Amputee Care conference about his latest

inventions which allow him to build banjos

with one hand. Three years ago, Bill was

critically injured in a motorcycle accident.

After a long and difficult recovery, Bill is

experiencing a new beginning. He is using

technical and business acumen to advance

his company’s automation systems for

global clients and to create new devices

that can be used by individuals with upper

limb amputations. Bill says rebuilding lives

after life-changing events should be about

focusing on “abilities”.

MESSAGE FROM THE CHAIR OF THE BOARD AND THE PRESIDENT AND CEO

Each year patients at St. John’s Rehab experience remarkable recoveries. They attribute their success to personal determination, support of family and having ahighly-skilled, committed team of health care professionalsassisting them to maximize function and independence.This 2004-2005 annual report celebrates the new beginningsthey have achieved after life-changing injuries or illness.Patients like Bill Rickard, pictured on our cover, exemplifyhow individuals reach beyond traditional boundaries inorder to achieve their rehabilitation goals.

As Ontario’s only hospital dedicated to specialized rehabili-tation, St. John’s Rehab has made great strides this past yearin introducing a number of care innovations while achiev-ing a balanced budget. This is due in part to increasedannual operating funding from the Ministry of Health andLong-Term care which recently recognized St. John’s as oneof the “pacesetter” hospitals in Ontario.

Having established a special niche in the health care sector,St. John’s Rehab began implementation of a seven-dayadmission and treatment rehabilitation care model.Currently, rehabilitation programs in Ontario are providedfive days a week. Our medical and clinical leadership groupagreed that by investing resources into this rehabilitationservice expansion, we can make it possible for many individ-uals to speed up and improve their recovery from strokes,orthopaedic and cardiovascular surgery. This initiative, andothers featured in the report, build on strong links estab-lished with our acute care referring hospitals which hasbeen a key strategic focus since 1999.

The opening of our transplant rehabilitation pro-gram, an initiative made possible through a part-nership with University Health Network, TorontoGeneral Hospital, will both improve patient out-comes and support more efficient use of health sys-tem resources. At the official launch ceremony inNovember 2004, Health Minister GeorgeSmitherman noted, “This is a good news story forpatients as well as the health care system…”

Another significant step forward in achieving ourvision is in the provincial government’s announce-ment of $3.5 million in funding to prepare to go totender for our $30 million redevelopment project.This major enhancement and expansion of theHospital’s outpatient and inpatient care areas willenable the organization to increase patient accessto specialized rehabilitation care thereby relievingacute care bed shortages and waiting lists.

Leading the campaign to support this redevelop-ment is St. John’s Rehab Foundation and a committed group of community and business lead-ers under The Honourable Art Eggleton. Throughtheir efforts, Rebuilding Lives, the first publicfundraising campaign in support of St. John’sRehab since it opened in 1937, is well on its way toreaching the $15 million goal.

Confident that the transformation of Ontario’shealth care system is moving in the right direction,the Hospital Board of Directors has begun a processto renew the Hospital’s vision and strategic direc-tions. Our vision, mission and future directions arebeing updated to reflect our highly specializedrehabilitation focus; to enhance our role as aunique health resource, to increase our teachingand research programs; and to build on our cultureof care that supports program and service excellence.

Nurturing and healing people whose lives havebeen changed as a result of major illness or injuryrequires a team who have a passion for specializedrehabilitation. Our staff, physicians, volunteers andSisters focus on people’s abilities rather than dis-abilities. Their commitment and dedication is at the heart of our success as a health care organization.

Before St. Catherines resident Randy Smith could start hislife again following a double lung transplant, he needed special help along the way. His surgery had been a great success, but a post-operative complication left him in a comafor several weeks. When he arrived at St. John’s Rehab in2004, days after discharge from intensive care, he was seri-ously deconditioned and anxious about his future. Today,Randy is running a successful family business with his wifeKate and being a lacrosse dad to son Christopher.

What made the difference for Randy was a smooth transitiondirectly from acute care to Canada’s first dedicated inpatientrehabilitation program for organ transplant recipients. St. John’s Rehab developed and launched the program in 2004 in partnership with the Multi-Organ Transplant program at Toronto General Hospital, University HealthNetwork.

Transplant patients can require specialized rehab because offactors such as a lengthy illness prior to surgery, respiratoryissues, loss of muscle mass and managing a complex regimen

of transplant drugs. “Our goal is to achieve better patientoutcomes,” says President and CEO Malcolm Moffat. “Theintegrated model of care created through this partnershipwith University Health Network will allow people recoveringfrom organ transplant surgery to make a smoother transitionback to good health, so they can return to their home andfamily as soon as possible, and regain their independenceand quality of life.”

St. John’s Rehab has six inpatient beds, which will serve upto 60 inpatients per year, as well as a comprehensive outpa-tient service to educate, monitor and support transplantrecipients.

The integrated model of care between St. John’s Rehab andUniversity Health Network will be supported through patientand team consultations using telemedicine technology andjoint research and education initiatives. Our long-term goalis to expand program links to other transplant surgical centres in Ontario.

INTEGRATING HEALTH INFORMATION –REHAB, COMMUNITY AND ACUTE CARE

With the advent of Local Health Integration Networks inOntario, and the evolution of St. John’s Rehab as a specializedrehab facility, it is expected that more patients with increasinglycomplex health needs from different geographic regions willbe referred to the Hospital. As a result, smooth referral systemsbecome critically important, which means fast, accurate sharing of information.

This is the objective of a unique collaboration with theUniversity Health Network, the Toronto and North YorkCommunity Care Access Centres and Bridgepoint Health.Through a shared CIO and management structure, the“Shared Information Management Services (SIMS)

Partnership” will focus on data integration andinformation sharing resulting in eReferrals,

an integrated care portal, wait timesimprovement, and integrated performance

management that are aligned with provincialand federal eHealth strategies. Partnership

projects will also create unique learning and advance-ment opportunities for professional staff not possible in

single organizations.

Until recently, patient transfers between acute care hospitals and special-ized rehabilitation facilities in the Greater Toronto region occurredMonday to Friday. St. John’s Rehab has introduced a seven-day admissionand treatment program that will allow patients to begin specialized reha-bilitation when they need it in order to achieve the best possible functionaloutcome.

The Seven Day Admission and Treatment Program will be completelyphased in by the end of 2005. For our patients, this means within days oftheir surgical procedure and treatment in intensive care or the emergencyroom, they will receive immediate rehabilitation treatment to speed up andimprove their recovery. Our acute care referring partners are supportive ofthis initiative, as it will assist them in relieving acute care bed pressures.

Total Joint Replacement Initiative

St. John’s Rehab is participating in a Ministry of Health funded initiativeto implement an integrated model of care for GTA total joint replacementpatients. This initiative will standardize best practices in orthopaedic care,reduce the total length of hospital stay and improve patients’ experienceacross the continuum of care. This project involves acute and communityhospitals, rehab hospitals, CCACs, the GTA Rehab Network, the OntarioJoint Replacement Registry and the Arthritis Society.

Joint replacement patients are being assigned to two care streams: threedays in acute care, seven days in inpatient rehab; or five days in acute care,discharge and rehab delivered at home. The Toronto Joint Network willtrack progress along care pathways and compare complication and read-mission rates, functional health status and patient satisfaction at threemonths post-surgery. It is anticipated that 50 percent of eligible patientswill be on the protocol by July 2005, with that percentage increasing in thelong term.

St. John’s Rehab teamed up this past year with the BaycrestCentre for Geriatric Care and Sunnybrook and Women’s CollegeHealth Sciences Centre to create a new partnership called the“Neuroscience Alliance”. Alliance members are committed toworking together to evolve as international leaders in cognitiveand rehabilitation neuroscience related to aging and brain dysfunction from stroke, mood disorders, head trauma, and neurodegenerative disorders such as Alzheimer’s disease.

The Neuroscience Alliance will help people gain access to thebest in neurological care, when and where they need it. As aresult of our collaboration with Baycrest and Sunnybrook &Women’s, patients and their caregivers will have access to morecomprehensive and effective care through a continuum of serv-ices from health promotion, to the acute phase of their disease,to rehabilitation and long-term management in the community,integrated with and supported by research and education.

The alliance will also help St. John’s Rehab address one of its strategic priorities – the development of a formal researchprogram.

Otis Wilson was faced with having to give up his passion for amateur basketball until he could find asupplier for a made-to-order face mask. Having broken his nose four times over the past two years, heneeded a face protector to continue his love of thesport. One of the Hospital’s most important objectives is to focus on ouremployees. In building a culture and environment that promotesemployee achievement, commitment to quality and continuouslearning, we are pleased to introduce the 2005 recipients of the St. John’s Rehab Hospital Award of Excellence. The award repre-sents the organization’s highest honour for employees who havedemonstrated an achievement that has gone above and beyondexpectations, while demonstrating our values, vision and mission in completion of a goal. Award recipients are nominated by their colleagues.

THE STAFF CAMPAIGN CABINET

Will Cachia, Linda Lepine, Cathy Pupo, Tania Ferrante, Jack Clarke, Elaine Murphy, Pat Lee, Kitty Liu, Barb Wesley,and Linda Nasturzio

The Rebuilding Lives Staff Campaign Cabinet successfully planned,organized, and carried out a fundraising campaign within a veryshort period of time in the spring and summer of 2004. The teamachieved 57% employee participation and raised $247,000. Moreimportantly, the achievement raised community awareness andexpanded the visibility of the wonderful work done at St John’sRehab.

THE MASSAGE THERAPISTS

Kimberly Boersen-Gladman, Leah Fisher, Zoran Jelicic, and Anna Kania

This dynamic team of therapists has evolved the Massage Therapyclinic into a recognized centre of excellence for specialized hospital-based massage therapy. They have gone above and beyond to raiseawareness of the massage therapy profession, and to gain credibilityand recognition as valued members of the interdisciplinary rehabili-tation team. This has been done within the Hospital’s inpatient andoutpatient programs, and externally by conducting numerous educational sessions, publishing various articles, and presenting at conferences. The team also developed an innnovative aquatic massagetherapy program now available as a specialized community service.

THE TRANSPLANT REHABILITATION PROGRAMIMPLEMENTATION TEAM

Betty Cheung, June Li, and Merlie Uy-Polintan

The Hospital successfully implemented a new TransplantRehabilitation Program in collaboration with the University HealthNetwork in November of 2004. Betty Cheung (physiotherapist), JuneLi (occupational therapist), and Merlie Uy-Polintan (registerednurse) led the clinical team in developing this program which is thefirst of its kind in Canada. Through their research, teamwork, and aconstant drive to ensure that our patients would have the best possi-ble treatment plans, the program was launched in 2004. Betty, June,Merlie have been true leaders in promoting an innovative servicethat continues to develop as one of the Hospital’s ground-breaking programs.

In the summer of 2005, the Ontario Hospital Association andthe Ministry of Health and Long-Term Care will release forthe first time individual rehabilitation hospital performanceresults. Last year, the overwhelming majority of our patientssurveyed by the Ontario Hospital Association and the Ministryof Health judged our overall quality of care as excellent, very good or good. A significant number of our patientswould recommend St. John’s Rehab to friends and family.

The Ministry of Health and Long-Term Care andOntario hospitals are working collaboratively to devel-op hospital – specific accountability agreements forthe 2005/06 fiscal year. In 2004-2005 St. John’s Rehabsigned an interim accountability agreement with theprovince. We are proud to have been one of the firsthospitals to enter into such an agreement.

St. John’s Rehab Hospital’s student vol-unteers and Auxiliary have always beenvital to our success. With more than 146Auxilians and 90 student volunteers,these spirited members of the communi-ty gave more than 21,000 hours of valu-able service last year. Volunteers assist inmore than 20 hospital functions, includ-ing daily services on the patient careunits, delivering meal trays, portering

patients, gift shop operations and muchmore. Organizing several fund-raisingevents, the Auxiliary has pledged an out-standing $500,000 to the RebuildingLives campaign and in just two years hasalready raised almost one third of thatgoal. Individual members of theAuxiliary have also made personal con-tributions to this first fundraising initia-tive since the Hospital opened in 1937.

Rebuilding Lives through charitable giving. That’swhat St. John’s Rehab Foundation is all about. Ourmission is to ensure St. John’s Rehab Hospital hasthe resources it needs to fulfill its mandate – to beat the forefront of specialized rehabilitation care.

This year the Foundation made great strides inassisting the Hospital in that important goal. Earlyin 2004, the Foundation began the first publicfundraising campaign in support of the Hospitalsince it opened in 1937. This incredible effort, theRebuilding Lives campaign, has already reached 62%of the $15 million necessary to help fund a majorenhancement and expansion of the Hospital’s outpatient and inpatient treatment facilities. Thisis thanks to a committed group of business and community leaders under The Honourable ArtEggleton, and to countless donors and volunteers.

St. John’s Rehab Foundation is extremely gratefulto our donors, because it is through your on-goingsupport that we will be able to continue to raise

much-needed funds for this important buildingproject as well as for future investments in equip-ment, research and teaching at the Hospital. Yourcommitment to St. John’s and your willingness toinvest in these crucial areas of need serves to recog-nize the efforts of Hospital staff, physicians, theSisters and volunteers at the Hospital.

A special tribute of appreciation goes to our manyloyal and generous contributoers and participantswho once again made both the Annual Rehab GolfTournament, and the Hip Hip Hooray walk fororthopaedic care, very successful fundraisingevents.

As a prominent member ofOntario’s manufacturing sector, Bill is helpingSt. John’s Rehab Foundation reach out to cor-porate and business leaders to enlist theirinvolvement in building the future for special-ized rehabilitation care for the Greater TorontoArea and the province of Ontario. Having beentouched by St. John’s Rehab after life-changinginjury, he knows that in order to maximizerecovery and find new meaning in living, indi-viduals need access to a highly skilled team ofhealth professionals.

Rehabilitation medicine is the new medicalfrontier. We are living longer and more activelives. Advancements in medicine have made itpossible to survive strokes, complicated heartsurgery, organ and joint failures. With thisgood news comes greater demand for intensiverehabilitation services and programs providedby a highly-skilled, multidisciplinary team ofhealth professionals committed to giving peo-ple back their lives.

To move into the future and respond to thatdemand, St. John’s Rehab urgently needs to replace the Hospital’s existing outpatient

treatment facility and significantly improveinpatient treatment space. Our redevelopmentproject will:• meet Ontario’s urgent need for improved

access to specialized rehabilitation care; • assist in reducing wait times for surgery and

emergency room back-logs in GreaterToronto Area hospitals;

• create a state-of-the-art facility for patients,our staff and volunteers

• create an exceptional academic environmentfor future health care professionals;

• contribute to the further development of theGTA as a healthy, sustainable community.
