The following clarification is issued in  Affidavit under Annexure-V of the Admission Notice 1. The Annexure-V has to be submitted along-with the application. 2. The language of the Annexure has been suitably revised. The revised Annexure-V to be executed and submitted along with the application is enclosed. 3. Candidates, who have already submitted their applications, may only send copy of the revised Annexure-V and bring the same in original at the time of counselling. There is no change in the existing schedule of events notified earlier.     This is in continuation of the admission notice issued on 13/8/14, inviting

    applications from Wards of Insured Persons IPs) for admission in UG course

    MBBS) in ESIC Medical Colleges for academic session 2014-15. The following

    Annexure - V

    REVISED AFFIDAVIT By All Candidates

    On Non-Judicial Stamped paper of Rs. 50/-

    I, Ms./Mr , 5/0/0 Shri , resident

    of hereby declare as under

    1. That I understand that if I am granted admission to MBBS course at

    ESIC Medical College for the academic year 2014-15, the fees and

    bond amount being charged/prescribed presently is subject to

    revision of fee structure and Bond amount.

    2. That I understand that the tuition fee amount

    Rs. 24,000/- per

    annum to be paid at the time of admission is on provisional basis.

    That it has been explained to me that the fee structure is under revision and

    fees are likely to be increased.

    That I have been informed that the final fee to be deposited would be

    subject to the decision of the Fee Fixation Committee / Competent Authority

    which decides the fee structure of Government and Private

    Engineering/Medical Colleges keeping in view the infrastructure, investment,

    expenditure, etc.

    5 That I have also been informed that there could be substantial difference in

    fee paid provisionally at the time of admission and the fee amount as

    decided by the Fee Fixation Committee / Competent Authority.

    That I hereby declare that I shall abide by the decision of the Fee Fixation

    Committee / Competent Authority on the amount of fee and undertake to

    deposit the difference, if any.

    7 That I have also been informed that the amount of Rs. 7.5 lacs towards

    failure to fulfill obligation under Bond is likely to be revised by the

    Competent Authority and I hereby declare and undertake that I will abide by

    the decision in respect of revision in amount of Bond.

    8 I hereby state that I have given the aforesaid undertaking to pay the

    difference in the Fees, if so decided by the Fees Fixation Committee/

    Competent Authority, and pay revised amount as decided by the Competent

    Authority, towards failure to fulfill obligation under Bond, out of my own free

    will.
