I have devoted the past 20 years of my career developing my specialist role in the field of palliative and end of life care. As a highly qualified, experienced and motivated pharmacist I aspire for people with cancer or other life limiting condition and those important to them to experience medicines treatment to the level of the very best by offering high quality, contemporary care in ways that avoids harm and is delivered in a seamless and timely fashion in the place that's right for them. Using excellent clinical management, collaborative and influencing skills at a professional leadership level within the multidisciplinary team has empowered me to deliver high quality people-focussed care that is evidence-based and meets the requirements of the regulator at all times. Integrating my specialist role into high level strategic teams accountable for quality within the organisation and its partners allows me to drive pharmacy issues into models for improvement. This is particularly so in the area of opioid safety that my founding role and co-chair position for the London Opioid Safety and Improvement Group has enabled me to lead on. Managing services to organisations that offer care across different settings allows me to understand the complex processes that need to be addressed so that provision remains safe, effective and well- led, and is delivered in a seamless fashion that meets the expectations of the individual person and those important to them of what good quality care at the end of life means and, importantly, is offered to them in the place of their choice. Specialist areas of practice 1. Hospice pharmacy services – developing services across adult and children’s hospices. Services focus on optimising the use of medicines, including the use of a person’s own medicines including self-administration and young adult transition schemes, and sharing information about a person’s medicines between care settings so that admission and discharge is not delayed. Outcomes are monitored against indicators of quality and safety to meet the requirements of the regulator and the expectations of the patient and those important to them at all times.

medicinal practice for specialist palliative and end of life care services to Guy’s and St. Thomas’ NHS Foundation Trust, Royal Trinity Hospice, St. Joseph’s Hospice, Richard House Children’s Hospice and Haven House Children’s Hospice.

Chair the incident surveillance group for specialist palliative and end of life care that reports quality assurance to the Trust end of life clinical governance committee.

Provide consultant pharmacist support, including clinical supervision and assessment to other pharmacy and healthcare professional leads across the organisation, primary care and independent sectors in the delivery of specialist palliative and end of life care.

Produce clinical guidelines for use within the organisation that provide examples of best practice and that can influence the practice of others locally, nationally and internationally.

Provide a consultant outpatient clinic and domiciliary home visiting service to palliative care patients.

Member of the specialist palliative care working groups within integrated networks including the Royal Marsden Vanguard, London Cancer PallE8, and the South East London Accountable Clinical Network as this develops.

The RPS Advanced Pharmacy Framework (APF) is a framework for identifying and recognising the stage of practice. It is applicable to all sectors and nations, building on what have previously been shown to work. This can form the basis of the professional portfolio.

The APF builds on the widely used Advanced to Consultant Level Framework (the ACLF), to ensure it is applicable to all sectors and specialisms across pharmacy in Great Britain. The evidence supports its use across the profession, and more widely, for the development of both advanced and specialist practice, applicable to all who work in pharmacy. The APF is intended for use once early or foundation years have been completed. It forms a useful supportive framework to gather evidence of advancement across the core competencies

10.1 The RPS Faculty Professional Recognition Professional recognition involves formal acknowledgement of professional development and advancement in the scope of professional practice. Many professions have established methods to recognise advanced competencies (knowledge, skills, experiences, and behaviours) set against career pathways, where career progression is aligned with the ability to deal safely with increasingly complex patients and situations in a safe and appropriate way. There are four main elements required for formalised professional recognition: i. Access to the knowledge, skills, experiences and behaviours needed for careers advancement (i.e. frameworks). ii. Access to professional curricula to support the development of knowledge, skills, experiences and behaviours across the clusters underpinning advanced practice. iii. Access to high quality accredited or endorsed educational, training, research, management and leadership resources. iv. Credentialing of an individual’s attainment, i.e. awarding of post-nominals. All of these are provided by the Faculty and enable you to work towards and obtain consistent and validated professional recognition of advanced practice. What does the Faculty Provide? The Faculty provides support, networks, access to experts and mentors across all sectors, and at all stages of your professional careers, alongside opportunities to develop professionally, to build a portfolio of transferable knowledge and skills that is widely recognised. The RPS manages and leads the Faculty, providing governance to ensure quality and consistency across all sectors and at all levels. The Faculty will help you to demonstrate excellent patient care by supporting your development. It will enable you to access a range of quality assured and validated professional curricula, support tools and frameworks which support your professional development, enabling you to focus your learning, professional development and Continuing Professional Development (CPD) and gain recognition for your enhanced knowledge and experience. The Faculty will also signpost to education/learning opportunities that support your development. The Advanced Pharmacy Framework (APF) has been developed to support professionals across all sectors to identify what they need to know or do to advance. This forms the backbone of the Advanced Practice Portfolio (APP). Building your APP and peer review will enable you to reflect on your practice and support your ongoing professional development in

order to benefit your practice whether in a patient facing or wider pharmacy role. You can also use your portfolio entries for CPD records. Being a member of the Faculty will also help you to stand out in increasingly competitive jobs markets and will support your career progression. It will provide you with a mark of quality and assurance of progression which will be valued by employers and commissioners of services. My Faculty portfolio will be a key component in the assessment of my skills, knowledge, experience and behaviours, it will count towards professional recognition of my practice and in turn, the awarding of post-nominals, according to my stage of professional development. Whether or not I am ready to join the Faculty, building a portfolio will be an effective way of developing my practice, supporting my career progression and may help prepare me for revalidation in the future.
