ABOUT BED POSITIONING

Positioning the Patient

One of the primary purposes of moving and positioning patients is to prevent pressure sores. The incidence of pressure sores usually develop as the result of pressure over bony prominences. Sores are the result of inadequate blood and oxygen flow to the area and can develop in a short time. These sores can develop in the bed and the best way to prevent them is to keep pressure off the skin cov-ering the bony prominences. This can be done by proper positioning and padding by moving the patient frequently. A high mechanical load for a brief period or a low mechanical load for a prolonged period of time increases the risk for skin breakdown.

Determine Repositioning Frequency with Consideration to the Individual

• Tissue Tolerance • Level of activity & mobility • General medical condition • Skin Condition • Comfort

Teach individuals to do pressure relief lifts or other pressure relieving manoeuvres.

Avoid positioning the individual on bony prominences with existing non-blanchable erythema

Avoid subjecting the skin to pressure and shear forces

Use manual handling aids to reduce friction and shear lift, don’t drag the individual while repositioning.

Repositioning Individuals in Bed

Use the 300 tilted side-lying position (alternately right side, back, left side) or the prone position if the indi-vidual can tolerate this and his/her medical condition allows.

Encourage individuals who can reposition themselves to sleep in a 300 to 400 side lying position or flat in bed if not contraindicated.

Limit head of bed elevation to 300 for an individual on bed rest unless contraindicated by medical condition or feeding and digestive consideration.

Positioning the patient in good alignment is very important. This means that the spine is straight and other parts of the body are in good alignment. When the patient is positioned correctly, function of the bodysystems is improved. Using a pull sheet is easier to position the patient than pulling the body directly. Using these devices prevents accidental injury to the patients skin and trauma to the muscles, bones and joints. These slide sheets also help prevent back injury of the caregiver. Use of foam wedges are also useful in helping the patient maintain a side lying position and eliminating pressure from the coccyx. A patient who is using a pressure relieving pad or mattress still requires turning. Although some pressure is relieved, the devices do not remove it entirely, so further breakdown is possible. In some cases, in addition to the side lying wedges, other devices such as heel and elbow protectors, heel elevators, side rail pads, foot cradles to keep the weight of the blanket off the feet and special mattresses may be requiredText above is from BARBARA ACELLO, Basic Skills for the Healthcare Provider

The four handle side lying positioning system described in this flyer provides all of the components to properly position the patient to off-load the coccyx area. In addition, the low friction, four handle slide sheet facilitates in moving and positioning the patient while minimizing the risk of caregiver back injury.

Purpose This transfer system provides a secure positioning surface for side lying residents which is an important feature when it’s necessary to off-load pressure from the coccyx area. Also when one or both of the bed rails are lowered, the bolster system helps prevent the resident from rolling off the bed. Note that a standard wedge tends to migrate when a resident leans against it whereas this inter-locking system (wedge and transfer sheet) prevents the wedge from moving. The system also prevents entrapment between the bed and mattress.

Easy to Use A standard wedge tends to move or migrate as the residents applies a side lying force. Utilizing an inter-locking system when the transfer pad is placed over the wedge, the two mating (hook and loop) strips of Velcro® mate and as a result, the wedge remains in place as pressure is applied. The offering of products allows you to utilize one wedge and one sheet (models 556135,556136 and 556137) or if desired, the same sheet can mate on both sides with an additional wedge (models 556040, 556041 and 556056). Three lengths of wedges (16”, 24”& 34”) are offered to accommodate a wide range of body shapes and sizes. An additional nylon sheet is offered for the low-friction easy slide characteristics that is placed over the bed sheet and under the slide system. The mesh sheet is offered in the event the system is used above a low air loss mattress. Either of the bolsters or sheets can be used as components without incorporating the components into a system.

Easy to Order Each set comes with one bolster and one slider sheet. The bolster and slider sheets (system) sizes are annotated above (model numbers 556135, 556136 & 556137). When ordered, to minimize shipping costs, two sets are packaged together and the unit of shipping is comprised of two sets. Under normal use the low fricition bottom, slider sheet is placed over the mattress cover that is generally cotton, which has a high coefficient of fricition. To relieve additional stress from the caregiver, additional EZ Glide sheets (model numbers 556065, 556066 & 556067) can be purchased and placed over the mattress covering and under the slider sheet.
