    1 Introduction

    1.1 Purpose & Salient features

    This Project Health and Safety Plan are specifically designed for the Project Empire. Itrepresents a guide on the philosophy and strategy to be adopted for the safe execution of theconstruction activities & machine installation related to the project and it reflects the minimumexpectations of GSK. It is a basic communication tool for the project organization and it shallbe continuously updated to incorporate the local contractors and subcontractors own plansand the procedures compliant to the legal requirements.

    This document is prepared complying with the requirements of GSK Construction SafetyGuidelines, Project Management Guidelines & tools on Project Safety, ISC Division and theSites own safety practices, OSHA/FM Global/NFPA and other National/Internationalstandards/guidelines & Statutory requirements. These documents are shown in the reference

    document list (Section - 5) and will be available with the Project Team Document Library forreference and records.

    A number of GSKs standard formats and documents related with safety implementation,monitoring and observations are attached with this Document as Attachments. In case thePMC/Contractors have their own standard formats/documents, the same shall also be usedsubject to prior approvals from PMC/GSK. These are attached to the Document asAttachement-1

    This document specifies the minimum safety requirements and the Contractors safetymanual/procedures will be more elaborate and specific to their area of work.

    This document contains the information as available during scheme design and will undergo

    changes during Detail Design and prior to commencement of Construction activities as wellduring construction phase, incorporating any changes/learnings

    The project team also intent to visit one of the major ongoing project sites at Sonepat (projectDestiny) and the learnings on Construction safety from this project would be incorporatedduring the next revision of this Document.

    This document would be circulated to all key stake holders, project team and contractors, asrequired.

    1.2 Definitions

    Within this document, the term

    Owner means GlaxoSmithKline Consumer Healthcare Ltd (GSK CH)

    PMC means Project Management Consultant

    Contractor - means the organization of the primary /main Contractor/Vendors and theorganizations of the secondary contractors, consultants, sellers or suppliers. The termcontractors management means the personnel responsible for the management,supervision or leading of the contractual activities and personnel. The term employeemeans an individual who is employee of the Main Contractor or belonging to theSecondary Contractors, Consultants, Sellers or Suppliers.

    1.3 Accident Prevention and GSK Commitment

    The OWNER and PMC are fully committed for providing safe and healthy working conditionsfor all employees working on the project and our goal is zero incidents in our operation. Safetyis an overriding value that the operation shall be conducted in such a manner that allpracticable measures are taken to protect people, not only in our employment, but also otherswho may be affected by our activities.

    Owners /PMCs Project team is committed to the protection of personnel, property andthe environment. Owners EHS manual and other E & S, standard operating proceduresspecify the minimum standard safety requirements applicable for project execution and theSupplier shall comply with the same.

    The concept of GSK is that all incidents and accidents can be and shall be prevented.This requires a proactive involvement of all employees throughout all phases of the project:

    Contractor safety planning for all jobs

    Effective field supervision Awareness of all people involved in the construction activities

    Therefore the EHS Objectives for the Project Execution shall be:

    To execute all activities in a safe, healthy and environmentally responsiblemanner

    To achieve the project safety targets

    Protect self and others at all times

    To ensure full compliance with all safety guidelines, standards & instructions

    The Contractors are required to implement EHS standards, and any additional requirements,as per local laws and statutory requirements and best practices in industry, whichever are

    higher. OWNER and PMC may issue further changing requirements from time to time as thework progresses to fit changing site conditions, which will be binding on the CONTRACTOR.

    Safety is the responsibility of every employee but line management is ultimately responsiblefor the implementation of safety requirements.

    Everyone involved must be committed to achieving high standard of EHS Performance withproactive approach at all levels in the organization with excellent teamwork.

    All activities shall be in compliance with relevant local laws, statutory requirements, codes and

    practices pertaining to health & safety of employees.

    All personnel should share a sense of empowerment on safety matters with an effectivecommunication system to facilitate the flow of safety information both up and down throughthe organization

    This document does not exonerate the contractors from their own responsibility with regardto the Occupational Health and safety in compliance with the law.

    This Health and Safety Plan shall be updated while the strategy and plans are improved.

    The Project Empire primary objective is to set-up a new GMP compliant tablet compressionfacility in Mt. Lavinia Sri Lanka.Currently the site receives bulk supply of Panadol tablets fromGSK Dungarvan Site, which are blister packed at the site and marketed for the local Sri Lankamarket.

    The Project proposal is to construct a new compression facility at the site to host two

    compression machines to compress Panadol tablets, storage area for the directlycompressible granules and all the related auxiliary services. The existing blister packing willcontinue without any change. The directly compressible granules shall be procured fromGranules India India.

    1.6 EHS Objectives & General Requirements

    General

    Everyone working on the Project shall be committed to this policy of ensuring that thehighest standards of Health, Safety and Welfare, are adopted. To this end, thefollowing objectives have been set for the Project:

    Successfully implement the Safety Policy.

    Unsafe acts and conditions must be identified and corrected, with action aimed to

    prevent recurrences.

    Achieve Safe and Healthy Workplace.

    The Safety & Health of all employees must receive prime consideration throughout

    all phases of work.

    Ensure compliance with all applicable laws, statutory requirements, codes of

    practices and standards set forth by OWNER.

    In essence NO SAFETY- NO WORK.

    Targets

    The following Safety targets apply to the Project:

    Lost Time Accident (i.e. LTA=0)-Zero

    Occupational Illness- Zero

    Environmental Incidents -Zero

    Injury and Illness-Zero

    Property damages-Zero

    Vehicle Incidents -Zero

    100% Compliance with GSK safety standards, guideline, instructions & Statutory

    Rules & Regulations

    1.7 Contractors Selection & Safety Management

    Safety, Health & Environment Management is a major element and GSKs commitment in this

    regard is global. The Contractors & vendors shall therefore ensure that all PLANT equipmentand system is evaluated for safe operation and maintenance. Special attention shall be givento the requirements of operational safety and health.

    GSK is committed to ensure that no accident or safety incidents took place at any of itsoperations worldwide. Contractor shall be committed to ensure that the job is executed in amost safe and professional manner without any accidents or incidents.

    Contractors EHS policies, practices, past safety performances and commitments shallbe considered as a major factor in Tender evaluation & Contractors selection.Contractor shall provide the following data for the previous three years (Project wise)& Documents for PMC/Owners review:

    Contractors EHS policy, EHS Manual, Construction Safety Plans,

    Details of the safety staff considered for the project

    Any convictions or Regulatory improvement notice served from theenforcement offices.

    Photos of other construction sites especially showing PPE being

    managed. Safe Systems of Work PTW, RAMS task analysis etc. how they are

    implemented and communicated to the workers.

    Working in the Pharma industry which companies and type of workunder taken.

    1.8 General Safety Rules

    Contractor/PMC shall ensure/adhere to safe construction practices and guard againsthazardous and unsafe Working conditions and shall comply with Owners safety rules as

    set forth herein PMC to ensure that CONTRACTOR should have a written statement of policy in

    respect of safety and health of workers, copy of the policy should be signed by an

    authorized signatory. CONTRACTOR shall prepare a comprehensive SAFETY

    ASSURANCE PLAN for implementation and monitoring of EHS requirements. This

    shall be submitted to PMC for approval and implementation. CONTRACTOR shall

    appoint safety personnel as given below for every work shift:

    Safety Supervisor: At all times, minimum one supervisor for workers not

    exceeding 50 in numbers. That means CONTRACTORs are expected to deployadditional safety Supervisors when number of workers crosses beyond 50 and

    thereon. However, in case the no of workers, as deployed is less than 10 Nos, the

    contractors construction supervisor, after undergoing the required safety training may

    be allowed to be the Safety Supervisor, solely at the discretion of PMC/GSK.

    Safety Officer: In addition to above, one safety officer for every 250 workers.

    Safety Manager: Minimum of one Safety Manager at all times required to be at site

    for all Contracts deploying more than 10 workers

    Working hours per shift should not exceed 12 hours per day / 72 hours per week.

    As a minimum wearing of Safety shoes (anti static), Safety helmets, high viz-vests

    and safety glasses are mandatory for all personnel entering the designated

    construction areas and includes all workers, supervisors, managers, GSK/PMC

    personnel and any visitors.

    The security guards posted at the main entry to the barricaded construction area will

    physically check & verify all personnel entry to this area to ensure the minimum safety

    requirements compliance. Any personnel not complying with these minimum

    requirements will not be allowed to enter the construction areas.

    Contractors personnel shall be restricted to the areas required for the performance of

    their WORK. Wandering of Contractors personnel through PLANT areas other than

    the immediate WORK area and other permitted areas, is not permitted

    Individuals violating safety rules and measures can be removed from the JOBSITE by

    PMC.

    Contractor is bound to correct for his own account unsafe working methods or unsafe

    situations for which he is responsible. If Contractor fails to do so and in the event that

    Contractor is performing in what OWNER considers to be an unsafe manner, PMC is

    entitled to suspend the WORK and/or terminate the Contract, in which case all related

    costs will be for Suppliers account.

    Contractor shall conform compliance to all statutory laws governing safety,Environment, storage of radiographic sources, petroleum and other explosive

    products etc., which may be required for carrying out his job

    Qualifications/experience of Safety supervisors should not be less than as prescribed

    in local regulations.

    Contractor shall arrange for initial Site orientation/ induction of all Workmen/Supervising personnel on Safety practices before commencing work at site. Thisshall include brief about project site, safety policy, site safety rules, Permit To Work(PTW) systems, .Safe Systems of work, RAMS and site facilities.

    Contractor shall conduct toolbox talks for all workers each week about ongoing work

    activities and precautions to be taken.

    Contractor shall arrange regular training classes on safety to all his

    employees/workmen. Newly joined personnel shall be given proper safety training

    prior to deploying them at job

    Contractor shall ensure participation of his site in-charge and safety

    Manager/Officer/Supervisor in the safety meetings arranged at intervals decided by

    PMC / owner.

    Contractor shall submit Health & Safety reports on Daily/Weekly/monthly basis to

    PMC in the formats provided.

    The safety conditions specified and recommended here are being issued for

    guidance of the contractor. It is the primary responsibility of the contractor to ensure

    that jobs are executed in absolutely safe manner. These, however, do not absolvethe contractor from any obligations or liabilities he might incur or transfer such

    obligations or liabilities to his subcontractors or agent or to the Owner/ Consultant.

    These rules do not exempt the contractor from statutory duties on health and safety

    but are intended to assist in attending a high standard of compliance with those

    duties in order to provide a safe and healthy working environment.

    The contractor should obtain a Work Permit from PMC Engineer-in-charge before

    starting any work. These permits are issued to prevent contractors working in

    unauthorized area, and will be valid for specific area and for limited period.

    The contractor shall not store construction material, consumable, tools and tackles

    etc. at any place other than area allocated by the Engineer-in-charge.

    The contractor shall be responsible and shall indemnify the owner, against all injuriesto persons both his own workmen and others.

    The contractor should take all precautions for safeguarding existing structures,

    equipment and / or parts of the owners property during construction, erection and

    commissioning. The contractor shall repair / reinstate all such damages

    Carrying/striking of matches, lighters inside the PLANT area & smoking within thePLANT area are strictly prohibited. Violators of the "No Smoking", rules shall be

    discharged immediately. Within the operation area, no hot WORK shall be permittedwithout valid gas safety/fire permits The Contractor shall be held liable andresponsible for all lapses of his Sub Contractors/ employees in this regard..

    Chewing of tobacco, pan, or other items containing tobacco is not permitted insidethe complex.

    Mobile phones are not allowed in the construction areas

    Supplier shall ensure that their personnel are fit for duty

    Personnel protective equipment shall comply with the relevant National standardsand Owners specifications. Wearing of PPE, as specified, is mandatory.

    No drugs, alcohol, or alcoholic beverages are permitted inside the complex.

    Electrical work shall only be carried out by qualified and authorized personnelsatisfying all statutory requirements

    Contractors electrical installations shall comply with the requirements of all governinglaws. Contractor shall employ full time authorized electricians to maintain histemporary electrical installations. All electrical connections, installations shall becertified Safe by an approved Electrical Inspector before connecting it to source

    All electrical installations shall be connected with ELCB for protection against shocks

    All accidents, hazards and near misses shall be reported, investigated andcorrective/.preventive actions taken to avoid recurrence, as elaborated separately

    All injuries regardless of severity shall be reported for medical treatment and informed

    to PMC/Construction Safety Supervisor & GSK Project Implementation Manager.

    Cameras shall not be brought to site, without prior permission from Owner.

    All employees working at site shall possess valid ID card, issued by Owner.Personnel having valid ID cards only are allowed to enter the site.

    Daily attendance shall be reported on a daily basis before 10.00 Hrs

    Work shall be carried out in accordance with the applicable permit regulations

    Job site shall be positively isolated from the existing operating facilities.

    For all major construction jobs a Job Safety Analysis shall be performed by theSupplier and method statements, detail work instructions prepared, prior to taking up

    the job. Immediately after the kick off meetings with the selected Contractors and before the

    commencement of the construction work the Contractor jointly with his safetymanager, the PMC, GSK Project Implementation manager, Safety Engineer/safetySupervisor, shall identify all critical activities requiring detail analysis and methodstatements and complete the preliminary requirements, before the commencement ofconstruction activities

    Operators of vehicles must possess a valid SLn Drivers license. Permitted vehicleshall only be allowed to enter the Complex.

    No foodstuff will be allowed at the work site. Food will be taken in approved controlledconditioned facilities. All scrap/waste food stuffs will be disposed of immediately as

    per the approved waste management plan

    A well documented waste management plan shall be developed and disposal ofwaste shall be carried out accordingly.

    All lifting and hoisting equipment, tools, tackles etc. shall be certified before use andtested periodically and records maintained, as required. All personnel operating liftingequipment must be competent and certified.

    All portable and fixed construction equipment, tools tackles etc. shall be in safeworking condition.

    Zero access guards, as per GSK standards shall be provided for all constructionequipment

    Safety, Health and environmental issues shall be considered for the contractorevaluation, selection, qualification and control

    Contractors shall have a programme on safety rewards & recognition as wellpunishments for safety violations.

    2 EHS Management Communication

    2.1 Organization, Roles & Responsibilities

    2.1.1 Overall Responsibility

    The responsibility for the EHS management at the site level belongs to GSK through the PMC& the Construction Safety Supervisor designated for the construction work. The SafetySupervisor must be experienced in construction safety & trained in compliance with the EHSrequirements of GSK.

    The Construction Safety Supervisor shall be assisted and fully supported by the GSK EHS

    Engineer/Manager. The Safety Supervisor and the EHS Manager of GSK have the right todecide upon the partial or total suspension of the activity or the removal of the employees offthe construction site.

    The high level RACI (Responsible/Accountable/Consulted/Informed) for Client (OWNER)CONSULTANT/PMC and Contractor shall be developed, discussed during pre-bidconference and agreed with all Contractors, before the award of contract by the PMC.

    2.1.2 EHS management & staffing

    2.1.3 Roles & Responsibilities

    Project team shall support this EHS Plan and hold their members of team accountable for the

    proper execution of EHS activities.

    Roles & responsibilities of GSK Project implementation team are well defined in the ProjectPlan and are not duplicated in this Safety Manual. GSK project management team fullysupports the PMC in effectively implementing the Safety management plan for the project

    GSK Project Implementation Manager & EHS Manager/EHS Engineer works closely with thePMC and his Safety Supervisor in effectively implementing the Safety Management and toensure that the same is in place and in use.

    2.1.3.1 The Specific EHS related Roles and Responsibilities of PMC are listed below;

    Effectively communicate and implement EHS policies and procedures;

    Generate the project-specific HASAP(Hazard Assessment and Safety Action

    Plan), in coordination with the responsible GSK EHS Manager;

    Provide the resources necessary to maintain a safe and healthful work

    environment and ensure implementation of Company EHS policies and

    procedures;

    Establish program, project goals for EHS performance and verify that they are

    Assign clear responsibilities that are necessary to achieve the EHS goals and

    objectives;

    Verify that the project team are executing these responsibilities properly;

    Enforce EHS procedures, award & recognize good safety behaviors/actions

    and issue disciplinary actions, when required;

    Ensure that EHS deficiencies identified in audits and inspections are promptly

    corrected;

    Coordinate with the OWNER and other third parties on EHS matters.

    Ensure safety awareness and promotion activities and effectively participate in

    it.

    Responsible for issuing GSK-PTWs

    2.1.3.2 PMCs Construction Safety Supervisor Responsibilities

    Support and advise site management in their duty to implement site-specific EHS

    policies and procedures;

    Assure the adequacy of site and OWNER EHS procedures;

    Assess the work to determine compliance with EHS policies and procedures,

    promptly communicating concerns and recommendations to managers and/or

    supervisors for their action;

    Take immediate action on any imminent danger situations observed;

    Review and approve personal protective equipment, safety equipment, and first

    aid supplies;

    Educate management and supervision in the applicable EHS policies and

    procedures;

    Ensure EHS orientation of site employees and sub CONTRACTOR personnel;

    Ensure prompt and adequate treatment for injured employees and sub

    CONTRACTOR personnel;

    Ensure adequate investigation and analysis of accidents/ incidents (non-injury

    and injury); Provide EHS information for the education of management, supervision, and

    employees concerning:

    Incident/ Accident analysis (cause, trends, results, and corrective action), Nearmiss

    Analysis of inspection results and needed corrective actions,

    Safe work procedures, and

    EHS training requirements;

    Communicate and promote EHS excellence recognition programs;

    Ensure preparation, submittal, and maintenance of required EHS recordkeeping

    documentation, such as inspection reports, training records, SPAs, SORs ,

    accident/ incident/ near misses investigation reports, safety reports etc.; and

    2.1.3.3 Contractors Construction Manager

    Contractors Resident Construction Manager shall provide his workers with a safe and healthyworking environment. He shall be responsible to promote safety and health consciousnessamong all his workers at all times. He is responsible to ensure that the HSE Plan dulyapproved by PMC / owner is implemented in its entirety and constantly monitored.

    He shall:

    o Familiarize himself and comply with all government, consultant and owners

    HSE procedures and practices.

    o Provide adequate safe construction equipment, tools suitable for the work

    and personal protective equipment required for the workmen.

    o Ensure the construction work is being executed in the safest manner based

    on approved Method Statement.

    o Inspect and maintain the entire work area in safe and healthy condition.

    Instruct his supervisors to take immediate corrective actions if he detects

    discrepancies.

    o Ensure valid PTW / RAMS are in placeo Toolbox talks on PTW / RAMS have been communicated to workers and

    workers signed on to PTW.o Appropriate boundaries are in place to cover PTW etco All safety / PPE equipment is available in working ordero Appropriate signage is in place

    2.1.3.4 Contractors Safety Manager/Engineer / Safety Officers

    o Assist contractors Construction Manager and coordinate with consultants /

    Owners Safety Supervisor in implementing EHS procedure within theirrespective work groups.

    o Familiarize them with all Government, PMC and owners safety and health

    regulations, including reporting and work permit procedures.

    o Inspect the construction area daily to examine appropriate corrective actions

    and prepare reports to their Construction Manager.

    o Review SPA prepared by line supervisors.

    o Co-ordinate with supervisors and foremen, participate in safety meetings and

    lead daily safety walks of the site.

    o Assure the adequacy of site and PMC/OWNER EHS procedures;

    o Assess the work to determine compliance with EHS policies and procedures,

    promptly communicating concerns and recommendations to managers and/or

    supervisors for their action;

    o Take immediate action on any imminent danger situations observed;

    o Review and approve personal protective equipment, safety equipment, and first

    aid supplies

    o Ensure EHS orientation of Companys employees and sub CONTRACTOR

    personnel;

    o Ensure prompt and adequate treatment for injured Company employees and

    sub Contractor personnel

    o Ensure adequate investigation and analysis of accidents/ incidents (non-injury

    and injury);

    o Provide EHS information for the education of management, supervision, and

    employees concerning:

    Incident/ Accident analysis (cause, trends, results, and correctiveaction), Near miss

    Analysis of inspection results and needed corrective actions,

    Safe work procedures, and

    EHS training requirements;

    o Communicate and promote EHS excellence recognition programs;

    o Ensure preparation, submittal, and maintenance of required EHS recordkeeping

    documentation, such as inspection reports, training records, SPAs, SORs ,

    accident/ incident/ near misses investigation reports, safety reports etc.; and

    o Provide EHS evaluations of employees and supervisors to management, as

    necessary.

    2.1.3.5 Contractors Safety Manager/Supervisor Responsibilities

    Contractors safety Supervisors are accountable for the health and safety of those whothey supervise.

    Supervisors shall:

    Learn Company and project-specific EHS procedures;

    Implement the EHS program;

    Effectively communicate the Companys EHS goals, objectives, policies and

    procedures to each member of their work group;

    Establish an environment of zero tolerance for safety non-compliance and

    promote a positive atmosphere for EHS excellence;

    Plan each task with EHS as an integral part, and conduct pre-job risk assessments

    of all work activities;

    Orient the worker with respect to:

    The work group (the new worker),

    The work area and hazards that may exist,

    General and specific safety hazards,

    Incident (non-injury and injury) reporting requirements, and

    The employees EHS responsibilities;

    Assign employees only to tasks for which they are trained and qualified;

    Monitor worker and work group performance for quality and safe work practices;

    Correct any deficiencies noted and educate the worker to improve work methods;

    Report and investigate all non-injury and injury incidents and

    - Secure incident scene,

    - Actively participate in the investigation, and

    - Implement corrective actions;

    Train the employees and work group in safe work procedures and

    - Lead daily safe work planning sessions (SPA),

    - Participate in safety meetings, and

    - Inform employees of potential health hazards;

    Assure good housekeeping at all times;

    Encourage crew members to actively participate in the EHS program; and

    Enforce company and project EHS policies and procedures at all times.

    Ensure valid PTW/RAMS/SPA are in place

    2.1.3.6 Contractors/Subcontractors Employee/workers Responsibilities

    Employee acceptance of the Company EHS procedures is the ultimate key to thesuccess of the EHS program.

    Employees and sub CONTRACTOR employees shall:

    Comply with procedures established for his/her safety and health and for

    preservation of the environment;

    Assist management and supervision in positive development of their co-workers

    attitudes toward EHS and workplace morale;

    Suggest improvements in methods or procedures that will prevent incidents and

    protect the environment;

    Stop, or bring to the attention of management, any unsafe acts or conditions and

    any potentially harmful environmental practices;

    Correct immediately any imminent danger situations, e.g., exposure to falls from

    elevations, electrocution hazards, exposure to improperly protected excavations,

    etc.;

    Notify their supervisor if they have an insufficient understanding

    - of the task to be performed,

    - of EHS procedures, and

    - of assigned safety equipment;

    Promptly report incidents or accidents involving personnel or property to your

    supervisor, no matter how minor;

    Learn the approved EHS safe practices that apply to your work and practice them

    at all times;

    Ask for assistance from a supervisor or the EHS department when you are unsure

    of a work or safety practice;

    Participate in and maintain an active interest in the EHS program;

    Be attentive to safety discussions that your supervisor may lead and request

    explanation of points that you do not understand;

    Never bypass the function of a safety device (Promptly report safety device

    malfunctions to your supervisor and flag the device immediately to warn others of

    the hazard.);

    Promptly report to your supervisor any potential work-related injury or illness, no

    matter how slight you might consider the injury to be;

    Never attempt to do a job alone when good judgment tells you ,assistance is

    needed;

    Inspect, use, and maintain the personal protective equipment provided to you;

    Do not participate in clowning, scuffling, practical joking, or horseplay of any kind

    (Running on the job is prohibited.);

    Be cautious in walking or moving about the work area to avoid slipping, tripping, or

    falling (Be especially cautious when weather conditions create a potentially slippery

    walking surface.);

    Plan your work and try to anticipate any hazards you might encounter; and

    Observe safe practices off the job as well as on the job.

    2.2 Leadership Engagement

    A Pro-active Safety Monitoring Programme shall be used on the project.

    2.3 Induction, Orientation, Training & Tool Box Talks

    The access of all contractors is only permitted to the site area after the completion of the EHSinduction training. A person shall only receive the access pass (Gate pass) only after thedocumented completion of an induction session. Inductions shall be regularly organized andthe PMC/Project Implementation Manager and shall notify GSK EHS Engineer before thecontractors/subcontractors arrival. When the session is completed, each employee shallreceive the formal access card and a copy of the Basic Construction safety guidelines in local

    language.

    2.3.1 Induction & orientation

    All new employees entering the site shall be given induction training. The theme of theinduction shall cover at the minimum:

    Introduction to the site and project, with a brief overview of the project that is being

    constructed

    Site layout

    OWNER and PMCs EHS policy and safety philosophy

    Site access / egress

    Emergency procedures for site and for the GSK facility Site Emergency Number

    EHS Regulations

    GSK PTW System & mandatory Personal Protection Equipment

    Housekeeping

    Working in and around excavations

    Working at height (ladders, scaffolds, free edges and openings, etc.)

    The Safe Plan of Action (SPA)

    First aid facilities, Accident reporting system

    Smoking restriction, prohibition of alcohol and drugs.

    Gate pass shall be issued only after completing the site orientation / induction and

    local statutory regulation formalities.

    Besides initial orientation and induction at site, training on following topics should be

    given:

    o Scaffolding, Ladders, Safety nets etc., Fall Protection.

    o Cranes, Hoists and Lifting Equipment

    o Electrical safety

    o Gas Cutting and Welding

    o Hazardous material handling

    A current list of the participants to the induction session shall be kept with the PMC/GSKSafety Department.

    2.3.2 Specific hazard training

    Specialized training on following topics shall be given to the employees likely to getinvolved:

    Hot work

    Material handling

    Pressure testing

    Heavy equipment lifting & rigging

    Work permit system

    Lockout & tag out procedures

    Entry into confined spaces

    Fire prevention and control

    Excavation

    Emergency Response Team (ERT) etc.

    These courses should be having adequate time required to properly address the

    topic. Completion of specific hazard training is mandatory before working on such

    tasks. A list of all trained persons shall be maintained at site.

    2.3.3 Toolbox Talks

    CONTRACTORs shall conduct toolbox talks for all employees and workers each

    day.

    Topics should include current issues and precautions to be taken for works in

    progress.

    CONTRACTOR shall maintain records of Toolbox Talks and shall forward the same

    An EHS Meeting shall be organized on a monthly basis with the participation of thecontractors representatives, Site Safety Coordinator, GSK Project Manager & GSK EHSManager.

    In addition to the above all contractors will attend regular weekly safety meeting with PMCSite management. At these meetings previous week Safety performance would be analyzed

    and the activities planned for the forthcoming week would be reviewed.

    Additionally, an EHS review shall be the no. 1 topic on the agenda of the following projectmeetings

    Monthly progress review Meetings

    Steering Committee meetings

    Daily field meetings/discussions with Contractors

    Project Team Meetings

    Meeting with Contractors

    PMC Meeting with Owner

    In each meeting the latest EHS events, performance indicators and risks shall becommunicated and discussed.

    If required, ad-hoc meetings shall be organized, in which EHS events shall be communicatedand discussed.

    2.5 Safety Recognition and Incentive Scheme

    In consultation with Owner, CONTRACTOR shall prepare a procedure for evaluating monthlysafety performance rating of the Contractor. Contractor s EHS manual shall contain thestandard procedure adopted by him. Contractor shall also specify the Safety incentiveprogram to be adopted by him for his/sub suppliers personnel.

    In order to develop safety consciousness and awareness amongst the Contractors & theiremployees, the site management shall organize suitable Safety campaigns, Safetycompetitions and Safety rewards. These shall be awarded on a monthly basis and any time,in case of specific major achievements.

    The CONTRACTOR shall take every effort to communicate the safety measures throughdisplaying posters, banners and sign boards around the work site.

    Posters should be in local language, English and other suitable languages deemedappropriate

    In order to promote safety culture an incentive scheme shall be developed by all Contractorsjointly with PMC/Owner for rewarding their employees for good safety conducts, behaviours

    and actions

    The following are the minimal requirements considered for the project

    Each CONTRACTOR shall formulate safety reward program for individuals safetyperformance excellence as

    Worker of the Month

    The respective employees shall be rewarded during the monthly safety meetings in the

    presence of PMC and GSK Senior management. Certificate of recognition along with a cash

    award/gift (with a value of approximately SLR 1000/-) shall be rewarded

    Award of Merit In addition to the above monthly award and recognition system, recognition

    shall be given formally to employees, who perform commendable safety acts or deeds, by

    GSK. Examples of these would be;

    Extinguishing or controlling of fire

    Reporting a hazard, near miss or giving a suggestion to improve safety aspects

    Active participation in Critical Incident Technique Review, safety committees, safetycampaigns etc.

    The recognition shall be a certificate and a gift.

    Contractors Safety Performance Rating GSK jointly with PMC shall conduct regular safetyaudits of the Contractors and the Contractors shall be rated based on their safetyperformance. The average scores of the contractors shall be displayed in the safetyinformation sign board located at a prominent position. . Based on the scores, the Contractorssafety performance shall be rated on a quarterly basis and the best performing Contractor ofeach quarter shall be presented with a plaque or rolling trophy etc, by GSK/PMC

    Additional promotional process shall be implemented for achieving site wide key milestones

    on safe man hours etc. All employees shall be gifted with promotional gifts such as Caps, T-shirts etc on such achievement.

    2.6 Disciplinary Actions related to EHS

    Failure to meet the EHS requirements may result in the suspension of works and therejection/removal of the supervisor and/or of the employees in the area. Intentional orrepeated breach may lead to the dismissal of the contractor and the termination of thecontract.

    Temporary or permanent interruption of the activity in the GSK facility may occur if thecontractors manager, the supervisor or the person in charge with the works requests, asks,

    allows or finds excuses for the employees working in unsafe conditions.

    Permanent interruption of the activity shall occur if the contractor s manager, the supervisor orthe employee:

    Obviously disdains, defies or breaks the EHS regulations;

    Deliberately counterfeits investigation documents or makes false statements in aninvestigation;

    Is involved in physical conflicts, violence, violence threats, robbery or propertydamage;

    Breaks the EHS regulations imposed putting in jeopardy own person or other workmates

    Minor infringements of the safety regulation shall result in a written warning addressed to the

    employee. After three such written warnings, the employee shall be dismissed with no furtheractivity in the GSK facility.

    Immediate dismissal results for major infringement of safety regulations.

    2.7 EHS Reports & Periodic EHS Inspections

    CONTRACTORs shall prepare all EHS documents in assistance with PMC and shall forwardall these documents to PMC Construction Supervisor/GSK EHS Engineer.

    A periodic formal inspection shall be carried out at least on a monthly basis by the PMCConstruction Safety Supervisor/GSK EHS Engineer and the Inspection Report form shall beused to record the observations and the corrective actions to be taken.

    In addition to the above informal weekly safety inspections of the Contractors premises shallbe undertaken by the PMCs Safety Supervisor.

    The EHS Manager of GSK and the EHS Staff of GSK are entitled to carry out inspections.The Site Safety Coordinator shall record the inspections in an inspection log permanentlyupdated.

    EHS Audits shall be carried out on a monthly basis and shall be conducted by the PMC jointlywith EHS Manager of GSK and the GSK EHS staff.

    2.8 Hazard Identification and Risk Assessment

    During the kick off meeting, as a part of the agenda, the Contractor shall submit a detail list of

    critical activities, requiring specific safety strategies and Hazard Assessment. These shall be

    discussed in the meeting. Before start of the project HASAP (Hazard Assessment and Safe

    Action Plan) shall be prepared and circulated to all concerned. This is the part of the pre

    task planning / fore- sighting mainly on the critical activities and for resources deployed and to

    formulate safety strategy how to handle such risks. Construction safety will be aligned to

    match the design safety strategy.

    2.9 Task Assessment

    Contractors shall submit method statements for each work to PMC well beforecommencement of the work. The method statement shall detail:

    The job to be undertaken

    The description of activities to be carried out The individual activities required for completing the job.

    The individual trades/disciplines involved in each activity

    Activity Risk Assessment

    List of equipment

    Plant, equipment, tools to be used in each activity.

    Any hazardous substances/chemicals to be used along with their MSDS.

    A detailed description of how the work will be done including control measures andprocedures to complete each activity and the overall job safely.

    Permits for the task (PTW, hot work confined space)

    Certificate for equipment

    Certificates for competent certified personnel

    Locate copy of PTW/RAMS/Certs local to job site.

    The method statement shall be reviewed by the Safety Coordinator, the Project Manager &GSK EHS before issuing of Work Permit.

    Any modification of the activities and/or schedule shall require a review of the methodstatement before the changes become effective.

    2.10 Accident & Incident Reporting

    The PMC Construction Safety Supervisor shall collect, sum up and record all safety statisticsand shall review the data to decide upon the required development and corrective actions.

    The data shall be submitted to the PMC/GSK EHS Manager/Engineer for review and decisionupon the required development and corrective actions.

    The Construction Safety Supervisor shall also be responsible for data recording in compliancewith local and GSK requirements.

    Any incident at site, however small it is or which entails a person reporting to hospital orleaving site for treatment shall be verbally reported immediately to PMC Safety Supervisor /

    OWNERs EHS Engineer. This will enable the PMC/OWNER to reach to the scene of incident/ dangerous occurrences to monitor/assist any rescue work and/or start conducting theinvestigation process so that the evidences are not lost.

    The return date of injured workmen back to the work shall be reported to OWNER/ PMCpromptly.

    Written reports of all incidents and dangerous occurrences shall be submitted within 6 hoursas per format.

    CONTRACTOR shall ensure that any incident that occurs is fully investigated to find rootcause and preventive measures are adopted to prevent its reoccurrence. OWNER/PMC shallhave a right to conduct independent inquiry of the incident.

    Near misses and minor incidents should also be reported and investigated by theCONTRACTOR as soon as possible as they are signals that there are inadequacies in thesafety management system. The contractor shall formulate a formal reporting system forreporting any near misses and maintain a record of all near misses. The near misses asrecorded during every month shall be discussed in the monthly safety meeting and analysedin detail.

    Incident reporting chain and emergency contact numbers shall be displayed at conspicuouslocations at site and CONTRACTOR offices.

    Any accident that appears to be serious or which entails a person reporting to hospital orleaving site for treatment shall be reported immediately to PMC / OWNERs EHSrepresentative in the specified format. CONTRACTOR shall ensure that any incident/accident that occur are fully investigated to find root cause and preventive measures areadopted to prevent its reoccurrence. PMC shall have a right to conduct independent inquiry of

    the accident. Subsequent to this report PMC will make incident/Accident Report and forwardto GSK

    2.11 Signs and Communication

    All the caution and signboards shall be displayed in local vernacular language, English and to

    cover the majority of the languages understood by the work force.

    Contractors Safety coordinator shall make sure that signs, barricades and signals are used,

    erected and maintained as required to ensure the safety and health of site personnel

    Main sign to the construction area, local to security office shall include

    o Mandatory PPE signs for Hat, shoes, glasses and hi-viz

    o Prohibited: Smoking, eating, mobile phones.

    o Advisory: speed limit 10KMPR

    Hazard warnings come in different forms, which include signs, tags, permits, barriers etc. Theprimary factor with any hazard warning is that all personnel; visitors etc. shall be educated asto the types of warnings and their definition.

    Tags

    o Various tags may be utilized during the construction and commissioningphases, e.g.

    o Electrical - to designate that electrical equipment has been taken out ofservice, or men are working on the equipment.

    o Danger - designating a possible hazard such as valve open, do not use,and spade inserted.

    o Defective - designating unsafe materials and/or equipment, e.g. out of order,

    failed safety check.

    Warning Tapes / Flags

    Hazard warning tapes should be utilized to attract the eye to avoidance of hazards orlocal services. Tapes, whilst warning of potential hazards, should not be employedas safety barriers and are considered as a guidance indicator

    3 General EHS Information - Site Facilities & Emergency Procedures

    3.1 Working Hours

    The regular working time for the construction works is of 6 (six) days / week, from Mondayto Saturday.

    Normal working hours are 8 hours/shift and Working hours per shift should not exceed 12hours per day / 72 hours per week per person. Additional working hours beyond 8 hrs perday or beyond 64 hours per week, shall be compensated through overtime, as perStatutory/GSK requirements.

    Extra working time beyond 12 hours/day / 72 hours per week or work on Sunday shall beauthorized by the Project Management Consultant and coordinated together with the SiteSafety Coordinator.

    National holidays as applicable for Industrial Establishments in Sri Lanka shall beapplicable for the project also.

    3.2 Food and drinks & other welfare amenities

    Consumption of food or drinks is forbidden in the area designated for construction, exceptfor bottled drinking water

    Tea/Coffee, snacks/refreshments, Lunch etc shall be taken outside the construction area.Necessary provision for the same shall be provided by GSK during specified timings andContractors employees can utilize the same during the notified timings.

    GSK will provide workers rest room and necessary toilet/wash room facilities, for theContractors employees in accordance with the Contract. Wherever, these are to be providedby the Contractor, as per the Contract, the same shall be provided by the Contractor.

    Necessary drinking water & service water facilities shall be provided by GSK

    3.3 First Aid and HospitalsThe owner at project site may provide a first-aid centre for the treatment of minor injuries andillness. However all major injuries and sickness cases shall be referred to the Hospitals andcontractor shall make his own arrangement for such treatment as required.

    The contractor must have arrangement for rendering necessary first-aid in case of accidentalinjuries. Work site must be provided first-aid-box containing items as specified in the Building& other Construction Workers (Regulation of Employment and Conditions of Service Rules,1998) and keep the same in a conspicuous place where it is easily accessible.

    Contractors shall maintain first aid facilities for his employees and those of his SubContractors. Contractors First Aid facilities shall have adequate facilities for treating industrial

    injuries and shall meet the requirements, specified by Owner/PMC.The contractor shall make arrangements for the emergency care and treatment of allcontractors employees at the nearest (or reasonably nearby) hospital.

    Owner will provide the available Ambulance services to the Contractor. However, Contractorshall have vehicles available at site for transportation of employees to the First Aid Centre fortreatment.

    Contractor shall also make outside arrangements for ambulance service, in case GSKfacilities are not available during an emergency and for the treatment of industrial injuries.Names of those providing these services shall be furnished to PMOC/GSK prior to start ofconstruction, and their telephone numbers shall be prominently posted in Suppliers field office.

    The location of Contractors first aid kit shall be visibly marked and the GSK First aid roomlocation & contact person shall be displayed at the site.

    3.4 Smoking

    Smoking is completely forbidden inside the GSK premises.

    3.5 Drugs and Alcohol

    Drugs or alcohol consumption is strictly forbidden inside the GSK premises. Break of thisregulation shall lead to immediate dismissal. No person who is under the influence of alcoholor drugs should be allowed to enter the premises.

    3.6 Emergency Procedures

    The contractor shall work out a detailed site emergency plan specific for the category of thecontractual works and communicated to all employees at site. Necessary arrangements shallbe made to deal with such emergency situations. An emergency response team will beformed to tackle such situations.

    Since the construction activities are to be carried out adjacent to the operating plant, all the

    emergency procedures of the OWNER shall be followed for the project activities also.

    Contractors employees shall be trained for Owners emergency procedures.

    Mock drills shall be conducted to ensure all the project personnel are familiar with the

    emergency procedures and plant assembly points.

    The details of Owners emergency procedures are briefly indicated below.

    The following aspects shall be taken into consideration:

    3.6.1 Specific Requirements for Evacuation:

    Fire mock drill shall be conducted to familiarize with procedures

    All the workers shall be trained to operate various fire extinguishers and itslocations at site

    Workers shall be trained for emergency escape routes and how to reach theemergency assembly point

    Evacuation from Site:

    The muster points from the construction site shall be the same as for the facility employees.There are 2 muster points in front of the OTC tablet packing area. The emergency evacuation

    routes and the assemble points shall be displayed at the construction sites

    Necessary fire alarms shall be provided. The adequacy of the site fire alarm within theconstruction area would be verified and if required additional alarm provided.

    The emergency evacuation plans shall be reviewed according to work progress, dependingon the requirements of each construction stage.

    Medical Emergency Response:

    OWNER shall be providing first aid facilities comprising of the following:

    Ambulance

    Trained first aider of the existing plant

    Training to selected contractors employees to conduct first aids.

    Each CONTRACTOR has to arrange :

    Training on first aid and emergency first aiders within the workers to acton an emergency

    Required number of first aid boxes to the construction site

    Tie up with the nearest hospital to deal any medical emergency

    3.6.2 Emergency Services

    The estimated time for the fire brigade arrivals, according to GSK simulations, is 10 minutes.The access of the fire engine, ambulance is through the GMS Mount Lavinia site mainentrance.

    3.6.3 Emergency Contacts

    A directory of the site personnel to be informed in case of fire / emergency shall be displayedshall be displayed at prominent locations at the site as well at Contractors site office and shallbe available with the PMC Safety Supervisor and GSK EHS Manager/Engineer.     4 Construction Safety & Security

    4.1 Personal Protective Equipment (PPE)

    The minimum protective equipment required for any person within the designatedconstruction area or performing project work in the GSK facility is as follows:

    Additional protective equipment shall be required depending on the activities performed. Thecontractors/ subcontractors have the responsibility to ensure the protection equipmentappropriate for the risks (few of which are indicated below) associated with the activityperformed is provided.

    The CONTRACTOR shall:

    Maintain all personal protective equipment in good working condition.

    Periodically check and replace all defective / broken personal protective equipment.

    The followings, but not limited to these, are considered to be personal protective facilities:

    Safety Nets

    Work Platforms

    Shoring

    Cages, gondola and crane suspended working platforms

    Insulating Screens and Barriers

    All necessary personal safety/protection equipment as considered adequate by thePMC/OWNER should be made available for the use to the persons employed on the site andmaintained in a condition suitable for immediate use, and the CONTRACTOR should takeadequate steps to ensure proper use of equipment by those concerned.

    Personnel working at a height of more than 2m above ground level shall wear and use safetyharness, if not working on a completed platform/scaffolding. All personnel working at site shallwear appropriate hand gloves, safety foot wear and safety hard-hat.

    Personnel shall use hearing protection when the noise level is above 85dBA (8-hour timeweighted average). Face shield, Respiratory protection, and other body protection as requiredshall be provided for specific jobs.

    Workers employed on mixing asphaltic materials, cement and lime mortars shall be providedwith protective footwear and protective gloves.

    Those engaged in white washing and mixing or stacking of cement bags or any materials,which are injurious to the eye, shall be provided with protective goggles.

    Those engaged in welding and cutting WORKS shall be provided with protective face and eyeshields, and gloves, etc.

    Stone -breakers shall be provided with protective goggles and protective clothing, and seatedat sufficiently safe intervals.

    When Workers are employed in sewers and manholes, which are in use, the Contractor shallensure that the manhole covers are opened and are ventilated at least for an hour before theWorkers are allowed into the manholes so opened which be cordoned off with suitable railingand provided with warning signals or boards to prevent accident to the public.

    The following precautions should be taken on the WORK of lead painting:

    No paint containing lead or lead product shall be used except in the form of pasteor ready-made paint.

    Suitable face masks shall be supplied for use by the Workers when paint Isapplied in the form of spray or a surface having lead paint dry rubbed andscraped.

    Overalls shall be supplied by the Contractor to the workmen and adequatefacilities shall be provided to enable the Working painters to wash during and oncessation of WORK.

    4.2 House Keeping

    An orderly workplace is a safe workplace

    This principle shall apply during the entire project period and frequently and periodicallyreiterated. Good Housekeeping shall be maintained at all times. The daily inspections carriedout by the Site Safety Coordinator will include this aspect as well. Work shall be suspendedas long as necessary to correct any fault.

    CONTRACTOR shall WORK in a clean and orderly fashion and shall be responsible formaintenance and cleaning of his assigned part of the JOBSITE.

    Very high standard of housekeeping shall be maintained at all times by the contractor.

    * Keep the site neat and tidy. Keep adequate number of skips / waste bins.

    * Keep the access clear of all obstructions.* Remove the nails or bend them down from the wooden scrap and remove them fromjob site.

    * Store the material in an orderly manner.

    The contractor shall on daily basis keep the premises and surrounding area free fromaccumulation of waste materials or rubbish caused by activities.

    At the completion of days work contractor shall remove these materials to avoid Slip/ Triphazards and provide safe areas for movement of all workers and supervisors.

    Contractor should establish regular maintenance program of sweeping and hosing tominimize accumulation of dirt and dust in such areas.

    If the contractor fails to Clean-up as provided in this document, the owner may do soand cost thereof shall be charged to the contractor.

    On completion of the job the contractor shall remove all his construction material, tools etc.and demolish all temporary constructions and leave the job site broom clean - thoroughlycleaned up and ready for use.

    When required contractors have to suspend other operations and do housekeeping

    PMC/OWNER has the right to demand a general cleanup of the JOBSITE whenCONTRACTOR does not comply with the cleaning instructions as requested.

    For such general site cleanup the cleaning crew of CONTRACTOR will be at the disposal ofOWNER without additional compensation. Alternatively, OWNER may have the cleanupperformed by another party and back-charge CONTRACTOR.

    Burning of waste is not allowed at the JOBSITE without the written permission of theOWNER.

    4.3 Waste Management

    Waste generated at site shall be controlled and disposed off in accordance withenvironmental control regulations, municipality rules & Ownersguidelines/standards/instructions. PMC shall be informed of procedure followed by thecontractor before disposal

    Controlled waste is any kind of household industrial or commercial waste. This includes forexample:-

    Scrap metal

    Building, construction, demolition and excavation waste, including waste from any

    repair or renovation. Clinical waste.

    Anything, which is unwanted because it is surplus, broken, worn out, contaminated or

    spoiled in some other way.

    Where there is any doubt of the composition of excavation spoils, it must be analyzed

    before it is removed off-site.

    Waste Disposal

    All receptacles shall be constructed of metal or other suitable types of containers of non-flammable materials.

    Cardboard, wooden boxes or crates and similar types of containers shall not be used for thecollection of combustible waste.

    The removal of waste shall only be taken in accordance with the rules prescribed by localPublic Health Department.

    Separate waste skips should generally be provided for:

    Scrap metal

    General construction waste

    and fully enclosed skips for

    food waste (to deter vermin)

    waste and rags saturated with oil, grease and the like

    Skips containing combustible waste should be strategically placed to ensure fire does notspread in the case of combustion within a skip.

    4.4 Fire Prevention and Control

    Fire hydrants with fire hose reels are provided at different locations at the site and would bemade available for the construction area. Necessary fire extinguishers shall also be providedat different locations for emergency purposes.

    The contractors shall plan and chalk out the measures for the elimination of possibleconditions, which may lead to fire.

    The contractors shall provide adequate number of well-maintained fire extinguishers at eachwork area and for any hot works.

    The contractor shall ensure that workmen are trained in the use of fire extinguishers,understands the basics of fire fighting and train the workmen to become familiar with suchFire Fighting Procedures. A competent person shall periodically inspect all fire extinguishers.

    Fire extinguishers should be located in designated areas and clearly identified.

    The contractor shall take every precaution and use all reasonable means to prevent anoutbreak of fire and shall tender immediate assistance in case of fire. Any outbreak of fire inor near the workplace shall immediately be reported to the engineer in-charge of Owner /Consultants.

    Fire drills should be held at intervals to ensure personnel are familiar with the location, ofAssembly Point and response expected of them in case of emergency.

    Smoking on site is not permitted, but may be allowed in restricted areas as may be authorisedby the engineer. No smoking boards shall be displayed at prominent location includingstores/ storage places.

    Care of flammable materials, oily rags, waste etc. must not be left lying around or allowed toaccumulate. Covered metal skips should be used for disposal of flammable waste.

    If welding / cutting operations are to be carried out, non-flammable screens and blankets shallbe used to confine sparks hot metal or slag. A fire extinguisher should also be readilyavailable.

    Material Storage

    Construction materials such as steel, rubble, metal, cement, sand etc, shall be stored only inthe designated lay down area as shown in the site layout, within the barricaded constructionsite.

    The storage of combustible materials in buildings/ barracks shall be maintained at theminimum level, within the limits of the daily quantity required. Barracks where such material isstored shall be marked with warning signs and provided with appropriate extinguishers.

    Oxygen, acetylene, argon etc. cylinders shall be stored outside the buildings, in areas ofspecial security.

    Chemicals and other such goods should be stored in stable racks properly labeled. Mutuallyreactive chemicals should be kept away from each other. Storage place should have properventilation.

    4.5 Working at Height

    During the assessment of a task to be carried out risks must be taken into consideration inorder to identify the appropriate precautions. This includes the hierarchical consideration ofthe following control measures:

    Avoidance of the work at height

    Prevention of falls where the work at height cannot be avoided

    Appropriate equipment for fall prevention where risks cannot be prevented.

    Working at heights has proven to be amongst the most hazardous activities duringconstruction. Therefore, Contractor shall ensure proper Tie-off policy for working at heights.GSK ISC Technical specifications and standard guidelines for Scaffolding is attached asAttachment- 2

    Safe platforms include: removable platforms, fixed scaffold, removable tower scaffold, mobileplatforms. The working equipment (platforms, scaffold, etc) shall be adjusted / fixed withdecks with access trap, handrails (at about 1m height from deck level), middle rails and toeboard of at least 10 cm height. The access from one level of the scaffold (fixed or mobile) tothe next shall only be possible through inside stairs specially designated

    Wearing of safety belts is mandatory for working at height. Requirement of fall arrestor shallbe assessed & enforced, as required, prior to taking up the job.

    Bamboo and wooden scaffolding is prohibited. Only pipe scaffolding is permitted.

    It is mandatory that approved and competent scaffolders erect all scaffolding.

    During erection, dismantling or modification, scaffolds must be cordoned off and adequateprecautions must be taken to prevent materials falling. People unconnected with the workshould be kept away from the area by the use of signs and barriers.

    All materials must be raised and lowered in a controlled manner, and under no circumstances

    should components be dropped from heights during dismantling.

    Properly positioned and secured ladders must be used for access up or down the scaffolding.Climbing using the framework is not permitted and this includes scaffolders.

    Suitable standard metallic scaffolding shall be provided for Workmen for all WORKS thatcannot safely be done from the ground or from solid construction except such short periodwork as can be done safely from ladders. CONTRACTORs scaffolding design; material ofconstruction etc. shall be submitted to PMC/OWNER for his approval. Tall scaffolds shallhave a landing (Platform) at the ladder every six meters.

    All scaffolding will be designed and erected to support itself and four times intended load. Allscaffolds platforms and staging must be completely decked with decking secured and shall be

    built with standard handrails and toe boards on open sides and ends. The footing ofscaffolding shall be sound and capable of carrying four times maximum intended load.

    When scaffold heights exceed three times the smallest base dimension (or 2.5 meters), itmust be secured to a building or structure at the second lift and every lift thereafter. Minimumone access ladder shall be provided for every 25m of scaffolding length.

    Prior to deploying workers on any erected scaffolding the same shall be checked and certifiedby CONTRACTORs Safety Engineer. PMC/OWNERs Safety department shall inspect thesescaffoldings on a regular basis and any unsafe scaffolding, shall be rejected. Scaffold will betagged to show status:

    Red : Dont Use

    Yellow : Safety Harness Required Due To Some Type Of Physical Restraint, AComplete Scaffold Could Not Be Built. Tie Off Is Required and Mandatory.

    Green : Safe for Use

    Scaffolding or staging more than 4 meters above the ground or floor or suspended from anoverhead support or erected with stationary support shall have a guard rail properly attached,bolted, braced or otherwise secured at least 1 meters high above the floor or platform ofscaffolding or staging and extending along the entire length of the outside and ends thereofwith only such openings as may be necessary for the delivery of materials. Such scaffoldingor staging shall be so fastened as to prevent it from swaying from the building or structure.

    Working platforms, gangways and stairways should be so constructed that they should notsag unduly or unequally and if the height of the platform or the gangway of the stairway ismore than 4 meters above ground level or floor level, they should be closely boarded, shouldhave adequate width and should be suitably fastened as described above.

    Every opening in the floor of a building or in a Working platform be provided with suitable

    means to prevent the fall of persons or materials by providing suitable fencing or railingwhose minimum height shall be 1 meters.

    Safe means of access shall be provided to all Working platforms and other Working places.Every ladder shall be securely fixed. No portable single ladder shall be over 9 meters inlength while the width between the side rails in rung ladder shall in no case be less than 0.30meters for ladder up to and including 3 meters in length, for longer ladders this width would beincreased at least 0.06 meters for each additional foot of length. Uniform step spacing shallnot exceed 0.30 meters.

    The mobile scaffolds shall be provided with the same safety elements as the fixed scaffolds(double rails, toe board). Work on mobile scaffold is strictly forbidden if they are not fixed or ifthe castors are not secured in compliance with the construction type

    During the erection / dismantling of the scaffold safety harnesses shall be used, secured instable points. The use of safety harness is mandatory for works on the scaffold unlessotherwise certified by PMC/GSK Engineer.

    4.5.1 Ladders, Staircases & Safety nets and others

    Ladders must be of good construction, sound material and made as per standardspecifications. Wooden ladders will not be permitted at site. Ladders shall be checked fordefects such as worn rungs, loose tie rods, and split or frayed feet.

    Ladders are a means of access, and they should not be used as working platforms. Carryingof load while ascending and descending would not be allowed.

    Ladders shall be avoided where other safe platforms are more practical. Ladders may betaken into consideration for short-time work performed in daylight and they shall be securedduring the entire period of use and an extra labour provided for holding the same properly

    Ladder can be put in use only after inspection and approval. Each approved ladder shall betagged and a ladder inspection register maintained. Date of inspection and next due dateshall be indicated on the ladder.

    When a ladder is erected for use it must:

    * have a good footing (level)* be fixed at both top and bottom points of rest* extend at least 36 above the top landing level

    Ladders to scaffolding shall be clamped at a minimum of [two (2)] points and shall extend aminimum of [1.0 m] above the level of the platform it accesses.

    All Staircases should have guardrails (Waist and Knee rails) till permanent parapet or railingsare installed.

    Rope ladders may be used in special cases along with fall arrestor..

    If the ladder is used for carrying materials as well, suitable footholds and handholds shall beprovided on the ladder and the ladder shall be given an inclination not steeper than 1 in 4

    (1 horizontal and 4 vertical). Ladders are to be tied or secured at the top and base. Noextension ladder should extend its full length; it should overlap at least four rungs.

    Ladders should be inspected periodically and defects corrected or defective ladders shouldbe taken away from the site

    Other protective means: nets. Such systems shall be used only as last solution butconsideration shall be given to safety ropes for the work on flat roofs. 2 safety ropes shallalways be used for moving, so that one rope is secured on a safe element at any moment.

    Safety cables shall be attached to secure the structural elements and to properly position theropes of the safety harness so that the workers movements should be fully safe.

    The access roads shall be maintained clear and clean.

    In the Instructions for the Execution of the Work the contractor shall include the rescue plan incase of incidents / events of personnel working at height or on the roof.

    All working at heights shall be treated as a critical activity and Hazard analysis & method

    statements completed prior to taking up the job. Special permits should be obtained for workat height. Check list shall be used for inspecting the scaffoldings and the safety preparednessfor working at height.

    4.6 Hot work

    Hot work (open flame/ high temperature) includes the following operations:

    Welding

    Flame cutting

    Any other work that involves open flame or sparks

    All these works shall be carried out based on the appropriate work permit in which the

    preventive actions to be taken and the permit validity time are stated. All combustiblematerials shall be removed from the work area before the operation begins. The Site SafetyCoordinator shall inspect the area before the commencement of work to ensure that thesuggested precautions in the permit are fully complied with.

    Hot work shall be stopped one hour before the shift end and extension obtained forcontinuation in the next shift, from authorized personnel.

    1. All oxygen cylinders MUST be transported and stored in vertical position.2. Reducers and cylinders MUST be in good operational conditions (no cracks, faults

    etc.)3. All cylinders MUST be provided with backfire arrester.4. Cylinders not in use MUST be secured and stored in specially designated areas.

    5. Cylinders MUST NOT BE STORED inside the building without prior approval.6. The welding equipment MUST be compliant with the requirements in force.7. Welding & cutting operations shall comply with the specific requirements specified

    under section 4.12 & 4.13

    Minimum safe distance from hot work to other works must be of 1.5 m.

    Installations (utilities) that may be affected by such works must be isolated / separated beforethe operations begin.When the work is performed in the ventilation column or at high levels the cylinders shall beproperly secured and additionally protected with fire resistant or non-combustible material.

    Workers MUST remain within the work area for 15 minutes after the end of the operation to

    make sure there is no fire risk.

    The work area MUST be provided with an extinguisher fitted for the type ofoperation.The supervisor MUST ensure thatthe location is prepared with the PPE required for the workand that the workers involved WEAR this equipment. (Welding mask, welding panel, gloves,specific footwear, etc)

    To avoid dust, water shall be sprayed from the hydrant H2, as in the site layout plan.

    4.7 Work in Confined Space

    Confined or limited space is any area with limited exit ways or limited ventilation resulting inthe accumulation of vapors or inflammable gas, toxic substances or any atmosphere lackingoxygen. Such areas may be storage tanks, vessels, ventilation or evacuation ducts, collectingmains, underground utility vaults, tunnels, pipe networks, basements, etc.

    All such work shall be carried out based on the appropriate work permit with mention of thepreventive action to betaken and the permit validity period.All workers operating in such areas shall be appropriately trained on the nature of the risksinvolved, on the required preventive actions and on the use of the protective and emergency

    equipment, especially the respirator.

    Before allowing the workers access in any confined or limited space the atmosphere in suchspace shall be tested to identify the level of oxygen and the concentration of flammablevapors, gas or toxic substances.

    Communication (visual, voice, signals) shall be maintained between the workers outside theconfined space and those operating within the confined space, to provide help in case ofemergency.

    4.8 Noise

    Noise must be kept to a minimum at all times and must not exceed acceptable and/or locallyspecified rules and conditions relating to noise imposed by the construction activities. Dueregard must always be given to noise levels, permissible duration for noisy work operationsand other restrictions. Any work activity which is likely to expose any employee on site to anoise level of 85 db(A) or above, assessments should be carried out. In such circumstances,the contractor must keep stocks of ear defenders or other suitable hearing protection andissue to the workmen who are exposed to the higher levels of noise.

    The legal requirements for the protection of workers hearing are contained in the Building &Other Construction Workers (Regulation of Employment and Conditions of Services) CentralRules 1998. GSK standards & guidelines on Noise control shall be adhered to.

    General guide at a construction site is, if one has to raise his voice to be heard, place is toonoisy.

    Grinders, Cutters, Jack Hammer (pneumatic drills), Engine driven plant and the like, all giveoff levels of noise that is harmful.

    Earplugs and earmuffs for hearing protection to be used, wherever required and as instructedby PMC.

    If there is a concern that personnel cannot hear warnings, shouts or gas monitor alarms, thena method statement/ safety system of work should be provided for those operations

    4.9 Work Permit System

    No work shall be undertaken without a work permit. The contractor s work permit system shallfully comply with the Owners requirement and Owners PTW systems.. Owners relevantSOPs in this regard shall be shared with the successful bidders.

    The Contractors designated construction Engineers/Supervisors shall be responsible forensuring that the permits are issued prior to taking up the jobs. The permits are to bereviewed by the Contractors safety engineer/Supervisor and authorized by PMCsConstruction Supervisor as well GSK designated personnel

    For working in any of the operating areas (during tie in or post mechanical completion), orwherever LOTO requirements are there, the permits are to be authorized by GSK Customerrepresentative/his authorized representative.

    The permit system shall fully comply with the requirements of GSK Global Engineering

    Standard GES -110, Machinery Safety and GES 102 Plant Modifications.

    Special work permits shall be issued for the following types of operations

    hot work, work at height, excavations, LV electrical work, work on

    pressurized systems

    demolition work

    work in confined spaces

    Electrical HV and working on live circuits is only allowed by absolute exception and needsspecial approvals, special supervisions and to be approved by GSK EHS Manager.

    The PMC Safety Supervisor is responsible for the observance of the measures mentioned inthe above permits

    The work permit shall be visibly displayed by the CONTRACTOR at the entrance into theWORK AREA

    4.10 Asbestos Handling and Disposal

    Asbestos handling is not envisaged for this project..

    4.11 Electrical Supply (Construction Power) & Electrical Safety

    4.11.1 Electrical Supply & Construction Power

    The contractor shall appoint a competent person to the satisfaction of the engineer, on sitewho will be responsible for the control of all maintenance and repairs to any electricalswitchboard, distribution board, hand tools, etc

    The OWNER supply electricity for construction power, at one point, which would bedetermined by the PMC. The supply would be of 230V, 50Hz, and single phase or of 415/433V, 50Hz, three phase. Power dis
