B) HMSA REVIEW OF ARCHITECTURAL DRAWINGS (1 WEEK)

1. THE ARCHITECT SUBMITS CONSTRUCTION DOCUMENTS TO THE SITE PLANNING

SPECIALIST.

2. THE SITE PLANNER DOES A CURSORY REVIEW* OF THE ARCHITECTURAL DRAWINGS,

NOTES DISCREPANCIES, AND ADVISES THE ARCHITECT.

*NOTE: THIS IS A REVIEW AND NOT AN APPROVAL. HMSA ACCEPTS NO LIABILITY FOR

ERRORS OR OMISSIONS. IT IS THE RESPONSIBILITY OF THE CUSTOMER AND THEIR

DESIGN AND CONSTRUCTION PROFESSIONALS TO PREPARE THEIR SITE TO MEET THE

GUIDELINES CONTAINED IN THIS DOCUMENT AND ALL APPLICABLE CODES.

D) PRE-CONSTRUCTION MEETING

1. ALL PLANS ARE REVIEWED AND OUTSTANDING QUESTIONS ADDRESSED. A

CONSTRUCTION TIM ELINE SHOULD BE AGREED UPON AT THIS MEETING AND A

DELIVERY TIME ESTIMATED. TYPICALLY, MEMBERS OF THE FIELD SERVICE TEAM WILL

BE INTRODUCED TO THE CUSTOMER, ARCHITECT, AND CONTRACTOR AT THIS

MEETING. THE FIELD SERVICE TEAM WILL CONDUCT PERIODIC SITE VISITS FOR

SITE EVALUATION BEGINNING WITH THIS MEETING.

SITE CONSTRUCTION (4 WEEKS)

A) ON SITE EVALUATION

1. THE FIELD SERVICE TEAM WILL CONDUCT PERIODIC SITE VISITS TO EVALUATE

PROGRESS. THEY WILL ASSIST BY ANSWERING SITE PREPARATION QUESTIONS

OR FORWARDING THOSE QUESTIONS TO OTHER HMSA TEAM MEMBERS IF

NECESSARY. THEY WILL ALSO PASS ON THE SITE STATUS AND ANY OPEN

CONCERNS TO SITE PLANNING. SITE PLANNING WILL ADDRESS THOSE CONCERNS

WITH THE ARCHITECT OR CONTRACTOR AS NEEDED.

B) CONSTRUCTION PRIOR TO DELIVERY

1. THE SITE PLANNER AND THE CUSTOMER'S CONTRACTOR MUST MAINTAIN TIMELY

COMMUNICATION THROUGHOUT THE CONSTRUCTION PHASE.

2. THE CONTRACTOR SHOULD REFER TO THE HMSA SUPPLIED DRAWING, AS WELL AS

THESE STANDARD DETAILS, DURING CONSTRUCTION. ALL QUESTIONS AND CONCERNS

NOT COVERED IN THESE DOCUMENTS MUST BE RESOLVED BETWEEN THE SITE PLANNER,

CONTRACTOR AND THE CUSTOMER'S DESIGN PROFESSIONALS.

3. THE CONTRACTOR SHOULD REFER TO THE PRE-DELIVERY CHECKLIST (13 of 13) DURING

CONSTRUCTION AND IMMEDIATELY CONTACT THE SITE PLANNER WITH ANY CONDITIONS

THAT MIGHT AFFECT THE DELIVERY SCHEDULE.

TYPICAL PLANNING PROCESS (CON'T) 1. VERIFICATION AND COORDINATION OF THE SYSTEM DELIVERY MUST BE FINALIZED AT

LEAST lWO (2) WEEKS PRIOR TO THE SCHEDULED DELIVERY DATE. ALL PARTIES,

(CONTRACTOR, CUSTOMER, FIELD ENGINEER, RIGGER, SITE PLANNER, AND SALES

ADMINISTRATION) MUST BE KEPT INFORMED REGARDING THE DELIVERY DATE. ANY

CHANGE SHOULD IMMEDIATELY BE FORWARDED TO ALL PARTIES.

2. PRIOR TO DELIVERY, THE SUPRIA PRE-DELIVERY CHECKLIST (SEE PG.13 OF 13)

MUST BE COMPLETED BY THE CUSTOMER AND RETURNED TO HMSA. ALSO, ALL

FINANCING, WITH APPROPRIATE PAPERWORK, MUST BE APPROVED

AND IN PLACE WITH SALES ADMINISTRATION A MINIMUM OF 10 WORKING

DAYS PRIOR TO DELIVERY TO ALLOW FOR THE COORDINATION OF APPROPRIATE

DELIVERY TRANSPORTATION.

3. CUSTOMER MUST HAVE CONTRACTED FOR A RIGGER (IF REQUIRED) FOR THE DELIVERY

OF THEIR SYSTEM. SITE PLANNING CAN ASSIST IN IDENTIFYING A QUALIFIED RIGGER IN

YOUR AREA.

4. CUSTOMER MUST HAVE COMPLETED ALL FEDERAL, STATE AND LOCAL REGISTRATIONS

AND MET ALL APPLICABLE REQUIREMENTS TO ALLOW THE DELIVERY, INSTALLATION

AND SERVICING OF THE EQUIPMENT TO BE DONE IN CONFORMANCE WITH THOSE

REGULATIONS.

D) DELIVERY/INSTALLATION

1. ALL SUITE CONSTRUCTION MUST BE FINISHED.

2. COMPLETION OF MINIMAL PUNCH-LIST ITEMS MAY ONLY BE SCHEDULED WITH THE

APPROVAL OF HMSA FIELD SERVICE (INSTALLER). ANY ITEMS NOT AGREED UPON

MUST BE SCHEDULED FOR AFTER HOURS.

3. THE HMSA INSTALLER AND THE CUSTOMER'S RIGGER WILL HAVE FINAL APPROVAL OF

SYSTEM DELIVERY. THE DELIVERY MAY BE DELAYED SHOULD SITE CONDITIONS OR

WEATHER COMPROMISE THE SAFETY OF PERSONNEL OR THE SUPRIA SYSTEM.

4. THE ONE (1) WEEK INSTALLATION TYPICALLY BEGINS THE DAY OF DELIVERY. THE

CONTRACTOR AND THE ELECTRICIAN, WHILE NOT REQUIRED ON SITE, SHOULD BE

AVAILABLE DURING THE FIRST WEEK OF INSTALLATION. ANY INCOMPLETE

CONSTRUCTION RELATED ITEMS MAY CAUSE A DELAY IN EITHER THE START OR

COMPLETION OF THE INSTALLATION.

5. THE CUSTOMER/CONTRACTOR IS RESPONSIBLE FOR PROVIDING A 40 YARD DUMPSTER

AT THE TIME OF DELIVERY.

INSTALLATION (1 WEEK) 1. NO CONSTRUCTION MAY TAKE PLACE IN THE SCAN OR CONTROL ROOMS DURING

THE INSTALLATION.

2. WHILE NOT REQUIRED FULL TIME, THE CONTRACTOR AND ELECTRICIAN MUST BE

AVAILABLE DURING INSTALLATION. TYPICALLY, THE ELECTRICIAN IS NEEDED ON SITE

DURING THE FIRST lWO DAYS TO ASSIST WITH THE PRIMARY POWER CONNECTIONS

(PG 6 OF 13, NOTE 13 UNDER "PROVIDED BY CUSTOMER'S CONTRACTOR").

APPLICATIONS 1. AN HMSA APPLICATIONS SPECIALIST WILL WORK WITH THE SITE'S TECHNOLOGISTS TO

FAMILIARIZE THEM WITH THE OPERATION & PROTOCOLS OF THE SUPRIA SYSTEM. AN

ENTIRE WEEK IS SCHEDULED FOR INITIAL APPLICATIONS TRAINING WHICH INCLUDES A

LIMITED SCHEDULE OF PATIENT SCANNING BEGINNING THE SECOND DAY OF TRAINING.

AND THROUGH THE DESIGN, CONSTRUCTION, AND FINAL DELIVERY/INSTALLATION PROCESSES. PROPER PLANNING WILL HELP TO ENSURE HIGH SYSTEM PERFORMANCE STANDARDS, COMFORTABLE PATIENT MANEUVERING, AN EFFICIENT WORKING ENVIRONMENT AND SUFFICIENT STORAGE SPACE.

2. IN GENERAL, THE FOLLOWING SHOULD BE CONSIDERED WHEN DESIGNING ACT SUITE: A. WHAT ANCILLERY FUNCTIONS (DRESSING ROOM(S), TOILET(S), READING ROOM, ETC ... ) ARE REQUIRED OR DESIRED IN CLOSE PROXIMITY

TO THE CT SCAN SUITE. B. WHAT OPTIONAL COMPONENTS (CODONICS COLOR PRINTER, NETWORK LASER PRINTER, TERARECON WORKSTATION, ETC ... ) MIGHT BE

PLACED IN THE CONTROL ROOM. C. AMOUNT OF STORAGE REQUIRED TO ACCOMMODATE BOTH STAFF WORK SUPPLIES AND HOUSEKEEPING NEEDS (LINENS, CONTRAST,

MEDIAIV/INJECTION DISPOSIBLES, ETC ... ) D. CONVIENENCE OUTLETS MUST BE LOCATED IN THE CT SCAN ROOM AND THROUGHOUT THE SUITE FOR SERVICE AND TO ACCOMMODATE

RECOMMENDED AND GOVERNING REGULATIONS IN REGARD TO PATIENT PRIVACY AND SAFETY ISSUES.

PATIENT I OPERATOR SAFETY 1. A SINK SHOULD BE LOCATED IN OR NEAR THE CT SCAN ROOM FOR HYGIENE PURPOSES 2. BECAUSE THE CT SYSTEM PRODUCES X-RAYS, LEAD SHIELDING MAY BE REQUIRED. IT IS THE CUSTOMER'S RESPONSIBILITY TO CONTACT

A LICENSED RADIOLOGICAL HEALTH PHYSICIST OR OTHER QUALIFIED INDIVIDUAL TO PERFORM AN EVALUATION OF SHIELDING REQUIREMENTS. HMSA CAN NOT ESTIMATE, DESIGN, EVALUATE OR APPROVE LEAD SHIELDING DESIGNS.

3. DIRECT ACCESS BETWEEN THE CT CONTROL ROOM AND SCAN ROOM IS HIGHLY RECOMMENDED 4. SHUNT TRIP SWITCHES TO POWER DOWN THE SYSTEM IN EVENT OF AN EMERGENCY ARE HIGHLY RECOMMENDED (OPTIONAL UNLESS

ANCILLARY FUNCTIONS SUCH AS DRESSING ROOMS AND TOILETS SHOULD BE LOCATED ADJACENT TO OR IN THE VICINITY OF THE

SCAN ROOM. D. THE OPERATOR SHOULD HAVE DIRECT ACCESS TO THE PATIENT FROM THE CONTROL ROOM.

ENVIROMENTAL AND STRUCTURAL SPECIFICATIONS

LIGHTING 1. DIMMABLE LIGHTING IS RECOMMENDED IN BOTH THE SCAN AND CONTROL ROOMS. SOME CT SCAN PROCEDURES REQUIRE THE INSERTION

OF AN IV INTO THE PATIENT FOR CONTRAST MEDIA INJECTIONS. THIS WILL REQUIRE HIGHER LEVELS OF ILLUMINATION IN THE SCAN ROOM THAN ARE ARE TYPICALLY DESIRED DURING THE ACTUAL SCAN PROCESS. IN THE CONTROL ROOM, THE TECHNICANS OPERATING THE SYSTEM WILL BE PERFORMING TASKS THAT VARY FROM TYPICAL OFFICE TASKS TO VIEWING COMPUTER MONITOR SCREENS.

2. CARE SHOULD BE TAKEN WHEN DETERMINING THE PLACEMENT OF LIGHTING FIXTURES TO ENSURE PATIENT COMFORT IN THE SCAN ROOM AND A GLARE FREE ENVIRONMENT FOR THE TECHNICIANS IN THE CONTROL ROOM.

3. LIGHTING IN BOTH ROOMS MUST MEET OSHA AND/OR OTHER APPLICABLE STANDARDS FOR ILLUMINATION BASED ON THE TASKS TO BE PERFORMED.

HVAC 1. CLIMATE CONTROL MUST BE MAINTAINED 24 HOURS A DAY, 7 DAYS A WEEK. 2. THE CT SUITE TEMPERATURE MUST BE MAINTAINED BETWEEN 68-82•F; +/- 3•FtHOUR. 3. RELATIVE HUMIDITY MUST BE MAINTAINED BETWEEN 35% AND 80% WITHOUT CONDENSATION 4. HEAT LOADS FOR EACH ROOM MUST BE CALCULATED BY THE HVAC ENGINEER BASED ON SITE SPECIFIC EQUIPMENT PLACEMENT ALONG

WITH OTHER FACTORS SUCH AS LIGHTING, OCCUPANCY AND BUILDING EXPOSURE. 5. SEPARATE THERMOSTATIC ZONES FOR THE SCAN AND CONTROL ROOMS ARE HIGHLY RECOMMENDED DUE TO THE GREATLY VARYING

STRUCTURAL SPECIFICATIONS 1. ALL STRUCTURAL SUPPORT IS THE CUSTOMER'S RESPONSIBILITY. A QUALIFIED PROFESSIONAL MUST REVIEW THE SCAN ROOM FLOOR

DELIVERY I RIGGING INFORMATION

CUSTOMER/CONTRACTOR RESPONSIBILITIES

TO THE TERMS OF THE SALES AGREEMENT.

2. WITH THE ASSISTANCE OF THE SITE PLANNING SPECIALIST, DETERMINE AND CONFIRM THE DELIVERY PATH AND OFF LOAD AREA FOR

THE SUPRIA SYSTEM.

3. IDENTIFY, CONTRACT, AND SCHEDULE A RIGGER TO UNLOAD, UNCRA TE, AND DELIVER ALL SUP RIA SYSTEM COMPONENTS.

THE DELIVERY DATE WILL BE A COORDINATED EFFORT BETWEEN THE CUSTOMER, CONTRACTOR, HMSA, AND THE RIGGER.

THE DELIVERY DATE IS BASED ON SITE PREPAREDNESS, SYSTEM AVAILABILITY, RIGGER AVAILABILITY, AND ADMINISTRATIVE

APPROVAL. YOUR HMSA SITE PLANNING SPECIALIST CAN ASSIST IN IDENTIFYING AN EXPERIENCED RIGGER IN YOUR AREA.

4. ENSURE THE AREA REQUESTED BY THE RIGGER FOR DELIVERY TRUCKS AND WORK SPACE IS AVAILABLE AND FREE OF OBSTRUCTIONS.

5. ENSURE THE DELIVERY PATH FOR THE GANTRY AND OTHER SYSTEM COMPONENTS IS CLEAR AND FREE OF OBSTRUCTIONS.

6. HAVE A FACILITY REPRESENTATIVE AVAILABLE AT THE TIME OF DELIVERY.

7. SIGN SHIPPING DOCUMENTS UPON DELIVERY.

8. SECURE INSURANCE COVERAGE, EFFECTIVE UPON DELIVERY OF YOUR SYSTEM.

9. PROVIDE A SECURE STORAGE AREA NEAR THE CT CONTROL ROOM (MINIMUM 10' X 10') FOR ANCILLARY SYSTEM COMPONENTS UNTIL

THE INSTALLATION PROCESS IS COMPLETED.

10. IF A SECURABLE STORAGE AREA IS NOT AVAILABLE, HMSA CAN ASSIST IN ARRANGING WITH THE TRANSPORTATION COMPANY TO

STORE THE ANCILLARY COMPONENTS FOR A SECOND DELIVERY. THE CUSTOMER IS RESPONSIBLE FOR ANY ADDITIONAL COST OF

STORAGE AND DELIVERY/RIGGING. THIS MUST BE ARRANGED PRIOR TO THE INITIAL DELIVERY.

HMSA RESPONSIBILITIES

1. ASSIST THE CUSTOMER AND CONTRACTOR IN IDENTIFYING A RIGGER TO UNLOAD, UNCRATE, AND DELIVER THE SUPRIA SYSTEM.

2. ASSIST THE CUSTOMER, CONTRACTOR, AND RIGGER IN IDENTIFYING A DELIVERY PATH FOR THE SYSTEM.

3. COORDINATE THE DELIVERY DATE OF THE CT SYSTEM WITH THE CUSTOMER, CONTRACTOR, AND RIGGER.

4. PROVIDE THE CUSTOMER, CONTRACTOR, AND RIGGER WITH DELIVERY AND HANDLING PRECAUTIONS FOR THE CT SYSTEM.

5. ASSIST THE CUSTOMER AND CONTRACTOR IN LOCATING AN ADEQUATE STORAGE SPACE FOR THE ANCILLARY COMPONENTS.

6. AN HMSA SERVICE REPRESENTATIVE WILL BE AT THE DELIVERY TO PROVIDE DIRECTION FOR THE UNLOADING, UNCRATING,

AND PLACEMENT OF ALL SUPRIA COMPONENTS.

RIGGER RESPONSIBILITIES

1. SECURE INSURANCE COVERAGE FOR TRANSPORTATION, RIGGING, AND DELIVERY OF THE CT SYSTEM. PROVIDE THE HMSA SITE

PLANNING SPECIALIST WITH A CERTIFICATE OF INSURANCE PRIOR TO DELIVERY.

2. DETERMINE AREA FOR UNLOADING, RIGGING, AND DELIVERING THE SYSTEM COMPONENTS, INCLUDING CRATE UNPACKING AREA.

3. INSPECT SITE PRIOR TO DELIVERY TO DETERMINE THE BEST ROUTE AND CONFIRM ADEQUATE ACCESS FOR DELIVERY.

4. PROVIDE COST ESTIMATE TO THE CUSTOMER PRIOR TO DELIVERY.

5. TAKE NECESSARY PRECAUTIONS TO PROTECT THE EQUIPMENT AND THE FACILITIES (I.E. FLOORING, WALL COVERING, ETC.).

6. PROVIDE NECESSARY EQUIPMENT FOR DELIVERY OF THE SUPRIA SYSTEM, ( IE: CRANE, FORKLIFT, ROL-A-LIFTS, & PALLET JACKS).

7. APPROVE FINAL DELIVERY ROUTE.

8. UNLOAD, UNCRATE, AND DELIVER THE SYSTEM INSIDE AS DIRECTED BY HMSA SERVICE REPRESENTATIVE ON-SITE.

9. DISPOSE OF ALL PACKING AND CRATING MATERIALS AS DIRECTED BY THE HMSA SERVICE REPRESENTATIVE.

SUPRIA PRE-DELIVERY/INSTALLATION SITE CHECKLIST The following checklist notes items that must be completed prior to the delivery and installation of the SUPRIA CT system. “Per site drawing” on this checklist refers to the HMSA final site specific drawing provided to the customer and their contractor. It is the customer’s/contractor’s responsibility to provide masonite or other protective covering for the floors to prevent damage during the delivery and placement of the equipment. THIS CHECKLIST MUST BE COMPLETED AND RETURNED TO HMSA TWINSBURG PRIOR TO DELIVERY

GENERAL CONSTRUCTION: All room dimensions verified against the final HMSA site drawing and Site Planning notified of any discrepancies Scan room ceiling height is 8’-7” (min req’d for tube crane) or greater Counter for Operator Workstation installed in control room if applicable All other casework and sink installed and complete Scan and Control room construction complete All rooms free of dust and debris Protective floor covering (masonite or equal) in place or available for delivery

ELECTRICAL:

Disconnect installed at Image Processor location and ready to be energized: 208V, 200A, 3 phase, 4 wire Emergency stop buttons (Shunt trip) installed and functional “X-Ray On” warning lights installed at scan room doors and location in scan room visible from anywhere in room Door interlock switches (optional) installed Wiring for “X-Ray On” and door interlocks run through wireway w/ 6’ pigtails at rear of gantry Wall mounted wireway with 2 equal compartments and removable cover per site drawing (divider open @ vertical transition) On-floor/ in-floor wireway with 2 equal compartments and removable cover per site drawing Standard service outlets (120V) throughout suite (minimum 1 per wall) and required outlets (120V) per site drawing Telephone in control room for Technician and Service Engineer use Network (DICOM) drops located per site drawing Lights with adjustable brightness levels installed and functional in scan and control rooms Electrician scheduled for the morning after delivery to connect power and provide wireway access openings per installer ENVIRONMENTAL: HVAC system with separate thermostatic zones for scan and control rooms operational

FACILITIES:

HMSA must have 24 hour access to facility Chairs and a restroom must be available in the work area Convenient storage area is available for miscellaneous equipment A dumpster will be available for disposal of packing debris Delivery path is accessible and free of obstructions (minimum 38” clearance in straight path) Area for parking delivery truck and off loading identified and arrangements made to keep it clear for delivery date All of these items have been checked and are complete. All of these items have been checked and exceptions with completion dates noted. Customer has completed all applicable Federal, State and Local registrations and met all applicable requirements to allow for the delivery, installation and service of the equipment in conformance with those regulations.
