or addition shall be made to the Regulated Stand(s) without the prior written consent of the Local Authority.

15. The siting in any part of a Regulated Stand of television cameras and equipment shall be agreed with the Local Authority prior to their use with particular reference to structural stability and safety. A minimum of 7 days notice shall be given to enable an inspection to be made before the contractor who erects and installs such cameras and equipment leaves the site.

16. The Holder shall produce a written policy of safety for spectators which shall be made known to and understood by all staff and voluntary workers involved in ground operations. The Holder shall keep the policy under review and revise it as necessary.

17. The Holder shall appoint a Safety Officer and a Deputy Safety Officer who shall be of sufficient competence, status and authority to ensure that all matters relating to spectator safety are carried out promptly and effectively. The Safety Officer or an appointed Deputy should be easily identifiable and shall be in attendance at a central control point when an event takes place. The sole duty of that person during the course of the event shall be that of safety.

18. While the public are in the Regulated Stand(s), the Holder shall ensure that sufficient staff are on duty to cover entrances, exits (including any emergency exits) and other strategic points or areas in accordance with Part 5 of this Schedule.

PART 3 - STAND CAPACITY

1. The number of spectators permitted to occupy the Regulated Stand(s) shall not exceed the following figures:-

EAST STAND - 892 (Eight hundred and ninety two)WEST STAND -715 (Seven hundred and fifteen)

2. Any proposed variations to this figure shall be notified to and approved by the Local Authority.

3. A spectator counting system shall provide continuous monitoring of spectator numbers in each area referred to in paragraph 1 of this part so that the duty safety officer can immediately give instructions to close any gate or turnstile if necessary. The numbers of spectators admitted shall be continuously passed to the control point.

4. Arrangements shall be made for the control point to be informed immediately that 90% of the total capacity figure referred to in paragraph 1 of this part has been admitted.

5. Where entrance is by ticket, the number of tickets issued for each particular stand shall not exceed the permitted capacity figure for that stand (set out in paragraph 1 of this Part) less the number of season tickets issued for that stand.

PART 4 – MEANS OF INGRESS AND EGRESS

1. The Holder shall ensure that all means of ingress and egress are maintained in good repair and in such condition as to fulfil safely their required functions and shall comply with the following requirements:

a. A comprehensive system of signposting which clearly indicates routes to exits from the ground, shall be provided. Signs relating to fire safety shall be in accordance with BS 5499 : Part 1 and other safety signs shall be in accordance with BS 5378: Part 1.

b. All gates in radial or perimeter fences of spectator areas or other self- contained areas shall be painted in a different colour from the rest of the fence and marked "Emergency Exit".

c. During a Specified Activity all doors and gates which form an egress route between any spectator accommodation and the public highway or other open space outside the Regulated Stand(s) shall be capable of being opened immediately and easily from the inside in an emergency. The steward for that area will be responsible for opening in an emergency.

d. For clearance of the Regulated Stand(s) at the end of the activity, gates or doors shall be secured open not less than 10 minutes before the end of the activity.

e. All means of ingress and egress shall be kept free from obstruction. The direction of opening of all doors shall not be altered without the prior written approval of the Local Authority.

f. Doors fitted with self-closing devices shall be maintained self-closing at all times and shall not be kept in an open position, except in the case of doors fitted with automatic release mechanisms approved by the Local Authority.

g. All stairs and steps comprising parts of the means of ingress and egress shall be maintained with non-slip even surfaces and stair coverings shall be secured and maintained so that they will not in any way be a source of danger to persons using them.

PART 5 - SAFETY OFFICER AND STEWARDS

Safety Officer

1. The Safety Officer shall be a person of sufficient competence, status and authority to take responsibility for safety at the Regulated Stand(s) and be able to authorise and supervise safety measures. The Safety Officer shall have direct access to the Club Management.

2. The Safety Officer shall be responsible on behalf of the Holder for all safety matters, I inspections, tests and training and shall examine and sign all records which are required by Part 11 of this Certificate to be made before, after or between every Specified Activity.

3. The Safety Officer shall be easily identifiable and shall be in attendance at the central control point. The Officer shall not be engaged in any other task whilst the Stadium is open for admission of spectators and his/her sole duty shall be that of safety.

Stewards

4. Effective stewarding should prevent overcrowding in sections of the ground, reduce I the likelihood and incidence of disorder, help disperse spectators in such a way as to reduce crowd pressure and provide the means to investigate, report and take early action in an emergency.

5. The Holder shall appoint a Chief Steward responsible for co-ordinating the responsibilities of all stewards. The Chief Steward should attend any police pre match briefing. Control of Stewards should be from a central point with an efficient means to communicate with them.

6. Staffing levels and stewarding requirements will vary depending on the size and configuration of the ground, the nature of the event and crowd levels, but there are six basic duties which stewards must carry out. These are:

a) to control or direct spectators who are entering or leaving the ground in order to help achieve an even flow of people to the viewing areas;

b) to recognise crowd densities, signs of crowd distress and crowd dynamics so as to help ensure safe dispersal of spectators on the terraces or viewing slopes;

c) to patrol the ground to deal with any emergencies, e.g. raising alarms or tackling the early stages of fires;

d) To staff entrances and exits to and from the viewing accommodation and other strategic points, especially exit doors and exit gates from the ground which are not continuously open whilst the ground is in use;

e) to assist police as appropriate or as requested with crowd control; and f. to undertake specific duties in an emergency.

7. Stewards shall be fit, active, not less than age 18 and ideally not more than 55 years old. They shall be fully capable of carrying out the duties allocated to them. Every Steward shall be properly trained in their duties.

8. All Stewards shall be readily identifiable, eg. by the wearing of luminous tabards which are distinctive from those worn by police, fire and ambulance services.

Armbands are not acceptable. Chief Stewards shall be distinguishable from the Stewards.

9. Before a specified activity each Steward shall be briefed thoroughly, handed a copy of their instructions and made fully aware of their general responsibilities and duties in the event of an emergency.

10. Any supplementary Stewards from visiting clubs who will be operational during an event must arrive before the ground is open to the public and in time to be fully briefed upon the safety arrangements and their specific duties.

11. All Stewards and persons employed in the vicinity of the Regulated Stand(s) shall receive practical instruction and training appropriate to their responsibilities in the event of an emergency. Exercises for Stewards and persons employed in the vicinity of the Regulated Stand(s) shall be carried out annually.

12. A minimum of 3 stewards is required by the Local Authority.

13. All stewards shall be fully briefed and trained in the recognition of crowd densities, in the recognition of signs of distress and in crowd dynamics.

14. A record of the training and instruction given and exercises held shall be kept and shall include the following matters:

a. the date and time of the instruction or exerciseb. duration of the instruction or exercisec. name of the person giving instruction or conducting the exercised. name of the persons receiving the instructions or taking part in the exercisee. nature of instruction or exercises.

15. After initial instruction, all Stewards and persons employed in the vicinity of the Regulated Stand(s) shall receive further instruction given by a competent person, at least once in every period of three months.

16. All Safety Stewards shall be made aware of the exact location of the Emergency telephone, communications equipment and fire equipment within the vicinity of the Regulated Stand(s) and how to operate the equipment. Stewards shall be capable of recognising potential fire hazards and suspect packages and reporting such findings immediately to the Safety Officer. Stewards shall understand the methods or signals used to alert staff that an emergency has arisen.

17. No steward shall leave their place of duty during a specified activity without the permission of the appropriate supervisor, who shall provide a replacement Steward before the position is vacated.

PART 6 – POLICING

1. The Police and the Local Authority shall be consulted if any match day crowd capacity could affect the safety of the occupants of the regulated stand/s.

2. In this event the Police may impose a special safety plan for the ground and/or seek a more suitable venue for the match to be played.

PART 7 - FIRST AID AND MEDICAL FACILITIES

1. The Holder shall ensure that during any Specified Activity the following medical facilities are available:-

a. At each event there shall be a minimum of 2 trained first aiders and at least one trained first aider per 1,000 spectators.

b. The Holder shall provide accommodation designated for the provision of first aid to spectators. Any room set aside for this purpose should be large enough to contain a couch with enough space for people to walk around; a sink providing hot and cold water and drinking water; a worktop; and sufficient room to store stretchers, blankets and pillows. Toilet facilities should be available. The recommended minimum size of the room is 15 square metres. Care shall be taken to ensure that the doorway is large enough to allow access for a stretcher or a wheelchair.

c. First aid rooms shall have heating, lighting, ventilation and electric sockets. The room shall be signposted throughout the ground and located in such a position that it is accessible to both spectators and emergency vehicles. A telephone line shall be installed allowing internal and external communication. Procedures for contacting emergency services and local hospitals shall be displayed in the first aid room.

d. The first aid room shall contain suitable storage for first aid materials and equipment. The holder shall appoint someone with sole responsibility for the equipment and materials in the first aid room and for the upkeep of the room. The Holder shall ensure that all appropriate standards of hygiene are maintained. These facilities shall be available at any time for inspection.

e. The Holder shall designate an area to be reserved exclusively for an ambulance, the area being clearly marked. The ambulance shall have clear access/ egress at all times.

PART 8 - ELECTRICAL SAFETY

1. The Holder shall ensure that electrical fittings, wiring, switches and all electrical apparatus or appliances shall be maintained safe and free of any hazard. Any alteration shall be subject to the prior written approval of the Local Authority.

2. All electrical installations shall comply with the Electricity at Work Regulations 1989 and also with the Electricity Supply Regulations 1988. New electrical installations shall also comply with the edition of the Wiring Regulations of the Institution of Electrical Engineers (IEE) current at the date of installation. The IEE Completion Certificate as prescribed for electrical installations in the IEE Regulations shall be retained by the Holder.

3. The installation shall be regularly inspected and an IEE Inspection Certificate as prescribed in the LEE regulations shall be obtained. The interval between inspections shall be no longer than five years, and necessary repairs promptly carried out.

4. A diagram of the main electrical circuit shall be fixed in a position easily accessible to the technical staff. All main switches or circuit breakers shall be clearly labelled to indicate the circuits which they control.

5. The base of any floodlighting tower shall be earthed in accordance with BS 6651 which also contains advice on bonding and earthing for lightning protection.

6. An alternative electricity supply which functions for a minimum of three hours in the event of a failure of the normal supply shall be provided to the following installations :-

a. Emergency lightingb. Escape lightingc. Emergency telephoned. Fire alarme. CCTV control and camera systems (if fitted)f. First Aid roomg. Public address system.

PART 9 - EMERGENCY LIGHTING AND COMMUNICATIONS

Emergency Lighting

1. Emergency lighting for use in the event of a failure of the general lighting shall be provided in all Regulated Stands and along all exit routes, with exit signs clearly illuminated. Installation shall be in accordance with BS 5266.

2. The emergency lighting shall operate automatically on the failure of the normal lighting.

3. Emergency lighting systems shall be tested before each specified activity in accordance with BS 5266: Part 1. (See Part 11 of this Schedule).

Communications

4. There shall be a communications system capable of clearly relaying essential messages to all Regulated Stands in both normal circumstances and in an emergency.

5. There shall be a central control point commanding a good view of each Regulated Stand for the use of the safety officer.

6. The public address equipment shall be installed so that broadcast messages can be heard under reasonable conditions (including emergencies) by all persons of normal hearing in any part of the Regulated Stands to which the public has access.

7. Installation shall conform to the relevant guidance contained in BS 6259 (Code of Practice for Planning and Installation of Sound Systems).

8. The public address system shall be staffed for the duration of each specified activity by a person who is capable of transmitting messages on behalf of the Safety Officer.

9. This member of staff shall be familiar with the emergency procedures for the Regulated Stands.

10. The Holder shall ensure that the communications and warning systems, including the public address system are maintained and inspected to the satisfaction of the Local Authority and inspections and/or tests shall be recorded and the results thereof available for inspection by an Authorised Person, in accordance with Part 11 of this Schedule.

PART 10 - FIRE SAFETY

1. Emergency and fire fighting equipment shall be provided and maintained by the Holder in accordance with the requirements of the Local Authority.

2. The Holder shall provide persons trained to operate the emergency and fire fighting equipment.

3. All fire equipment shall be maintained in efficient working order.

4. Portable fire extinguishers shall be examined at least once annually and tested by a competent person in accordance with the relevant British Standard.

5. The date of such test shall be clearly marked on the extinguisher.

6. The fire alarm where fitted shall be maintained in efficient working order, and shall be tested or examined in accordance with the relevant British Standard.

7. In addition it shall be tested weekly using a different call point for each successive test.

8. The results of such tests shall be recorded in a Log Book.

9. The Holder shall ensure that all fire precautions required by the Local Authority are suitably maintained and any defects rectified immediately.

10. The Holder shall ensure that all parts of the Regulated Stands are inspected before and after each event and any accumulation of litter removed. Sufficient litter bins shall be provided and arrangements made for the frequent emptying of them during an event.

11. Each member of staff and each Steward shall be fully trained by the Holder in the procedures to be adopted in the event of fire or other emergencies. Such training shall include:

a) action to be taken on discovery of a fire or other emergencyb) the method of calling the Fire Service c) the method of operating the fire alarms and procedures to be followedd) operation of the emergency telephone systeme) familiarisation with the location of emergency telephone and fire alarm call points

throughout the groundf) the correct method of using fire fighting equipment and the familiarisation with the

location of equipment throughout the Regulated Standg) evacuation of the ground the location of assembly points and specific action required

of individual members of staffh) familiarisation with all escape routes and the methods of maintaining such escape

routes free of obstructioni) the purpose of fire resisting doors and the need to maintain them in the closed

position.

PART 11 - SPECTIONS, TESTS AND RECORDS

1. The design and construction of buildings and installations shall be in accordance with good engineering and building practice, as set out in the relevant British Standard specifications and codes of practice. They shall be properly maintained and repaired by suitably qualified persons. Specialist advice from appropriately qualified engineers belonging to a recognised relevant Institution shall be sought to assess the safety and strength of load-bearing elements in buildings and installations such as crush barriers, brick/block walls etc.

Records2. The Holder shall keep records which shall be available at all reasonable times to any

Authorised Person.

3. The Holder shall retain the records for at least 6 years.

4. All entries in the Record Book shall show the name of the Approved Inspector who carried out the test or examination, the subject of the entry, the result of the test or examination and date of such test or examination.

5. The record Book shall be kept available for examination at any time within normal working hours by an Authorised Person.

Pre-event - Inspection Before Each Specified Activity

6. Before every Specified Activity to which spectators are admitted the Holder shall record the result of the following checks, inspections or tests and any remedial action taken. The results of checks, inspections and tests shall be in a form agreed by the local authority (see Annex D)

a) The general fabric of the stands, in particular, signs of damage, corrosion or deformation. Particular attention should be given to the condition of each terrace, stairway, exit route, access route, and directional signs, balustrades and handrails.

b) All gangways, passageways, ingress and egress routes for obstructions, damaged or malfunctioning fire doors, doors and gates, hazards under foot, missing or damaged signs or other impediments to their safe use.

c) All exit doors and gates which shall be capable of being easily and fully opened.d) All areas to which spectators have access for litter, debris, building materials and

potential missiles.e) Any void areas, kitchen and food preparation areas and other areas particularly

vulnerable to fire for combustible materials and other hazards.f) Any machine rooms, plant rooms and store rooms for litter, waste or unauthorised

storage of combustible materials or hazardous products.g) All artificial lighting which it is anticipated will be used during and immediately

following the event and all emergency lighting.h) The location of any TV equipment, vehicles, cables, gantries and supports if

applicable.i) The required fire fighting equipment is in position and available for use.j) The first aid rooms and equipment are ready for use, are in a clean condition and

have a supply of water, electricity and heating available.

k) The emergency telephones, fire alarms, fire detectors, CCTV systems (if fitted), public address system, including emergency tone and override facility (and the repeater panel in the stadium control room in accordance with the methods laid down in BS 5839: Part 1 (including a test from the secondary source of power supply).

Post-Event -Inspection after Specified Activities

7. The Holder shall ensure that after each Specified Activity the Regulated Stand(s) shall be inspected, the results of such inspection shall be entered in the Record Book. Any signs of damage which may create a danger to the public shall be rectified forthwith.

8. Attendance RecordsA record of the number of spectators admitted to the stands/terraces during each Specified Activity and in each separate area mentioned in Part 3 of this Schedule

9. Accident RecordsA record of any accident or incident occurring whilst the Regulated Stand(s) is in use for a Specified Activity which causes injury to a spectator, such records to include the name, address, age and gender of the spectator injured, the nature of the injury and how it was caused, (as far as can be ascertained by diligent enquiry) and the location of the incident.

11. A record of the names of Stewards on duty at each Specified Activity, positions allocated to them within the Regulated Stand(s) and the briefing on their general and emergency duties. A record of the training and instruction given and exercises held shall be kept and shall include the following matters:-

a. the date and time of the instruction or exercise;b. duration of the instruction or exercise;c. name of the person giving the instruction or conducting the exercise;d. names of the persons receiving the instructions or taking part in the exercise;e. nature of instruction or exercise.

12. Escape LightingThe Holder shall have available a certificate with the inspection and test results signed by a competent person of a 3 hour full load test of the escape lighting batteries. Where self-contained battery operated escape luminaries are installed, the Certificate shall confirm that each luminary has been energised from its battery for 3 hours.

13. Fire Protection EquipmentA record shall be maintained in a form agreed by the Local Authority of the maintenance and overhaul of all fire fighting equipment in accordance with BS 5274, BS 5306 : Part 1 and 3, BS 5423 and BS 6575 as appropriate by a recognised person or company of fire engineers.

14. Stands/TerracesA detailed inspection of the stands/terraces, including all components and installations, shall be arranged annually by the Holder in order to ensure that load-bearing elements are capable of withstanding the pressures to which they are likely to be subjected and that they are fit for their intended purpose. Inspection shall be carried out by a person with a relevant, recognised professional qualification and with experience of inspecting structures.

15. The Holder shall supply annually, within one month of the last specified activity of the season, a report (in the form set out in Appendix C or such other form as may be required by the local authority) duly completed and signed by an Approved Inspector and showing a programme of remedial works necessary to remedy any defects revealed in such report.

16. Crush BarriersAll crush barriers within the stands shall be tested by an Approved Inspector in the manner specified in Annex D of The Code. The results of such tests shall be sent to the County Emergency Planning Officer and entered in the Record Book by 1st July each year.

17. All other barriers and handrails shall be visually inspected each year for signs of wear, distortion, corrosion or other signs of weakness and the results of the inspection recorded.

5 YEARLY

18. Electrical InstallationsThe results of inspections and tests of the electrical installations shall be kept available for inspection. The Certificate shall be in the form prescribed in the current edition of the "Regulations for Electrical Installations" issued by the Institution of Electrical Engineers. The Certificate shall confirm that the installation is in a safe working condition and shall be signed by a competent person.
